    Population Based Screening for Prostate Cancer: assessment of diagnostic tools and cancers detected

    Over the past decade, considerable debate has occurred over the question whether or not to screen asymptomatic men for prostate cancer. It is unknown whether early detection and treatment of the disease will decrease the disease specific mortality. On theoretical grounds screening may prove to be successful. If the disease is diagnosed at an earlier stage of its development in which it is still organ-confined; treatment of the disease has a higher chance of being curative. Comparisons have been made [1, 2]with the effective strategy [3, 4J of breast cancer screening. Similar parallels however, can be drawn between prostate and lung cancer screening in which a shift towards earlier (potentially curable) stage did not reduce ti,e disease specific mortality in screenees [5]. To conduct a randomized screening study with prost.lte cancer mortality as the major endpoint is one possible solution to the present controversy. For this purpose the European Randomized Study of Screening for Prostate Cancer (ERSPC) has been initiated [6]. The studies presented in this thesis are conducted within the Rotterdam section of the ERSPC to investigate the feasibility of screening and early detection of prostate cancer in the general population. Intermediate endpoints of the study are: 1. Assessment of the efficiency of the screening tests. Serum Prostate Specific Antigen (PSA) Digital Rectal Examination (DRE) and Transrectal Ultrasonography (TRUS). 2. To evaluate the morbidity related to the screening procedure. 3. To study tumor extent at the time of diagnosis

Every day, between 7 a.m. and 7 p.m., over 125,000 cyclists ride to their work, school, university, public transport, shops or home through the city centre. The municipality wants to make cycling even more attractive for these and other cyclists. Consequently, the bicycle is given precedence in the mobility policy of the municipality of Utrecht. We want to be the most bike-friendly city in the world. We want to keep our growing city livable, accessible and economically strong, and we are convinced that the bicycle can and should play a major role in this.     Prospects of computer models for the prediction of osteoporotic bone fracture risk

    Bone fractures are major problems for osteoporosis patients. To avoid such fractures, more information is needed about the factors that determine the bone fracture risk. In this chapter, it is discussed how recently developed finite element computer models that can represent the trabecular architecture in full detail can provide such information. It is concluded that a computer modeling approach to this problem is feasible, required and promising. It is expected that, eventually, such models can be used as a basis for an accurate diagnosis of the bone fracture risk

    Mechanical stability of trabecular bone morphology as a measure for osteoporosis

    A new technique for assessing the mechanical stability of trabecular bone was introduced. This technique uses a full three dimensional reconstruction of a trabecular bone specimen and a finite element model to calculate the local stress distribution within the trabeculae. A little bone was artificially removed in the model at highly loaded locations and the changed stress distributions were determined. The changes in these distributions are indicative for the mechanical stability (change in fracture risk) of the trabecular architecture with respect to small changes in mass. The authors propose that this method can be used to measure the mechanical efficacy of a trabecular architecture in terms of fracture risk, thereby defining osteoporosis in a quantitative wa
