Avaliação Microbiológica da Antissepsia Pré-Operatória das Mãos de Profissionais de Enfermagem de um Centro Cirúrgico
Justificativ a e Objetivo: Avaliar a microbiota nas mãos dos profissionais de saúde do Centro Cirúrgico de um hospital de ensino no interior do Rio Grande do Sul. Realizou-se estudo transversal. Método: A amostra foi composta por Técnicos de Enfermagem e Enfermeiros, 15 realizaram a técnica de escovação tradicional com digliconato de clorexidina 2% e 18realizaram a antissepsia das mãos utilizando uma solução alcoólica. A coleta foi realizada através de swab estéril das mãos antes e após a higienização,foram realizadas as provas de identificação das espécieseantibiograma. Resultados:Das 18 amostras,apenas 5 apresentaram contaminação após a higienização das mãos com solução alcoólica. Já,as 15 amostras queutilizaram a escova de digliconato de clorexidina 2%, 12 apresentaram contaminação, após a antissepsia pré-operatória. Considerações finais: a solução antisséptica alcoólica foi mais eficaz se comparada à técnica tradicional de escovação com digliconato de clorexidina 2%
STEWARDSHIP PROGRAMS NO CENTRO CIRÚRGICO: identificando as demandas de uma instituição do Sul do Brasil: Stewardship programs on the surgicenter: identifying the demands of an institution of south Brazil
Objective: The purpose of this study was to verify the adequacy of the antimicrobial prophylaxis realized at an operating toom of Rio Grande do Sul. Methods: transversal, observational and retrospective study where secondary data of patients submitted to surgical procedures were analyzed. The therapeutic scheme and moment of administration were characterized, as well as the demographic information of the patients. Results: 54 procedures were analyzed, of which 51 patients received Cephazolin and were mostly heart surgeries. There was variation in the doses administered and 16 patients received the prophylaxis within 30 minutes before the incision. 52 patients used postoperative prophylaxis, of which 48 received Cephalothin as antimicrobial and 35 extended the prophylaxis for 2 days. It was also observed inconsistency of the posology prescribed. It was verified the incorrect filling of the electronic medical record of some patients and absence of data. Conclusion: the prophylaxis of preoperative was appropriate to the institutional protocol for the selection of the antibiotic. There was inconsistence regarding the moment and dose, as well as the posology and days of use of the extended prophylaxis.Objetivo: O presente trabalho tem por objetivo verificar a adequação da profilaxia antimicrobiana realizada em um centro cirúrgico do Rio Grande do Sul. Métodos: estudo transversal, observacional e retrospectivo onde foram analisados os dados secundários de pacientes submetidos a procedimentos cirúrgicos. Foram caracterizados o esquema terapêutico e o momento da administração, bem como informações demográficas dos pacientes. Resultados: foram analisados 54 procedimentos, dos quais 51 pacientes receberam Cefazolina e eram em sua maioria cirurgias cardíacas. Houve variação quanto às doses administradas e 16 pacientes receberam a profilaxia em até 30 minutos antes da incisão. 52 pacientes utilizaram profilaxia no pós-operatório, dos quais 48 receberam Cefalotina como antimicrobiano e 35 estenderam a profilaxia por 2 dias. Foi observada, ainda, inconsistência dos esquemas posológicos prescritos. Verificou-se o incorreto preenchimento e ausência de dados no prontuário eletrônico de alguns pacientes. Conclusão: a profilaxia no pré-operatório estava adequada ao protocolo institucional quanto à seleção do antibiótico. Quanto ao momento e dose houve inconsistência, bem como quanto à posologia e os dias de uso da profilaxia estendida
ANÁLISE DO USO DE MEDICAMENTOS POTENCIALMENTE INAPROPRIADOS PARA IDOSOS EM UMA UNIDADE DE URGÊNCIA DE UM HOSPITAL DO RIO GRANDE DO SUL
Objective: to Evaluate the prevalence of potentially inappropriate medications (MPI) prescription in an emergency unit of a hospital in Rio Grande do Sul. Methodology: a retrospective cross-sectional study through secondary data on medical prescriptions containing MPI of elderly hospitalized by the Unified Health System in a teaching hospital in the interior of Rio Grande do Sul. Results: a whole of 98 medical prescriptions were observed in which 21 MPIs were found for the elderly, with metoclopramide, regular insulin, omeprazole, clonazepam and clonidine being the most prescribed. In this context, the use of 10.2 drugs per patient was found, constituting an important polypharmacy picture in the elderly. Conclusion: thus, the rationalization of the prescription of MPI for the elderly is relevant because it offers more safety and quality in the drug treatment of geriatric patients in order to avoid possible unnecessary polypharmacy.Objetivo: avaliar a prevalência de prescrição de medicamentos potencialmente inapropriados (MPI) em uma unidade de urgência de um hospital do Rio Grande do Sul. Metodologia: estudo transversal retrospectivo através de dados secundários das prescrições médicas contendo MPI de idosos hospitalizados pelo Sistema Único de Saúde em um hospital de ensino no interior do Rio Grande do Sul. Resultados: foi observado, ao todo, 98 prescrições médicas na qual encontrou-se a prescrição de 21 MPIs para idosos,sendo metoclopramida, insulina regular, omeprazol, clonazepam e clonidina os mais prescritos. Nesse contexto, encontrou-se, em média, o uso de 10,2 remédios por paciente, configurando-se importante quadro de polifarmácia no idoso. Conclusão: desse modo, a racionalização da prescrição de MPI para idosos é relevante pois oferece mais segurança e qualidade no tratamento medicamentoso de pacientes geriátricos a fim de evitar possível quadro de polifarmácia desnecessário
PREVALÊNCIA DE USO DE MEDICAMENTOS E REMÉDIOS CASEIROS PARA PREVENÇÃO DE Covid-19: estudo de base populacional
Introduction: the Covid-19 pandemic brought a behavior of the population regarding the use of medicines without proven effectiveness. Objective: to measure the prevalence and profile of use of medicines and home remedies for the prevention of Covid-19 in the Vale do Rio Pardo region, Rio Grande do Sul, Brazil. Method: a population-based cross-sectional study carried out in four stages, whose collection was carried out at home from August to October 2020. Questions were asked about the use of medicines and home remedies, and demographic, socioeconomic, clinical and behavioral data were also collected, with rapid test for IgM and IgG antibodies. Descriptive and bivariate analysis was performed between medication use and sociodemographic data. The linear trend and the heterogeneity of proportions were evaluated using the chi-square test, considering the value of p<0.05. Results: the sample consisted of 4252 individuals. The prevalence of use of medicines and home remedies was 7.8% (95%CI: 6.5-8.17). The most used medications were German Balm® (Vitamin E) (43.7%), ivermectin (26.6%) and other vitamins (17.9%). The most reported home remedies were propolis (70.4%) and teas (25.6%). Medication use was higher in individuals aged 60 years or over (8.1%), with higher education (10.4%), family income greater than R6.271 (10,3%), moradores da zona rural (6,9%) e com pelo menos uma morbidade (8,0%) (p<0,05). Conclusão: apesar de baixo, destacou-se consumo de medicamentos e remédios caseiros sem comprovação científica para prevenção da Covid-19. É necessária uma discussão sobre o tipo de acesso às informações e a necessidade de uma assistência farmacêutica mais difusa e efetiva no cuidado da população, viabilizando o uso racional de medicamentos
Adesão aos protocolos de atendimento para a não infecção de sítio cirúrgico de coluna
Objective: to evaluate adherence to treatment protocols for non-surgical site infection (NISC) of the spine and associated factors. Method: cross-sectional study conducted through the review of 60 medical records of patients undergoing spinal surgery from 2015 to 2019, its non-infection outcome and related conditions. The study was carried out in the first half of 2020. Results: some variables related to NISC were: antimicrobial prophylaxis from 30 to 60 min before surgery (RR= 0.97; p=0.026), normothermia (RR= 0.80; p =0.050), post-surgical hospitalization in the Intensive Care Unit (up to 3 days) (RR=2.00; p=0.040). The frequency of NISC was 83.3% (50/60) p= 0.728). Conclusion: only normothermia was a factor associated with non-infection for NISC in linear regression. It is noteworthy that adherence to work processes is essential to protect against infections, reduce adverse events and ensure safety.Objetivo: avaliar a adesão aos protocolos de atendimento para a não infecção de sítio cirúrgico (NISC) de coluna e os fatores associados. Método: estudo transversal realizado por meio da revisão de 60 prontuários de pacientes submetidos à cirurgia de coluna de 2015 a 2019, seu desfecho de não infecção e as condições relacionadas. O estudo foi executado no primeiro semestre de 2020. Resultados: algumas variáveis relacionadas para NISC foram: profilaxia antimicrobiana de 30 a 60 min antes da cirurgia (RR= 0,97; p=0,026), normotermia (RR= 0,80; p=0,050), internação pós cirúrgica em Unidade de Terapia Intensiva (até 3 dias) (RR=2,00; p=0,040). A frequência de NISC foi de 83,3% (50/60) p= 0,728). Conclusão: apenas a normotermia foi fator associado a não infecção para NISC na regressão linear. Ressalta-se que a adesão aos processos de trabalho são primordiais para a proteção das infecções, reduzir eventos adversos e garantir a segurança

Development and validation of the MMCD score to predict kidney replacement therapy in COVID-19 patients
Abstract Background Acute kidney injury (AKI) is frequently associated with COVID-19, and the need for kidney replacement therapy (KRT) is considered an indicator of disease severity. This study aimed to develop a prognostic score for predicting the need for KRT in hospitalised COVID-19 patients, and to assess the incidence of AKI and KRT requirement. Methods This study is part of a multicentre cohort, the Brazilian COVID-19 Registry. A total of 5212 adult COVID-19 patients were included between March/2020 and September/2020. Variable selection was performed using generalised additive models (GAM), and least absolute shrinkage and selection operator (LASSO) regression was used for score derivation. Accuracy was assessed using the area under the receiver operating characteristic curve (AUC-ROC). Results The median age of the model-derivation cohort was 59 (IQR 47–70) years, 54.5% were men, 34.3% required ICU admission, 20.9% evolved with AKI, 9.3% required KRT, and 15.1% died during hospitalisation. The temporal validation cohort had similar age, sex, ICU admission, AKI, required KRT distribution and in-hospital mortality. The geographic validation cohort had similar age and sex; however, this cohort had higher rates of ICU admission, AKI, need for KRT and in-hospital mortality. Four predictors of the need for KRT were identified using GAM: need for mechanical ventilation, male sex, higher creatinine at hospital presentation and diabetes. The MMCD score had excellent discrimination in derivation (AUROC 0.929, 95% CI 0.918–0.939) and validation (temporal AUROC 0.927, 95% CI 0.911–0.941; geographic AUROC 0.819, 95% CI 0.792–0.845) cohorts and good overall performance (Brier score: 0.057, 0.056 and 0.122, respectively). The score is implemented in a freely available online risk calculator ( https://www.mmcdscore.com/ ). Conclusions The use of the MMCD score to predict the need for KRT may assist healthcare workers in identifying hospitalised COVID-19 patients who may require more intensive monitoring, and can be useful for resource allocation
