    Implantation of paclitaxel-eluting stents in saphenous vein grafts: clinical and angiographic follow-up results from a multicentre study.

    Objective: To define the clinical and angiographic follow-up results after implantation of paclitaxel-eluting stents (PESs) in stenotic saphenous vein grafts (SVGs). Design: Prospective multicentre study. Comparison with a control group. Methods: 60 consecutive patients with 65 lesions located in 65 SVGs (mean (SD) age of vein grafts 11.3 (5.7) years) treated with PES (V-Flex Plus, 2.7 mg/mm2 paclitaxel, Cook) and 60 patients with 60 SVG lesions treated with bare metal stent (BMS) were included. Lesions had to be ,20 mm in length and in grafts of 2.75–3.5 mm diameter. The 6 month angiographic follow-up was obtained on 51 lesions (79%) of the PES group and on 51 lesions (85%) of the BMS group. Results: Baseline clinical and angiographic characteristics were comparable between both groups. At angiographic follow-up, three vein grafts in the PES group and five vein grafts in the BMS group were occluded. In-stent late lumen loss was lower in PES than in BMS (0.61 (0.81) vs 1.06 (0.72) mm, respectively; p = 0.021). In-stent binary restenosis rates were 12% vs 33%, respectively, (p = 0.012). Linear regression analysis showed BMS to be the only factor with an effect on late lumen loss (p = 0.011). Target-vessel failure rates were 18% in the PES group and 41% in the BMS group (p = 0.019), whereas major adverse cardiac event (MACE) rates at 180 days were 15% and 37%, respectively (p = 0.014). Conclusions: Implantation of non-polymer-based PES in SVG lesions is associated with a lower late lumen loss and restenosis rate than those of BMS. There remains a substantial target-vessel failure rate and MACE rate even at 6 months owing to graft occlusion or new lesions in the graft

    mHealth Engineering: A Technology Review

    In this paper, we review the technological bases of mobile health (mHealth). First, we derive a component-based mHealth architecture prototype from an Institute of Electrical and Electronics Engineers (IEEE)-based multistage research and filter process. Second, we analyze medical databases with regard to these prototypic mhealth system components.. We show the current state of research literature concerning portable devices with standard and additional equipment, data transmission technology, interface, operating systems and software embedment, internal and external memory, and power-supply issues. We also focus on synergy effects by combining different mHealth technologies (e.g., BT-LE combined with RFID link technology). Finally, we also make suggestions for future improvements in mHealth technology (e.g., data-protection issues, energy supply, data processing and storage)

    Community stroke knowledge: a new information strategy using a joint project of the public health service and the hairdressers' guild of the Wesel district

    Objectives: The public health programme “Healthy Lower Rhine
against Stroke” is aimed at improving the population’s knowledge about stroke and thus at reducing the prehospital phase in patients with suspected stroke. First evaluation results indicate that apart from providing information through the mass media, there is an urgent need to further develop the face-to-face communication approach. This has to be achieved by efficient but also effective means, given that financial and personnel resources are scarce. Study design: In cooperation with lögd Bielefeld, the Lower Health Authority of the Wesel District (health department) developed a postcard-sized quiz card containing exclusively correct answers on the issue of stroke, risk factors as well as symptom and action knowledge. For face-to-face communication, the hairdressers could be convinced to be included in the project. The hairdressers posed the corresponding questions and marked those answers of the clients that were identical with the quiz card answers with a cross. Answers not given by the clients were read out loud to them by the hairdressers, who were thus “styling up” the knowledge of their clients. To increase participation in the project, prizes were offered for the hairdressers with the most filled-in quiz cards as well as for three of the participating clients (drawing of prizes 1–3). More than 380 hairdressers in the Wesel district were sent a letter inviting them to participate as facilitators in this project, which is probably the first of its kind worldwide. Methods: The machine-readable quiz cards were collected and statistically evaluated including data regarding age and gender of the participants. Results were to be presented in the form of a descriptive statistic. Results: Thirty-three hairdressers from 12 cities and municipalities of the Wesel district participated in this joint action of the Wesel district Department of Health and the Wesel hairdressers’ guild, dealing with the monitoring and imparting of basic knowledge on the issue of stroke. Almost 2,000 clients were interviewed by the participating hairdressers, and knowledge gaps were closed by information read out to them. Discussion: This innovative approach of imparting knowledge can be regarded as the model of an effective and economical way of communicating health information to the broader public

    Towards Evaluating Ambient Assisted Living: State-of-the-Art Use of Measures

    In order to determine the current evaluation of Ambient Assisted Living (AAL) solutions’ benefits, a systematic review of state-of-the-art literature was conducted. Four scientific online libraries were searched, articles combining AAL and controlling were considered relevant. In total, 223 abstracts were identified. Each of them was assessed regarding its suitability for the research at hand. The subsequent review contained analysis of 13 heterogeneous relevant articles: two of them discussed instruments and indicators for efficiency, eleven focused on quality aspects. Evidence on concrete measurement of benefits is specific and limited. Analyses of outcomes for stakeholders have not been standardized, yet. Demand for larger studies of AAL solutions and more focus on patients’ and economic perspectives exists

    Faraday rotation maps of disk galaxies

    Faraday rotation is one of the most widely used observables to infer the strength and configuration of the magnetic field in the ionised gas of the Milky Way and nearby spiral galaxies. Here we compute synthetic Faraday rotation maps at z=0z=0 for a set of disk galaxies from the Auriga high-resolution cosmological simulations, for different observer positions within and outside the galaxy. We find that the strength of the Faraday rotation of our simulated galaxies for a hypothetic observer at the solar circle is broadly consistent with the Faraday rotation seen for the Milky Way. The same holds for an observer outside the galaxy and the observed signal of the nearby spiral galaxy M51. However, we also find that the structure and angular power spectra of the synthetic all-sky Faraday rotation maps vary strongly with azimuthal position along the solar circle. We argue that this variation is a result of the structure of the magnetic field of the galaxy that is dominated by an azimuthal magnetic field ordered scales of several kpc, but has radial and vertical magnetic field components that are only ordered on scales of 1-2 kpc. Because the magnetic field strength decreases exponentially with height above the disk, the Faraday rotation for an observer at the solar circle is dominated by the local environment. This represents a severe obstacle for attempts to reconstruct the global magnetic field of the Milky Way from Faraday rotation maps alone without including additional observables.Comment: 10 pages, 10 figures, accepted by MNRA

    The bimodality of the East Siberian fast ice extent: mechanisms and changes

    Using operational sea-ice maps, we provide first insight into the seasonal evolution of fast ice in the East Siberian Sea for the period between 1999 and 2021. The fast ice season tends to start later by 4.7 d per decade and to end earlier by 9.7 d per decade. As a result, there is a trend towards a shorter length of fast ice season by 2 weeks per decade. The analysis of air temperatures indicates that onset and end of the fast ice season are largely driven by thermodynamic processes. Two spatial modes (large, L-mode and small, S-mode) of East Siberian fast ice cover which have significant areal differences were distinguished. The occurrence of L- and S-modes was linked to the polarity of the Arctic Oscillation (AO) index. Negative AO phase leads to increased sea-ice convergence in the region, which in turn favours sea-ice grounding and promotes the development of large fast ice extent (L-mode). Lower deformation rates in the region during positive AO phase does not allow the formation of grounded features which results in small fast ice extent (S-mode). An analysis of sea-ice divergence confirms that L-mode seasons are characterised by higher on-shore convergence compared with S-mode seasons

    We study experimentally the flow of a liquid metal confined between differentially rotating cylinders, in the presence of externally imposed axial and azimuthal magnetic fields. For increasingly large azimuthal fields a wave-like disturbance arises, traveling along the axis of the cylinders.     The RIN: an RNA integrity number for assigning integrity values to RNA measurements

    BACKGROUND: The integrity of RNA molecules is of paramount importance for experiments that try to reflect the snapshot of gene expression at the moment of RNA extraction. Until recently, there has been no reliable standard for estimating the integrity of RNA samples and the ratio of 28S:18S ribosomal RNA, the common measure for this purpose, has been shown to be inconsistent. The advent of microcapillary electrophoretic RNA separation provides the basis for an automated high-throughput approach, in order to estimate the integrity of RNA samples in an unambiguous way. METHODS: A method is introduced that automatically selects features from signal measurements and constructs regression models based on a Bayesian learning technique. Feature spaces of different dimensionality are compared in the Bayesian framework, which allows selecting a final feature combination corresponding to models with high posterior probability. RESULTS: This approach is applied to a large collection of electrophoretic RNA measurements recorded with an Agilent 2100 bioanalyzer to extract an algorithm that describes RNA integrity. The resulting algorithm is a user-independent, automated and reliable procedure for standardization of RNA quality control that allows the calculation of an RNA integrity number (RIN). CONCLUSION: Our results show the importance of taking characteristics of several regions of the recorded electropherogram into account in order to get a robust and reliable prediction of RNA integrity, especially if compared to traditional methods
