Fibrosis glútea: ¿esencial o secundaria?
Constatamos la rara frecuencia de esta afección presentando 5 observaciones
de Fibrosis Glútea recogidas en tres casos clínicos estudiados. Dos son bilaterales y uno afecta
a un solo glúteo. Habiéndose invocado, por los autores consultados, etiologías diversas, actualmente
sigue siendo desconocida. Creemos de interés resaltar que los casos presentados se trataron
en las etapas finales de crecimiento, mientras que los referidos en la literatura consultada
todos han recibido tratamiento quirúrgico en edad temprana. El tratamiento es de resultado satisfactorio,
independientemente de la edad y de la técnica quirúrgica empleada.Three patients affected with gluteal fibrosis are colected by the authors. Two
cases had bilateral presentation and one patient was unilateral. According to the literature, the
aetiology continue to be unknow. All patients were treated at the end of growthing period, while
all cases collected in the literature were treated early. The results have been satisfactory without
relation to age or the surgical procedure employed
Resultados clínicos de la artroplastia unicompartimental de rodilla en un hospital universitario
Se ha realizado un estudio retrospectivo de 25 artroplastias unicompartimentales de rodilla intervenidas
de 1989 a 2008, En 15 casos (16 prótesis) se obtuvieron datos completos para su estudio. Once artroplastias se
realizaron en mujeres y 5 en hombres con una edad media de 65 años. El tiempo de seguimiento fue de 65 meses
(rango 14-202). Analizamos el rango de movilidad, la necesidad de cirugía de revisión y la puntuación en el Knee
Society Store. Hubo de casos de infección superficial resueltos favorablemente y ningún caso de infección profunda.
La cirugía de revisión fue necesaria en el 37,5% de pacientes en una media de tiempo de 37'5 meses (rango 18-128).
Los resultados fueron similares a aquellos que hicieron que se abandonara la técnica en los años 90.Unicompartmental knee arthroplasty appeared at the 70s as an alternative to the proximal tibial osteotomy
in young patients with pain and medial or lateral compartment osteoarthritis of the knee. At the 90s this technique
began to be abandoned because the clinical results were not the expected. In the last years, lots of articles have been
published showing results that can be compared with the total knee arthroplasty thanks to the news designs and the
improve of the surgical technique. A retrospective study was made of the 25 unicompartmental knee arthroplasties performed
in our hospital, with a mean follow up of 65 months. We analysed the range of mobility, the need of revision
surgery and the Knee Society Score, getting similar results to that that made abandon the technique in the 90s
Resección compartimental mediante abordaje quirúrgico modificado en sarcomas de partes blandas del deltoides
Se describe una técnica quirúrgica para la resección radical de sarcomas de partes blandas confinados
al compartimento deltoideo en tres pacientes, así como los resultados clínicos y funcionales, además de
la evolución oncológica de su patología. Se realiza un abordaje en “Y” que permite la desinserción proximal y
distal del deltoides, incluyendo en la pieza tumoral una isla cutánea lateral con la zona de biopsia previa, evitando
la necesidad de técnicas de cobertura. No se desarrollaron complicaciones de la herida quirúrgica ni casos
de recurrencias locales. Todos los pacientes presentaron un rango de movilidad mínimo de 130º de abducción y
una pérdida de fuerza para la abducción y antepulsión. Concluimos que los pacientes con neoplasias malignas
confinadas al compartimento deltoideo tratados con la resección radical según la técnica descrita presentan unos
resultados funcionales excelentes, en ausencia de lesiones del manguito concomitantes, con un menor riesgo de
recidiva local.A surgical technique for radical excision of soft tissue malignant sarcomas is described and functional
and oncologic outcomes of this procedure in three patients diagnosed of soft tissue sarcoma are reported.
A “Y” approach is used to perform a complete release of deltoid attachments in the scapula, clavicle and proximal
humerus. This approach also allows an excision of the biopsy site, avoiding the establishment of large flaps
coverage. No local recurrence or surgical wound complications were found. All the patients showed a good range
of motion (130º of abduction) but loss of strength in abduction and antepulsion was found. We conclude that patients
with malignant soft tissue tumors involving the deltoid muscle which undergo a wide resection of the bump
through the previously described technique have an excellent functional outcome with low risk of recurrence, in
absence of concomitant rotator cuff injuries
Artroplastia de rodilla infectada: Revisión del tratamiento y valoración de resultados
La infección de la artroplastia total de rodilla representa su complicación local más temida con una incidencia actual entre el 1-5%. Se ha realizado un estudio retrospectivo del tratamiento empleado en 16 artroplastias de rodilla infectadas (3,1%) en los últimos seis años entre 516 artroplastias realizadas en ese mismo periodo de tiempo valorando la ausencia de infección asó como la evaluación de la rodilla tanto funcional mediante las escalas de la Knee Society como subjetiva del paciente mediante una escala analógica visual en función del tratamiento empleado. En todos los casos se erradicó al final del seguimiento aunque los resultados variaron en función del tratamiento. Los mejores resultados lo obtuvieron las infecciones agudas en las que se realizó un desbridamiento precoz o una revisión en 1 tiempo. La revisión en 2 tiempos obtuvo buenos resultados en infecciones tardía aunque fracasó cuando se realizó en infecciones hematógenas y agudas. La artrodesis se utilizó ante el fracaso de otras técnicas obteniendo resultados regulares. La artroplastia de resección obtuvo malos resultados en todos los casos.Infection of knee arthroplasty in the most local ugly complication with an incidence of 1 to 5%. We retrospectively studied 16 infected knee arthroplasties out of of 516 primary artrhoplasties performed in our department the last 6 years (3.1%), evaluating the absence of infection and the functional knee with the Knee Society Score. Subjective assessment was made using a visual analog scale. At the end of the follow-up the cases were free of infection although results changed depending on treatment performed. Best results were found on acute infections, in which early debridement or one-stage revision were performed. Two-stage revision obtained good results on late infections although it failed when it was attempted on hematogeneus and acute infections. Arthrodesis was done in cases where other techniques failed obtaining fair results. Resection arthroplasty obtained poor results in all cases
Tratamiento de fracturas de húmero proximal en hueso osteoporótico mediante fijación con sistema de aumentación
Introducción. Para el tratamiento de fracturas de húmero proximal en pacientes osteoporóticos, se
han desarrollado fijaciones con aumentación que mejoran la estabilidad del implante. El objetivo de este estudio
es analizar los resultados obtenidos con la técnica empleada. Material y métodos. Estudio descriptivo retrospectivo
de 7 pacientes con fracturas en 3-4 fragmentos intervenidos mediante placa Philos (Synthes)® de aumentación.
Seguimiento medio de 12 meses. Resultados. La puntuación media en la Escala Visual Analógica de dolor
fue de 4,4. En la escala Quick Dash de 47,47. En la Escala Constant-Murley de 36,36. La abducción activa media
fue de 95º; la flexión anterior media de 105º; la rotación externa media de 44,14º; la rotación interna alcanzó en
tres pacientes S1, en dos L5, en uno L2 y en otro T10. En los controles radiográficos se mantuvo la reducción.
Nos encontramos con un caso de consolidación parcial. Conclusión. Las técnicas de aumentación disminuyen el
riesgo de fallos de fijación manteniendo la reducción.. Introduction. To treat proximal humerus fractures in patients with osteoporosis, the last few years,
systems to improve implant stability by augmentation techniques have been developed. The aim of this study
is to analyze the results obtained with the surgical technique employed. Materials and methods. Retrospective
descriptive study. We selected 7 patients with 3 or 4- part fractures treated operatively with Philos Augmentation
plate (Synthes)® fixation technique. Average follow-up was 12 months. Results. Pain measured with a Visual Analogical
Scale (VAS) got a average value of 4,4. The mean Quick Dash Score was 47,47. The mean Constat-Murley
modified was 36,36. The active articular balances showed a mean abduction of 91º; mean anterior flexion of 105º;
mean external rotation of 44,14º; and finally the internal rotation achieved in three patients got to S1, in two patients
to L5, in one patient to L2 and in other patient to T10. In X- Ray post-operative controls we can appreciate
the reduction maintenance. In one case we observe a partial consolidation of the fracture. Conclusion. Surgical
techniques that imply implant augmentation reduces the risk of implant failure and maintains the fracture reduction
Evaluación de resultados en el tratamiento quirúrgico de las fracturas aisladas de cabeza de radio en el adulto
Presentamos una serie de 35 pacientes intervenidos en nuestro servicio por fractura aislada de cabeza de radio. El objetivo ha sido evaluar el resultado funcional del tratamiento quirúrgico de estas fracturas en el adulto, la correlación de los diferentes sistemas de evaluación de resultados y determinar los factores pronóstico. Las fracturas se clasificaron según Mason evaluando los resultados utilizando la escala de valoración del hospital par cirugía especial (HSSA) de Inglis y Pellici, el índice de clasificación funcional de Broberg y Morrey y la escala de evaluación funcional del codo de la Clínica Mayo. En más del 80% de los casos se obtuvieron resultados buenos ó excelentes, con una flexión media de 139% y un déficit de extensión de 10º. Los tres métodos de valoración mostraron un índice de concordancia estadísticamente significativo resultado por tanto ser equiparables. El retraso en la cirugía resultó ser un factor pronóstico en el resultado final (p<0.05) mientras que el tipo de fractura y de cirugía muestran tendencia a influir en el resultado final.Functional outcome after surgery for isolated radial head fractures in adults was assessed using three different outcome assessment approaches and favourable prognostic factors were determined. Thirty-five patients with radial head fractures were operated. Fractures were grouped according to the Mason classification, and the results obtained were assessed by means of the hospital for special surgery assessment (HSSA) system. Broberg and Morrey functional classification index and the Mayor Clinic elbow functional in over 80% of cases, with an average flexion of 139 degrees and an extension defect of 10 degrees. All three assessment system yielded statistically significant concordant scores, and was thus considered to be equivalent. Surgical delay proved to be a significant prognostic factor of the end result (p<0.05), while the type of fracture and surgery employed tended to influence outcome
Resultados de la autotransfusión postoperatoria en artroplastia total de rodilla
Introducción. El propósito es evaluar si la utilización de los drenajes autotransfusores en la artroplastia
total de rodilla primaria minimiza la caída de la hemoglobina en el postoperatorio, así como la necesidad
de transfusiones sanguíneas alogénicas. Material y métodos. Estudio retrospectivo, analítico observacional que
incluyó 100 pacientes intervenidos de artroplastia de rodilla entre enero 2014 y enero 2015 en el Hospital Dr.
Peset. Compara tanto los parámetros analíticos de sangrado (hemoglobina y hematocrito) como la necesidad de
transfusión sanguínea entre pacientes reinfundidos y no reinfundidos. Resultados. Se reinfundieron 50 pacientes
con un volumen medio de 505 ml. La necesidad de transfusión sanguínea alogénica en los reinfundidos fue de
4% mientras que en los no reinfundidos fue de 24%. La caída media de hemoglobina postquirúrgica fue 1,12 g/
dl menor en el grupo reinfundido. Conclusiones. El redón autotransfusor es una técnica segura que mejora el
rendimiento analítico postransfusional y, por consiguiente, reduce el número de transfusiones alogénicas.Introduction. The purpose is to evaluate the use of autotransfusors drains in total knee arthroplasty
primary minimizes the drop in hemoglobin in the postoperative period, and the need for allogeneic blood
transfusions. Material and methods. A retrospective, analitic and observational study, have been included 100
patients undergoing knee arthroplasty between January 2014 and January 2015 at the Dr. Peset Hospital. Compare
the analytical parameters of bleeding (hemoglobin and hematocrit) and the need for blood transfusion among
patients and not reinfused reinfused. Results. Fifty patients with Total Knee Arthroplasty (TKA) were reinfused
with a mean volume of 505 ml. The need for allogenic blood transfusions was 4%, while in patients not reinfused
was 24%. The average drop in postoperative hemoglobin was 1.12 g / dl lower in the group reinfused. Conclusions.
The autotransfusion drain is a safe technique which improves post-transfusional values and therefore
reduces the number of allogenic blood transfusions
Manejo de la infección protésica de hombro con gran defecto óseo: descripción de un caso complejo
La infección periprotésica de hombro tiene una incidencia de entre el 0.4% al 2.9%; su tratamiento
es controvertido y se basa en la experiencia sobre infecciones protésicas de rodilla y cadera. Presentamos el caso
de una paciente quien fue tratada mediante recambio en dos tiempos, la cual presentó como complicación añadida
un gran defecto óseo metafisodiafisario tratado siguiendo pautas establecidas en la literatura actual, con buen

DETAIL DESIGN OF A BALLAST CONTROL ROOM FOR AN UNDERWATER TIDAL ENERGY CONVERTER
The aim of this article is to design, precisely and fully detailed, the Control Room of a Ballast Actuation System of a marine Tidal Energy Converter. The design would respond to the detail design stage of any shipyard for its subsequent manufacture.
During the following lines, the authors have analysed the Ballast Actuation System, not only from a conceptual way, but also from the detailed design point of view, so it has been considered necessary to complete it, with a study of a technological nature, in which industrial (reducing the cost and increasing reliability) solutions have been developed for measuring and driving systems.
To elaborate the 3D detail design of the pump room, a topological design has been introduced for allowing a quicker evaluation of different design alternatives. All the machinery, piping, supports and auxiliary structures have been modelled in order to evaluate the quality of the conceptual and initial design. The primary structure, as well as the location of the equipment, have been carefully studied and analysed.
The P&ID (Piping and Instrumentation Diagrams) and access drawings have been followed in order to model the 3D pipe routing, supports and auxiliary structure. The 3D components have been defined from its main features (element ID, supplier, description, weight and centre of gravity) and its geometry (library or geometric macro model), provided by the supplier. All this work has been carried out with the intention of not only assess different alternatives, but also to generate at any time the information for manufacturing and mounting
