A Program for Parents in Children’s Literature and Prereading Activities
This project developed an educational program for parents of children in the lower economic classes. The program consisted of a series of workshops for parents to help them enrich the prereading background of their preschool and primary-grade children. The major emphasis was to acquaint parents with children\u27s literature and with the importance of their role in the reading development of their children
Antidepressant drug use in glioblastoma patients: an epidemiological view.
Background: Antidepressant drugs have shown antitumor activity in preclinical glioblastoma studies. Antidepressant drug use, as well as its association with survival, in glioblastoma patients has not been well characterized on a population level.
Methods: Patient characteristics, including the frequency of antidepressant drug use, were assessed in a glioblastoma cohort diagnosed in a 10-year time frame between 2005 and 2014 in the Canton of Zurich, Switzerland. Cox proportional hazards regression models were applied for multivariate analysis. Kaplan-Meier survival curves were used to estimate overall survival (OS) data and the log-rank test was performed for comparisons.
Results: A total of 404 patients with isocitrate dehydrogenase wild-type glioblastoma were included in this study. Sixty-five patients (16.1%) took antidepressant drugs at some point during the disease course. Patients were most commonly prescribed selective serotonin reuptake inhibitors at any time (N = 46, 70.8%). Nineteen patients (29.2%) were on antidepressant drugs at the time of their tumor diagnosis. No differences were observed in OS between those patients who had taken antidepressants at some point in their disease course and those who had not (P = .356). These data were confirmed in a multivariate analysis including age, Karnofsky Performance Scale (KPS), sex, extent of resection, O-methylguanine DNA methyltransferase (MGMT) promoter methylation status, and first-line treatment as cofounders (P = .315). Also, there was no association of use of drugs modulating voltage-dependent potassium channels (citalopram; escitalopram) with survival (P = .639).
Conclusions: This signal-seeking study does not support the hypothesis that antidepressants have antitumor efficacy in glioblastoma on a population level
Exercise Fails to Improve Neurocognition in Depressed Middle-Aged and Older Adults
Purpose: Although cross-sectional studies have demonstrated an association between higher levels of aerobic fitness and improved neurocognitive function, there have been relatively few interventional studies investigating this relationship, and results have been inconsistent. We assessed the effects of aerobic exercise on neurocognitive function in a randomized controlled trial of patients with major depressive disorder (MDD). Methods: Two-hundred and two sedentary men (n = 49) and women (n = 153), aged 40 yr and over and who met diagnostic criteria for MDD, were randomly assigned to the following: a) supervised exercise, b) home-based exercise, c) sertraline, or d) placebo pill. Before and after 4 months of treatment, participants completed measures of: Executive Function (Trail Making Test BA difference score, Stroop Color/Word, Ruff 2 & 7 Test, Digit Symbol), Verbal Memory (Logical Memory, Verbal Paired Associates), and Verbal Fluency/Working Memory (Animal Naming, Controlled Oral Word Association Test, Digit Span). Multivariate analyses of covariance were performed to test the effects of treatment on posttreatment neuropsychological test scores, with baseline neuropsychological test scores, age, education, and change in depression scores entered as covariates. Results: The performance of exercise participants was no better than participants receiving placebo across all neuropsychological tests. Exercise participants performed better than participants receiving sertraline on tests of executive function but not on tests of verbal memory or verbal fluency/ working memory. Conclusions: We found little evidence to support the benefits of an aerobic exercise intervention on neurocognitive performance in patients with MDD. The fragmentation of protostellar discs: the Hill criterion for spiral arms
We present a new framework to explain the link between cooling and
fragmentation in gravitationally unstable protostellar discs. This framework
consists of a simple model for the formation of spiral arms, as well as a
criterion, based on the Hill radius, to determine if a spiral arm will
fragment. This detailed model of fragmentation is based on the results of
numerical simulations of marginally stable protostellar discs, including those
found in the literature, as well as our new suite of 3-D radiation
hydrodynamics simulations of an irradiated, optically-thick protostellar disc
surrounding an A star. Our set of simulations probes the transition to
fragmentation through a scaling of the physical opacity. This model allows us
to directly calculate the critical cooling time of Gammie (2001), with results
that are consistent with those found from numerical experiment. We demonstrate
how this model can be used to predict fragmentation in irradiated protostellar
discs. These numerical simulations, as well as the model that they motivate,
provide strong support for the hypothesis that gravitational instability is
responsible for creating systems with giant planets on wide orbits.Comment: 11 page, 10 figures, submitted to MNRA

Pediatric patients with multi-organ dysfunction syndrome receiving continuous renal replacement therapy.BackgroundCritical illness leading to multi-organ dysfunction syndrome (MODS) and associated acute renal failure (ARF) is less common in children compared to adult patients. As a result, many issues plague the pediatric ARF outcome literature, including a relative lack of prospective study, a lack of modality stratification in subject populations and inconsistent controls for patient illness severity in outcome analysis.MethodsWe now report data from the first multicenter study to assess the outcome of pediatric patients with MODS receiving continuous renal replacement therapy (CRRT). One hundred twenty of 157 Registry patients (63 male/57 female) experienced MODS during their course.ResultsOne hundred sixteen patients had complete data available for analysis. The most common causes leading to CRRT were sepsis (N = 47; 39.2%) and cardiogenic shock (N = 24; 20%). Overall survival was 51.7%. Pediatric Risk of Mortality (PRISM 2) score, central venous pressure (CVP), and% fluid overload (%FO) at CRRT initiation were significantly lower for survivors versus nonsurvivors. Multivariate analysis controlling for severity of illness using PRISM 2 at CRRT initiation revealed that%FO was still significantly lower for survivors versus nonsurvivors (P < 0.05) even for patients receiving both mechanical ventilation and vasoactive pressors. We speculate that increased fluid administration from PICU admission to CRRT initiation is an independent risk factor for mortality in pediatric patients with MODS receiving CRRT.ConclusionWe suggest that after initial resuscitative efforts, an increased emphasis should be placed on early initiation of CRRT and inotropic agent use over fluid administration to maintain acceptable blood pressure

Non-perturbative dynamics of hot non-Abelian gauge fields: beyond leading log approximation
Many aspects of high-temperature gauge theories, such as the electroweak
baryon number violation rate, color conductivity, and the hard gluon damping
rate, have previously been understood only at leading logarithmic order (that
is, neglecting effects suppressed only by an inverse logarithm of the gauge
coupling). We discuss how to systematically go beyond leading logarithmic order
in the analysis of physical quantities. Specifically, we extend to
next-to-leading-log order (NLLO) the simple leading-log effective theory due to
Bodeker that describes non-perturbative color physics in hot non-Abelian
plasmas. A suitable scaling analysis is used to show that no new operators
enter the effective theory at next-to-leading-log order. However, a NLLO
calculation of the color conductivity is required, and we report the resulting
value. Our NLLO result for the color conductivity can be trivially combined
with previous numerical work by G. Moore to yield a NLLO result for the hot

Patientâ level Factors and the Quality of Care Delivered in Pediatric Emergency Departments
ObjectiveQuality of care delivered to adult patients in the emergency department (ED) is often associated with demographic and clinical factors such as a patient’s race/ethnicity and insurance status. We sought to determine whether the quality of care delivered to children in the ED was associated with a variety of patientâ level factors.MethodsThis was a retrospective, observational cohort study. Pediatric patients (<18 years) who received care between January 2011 and December 2011 at one of 12 EDs participating in the Pediatric Emergency Care Applied Research Network (PECARN) were included. We analyzed demographic factors (including age, sex, and payment source) and clinical factors (including triage, chief complaint, and severity of illness). We measured quality of care using a previously validated implicit review instrument using chart review with a summary score that ranged from 5 to 35. We examined associations between demographic and clinical factors and quality of care using a hierarchical multivariable linear regression model with hospital site as a random effect.ResultsIn the multivariable model, among the 620 ED encounters reviewed, we did not find any association between patient age, sex, race/ethnicity, and payment source and the quality of care delivered. Morphological Diversity between Culture Strains of a Chlorarachniophyte, Lotharella globosa
Chlorarachniophytes are marine unicellular algae that possess secondary plastids of green algal origin. Although chlorarachniophytes are a small group (the phylum of Chlorarachniophyta contains 14 species in 8 genera), they have variable and complex life cycles that include amoeboid, coccoid, and/or flagellate cells. The majority of chlorarachniophytes possess two or more cell types in their life cycles, and which cell types are found is one of the principle morphological criteria used for species descriptions. Here we describe an unidentified chlorarachniophyte that was isolated from an artificial coral reef that calls this criterion into question. The life cycle of the new strain includes all three major cell types, but DNA barcoding based on the established nucleomorph ITS sequences showed it to share 100% sequence identity with Lotharella globosa. The type strain of L. globosa was also isolated from a coral reef, but is defined as completely lacking an amoeboid stage throughout its life cycle. We conclude that L. globosa possesses morphological diversity between culture strains, and that the new strain is a variety of L. globosa, which we describe as Lotharella globosa var. fortis var. nov. to include the amoeboid stage in the formal description of L. globosa. This intraspecies variation suggest that gross morphological stages maybe lost rather rapidly, and specifically that the type strain of L. globosa has lost the ability to form the amoeboid stage, perhaps recently. This in turn suggests that even major morphological characters used for taxonomy of this group may be variable in natural populations, and therefore misleading
