    Picturesque Violence: Tourism, the Film Industry, and the Heritagization of ‘Bandoleros’ in Spain, 1905–1936

    This article examines the debates about the Andalusian ‘bandoleros’ (bandits) in the context of early tourism as a state-guided policy in Spain. As we argue, the development of tourism made Spanish intellectuals reconsider the real armed activity in Andalucía as part of Spanish national heritage and a tourist attraction. Consistent with the stereotypical image of Spain coined by the Romantic travelers, such an early heritagization of brigandry reveals the role of the élites in recasting exotic imagery into modern tourism-shaped identities: in the hands of early century writers, bandits were reshaped as part of the ‘modern picturesque’. Furthermore, the role given to brigands in early cinema allows one to see how the early heritage discourse bridged transnational and centralist interests at the expense of the regional ones, thus foreshadowing the debates about hegemony in present-day heritage studies

    Review article: Risk factors of infection following total knee arthroplasty

    22 PubMed articles in English were identified using the key words: ‘risk factors’, ‘infection’, and ‘primary total knee arthroplasty (TKA)’. The 10 most relevant articles were reviewed. In one study, obesity and diabetes were considered risk factors for infection following TKA. In another study, postoperative infection correlated with a history of open reduction and internal fixation, male gender, remnants of previous internal fixation material, and body mass index. In yet another study, the risk factors were (in decreasing order of significance): congestive heart failure, chronic pulmonary disease, preoperative anaemia, diabetes, depression, renal disease, pulmonary circulation disorders, obesity, rheumatologic disease, psychoses, metastatic tumour, peripheral vascular disease, and valvular diseas

    El papel del plasma rico en plaquetas (PRP) en las lesiones deportivas de rodilla

    El plasma rico en plaquetas (PRP) se está utilizando cada vez más. Usando como palabras clave “PRP” y “knee” (rodilla) se hizo una búsqueda en PubMed sobre el uso del PRP en las lesiones deportivas de la rodilla. Se encontraron 22 artículos. Muchos de ellos fueron estudios realizados en animales y el resto estudios sin grupo control llevados a cabo en humanos y publicados en revistas de rigor científico modesto. Dichos estudios animales y humanos sugieren que la colocación de un andamio en puente de colágeno-PRP en el defecto central del LCA podría estimular biomecánicamente e histológicamente su cicatrización. También que las inyecciones intra-articulares de PRP podrían mejorar la cicatrización de defectos meniscales. Por último, que las inyecciones de PRP podrían promover resultados satisfactorios en casos de rodilla de saltador (tendinitis rotuliana). Teniendo en cuenta la baja calidad de los datos publicados en la bibliografía y tras realizar una valoración objetiva del valor del PRP en las indicaciones previamente mencionadas, mi conclusión es que el PRP es todavía es un tratamiento experimental, que en muchos casos no ha demostrado tener un valor significativo en cuanto a sus efectosThe use of platelet-rich plasma (PRP) is becoming increasingly available. PubMed articles related to the use of PRP in sports injuries of the knee were searched using the following key words: PRP and knee. A total of 22 reports were found. Many of them were animal studies and the rest were human clinical studies with no controls and published in journals that do not expect as much rigour in the scientific validity. The aforementioned animal experiments and clinical studies suggest that placement of a collagen-PRP bridging scaffold in a central ACL defect can stimulate healing of the ACL histologically and biomechanically. Also that intra- articular injections of PRP can enhance the healing of meniscal defects. Finally, that PRP injections can have the potential to promote the achievement of a satisfactory clinical outcome in cases of jumper´s knee (patellar tendinopathy). Taking into account the low quality of the data reported in the literature and after performing an objective view of the value of PRP for the various indications, my conclusion is that PRP is a treatment that is still experimental and in many instances shown to be not significant in its effect

    Background Patellar tendinopathy (PT) presents a challenge to orthopaedic surgeons. The purpose of this review is to revise strategies for treatment of PT Materials and methods A PubMed (MEDLINE) search of the years 2002–2012 was performed using ‘‘patellar tendinopathy’’ and ‘‘treatment’’ as keywords. The twenty-two articles addressing the treatment of PT with a higher level of evidence were selected. Results Conservative treatment includes therapeutic exercises (eccentric training), extracorporeal shock wave therapy (ESWT), and different injection treatments (plateletrich plasma, sclerosing polidocanol, steroids, aprotinin, autologous skin-derived tendon-like cells, and bone marrow mononuclear cells). Surgical treatment may be indicated in motivated patients if carefully followed conservative treatment is unsuccessful after more than 3–6 months. Open surgical treatment includes longitudinal splitting of the tendon, excision of abnormal tissue (tendonectomy), resection and drilling of the inferior pole of the patella, closure of the paratenon. Postoperative inmobilisation and aggressive postoperative rehabilitation are also paramount. Arthroscopic techniques include shaving of the dorsal side of the proximal tendon, removal of the hypertrophic synovitis around the inferior patellar pole with a bipolar cautery system, and arthroscopic tendon debridement with excision of the distal pole of the patella. Conclusion Physical training, and particularly eccentric training, appears to be the treatment of choice. The literature does not clarify which surgical technique is more effective in recalcitrant cases. Therefore, both open surgical techniques and arthroscopic techniques can be use

An insufficiency of coagulation factor VIII produces hemophilia A, and lack of factor IX causes hemophilia B. Prevention and management of the disease require intravenous infusion of the deficient factor. Worldwide, hemophilia affects approximately 600,000 people, 20% of whom develop antibodies against the deficient coagulation factor. Hemophilic patients with inhibitors present with multiarticular joint degeneration (hemophilic arthropathy) secondary to recurrent hemarthroses. The availability of activated prothrombin complex concentrates and activated recombinant factor VII allows hemophilic patients with high inhibitor titers to undergo elective orthopedic surgery with a high expectation of success, and thorough individual case analysis by a multidisciplinary team allows surgeons to obtain satisfactory results. However, the rate of potential complications must not be underestimate

This review article summarizes the knowledge on the efficacy of using intra-articular injections of mesenchymal stem cells (MSCs) to treat KOA. PubMed (Medline) and the Cochrane Library were searched for literature related to MSC therapy and KOA up until January 31, 2014. The key search terms used were stem cells and knee osteoarthritis. One hundred thirty-five reports were found, but only the 25 fully focused on the topic were used for analysis. Only 3 randomized controlled trials (level II evidence) found pain relief and functional improvement over the short term. The other human studies also reported encouraging results, but their evidence level was very low (IV). Larger randomized controlled trials are needed to support these preliminary encouraging results.     Hemophilic pseudotumors: Diagnosis and management

    Prevention is essential for avoiding the complications of muscle hematomas (pseudotumors, compartment syndromes and peripheral nerve lesions) in hemophilic patients. This is achieved through early diagnosis of muscle hematomas and proper long-term hematological treatment until they have resolved (confirmed by image studies). Ultrasound-guided percutaneous drainage could be beneficial in terms of achieving better and faster symptom relief. When suspecting a hemophilic pseudotumor, biopsy will help us confirm the diagnosis and rule out true tumors (chondrosarcoma, liposarcoma, synovial sarcoma) that sometimes mimic hemophilic pseudotumor. Surgical removal of hemophilic pseudotumor is the best solution. As alternatives, there are curettage and filling with cancellous bone and radiotherapy (when surgery is contraindicated). Preoperative arterial embolization (ideally 2 weeks before surgery) helps control intraoperative bleeding during surgery for giant pelvic pseudotumors

    The quality is a degree of excellence, and what is excellent is new. The evaluations of the quality of the organizations, departments, functional areas, activities, products and services have been recognized by the goodness of their results beginning to be mandatory and of increasingly strict application in the search of the fulfilment of its purposes. The quality has been a goal of many countries. In Ecuador, the Constitution establishes that the public administration is governed by principles, among others: the quality. This involves the participation of the people to ensure the actions in the organizational context. The quality has an impact on people, activities, processes and outcomes; all guided toward excellence. An intention of quality (not exclusive from the point of view of the analysis of other models) and at the same time of the Government of the TSI is the application of good practices of the Ibero-American Model [3] and of the standard ISO/IEC 38500 [4], respectively. The proposed quality assessment model exposes the government of TIS as a functional team at the service of the organization of TIS. Therefore, the model is a response to the value-based quality concept on value due to the extent that Ecuador is gaining a growth in the use of technology. The existing literature on EFQM models, Malcolm Baldrige and other authors who support it, was available for this work [5] [6] [7] [8].
