Módosított Kleinsasser-laringoszkóp bemutatása két eset kapcsán
Összefoglaló. Az algaratot és a gégét érintő, minimálisan invazív transoralis műtéti eljárások elvégzésekor elsődleges ezen relatíve szűk anatómiai régiók megfelelő feltárása. A hagyományos operációs endoszkópok lehetővé teszik ugyan a műtéti terület kellő vizualizációját, kialakításukból adódóan azonban nem optimálisak a különböző mikrosebészeti eszközök koordinációja és szimultán használata szempontjából. A mikroszkóp és a feltáró eszköz közötti relatíve nagy távolság pedig negatívan hat a célterület megvilágíthatóságára és a lézernyaláb fizikai tulajdonságaira is. A szerzők egy, az algaratot, valamint a gégét érintő elváltozások lézer-mikrosebészetében használt feltáróeszközt módosítottak: az endoszkóp oldalain kialakított nyílások lehetővé teszik a mikrosebészeti eszközök oldalsó bevezetését, emellett a mikroszkóp és a célterület közötti távolság jelentős csökkentését. A glotticus területet és a nyelvgyököt érintő műtét kapcsán bemutatott módosított operációs endoszkóp lehetővé teszi a műtéti terület megfelelő feltárását, a lézer optimális használatát és több mikrosebészeti eszköz egyidejű, szabad mozgatását. Orv Hetil. 2022; 163(17): 688-692. Summary. In cases of transoral laryngeal and hypopharyngeal surgeries, the exploration of these relatively narrow anatomical regions is paramount. The conventional operational endoscopes allow the visualization of the surgical field, but - due to their structure - are not optimal regarding the coordination and simultaneous moving of microsurgical instruments. Furthermore, the relatively great distance between the surgical microscope and the endoscope has a negative impact on the illumination of the surgical area and the physical characteristics of the laser beam. The authors introduce a modified laryngoscope used in the field of laryngeal and hypopharyngeal surgery. The openings in the sides of the endoscope allow the lateral insertion of microsurgical devices, while the distance between the microscope and the target area can be significantly reduced. The endoscope is presented in addition to a tongue base and a glottic surgery. The introduced operational endoscope allows optimal exploration of the area of interest, the free and simultaneous moving of the microsurgical instruments, and the optimal usage of the laser beam. Congenital external auditory canal atresia and methods of rehabilitation
Congenital external auditory canal atresia is a disorder with a prevalence of one in 10 000-20 000 live births and is bilateral in one third of the patients. With a conductive hearing loss of 60 dB, even unilateral atresia restricts hearing related social skills. The degree of middle ear deformity may make reconstruction surgery impossible or too hazardous, thus bone-conduction hearing aids have become the first-line therapy. Children with unilateral cartilaginous and bony external auditory canal atresia were enrolled. High-resolution computed tomography with three dimensional reconstructions were made. Reconstruction surgery from retroauricular approach comprised maximal enlargement of the tympanic and mastoid cavities and their closure with adapted conchal cartilage. Hearing improvement reached the level above the social threshold. The auditory canal remained stable and widely patent and facial nerve function was unremarkable. The authors highlight that surgical reconstruction of the external auditory canal is possible in selected cases. The procedure is safe and effective with a reasonably short surgical time, if it is supported by deep anatomical knowledge, careful preoperative imaging and intraoperative facial nerve monitoring. Stable audiological benefits improve patients’ satisfaction and quality of life. If reconstruction surgery is not possible, bone-conduction hearing aids are beneficial
Endoscopic Arytenoid Abduction Lateropexy with Endolaryngeal Thread Guide Instrument for Bilateral Vocal Fold Paralysis
The treatment of upper airway stenosis is considered to be one of the most difficult fields in laryngology. In the 100-year-old history of airway stenosis surgery several important works of Hungarian authors (Rethi, Lichtenberger, Pytel) are found.At the Department of Oto-Rhino- Laryngology and Head- Neck Surgery, University of Szeged our workgroup has been working on the treatment of upper airway stenosis for more than 30 years.Hereby we introduce our surgical concept for bilateral vocal fold paralysis, the minimally invasive endoscopic arytenoid abduction lateropexy (EAAL), which provides an immediate adequate airway with acceptable voice quality, and good swallow function.A new Endolaryngeal Thread Guide instrument (ETGI) is also presented here, which is essential for a safe, accurate, and fast suture loop creation around the arytenoid cartilage for this surgical procedure

Módosított Weerda-laringoszkópok alkalmazási területei a fül-orr-gégészeti gyakorlatban = Application of modified Weerda laryngoscopes in the ear, nose, and throat practice
Absztrakt:
A légutak és a tápcsatorna felső szakaszának általános érzéstelenítésben történő,
mikroszkópasszisztált direkt endoszkópiája a fül-orr-gégészet egyik alapvető
diagnosztikai és terápiás módszere. A műtéti feltárás minősége a terület jó
vizualizálhatósága és a mikrosebészeti eszközök kellő manőverezhetősége
szempontjából kulcsfontosságú. A leggyakrabban alkalmazott
Kleinsasser-laringoszkóp mellett a retrocricoid régió feltárására a kettős
lapocú Weerda-laringoszkóp előnyösebb választás. A szűk anatómiai viszonyok, az
algarat és a gége több alrégiójára is kiterjedő elváltozások vizsgálata és
manipulálása bizonyos esetekben azonban még ezzel az eszközzel is problémás
lehet. A szerzők négy eset kapcsán bemutatják az általuk módosított,
aszimmetrikus lapochosszúságú Weerda-laringoszkópok alkalmazását. Az eszközökkel
a retrocricoid régió, az algarat hátsó falának distalis és a nyelőcső proximalis
része is jól vizualizálható. A módosítások lehetővé teszik új műtéti technikák
bevezetését az alkalmazott mikrosebészeti eszközök számának növelésével és még

Abstract:
Microscope-assisted direct endoscopy of the airways and the upper
gastrointestinal tract in general anesthesia is an essential diagnostic and
therapeutic method of otorhinolaryngology. The quality of surgical exploration
is a key for the good visibility and maneuverability of microsurgical tools.
Beside the most commonly used Kleinsasser laryngoscope, the distending Weerda
laryngoscope is a more favorable option for the exploration of the retrocricoid
region. Narrow anatomical situation, examination and manipulation of lesions
involving several sub-regions of the hypopharynx or larynx may be problematic in
some cases even with this endoscope. The authors present the application of
modified Weerda laryngoscopes with asymmetric spatula length in addition with
four cases. The retrocricoid region, the distal part of the posterior
hypopharyngeal wall and the proximal part of the esophagus can be easily
visualized with this device. The modifications allow the introduction of new
surgical techniques by increasing the number of applied microsurgical devices

Extended partial laryngectomy with functional preservation using the rotational crico-thyrotracheopexy
ObjectivesGlottic tumors with infiltration of the anterior/posterior commissure, and lesions with subglottic, cricoid, or cricoarytenoid joint infiltration have been highly controversial in the past, from the perspective of oncological safety. Although conservation laryngeal resection options exist, most are limited by the extent of resection proscribed by the technique and the postoperative functional results. Oncologically speaking, extended vertical hemilaryngectomy is often the optimal solution. However, limited reconstruction methods often compel total laryngectomy.MethodsEight patients with vocal fold malignancy, which infiltrated the anterior and sometimes the posterior commissure and with subglottic extension and resultant uni/bilateral vocal fold motion impairment, were treated by single stage extended vertical partial laryngectomy with rotational crico-thyrotracheopexy as a functional reconstruction of the laryngeal framework. Patients were evaluated with objective and subjective function tests.ResultsHistologic examination demonstrated tumor-free margins in every case. Definitive decannulation was successful in all cases within 2 weeks. All patients had a stable and adequate airway during follow-up and reported socially acceptable voice. Oral feeding was possible in seven patients.ConclusionRotational crico-thyrotracheopexy, as a single stage reconstruction technique, is based on well-vascularized, readily available, appropriately shaped local tissues, without significant donor site morbidity or need for long-term stenting to reconstruct large laryngeal defects after extended vertical hemilaryngectomy for advanced unilateral glottic tumors and is applicable even with supra/subglottic invasion or infiltration of the contralateral vocal fold. Endoszkópos arytaenoid abdukciós lateropexia foniátriai eredményei átmeneti kétoldali gégebénulás esetén = Phoniatric outcomes of endoscopic arytenoid abduction lateropexy in patients with transient bilateral vocal cord paralysis
Absztrakt:
Bevezetés: A kétoldali gégebénulások esetén kialakuló légszomj
azonnali sebészeti beavatkozást tesz szükségessé. A nervus laryngeus recurrens
esetleges regenerációja heteket, hónapokat vehet igénybe, előre nem jósolható
meg. Az elmúlt évtizedekben a tracheotomia helyett számos nyitott és endoszkópos
eljárást fejlesztettek ki a dyspnoe megszüntetésére, ugyanakkor ezek az
eljárások a gégestruktúra reszekciója miatt irreverzibilis károsodást okoztak,
mely a hangképzés minőségét hosszú távon gyengítette.
Célkitűzés: Az endoszkópos arytenoid abdukciós lateropexia
kétoldali gégebénulás esetén elfogadott reverzibilis eljárás, amely a kannaporc
lateralizált helyzetben történő rögzítésével megszünteti a nehézlégzést. Célunk
a hangszalagmozgások regenerációját követően a lateralizáló varrat eltávolítása
után a beteg hangminőségének és légzésfunkciójának meghatározása.
Módszer: Két, endoszkópos arytenoid abdukciós
lateropexiával kezelt, kétoldali gégebénulásban szenvedő betegnél a
hangszalagmozgások részleges, illetve teljes regenerációját követően
eltávolítottuk a lateralizáló varratokat. Objektív és szubjektív módszerekkel
vizsgáltuk a légzésfunkciót és a hangminőséget. Eredmények:
Mindkét esetben megfelelő légzésfunkciós eredmények mellett jó hangminőséget
sikerült elérni. Következtetések: Eredményeink bizonyítják a
minimálisan invazív, endoszkópos arytenoid abdukciós lateropexia nagyfokú
reverzibilitását. A hangszalagmozgások részleges vagy teljes visszatérése esetén
a lateralizáció megszüntethető, ezt követően közel fiziológiás hangképzés

Abstract:
Introduction: Dyspnea caused by bilateral vocal cord paralysis
often requires surgical intervention to prevent acute asphyxiation. The
regeneration of the laryngeal nerves may last weeks or months and it is
difficult to predict the outcome. In the past decades, several open and
endoscopic surgical techniques have been introduced for treatment to avoid
tracheostomy, however, these procedures with resection of the glottis resulted
in irreversible changes in the laryngeal structure, thus the voice quality
decreased over a long-term period. Aim: Endoscopic arytenoid
abduction lateropexy is an accepted reversible, minimally invasive technique
that provides an immediate patent airway by the lateralisation of the arytenoid
cartilage with a suture. The aim of our study was to analyze the phonatory and
respiratory outcomes of this treatment concept. Method: Two
patients suffering from bilateral vocal cord palsy were treated with endoscopic
arytenoid abduction lateropexy. After recovery of the vocal cord movements, the
sutures were removed. Spirometric and phoniatric results of the two patients
were analysed after suture removal. Results: Good spirometric
parameters and normal voice quality were detected in both cases.
Conclusions: These results prove the high reversibility of
the minimally invasive endoscopic arytenoid abduction lateropexy. Lateralization
suture can be removed in the case of vocal cord movement recovery, and phonation
