Geographical patterns of the incidence and mortality of colorectal cancer in mainland Portugal municipalities (2007-2011)

In Portugal, colorectal cancer is one of the most incident cancers; thus, it is crucial to act to fight it. Knowledge of the geographical distribution of the incidence and mortality of colorectal cancer can facilitate the execution of these actions and make them more effective.
Methods: Our paper aims to describe and discuss the geographical patterns of colorectal cancer incidence and mortality in mainland Portugal municipalities (2007–2011). We used the Besag, York and Mollié (BYM) model to compute the relative risk (RR) and posterior probability (PP). We performed a cluster analysis with Global Moran’s Index and Local Moran’s Index (LISA). We ran a geographically weighted regression (GWR) to compare incidence and mortality patterns.
Results: Incidence and mortality have different distributions of RR values. The interval of RR concerning incidence was higher than the interval of RR concerning mortality. PP values reinforce the finding of higher heterogeneity of the incidence of colorectal cancer.
The comparison of the cluster maps for incidence and mortality shows a few municipalities classified with the same cluster type in both maps. Additionally, the GWR results show that the percentage of RR mortality explained by RR incidence differs throughout mainland Portugal.
From the comparison of our results with the prevalence of risk factors (at NUTS II level), the need to be aware of smoking habits, alcohol consumption and the unhealthy diet of the Portuguese population stands out.
Conclusions: There are differences in the geographical distribution of the RR incidence and RR mortality of colorectal cancer in mainland Portugal municipalities. Likewise, it is relevant to highlight the cluster of two municipalities with high RR values concerning colorectal cancer’s incidence and mortality. Future research is necessary to explain the geographical differences in the distribution of colorectal cancer in mainland Portugal municipalities. Based on our findings, it may be interesting to examine the influence of smoking, alcohol consumption, diet and screening on colorectal cancer in greater detail. Mapeamento de índice de vulnerabilidade a ondas de calor
Introdução: Os efeitos diretos da temperatura ambiente na saúde humana são conhecidos e, no caso das temperaturas elevadas, podem ir desde irritações cutâneas moderadas ao denominado golpe de calor. O excesso de temperatura pode igualmente ter efeitos negativos sobre a saúde ao agravar doenças pré-existentes como as doenças cardiovasculares e cerebrovasculares, respiratórias, diabetes entre outras (1, 2).
Em Portugal, à semelhança de outros países, estão descritos efeitos do excesso de calor na saúde, especificamente na mortalidade, em situações de aumento de temperatura súbito e intenso, ou seja durante as chamadas ondas de calor (2, 3, 4, 5, 6, 7).
Objetivo: construir um modelo que permita identificar as áreas e populações mais vulneráveis às ondas de calor em Portugal continental. Para atingir o objetivo proposto utilizaram-se dados do Instituto Nacional de Estatística (INE), da Administração Central do Sistema de Saúde (ACSS) e da Direção Geral do Território (DGT), aos quais são aplicadas métodos estatísticos e de análise espacial.
Resultados: Como resultado obteve-se um indicador que permitiu identificar as áreas mais vulneráveis aos fenómenos de calor extremo.
Conclusão: O padrão de vulnerabilidade ao calor apresenta variações no território de Portugal continental. O seu conhecimento poderá permitir o desenvolvimento de ações destinadas a minorar o impacte do efeito do calor na saúde das populações
Self-reported hearing loss in Portugal: a cross-sectional comparison using evidence from National Health Surveys 2005/2006 and 2014
A incapacidade auditiva atinge grande parte da população com mais de
65 anos, diminuindo a sua qualidade de vida. Por conseguinte, é essencial
caracterizar a sua prevalência de modo a remover barreiras incapacitantes,
e promover a participação dos indivíduos afetados na vida em sociedade.
Utilizando a informação recolhida no âmbito do 5º Inquérito Nacional
de Saúde em 2014, e os dados apurados relativamente ao 4º Inquérito
Nacional de Saúde em 2005/2006, pretendeu-se estimar a prevalência de
incapacidade auditiva na população portuguesa em 2014, analisar a sua
evolução face a 2005/2006, e examinar as respetivas diferenças regionais.
Os resultados do presente estudo revelam que a incapacidade auditiva
afeta uma proporção considerável da população portuguesa, especialmente
nos grupos etários mais elevados. A prevalência de incapacidade auditiva
aumentou de 2005/2006 para 2014, e entre a maioria das regiões, sendo
mais elevada na região Norte. As menores prevalências registaram-se
na R.A. da Madeira, embora esta região, conjuntamente com a R.A. dos
Açores tenha sido a que registou um maior aumento proporcional da prevalência
de incapacidade auditiva.Hearing impairment affects a large part of the population over 65 year old,
diminishing their life quality. Therefore, it is crucial to examine the prevalence
of hearing impairment, in order to remove disabling barriers and promote
social inclusion. By using data gathered for the 5th National Health
Survey in 2014, and data gathered for the 4th National Health Survey in
2005/2006, this study aimed to estimate the prevalence of hearing impairment
in the Portuguese population in 2014, analyse its evolution against
2005/2006, and examine regional differences. Through the two years under
analysis, it can be seen that hearing impairment affects a considerable proportion
of the Portuguese population, especially in higher age groups. The
prevalence of hearing impairment increased from 2005/2006 to 2014, and
between most regions, being higher in the North. The lowest prevalence was
recorded in Madeira, although this region, together with Azores, was the

Spatial epidemiology of cancer : data types, spatial aggregation and geographical patterns at mainland Portugal
A thesis submitted in partial fulfillment of the requirements for the degree of Doctor in Information Management Specialization in Geographical Information SystemsThe importance of research on the spatial epidemiology of cancer has been progressively
recognized and the number of studies in this field has been increasing steadily, having
experienced significant growth over the last decade.
Overall, spatial epidemiology is a comprehensive research area that covers a variety of subjects.
Among those subjects, and particularly in the case of spatial epidemiology of cancer, one
important topic is the level of spatial data aggregation. The analysis of individual data may
present ethical data protection issues, so geographically-aggregated data are often used in
epidemiological research. Conversely, the analysis of aggregated data also raises certain
methodological issues, such as: a) the importance of calculating standardized rates (usually by
sex and age) to allow for the comparison of different geographical units; b) the need to take into
account the fact that the results obtained at a given level of geographical aggregation cannot be
extrapolated as true for other levels of aggregation; and c) the possible existence of the “small
numbers problem” and the relevance of using smoothed rates to try to solve it.
This thesis takes a first look at this subject through a systematic review of the literature on the
spatial epidemiology of cancer. The findings show that the questions which are considered most
frequently in the literature consist of: a) the methodology used; b) the spatial distribution of
cancer and its temporal evolution; and c) risk factors.
The thesis then seeks to address these three aspects. In order to do that, several methodological
issues related to the research of the spatial epidemiology of cancer are evaluated and the
mortality distribution of all types of cancer is analyzed. Given the high rates of incidence and
mortality due to colorectal cancer among the Portuguese population, this cancer type is taken
as a case-study, and the distribution of its incidence and mortality is studied and compared with
the distribution of the prevalence of the main risk factors. With regard to methodology, it is find that data aggregation at the municipality level makes it
possible to obtain detailed information on the geographical distribution of cancer in mainland
Portugal, and simultaneously address the small numbers problem. The use of statistical methods
to calculate smoothed rates based on the spatial vicinity of the units under analysis also
contributes to solving this latter problem. As far as cancer’s spatial distribution patterns are concerned, it is find significant heterogeneity
across the municipalities of mainland Portugal, both when examining overall cancer mortality
and when specifically looking at colorectal cancer incidence and mortality.
Regarding the mortality distribution of all cancer types, it is find that the highest mortality rates
tend to cluster around Oporto (on the north coast), Lisbon (on the west coast) and in the
municipalities of southeast mainland Portugal.
With respect to colorectal cancer, it are detected different patterns of incidence and mortality.
The distribution of colorectal cancer incidence is more heterogeneous than that of colorectal
cancer mortality. In addition, both colorectal cancer incidence and mortality exhibit especially
high values for men in two municipalities in the center of mainland Portugal, which seems to call
for a more detailed analysis of this area.
In terms of risk factors, the following main risks, which have been identified as having a possible
association with colorectal cancer, were considered: tobacco smoking; alcohol intake; and
dietary behavior. Screening, as a secondary prevention measure aimed at the early detection of
cancer and mortality reduction, was also considered in the analysis. However, data on these risk
factors are representative only at much more aggregated geographic units, such as NUTS II.
Therefore, a detailed analysis of association could not be performed. Finally, the natural
background radiation in mainland Portugal was also analyzed, computing a mean by
municipalities, and the conclusion is that its association with colorectal cancer is generally weak.
This work points to a variety of interesting topics to study in future research, such as: a) to apply
the same methodologies to other cancer types; b) to study the geographical association of risk
factors, like socio-demographic and environmental risk factors, with cancer incidence and
mortality; c) to test other spatial methods in the research of spatial epidemiology of cancer in
Portugal, like, for an instance, kriging or multilevel models; and d) to establish suitable
methodologies to study spatial epidemiology of cancer in the Portuguese islands.A importância da pesquisa sobre a epidemiologia espacial do cancro tem sido progressivamente
reconhecida e o número de estudos nesse campo tem aumentado constantemente, tendo
registado um crescimento significativo na última década.
Em geral, a epidemiologia espacial é uma área de pesquisa abrangente que inclui uma variedade
de assuntos. Entre esses assuntos, e particularmente no caso da epidemiologia espacial do
câncer, um tópico importante é o nível de agregação dos dados espaciais. A análise de dados
individuais pode apresentar problemas éticos de proteção de dados, pelo que é frequente
utilizar dados agregados geograficamente em pesquisas epidemiológicas. Por outro lado, a
análise de dados agregados também levanta certas questões metodológicas, tais como: a) a
importância de calcular taxas padronizadas (geralmente por sexo e idade) para permitir a
comparação de diferentes unidades geográficas; b) a necessidade de levar em conta o fato de
que os resultados obtidos em um dado nível de agregação geográfica não podem ser
extrapolados como verdadeiros para outros níveis de agregação; e c) a possível existência do
“problema dos pequenos números” e a relevância do uso de taxas suavizadas para tentar
resolvê-lo. Esta tese faz uma primeira abordagem ao tema através de uma revisão sistemática da literatura
sobre a epidemiologia espacial do cancro. Os resultados mostram que as questões consideradas
mais frequentemente na literatura estão relacionadas com: a) a metodologia utilizada; b) a
distribuição espacial do cancro e a sua evolução temporal; e c) os fatores de risco.
A tese procura então abordar esses três aspetos. Para isso, são avaliadas várias questões
metodológicas relacionadas com a investigação em epidemiologia espacial do cancro e é
analisada a distribuição da mortalidade por todos os tipos de cancro. considerando as elevadas
taxas de incidência e mortalidade do cancro colorrectal na população portuguesa, este tipo de
cancro é utilizado como caso de estudo, e a distribuição da sua incidência e mortalidade é
estudada e comparada com a distribuição da prevalência dos principais fatores de risco.
Em termos metodológicos, verifica-se que a agregação de dados ao nível do concelho permite
obter informação detalhada sobre a distribuição geográfica do cancro em Portugal continental,
e simultaneamente abordar o problema dos pequenos números. A utilização de métodos estatísticos para calcular taxas suavizadas com base na vizinhança espacial das unidades
geográficas analisadas também contribui para a resolução deste último problema.
No que diz respeito aos padrões de distribuição espacial do cancro, verifica-se uma
heterogeneidade significativa entre os concelhos de Portugal continental, tanto na análise da
mortalidade por todos os tipos de cancro como quando se analisa especificamente a incidência
e mortalidade do cancro colorretal. No que diz respeito à distribuição da mortalidade de todos os tipos de cancro, verifica-se que as
taxas de mortalidade mais elevadas tendem a agrupar-se em torno do Porto (na costa norte),
Lisboa (na costa oeste) e nos concelhos do sudeste do continente.
Em relação ao cancro colorretal, são detetados diferentes padrões de incidência e mortalidade.
A distribuição da incidência de cancro colorretal é mais heterogênea que a mortalidade por por
este tipo de cancro. Além disso, tanto a incidência como a mortalidade por cancro colorrectal
registam valores particularmente elevados para os homens em dois concelhos do centro de
Portugal continental, o que parece exigir uma análise mais detalhada dessa área.
Em termos de fatores de risco, foram considerados os seguintes factores, que foram
identificados como tendo possível associação com o câncer colorretal: tabagismo; ingestão de
álcool; e comportamento alimentar. O rastreio, como medida secundária de prevenção que visa
a deteção precoce do cancro e a redução da mortalidade, também foi considerado na análise.
No entanto, os dados relativos a estes fatores de risco são representativos apenas em unidades
geográficas muito agregadas, como as NUTS II. Assim, não foi possível realizar uma análise
detalhada desta associação. Para além disto, analisou-se a radiação natural em Portugal
Continental, calculando uma média por concelho, e concluiu-se que a sua associação com o
cancro colorretal é geralmente fraca. Este trabalho aponta para uma variedade de tópicos interessantes para estudar em pesquisas
futuras, tais como: a) aplicar as mesmas metodologias a outros tipos de cancro; b) estudar a
associação geográfica de fatores de risco, tais como fatores sócio demográficos e ambientais,
com a incidência e a mortalidade por cancro; c) testar outros métodos espaciais na análise da
epidemiologia espacial do cancro em Portugal, como, por exemplo, modelos de krigagem ou
multiníveis; e d) estabelecer metodologias adequadas para o estudo da epidemiologia espacial
do cancro nas ilhas portuguesas

Em Casa Observamos Saúde (ECOS)’ by telephone and web: results from the 2016 first wave implementation
Em 2016 o inquérito ECOS foi implementado, através de entrevista telefónica
assistida por computador, à semelhança de vagas anteriores, e
ainda por via eletrónica. O questionário foi respondido por um elemento
com 18 ou mais anos residente na unidade de alojamento, que prestou
informação sobre a sua saúde e dos restantes elementos do agregado
(por proxy). No total obtiveram-se 803 entrevistas concluídas e uma taxa
de participação global de 79,9%. Considerando os contactos realizados
via telefónica, a taxa de participação foi de 77,4% e de 26,5% por via
web. Em ambas as vias, a maioria dos respondentes era do sexo feminino,
encontrando-se a maior frequência de respondentes por telefone no
grupo etário 60-69 e via web no grupo 40-49. As taxas de resposta obtidas
em 2016 foram semelhantes às obtidas em outras vagas do ECOS
por via telefónica (aproximadamente 80% vs 79 a 86%), e na via web foi
superior às obtidas em estudos similares. O perfil dos respondentes em
cada uma das vias foi consistente com o observado em estudos que
utilizaram estas metodologias, nomeadamente, uma maior frequência
de respondentes do sexo feminino e uma maior adesão à via web dos
grupos etários mais novos.In 2016 the ECOS panel was implemented through both telephone
and electronic contact. The information was obtained using computer
assisted telephone and web. The questionnaire was answered by a
household aged 18 or more years living in the house unit, giving information
on himself and proxy to other household cohabitants. A total
of 803 completed inter views were obtained which corresponds to a
total participation rate of 79.9%. Considering the phone contacts the
participation rate by contact type was 77.4% by phone and 26.5% by
web. For both methods the highest percentages of respondents were
female; the highest frequency of telephone respondents was obtained
in the 60-69 age groups and by web in the 40-49 years individuals.
Response rates obtained by telephone in 2016 are comparable to
those obtained in other ECOS waves (80% vs 79-86%) and the rates
via web were higher than those obtained in similar studies. The profile
of respondents was consistent with that obser ved in studies using
these methods, including increased frequency of female respondents
and greater adherence to the web via the younger age groups

The 2013 June and July heat wave: a mortality analysis by district of mainland Portugal
[PT] Em 2013, entre o final de junho e início de julho, Portugal foi afetado
por um período de calor extremo detetado pelo sistema de vigilância
ÍCARO (Importância do CAlor: Repercussão sobre os Óbitos), em
funcionamento no Instituto Nacional de Saúde Doutor Ricardo Jorge
(INSA). O presente trabalho teve como objetivo apresentar uma análise
dos excessos de mortalidade, associados às temperaturas extremas
observadas nesse período, por distritos de Portugal Continental. O
período de calor extremo com possível impacte na saúde humana foi
definido pela análise conjunta do Índice-Alerta-ÍCARO, e das temperaturas
máximas registadas em cada capital de distrito (facultadas pelo
Instituto Português do Mar e da Atmosfera, IPMA). Os dados da mortalidade
foram obtidos pelo Sistema de Vigilância Diária da Mortalidade
(VDM), gerido pelo Departamento de Epidemiologia do INSA. Ao nível
distrital os excessos de óbitos variaram entre os 11 e os 40 óbitos por
100.000 habitantes no período identificado. Beja e Faro não apresentaram
excessos significativos. Em conclusão, verificou-se que durante
o período em análise, observaram-se excessos de mortalidade na
maioria dos distritos de Portugal Continental.[EN] In 2013, between late June and early July, Portugal was affected by an
extreme heat period detected by ICARO surveillance system (Importância
do CAlor: Repercussão sobre os Óbitos), in operation at National Health
Institute Doutor Ricardo Jorge (INSA). This study aimed to present
an analysis of excess mortality associated with extreme temperatures observed
in this period by districts of mainland Portugal, The extreme heat
period with possible impacts on human health has been defined by the
joint analysis of the Index-Alert-ICARO, and the maximum temperatures
recorded in each district capital (provided by the Portuguese Sea and
Atmosphere Institute, IPMA). The mortality data was obtained by the Daily
Mortality Surveillance System (VDM), managed by the Department of Epidemiology
at INSA. At district level the excess deaths, ranged from 11 to
40 deaths per 100.000 inhabitants in the identified period. No significant
excess mortality was observed in Beja and Faro. In conclusion, it was
found that during the period under analysis, it was observed an excess
mortality in most districts of mainland Portugal
A geographically weighted regression approach to investigate air pollution effect on lung cancer: A case study in Portugal
The risk of developing lung cancer might to a certain extent be attributed to tobacco. Nevertheless, the role of air pollution, both form urban and industrial sources, needs to be addressed. Numerous studies have concluded that long-term exposure to air pollution is an important environmental risk factor for lung cancer mortality. Still, there are only a few studies on air pollution and lung cancer in Portugal and none addressing its spatial dimension. The goal was to determine the influence of air pollution and urbanization rate on lung cancer mortality. A geographically weighted regression (GWR) model was performed to evaluate the relation between particle matter10 (PM10) emissions and lung cancer mortality relative risk (RR) for males and females in Portugal between 2007 and 2011. RR was computed with the BYM model. For a more in-depth analysis, the urbanization rate and the percentage of industrial area in each municipality were added. GWR efforts led to identifying three variables that were statistically significant in explaining lung cancer relative risk mortality, PM10 emissions, urbanization rate and the percentage of industrial area with an adjusted R2 of 0,63 for men and 0,59 for women. A small set of 8 municipalities with high correlation values was also identified (local R2 above 0,70). Stronger relationships were found in the north-western part of mainland Portugal. The local R2 tends to be higher when the emissions of PM10 are joined by urbanization and industrial areas. However, when assessing the industrial areas alone, it was noted that its impact was lower overall. As one of the first communications on this subject in Portugal, we have identified municipalities where possible impacts of air pollution on lung cancer mortality RR are higher thereby highlighting the role of geography and spatial analysis in explaining the associations between a disease and its determinants
