Objective To determine the possible additive effect of midazolam, a GABAA agonist, on the end-tidal concentration of isoflurane that prevents movement (MACNM) in response to noxious stimulation. Study design Randomized crossover experimental study. Animals Six, healthy, adult intact male, mixed-breed dogs. Methods After baseline isoflurane MACNM (MACNM-B) determination, midazolam was administered as a low (LDS), medium (MDS) or high (HDS) dose series of midazolam. Each series consisted of two dose levels, low and high. The LDS was a loading dose (Ld) of 0.2 mg kg-1 and CRI (2.5 μg kg-1 minute-1) (LDL), followed by an Ld (0.4 mg kg-1) and CRI (5 μg kg-1 minute-1) (LDH). The MDS was an Ld (0.8 mg kg-1) and CRI (10 μg kg-1 minute-1) (MDL) followed by an Ld (1.6 mg kg-1) and CRI (20 μg kg-1 minute-1) (MDH). The HDS was an Ld (3.2 mg kg-1) and CRI (40 μg kg-1 minute-1) (HDL) followed by an Ld (6.4 mg kg-1) and CRI (80 μg kg-1 minute-1) (HDH). MACNM was re-determined after each dose in each series (MACNM-T). Results The median MACNM-B was 1.42. MACNM-B did not differ among groups (p \u3e 0.05). Percentage reduction in MACNM was significantly less in the LDS (11 ± 5%) compared with MDS (30 ± 5%) and HDS (32 ± 5%). There was a weak correlation between the plasma midazolam concentration and percentage MACNM reduction (r = 0.36). Conclusion and clinical relevance Midazolam doses in the range of 10-80 μg kg-1 minute-1 significantly reduced the isoflurane MACNM. However, doses greater than 10 μg kg-1 minute-1 did not further decrease MACNM indicating a ceiling effect

Prosthetic alignment is a subjective concept which lacks reliability. The outcome responsiveness to prosthetic alignment quality could help to improve subjective and instrument assisted prosthetic alignment. This study was aimed to review variables used to assess clinically acceptable alignment in the literature. The search was done in some databases including: Google Scholar, PubMed, EBSCO, EMBASE, ISI Web of Knowledge and Scopus. The first selection criterion was based on abstracts and titles to address the research questions of interest. The American Academy of Orthotics and Prosthetics checklists were used for paper risk of bias assessment. A total of 25 studies were included in this study. Twenty-four studies revealed the critics of standing position or walking to locate clinically acceptable alignment, only one study measured outcomes in both situations. A total of 253 adults with transtibial amputations and mean age of 48.71 years participated in included studies. The confidence level of included studies was low to moderate, and before-after trial was the most common study design (n = 19). The joint angle, load line location with respect to joints and center of pressure-related parameters were reported as sensitive outcomes to prosthetic alignment quality in standing posture. The amount of forces at various parts of gait cycle and time of events were sensitive to prosthetic alignment quality during walking. Standing balance and posture and temporal parameters of walking could help to locate clinically acceptable alignment.N/

Probiotic's protection in ileal mucosa of mice after lipopolysaccaride Escherichia coliBackground: Gastrointestinal infection is one of the major causes of morbidity in children. Diarrhea is the common manifestation. Diarrheal prevention needs a balance mucosal immune system. Probiotic used in prevention of gastrointestinal infection needs to be considered. The aim of the study is to prove probiotic protection in ileal mucosa after lipopolysaccaride (LPS) Escherichia coli O55:B5 inoculation anatomically and immunologically.Method: Experimental study with male Balb/c, age 10-12 weeks, body weight 30-40g and randomized into treatment group and placebo group. Treatment group received mix probiotic for 14 days and on day 15 were inoculated by LPS Escherichia coli O55:B5, and day 16-21 were given mix probiotic again. Placebo group received LPS Escherichia coli O55:B5 on day 15. Mice necropsy were conducted at day 22. Immunohistochemistry examination used to look for amount expression cell for IL-2, IFN-γ, TNF-α, IL-4, IL-5,IL-6, IL-10 dan TGF-β in ileal mucosa. Scanning electron microscope was used to see ileal mucosal structure. Statistic analysis used in this study was multivariate analysis. Results: Sixteen Balb/c mices were randomized into 2 groups, each group consist 8 mices. There was significant difference on amount of cells expression IL-5 (p=0.022), IL-6 (p=0.05), and also on amount of cells expression TGF-β (p=0.036). On probiotic- LPS group, there was no Th1 domination but on the other hand, Treg became dominant. Th1 and Th2 response were still balance. Structural damages occurred in LPS group and did not occur in probiotic-LPS group. Conclusion: Probiotic protection in ileal Balb/c mice mucosa after LPS Escherichia coli O55:B5 inoculation is through anatomy and immunology changes
