Health co-benefits of climate change action in Italy
The climate breakdown is increasingly affecting the health of people around the world. As weather extremes intensify, the global health burden of unhealthy diets, sedentary lifestyles, and air pollution increases too. Against this backdrop, overstretched health systems can take stock of the co-benefits that a low carbon transition could deliver if health is prioritised in climate policies.1 Yet, as pointed out in a recent commentary,2 the UN Climate Change Conference (COP26) in Glasgow, UK, put little focus on health-related goals. Similarly, health is often disregarded in countries’ nationally determined contributions and in national climate policies. The dissociation between public health policy and climate action translates into millions of avoidable adverse health outcomes and deaths each year. For example, reducing greenhouse gas emissions also reduces the effect of air pollution, which is the largest environmental cause of disease and mortality in Europe in terms of measurable effect
Modello di tenuta della flangia bullonata, senza guarnizione, mediante l'analogia della meccanica della frattura di una fessura parzialmente aperta
I compressori centrifughi di elevate dimensioni non permettono l'utilizzo di guarnizioni deformabili, per cui le due meta della flangia di connessione sono forzate mediante bullonatura e la tenuta e affidata al contatto completo delle due superfici. La previsione della pressione di perdita e un aspetto di progetto di notevole interesse per questa tecnologia. L'azione della pressione interna sollecita la separazione delle superfici della flangia, che invece e contrastata dall'azione di serraggio dei bulloni. Il presente lavoro propone un modello per prevedere la condizione di perdita, basato sulla meccanica della frattura. Dato che le due superfici della flangia sono semplicemente a contatto, esse costituiscono una vera e propria fessura parzialmente aperta. Come ben noto il fattore di intensificazione di una fessura parzialmente aperta e nullo. Imponendo che le due superfici siano parzialmente separate ad una distanza fino al bordo del foro del bullone (che offre un canale di fuoriuscita per il fluido in pressione), e imponendo la condizione di fattore di intensificazione nullo, e possibile determinare la pressione di perdita, analiticamente, mediante la tecnica delle "weight functions" (o "funzioni peso"). Il presente lavoro riporta una positiva validazione del modello proposto mediante sia simulazione numerica sia risultati sperimentali in piena scala e in scala ridotta. Il modello analitico proposto offre uno strumento di progetto di immediata implementazione per comparare diverse geometrie di flangia bullonata
Collaborative Care Best Practices Model (CCBPM) Development for Nursing and Occupational Therapy Assistant Interprofessional Simulation
The use of simulation as a teaching and learning stratagem is well supported in the professional literature. The development of a collaborative care best practices model (CCBPM) was undertaken to help the creation and implementation of an interprofessional education (IPE) learning strategy between occupational therapy assistant (OTA) and pre-licensure nursing students at a private liberal arts college. The project goals and outcomes involved faculty development, CCBPM development, and OTA and nursing novice student competency acquisition in their respective professions. Twenty-three students (10 OTA and 13 nursing) completed the IPE learning simulation exercise. A mixed-method approach to data analysis was used to evaluate aggregate data for simulation fidelity, student feelings of support, and identification of simulation experience themes. For both scenario and overall fidelity, the aggregate rating of strongly agree and agree equaled 90.9%. The aggregate rating of strongly agree and agree equaled 95.45% for equipment fidelity. The number of participants who rated their feeling of being supported by selecting strongly agree or agree was 72.72%. The participants worked in teams to complete the simulation and the debriefing process. Using grounded theory, safety, communication, and teamwork were the three primary themes identified from the transcribed team debriefing worksheets
Bidirectional Communication System on Power Line Integrated on Electronic Board for Driving of LED and HID Lamps
We present the bidirectional power line communication system developed in parallel to an electronic board for driving and control of HID (high-intensity discharge) and LED (light-emitting diode) lamps. The communication system, developed to be applied in the sector of public illumination, is been designed to combine high efficiency and reliability with low production costs; it consists indeed of discrete cheap components. The communication system described in this paper implements the technique of transporting digital information over existing power lines, avoiding the issue of installing new cables. Digitized signals can use power line cables through the amplitude voltage and current modulation. The solution proposed is more advantageous compared to communication techniques currently on the market which are essentially two types, power line carrier (modem for high-voltage lines) or radio (zig-Bee transceiver)
Neuropsychological and neurophysiological correlates of fatigue in post-acute patients with neurological manifestations of COVID-19: Insights into a challenging symptom
More than half of patients who recover from COVID-19 experience fatigue. We studied fatigue using neuropsychological and neurophysiological investigations in post-COVID-19 patients and healthy subjects. Neuropsychological assessment included: Fatigue Severity Scale (FSS), Fatigue Rating Scale, Beck Depression Inventory, Apathy Evaluation Scale, cognitive tests, and computerized tasks. Neurophysiological examination was assessed before (PRE) and 2 min after (POST) a 1-min fatiguing isometric pinching task and included: maximum compound muscle action potential (CMAP) amplitude in first dorsal interosseous muscle (FDI) following ulnar nerve stimulation, resting motor threshold, motor evoked potential (MEP) amplitude and silent period (SP) duration in right FDI following transcranial magnetic stimulation of the left motor cortex. Maximum pinch strength was measured. Perceived exertion was assessed with the Borg-Category-Ratio scale. Patients manifested fatigue, apathy, executive deficits, impaired cognitive control, and reduction in global cognition. Perceived exertion was higher in patients. CMAP and MEP were smaller in patients both PRE and POST. CMAP did not change in either group from PRE to POST, while MEP amplitudes declined in controls POST. SP duration did not differ between groups PRE, increased in controls but decreased in patients POST. Patients' change of SP duration from PRE to POST was negatively correlated to FSS. Abnormal SP shortening and lack of MEP depression concur with a reduction in post-exhaustion corticomotor inhibition, suggesting a possible GABAB-ergic dysfunction. This impairment might be related to the neuropsychological alterations. COVID-19-associated inflammation might lead to GABAergic impairment, possibly representing the basis of fatigue and explaining apathy and executive deficits
The Axial Organ and the Pharynx Are Sites of Hematopoiesis in the Sea Urchin
Background: The location of coelomocyte proliferation in adult sea urchins is unknown and speculations since the early 1800s have been based on microanatomy and tracer uptake studies. In adult sea urchins (Strongylocentrotus purpuratus) with down-regulated immune systems, coelomocyte numbers increase in response to immune challenge, and whether some or all of these cells are newly proliferated is not known. The gene regulatory network that encodes transcription factors that control hematopoiesis in embryonic and larval sea urchins has not been investigated in adults. Hence, to identify the hematopoietic tissue in adult sea urchins, cell proliferation, expression of phagocyte specific genes, and expression of genes encoding transcription factors that function in the conserved regulatory network that controls hematopoiesis in embryonic and larval sea urchins were investigated for several tissues.Results: Cell proliferation was induced in adult sea urchins either by immune challenge through injection of heat-killed Vibrio diazotrophicus or by cell depletion through aspiration of coelomic fluid. In response to either of these stimuli, newly proliferated coelomocytes constitute only about 10% of the cells in the coelomic fluid. In tissues, newly proliferated cells and cells that express SpTransformer proteins (formerly Sp185/333) that are markers for phagocytes are present in the axial organ, gonad, pharynx, esophagus, and gut with no differences among tissues. The expression level of genes encoding transcription factors that regulate hematopoiesis show that both the axial organ and the pharynx have elevated expression compared to coelomocytes, esophagus, gut, and gonad. Similarly, an RNAseq dataset shows similar results for the axial organ and pharynx, but also suggests that the axial organ may be a site for removal and recycling of cells in the coelomic cavity.Conclusions: Results presented here are consistent with previous speculations that the axial organ may be a site of coelomocyte proliferation and that it may also be a center for cellular removal and recycling. A second site, the pharynx, may also have hematopoietic activity, a tissue that has been assumed to function only as part of the intestinal tract
Health care's response to climate change: a carbon footprint assessment of the NHS in England
Background: Climate change threatens to undermine the past 50 years of gains in public health. In response, the National Health Service (NHS) in England has been working since 2008 to quantify and reduce its carbon footprint. This Article presents the latest update to its greenhouse gas accounting, identifying interventions for mitigation efforts and describing an approach applicable to other health systems across the world. Methods: A hybrid model was used to quantify emissions within Scopes 1, 2, and 3 of the Greenhouse Gas Protocol, as well as patient and visitor travel emissions, from 1990 to 2019. This approach complements the broad coverage of top-down economic modelling with the high accuracy of bottom-up data wherever available. Available data were backcasted or forecasted to cover all years. To enable the identification of measures to reduce carbon emissions, results were disaggregated by organisation type. Findings: In 2019, the health service's emissions totalled 25 megatonnes of carbon dioxide equivalent, a reduction of 26% since 1990, and a decrease of 64% in the emissions per inpatient finished admission episode. Of the 2019 footprint, 62% came from the supply chain, 24% from the direct delivery of care, 10% from staff commute and patient and visitor travel, and 4% from private health and care services commissioned by the NHS. Interpretation: This work represents the longest and most comprehensive accounting of national health-care emissions globally, and underscores the importance of incorporating bottom-up data to improve the accuracy of top-down modelling and enabling detailed monitoring of progress as health systems act to reduce emissions.
