    Quality indicators for appropriate antibiotic prescribing in urinary tract infections in children

    BACKGROUND: The aim of this study was to define a set of urinary tract infections (UTIs)-specific quality indicators for appropriate prescribing in children and evaluate clinical practices in a district general hospital in Greece. METHODS: The aim of this study was to define a set of urinary tract infections (UTIs)-specific quality indicators for appropriate prescribing in children and evaluate clinical practices in a district general hospital in Greece. RESULTS: Twelve quality indicators were adapted or developed for prescribing in childhood UTIs. A broad variety of antibiotics were prescribed for UTIs, with a drug utilization (DU) 90% rate of 6 and 9 different antibiotics for febrile and afebrile UTIs, respectively. Despite the low incidence of multi-drug resistant UTIs in the study period (9/261, 3.4%), broad-spectrum antibiotics were prescribed in 33.5% (164/490) of prescriptions. A total of 62.8% (164/261) of patients were started on empiric combined therapies, while opportunities to de-escalate were missed in 37.8% (62/164) of them. One quarter (67/261, 25.7%) of patients did not fulfil the criteria for receiving treatment, while nearly half of those prescribed prophylaxis (82/175, 46.9%) could have avoided having a prophylaxis prescription. CONCLUSIONS: Our study identified substantial gaps for improvement in antimicrobial prescribing for UTIs in children. The application of the proposed quality indicators could help to limit unnecessary antibiotics use in children with UTI

    International audienceCompetitive sports coverage increasingly includes information on athlete or team statistics and records. Sports video coverage has traditionally embedded representations of this data in fixed locations on the screen, but more recently also attached representations to athletes or other targets in motion. These publicly used representations so far have been rather simple and systematic investigations of the research space of embedded visualizations in motion are still missing. Here we report on our preliminary research in the domain of professional and amateur swimming. We analyzed how visualizations are currently added to the coverage of Olympics swimming competitions and then plan to derive a design space for embedded data representations for swimming competitions. We are currently conducting a crowdsourced survey to explore which kind of swimming-related data general audiences are interested in, in order to identify opportunities for additional visualizations to be added to swimming competition coverage

    At a Crossroads: MidTown, Inc. 2005-2015

    Established in 2005, MidTown, Inc., a self-identified community development organization, celebrated its 10th anniversary in 2016. After a decade’s work, the organization is presently reviewing its activities as it takes a moment to consider its future trajectory. To facilitate this process MidTown, Inc. partnered with Columbus State University’s Columbus Community Geography Center to support MidTown, Inc.’s efforts. In fall 2015, nine students in CSU’s urban geography course, taught by Dr. Amanda Rees, researched and completed this report including: Celeb Ashburn, Cheltzie Brown, Eric Derhammer, Jessica Dixon, Kelsie Hughes, Rachel Knapp, Morgan Robinson, Anastasia Romain, and C.E. Sturgeon. An online report summary is also published at CSU’s Archives In approaching this project, students developed two questions: What is Midtown (the region), and what MidTown, Inc.? To address those questions students prepared a short overview of Midtown’s expansion between the 1830s and 1970s and developed demographic date exploring age, racial and ethnic mix, and levels of education in each of Midtown’s block groups to better understand the population within this 6-square mile district. Students created a short history of community development in the U.S. to place MidTown, Inc. within that broader context. Having conducted and then transcribed nine oral histories with people involved in the creation and evolution of MidTown, Inc., students drew upon those documents to develop a timeline of MidTown, Inc. along with a narrative history of the organization’s evolution

    “Art is the struggle to be, in a particular sort of way, virtuous.” —Iris Murdoch Indeed, when we embark on any creative adventure it is with the purpose of conveying a certain truth; an emotion, an idea that brings us together and reminds us of the innate consciousness that dwells within. Understory is emblematic of this; where individuals intent on honing their craft can join others to become a part of something greater, their work immortalized to become a time capsule. Decades from now a volume of Understory may be discovered on a dusty bookshelf in some forgotten corner and the lucky explorer who finds it shall be enlightened by pages of gold. We, as editors of Understory 2014, have worked very hard this year to put together a journal that showcases the very best of the University of Alaska Anchorage undergraduate student work. We hope you enjoy your journey through this issue as much as we have enjoyed ours.     Reflections on Visualization in Motion for Fitness Trackers

    International audienceIn this paper, we reflect on our past work towards understanding how to design visualizations for fitness trackers that are used in motion. We have coined the term "visualization in motion" for visualizations that are used in the presence of relative motion between a viewer and the visualization. Here, we describe how visualization in motion is relevant to sports scenarios. We also provide new data on current smartwatch visualizations for sports and discuss future challenges for visualizations in motion for fitness trackers

Frontiers in Marine Science, 6, (2019):257, doi:10.3389/fmars.2019.00257.The Indonesian seas play a fundamental role in the coupled ocean and climate system with the Indonesian Throughflow (ITF) providing the only tropical pathway connecting the global oceans. Pacific warm pool waters passing through the Indonesian seas are cooled and freshened by strong air-sea fluxes and mixing from internal tides to form a unique water mass that can be tracked across the Indian Ocean basin and beyond. The Indonesian seas lie at the climatological center of the atmospheric deep convection associated with the ascending branch of the Walker Circulation. Regional SST variations cause changes in the surface winds that can shift the center of atmospheric deep convection, subsequently altering the precipitation and ocean circulation patterns within the entire Indo-Pacific region. Recent multi-decadal changes in the wind and buoyancy forcing over the tropical Indo-Pacific have directly affected the vertical profile, strength, and the heat and freshwater transports of the ITF. These changes influence the large-scale sea level, SST, precipitation and wind patterns. Observing long-term changes in mass, heat and freshwater within the Indonesian seas is central to understanding the variability and predictability of the global coupled climate system. Although substantial progress has been made over the past decade in measuring and modeling the physical and biogeochemical variability within the Indonesian seas, large uncertainties remain. A comprehensive strategy is needed for measuring the temporal and spatial scales of variability that govern the various water mass transport streams of the ITF, its connection with the circulation and heat and freshwater inventories and associated air-sea fluxes of the regional and global oceans. This white paper puts forward the design of an observational array using multi-platforms combined with high-resolution models aimed at increasing our quantitative understanding of water mass transformation rates and advection within the Indonesian seas and their impacts on the air-sea climate system.     Vaccine breakthrough hypoxemic COVID-19 pneumonia in patients with auto-Abs neutralizing type I IFNs

    Life-threatening `breakthrough' cases of critical COVID-19 are attributed to poor or waning antibody response to the SARS- CoV-2 vaccine in individuals already at risk. Pre-existing autoantibodies (auto-Abs) neutralizing type I IFNs underlie at least 15% of critical COVID-19 pneumonia cases in unvaccinated individuals; however, their contribution to hypoxemic breakthrough cases in vaccinated people remains unknown. Here, we studied a cohort of 48 individuals ( age 20-86 years) who received 2 doses of an mRNA vaccine and developed a breakthrough infection with hypoxemic COVID-19 pneumonia 2 weeks to 4 months later. Antibody levels to the vaccine, neutralization of the virus, and auto- Abs to type I IFNs were measured in the plasma. Forty-two individuals had no known deficiency of B cell immunity and a normal antibody response to the vaccine. Among them, ten (24%) had auto-Abs neutralizing type I IFNs (aged 43-86 years). Eight of these ten patients had auto-Abs neutralizing both IFN-a2 and IFN-., while two neutralized IFN-omega only. No patient neutralized IFN-ss. Seven neutralized 10 ng/mL of type I IFNs, and three 100 pg/mL only. Seven patients neutralized SARS-CoV-2 D614G and the Delta variant (B.1.617.2) efficiently, while one patient neutralized Delta slightly less efficiently. Two of the three patients neutralizing only 100 pg/mL of type I IFNs neutralized both D61G and Delta less efficiently. Despite two mRNA vaccine inoculations and the presence of circulating antibodies capable of neutralizing SARS-CoV-2, auto-Abs neutralizing type I IFNs may underlie a significant proportion of hypoxemic COVID-19 pneumonia cases, highlighting the importance of this particularly vulnerable population

    DMTs and Covid-19 severity in MS: a pooled analysis from Italy and France

    We evaluated the effect of DMTs on Covid-19 severity in patients with MS, with a pooled-analysis of two large cohorts from Italy and France. The association of baseline characteristics and DMTs with Covid-19 severity was assessed by multivariate ordinal-logistic models and pooled by a fixed-effect meta-analysis. 1066 patients with MS from Italy and 721 from France were included. In the multivariate model, anti-CD20 therapies were significantly associated (OR = 2.05, 95%CI = 1.39–3.02, p < 0.001) with Covid-19 severity, whereas interferon indicated a decreased risk (OR = 0.42, 95%CI = 0.18–0.99, p = 0.047). This pooled-analysis confirms an increased risk of severe Covid-19 in patients on anti-CD20 therapies and supports the protective role of interferon
