    Expectation to Improve Cardiovascular Risk Factors Control in Participants to a Health Promotion Program

    Background: We assessed expectations to improve cardiovascular disease risk factors (CVD-RF) in participants to a health promotion program. Participants and Methods: Blood pressure (BP), blood glucose (BG), blood total cholesterol (TC), body mass index (BMI), and self-reported smoking were assessed in 1,598 volunteers from the general public (men: 40%; mean age: 56.7 ± 12.7years) participating in a mobile health promotion program in the Vaud canton, Switzerland. Participants were asked about their expectation to have their CVD-RF improved at a next visit scheduled 2-3years later. Results: Expectation for improved control was found in 90% of participants with elevated BP, 91% with elevated BG, 45% with elevated TC, 44% who were overweight, and 35% who were smoking. Expectation for TC improvement was reported more often by men, persons with high level of TC, and persons who had consulted a doctor in the past 12months. Expectations to lose weight and to quit smoking were found more often in younger persons than the older ones.     Assessing storm surge hazard and impact of sea level rise in the Lesser Antilles case study of Martinique

    In the Lesser Antilles, coastal inundations from hurricane-induced storm surges pose a great threat to lives, properties and ecosystems. Assessing current and future storm surge hazards with sufficient spatial resolution is of primary interest to help coastal planners and decision makers develop mitigation and adaptation measures. Here, we use wave–current numerical models and statistical methods to investigate worst case scenarios and 100-year surge levels for the case study of Martinique under present climate or considering a potential sea level rise. Results confirm that the wave setup plays a major role in the Lesser Antilles, where the narrow island shelf impedes the piling-up of large amounts of wind-driven water on the shoreline during extreme events. The radiation stress gradients thus contribute significantly to the total surge – up to 100 % in some cases. The nonlinear interactions of sea level rise (SLR) with bathymetry and topography are generally found to be relatively small in Martinique but can reach several tens of centimeters in low-lying areas where the inundation extent is strongly enhanced compared to present conditions. These findings further emphasize the importance of waves for developing operational storm surge warning systems in the Lesser Antilles and encourage caution when using static methods to assess the impact of sea level rise on storm surge hazard

    Chronic urinary tract infections by urease-producing bacteria may increase urine pH and promote thereby the formation of recurrent kidney stones made of highly carbonated calcium phosphate apatite and struvite, a magnesium ammonium phosphate. To date, there is no safe and effective treatment decreasing urine pH on a long term. We hypothesized that fludrocortisone, a mineralocorticoid, would decrease urine pH by increasing proton secretion in the kidney collecting tubule. We report three cases of patients with kidney stone suffering from chronic urinary infection by urease-producing germs, treated by fludrocortisone on a long term. Urine pH decreased sustainably over several months and tolerance was good

    Management of traumatic events: influence of emotion-centered coping strategies on the occurrence of dissociation and post-traumatic stress disorder

    Our aim was to assess the influence of the coping strategies employed for the management of traumatic events on the occurrence of dissociation and traumatic disorders. We carried out a 1-year retrospective study of the cognitive management of a traumatic event in 18 subjects involved in the same road vehicle accident. The diagnosis of post-traumatic stress disorder (PTSD) was made for 33.3% of the participants. The participants with a PTSD diagnosis 1 year after the event used emotion-centered strategies during the event more often than did those with no PTSD, P < 0.02. In the year after the traumatic event, our results show a strong link between the intensity of PTSD and the severity of the post-traumatic symptoms like dissociation (P = 0.032) and the use of emotion-centered strategies (P = 0.004). Moreover, the participants who presented Peritraumatic Dissociative Experiences Questionnaire scores above 15 made greater use of emotion-centered coping strategies than did those who did not show dissociation, P < 0.04. Our results confirm that the cognitive management of traumatic events may play an essential role in the development of a state of post-traumatic stress in the aftermath of a violent event

    Local Martingale and Pathwise Solutions for an Abstract Fluids Model

    We establish the existence and uniqueness of both local martingale and local pathwise solutions of an abstract nonlinear stochastic evolution system. The primary application of this abstract framework is to infer the local existence of strong, pathwise solutions to the 3D primitive equations of the oceans and atmosphere forced by a nonlinear multiplicative white noise. Instead of developing our results specifically for the 3D primitive equations we choose to develop them in a slightly abstract framework which covers many related forms of these equations (atmosphere, oceans, coupled atmosphere-ocean, on the sphere, on the {\beta}-plane approximation etc and the incompressible Navier-Stokes equations). In applications, all of the details are given for the {\beta}-plane approximation of the oceans equations
