    The amount of keratinized mucosa may not influence peri-implant health in compliant patients: A retrospective 5-year analysis

    AIM (a) To investigate the influence of the keratinized mucosa (KM) on peri-implant health or disease and (b) to identify a threshold value for the width of KM for peri-implant health. MATERIALS AND METHODS The total dataset was subsampled, that is one implant was randomly chosen per patient. In 87 patients, data were extracted at baseline (prosthesis insertion) and 5 years including the width of mid-buccal KM, bleeding on probing, probing depth, plaque index and marginal bone level (MB). Spearman correlations with Holm adjustment for multiple testing were used for potential associations. RESULTS Depending on the definition of peri-implant diseases, the prevalence of peri-implantitis ranged from 9.2% (bleeding on probing threshold: <50% or ≥50%) to 24.1% (threshold: absence or the presence). The prevalence of peri-implant mucositis was similar, irrespective of the definition (54%-55.2%). The width of KM and parameters for peri-implant diseases demonstrated negligible (Spearman correlation coefficients: -0.2 < ρ < 0.2). No threshold value was detected for the width of mid-buccal KM in relation to peri-implant health. CONCLUSION The width of KM around dental implants correlated to a negligible extent with parameters for peri-implant diseases. No threshold value for the width of KM to maintain peri-implant health could be identified

    Histologic and histomorphometric evaluation of peri-implant bone subjected to immediate loading: An experimental study with Macaca fascicularis

    Purpose: Immediately loaded splinted implants can become osseointegrated when they are placed in the anterior part of the mandible. The concept of immediate loading has not been well examined in the posterior mandible. The aim of this study was to evaluate the hard tissue reactions around immediately loaded implants placed in the posterior mandible in the monkey model. Materials and Methods: Six adult M. fascicularis monkeys were used in this study. Thirty-six Ankylos implants (Degussa Dental, Hanau-Wolfgang, Germany) were placed after extraction of the second premolar, first, and second molar teeth and complete healing of the sockets. Control (C) group implants were placed and, after osseointegration, were loaded for 1 month using temporary acrylic resin prostheses and later for 2 months using splinted metal crowns. In the contralateral region of the mandible, test (T) group implants were placed and loaded immediately with the same sequence as carried out for the C implants. After sacrifice of the animals, specimens were examined histologically and evaluated histomorphometrically. Results: All implants were osseointegrated. Compact, cortical bone In contact with the implant surface without any gaps or connective tissue formation was demonstrated. Discussion: Histomorphometric findings of the bone-implant-contacts showed no significant differences between the T and C group implants. Peri-implant mineralized bone areas presented statistically significant differences and showed a higher density of bone between the threads of immediately loaded implants (P < .05). Conclusions: Immediately loaded splinted implants in the posterior mandible can become osseointegrated with a hard tissue peri-implant response similar to that of delayed loaded implants. Moreover, immediate loading seems to increase the ossification of the alveolar bone around endosseous implants.     Preparation of fluorine-doped TiO2 photocatalysts with controlled crystalline structure

    Recommended by M. Sabry Abdel-Mottaleb Nanocrystalline F-doped TiO 2 powders were prepared by sol-gel route. The thermal behavior of the powders was recorded by DTA/TG technique. The crystalline phase of the fluorinated TiO 2 powders was determined by X-ray diffraction technique. It was demonstrated that F-doping using CF 3 COOH favors the formation of rutile along with anatase phase even at low temperature. Moreover, the rutile&apos;s phase content increases with the increase of the quantity of the fluorine precursor in the starting solution. The surface area of the powders and the pore size distribution were studied by N 2 adsorption-desorption using BET and BJH methods. X-ray photoelectron spectroscopy (XPS) revealed that the fluorine is presented in the TiO 2 powders mainly as metal fluoride in quantities ∼16 at %. The F-doped TiO 2 showed a red-shift absorption in UV-vis region which was attributed to the increased content of rutile phase in the powders.     In the race for viable solutions that could slow down carbon emissions and help in meeting the climate change targets a lot of effort is being made towards the development of suitable CO2 adsorbents with high surface area, tunable pore size and surface functionalities that could enhance selective adsorption. Here, we explored the use of silsesquioxane pillared graphene oxide for CO2 capture; we modified silsesquioxane loading and processing parameters in order to obtain pillared structures with nanopores of the tailored size and surface properties to maximize the CO2 sorption capacity. Powder X-ray diffraction, XPS and FTIR spectroscopies, thermal analysis (DTA/TGA), surface area measurements and CO2 adsorption measurements were employed to characterize the materials and evaluate their performance.     Peri-implant soft tissue integration of immediately loaded implants in posterior macaque mandible: a histomorphometric study

    Background: Today, one critical goal in implant placement is the achievement of optimal soft tissue integration. Reports thus far have demonstrated successful soft tissue preservation in delayed loaded implants placed in anterior jaws. The aim of this study was to histomorphometrically examine the soft tissues around immediately loaded implants placed in the macaque posterior mandible. Methods: Splinted crowns on screw-shaped titanium implants (8 mm length, 3.5 mm diameter) were utilized. Three implants each were placed in the premolar-molar edientulous mandibular segments of 6 adult monkeys (Macaca fascicularis); one side served as the control (delayed loading) and the other as the test sites (immediate loading). The animals were sacrificed after 3 months of loading. Histomorphometry of 6 soft tissue indices including the sulcus depth (SD), junctional epithelium (JE), connective tissue contact (CTC), biologic width (BW = SD + JE + CTC), DIM (distance between the implant top and coronal gingiva), and DIB (distance between the implant top and first implant-to-bone contact) was performed on non-decalcified sections. Results: No significant differences in the mean soft tissue scores (mm) between the test (SD = 0.68 +/- 0.63; JE = 1.71 +/- 1.04; CTC = 1.51 +/- 1.14; DIM = 2.27 +/- 1.18; DIB = 1.32 +/- 1.21; BW 3.9) and control (SD = 0.88 + 0.57; JE = 1.66 + 0.77; CTC 1.24 +/- 0.92; DIM = 2.38 +/- 0.81; DIB = 1.19 +/- 0.91; BW 3.78) groups were observed (P>0.01). Conclusion: These findings suggest that the dimensions of the peri-implant soft tissues were within the biologic range and were not influenced by immediate functional loading or posterior location of the implants in the macaque mandible

The local delivery of antimicrobial agents has demonstrated benefit in mild to moderate cases of periimplantitis. This study compared the safety and efficacy of Chlorhexidine gluconate 2.5 mg chip (CHX chips) as an adjunctive treatment to sub-gingival debridement in patients afflicted with periimplantitis. METHODS: A multi-center, randomized, single-blind, two-arm, parallel Phase-3 study was conducted. Periimplantitis patients with implant pocket depths (IPD) of 5-8 mm underwent sub-gingival implant surface debridement followed by repeated bi-weekly supra-gingival plaque removal and Chlorhexidine chips application (ChxC group) for 12 weeks, or similar therapy but without application of ChxC (control group). All patients were followed for 24 weeks. Plaque and gingival indices were measured at every visit while IPD, recession and bleeding on probing were assessed at 8,12,16,24 week. RESULTS: 290 patients were included: 146 in the ChxC group and 144 in the control. At 24 weeks, a significant reduction in IPD (p = 0.01) was measured in the ChxC group (1.76 ± 1.13 mm) compared to the control group (1.54 ± 1.13 mm). IPD reduction of ≥2 mm was found in 59% and 47.2% of the implants in the ChxC and control groups, respectively (p = 0.03). Changes in gingival recession (0.29 ± 0.68 mm vs. 0.15 ± 0.55 mm, p = 0.015) and relative attachment gain (1.47 ± 1.32 mm and 1.39 ± 1.27 mm, p = 0.0017) were significantly larger in the ChxC group. Patients in the ChxC group that were <65 years exhibited significantly better responses (p<0.02); likewise, non-smokers had similarly better response (p <0.02). Both protocols were well tolerated, and no severe treatment-related adverse events were recorded throughout the study. CONCLUSIONS: Patients with periimplantitis that were treated with an intensive treatment protocol of bi-weekly supra-gingival plaque removal and local application of Chlorhexidine chips had greater mean IPD reduction and greater percentile of sites with IPD reduction of ≥2 mm. as compared to bi-weekly supra-gingival plaque removal.
