Simultaneous multiwavelength observations of V404 Cygni during its 2015 June outburst decay strengthen the case for an extremely energetic jet-base
We present results of multiband optical photometry of the black hole X-ray
binary system V404 Cygni obtained using Wheaton College Observatory's 0.3m
telescope, along with strictly simultaneous INTEGRAL and Swift observations
during 2015 June 25.15--26.33 UT, and 2015 June 27.10--27.34 UT. These
observations were made during the 2015 June outburst of the source when it was
going through an epoch of violent activity in all wavelengths ranging from
radio to -rays. The multiwavelength variability timescale favors a
compact emission region, most likely originating in a jet outflow, for both
observing epochs presented in this work. The simultaneous INTEGRAL/Imager on
Board the Integral Satellite (IBIS) 20--40 keV light curve obtained during the
June 27 observing run correlates very strongly with the optical light curve,
with no detectable delay between the optical bands as well as between the
optical and hard X-rays. The average slope of the dereddened spectral energy
distribution was roughly flat between the - and -bands during the June
27 run, even though the optical and X-ray flux varied by 25 during
the run, ruling out an irradiation origin for the optical and suggesting that
the optically thick to optically thin jet synchrotron break during the
observations was at a frequency larger than that of -band, which is quite
extreme for X-ray binaries. These observations suggest that the optical
emission originated very close to the base of the jet. A strong H
emission line, probably originating in a quasi-spherical nebula around the
source, also contributes significantly in the -band. Our data, in
conjunction with contemporaneous data at other wavelengths presented by other
groups, strongly suggest that the jet-base was extremely compact and energetic

Utility of a routine ultrasound for detection of ectopic pregnancies among women requesting abortion:A retrospective review
INTRODUCTION: Routine ultrasound may be used in abortion services to determine gestational age and confirm an intrauterine pregnancy. However, ultrasound adds complexity to care and results may be inconclusive, delaying abortion. We sought to determine the rate of ectopic pregnancy and the utility of routine ultrasound in its detection, in a community abortion service. METHODS: Retrospective case record review of women requesting abortion over a 5-year period (2015–2019) with an outcome of ectopic pregnancy or pregnancy of unknown location (PUL) at a service (Edinburgh, UK) conducting routine ultrasound on all women. Records were searched for symptoms at presentation, development of symptoms during clinical care, significant risk factors and routine ultrasound findings. RESULTS: Only 29/11 381 women (0.25%, 95% CI 0.18%, 0.33%) had an ectopic pregnancy or PUL (tubal=18, caesarean scar=1, heterotopic=1, PUL=9). Eleven (38%) cases had either symptoms at presentation (n=8) and/or significant risk factors for ectopic pregnancy (n=4). A further 12 women developed symptoms during their clinical care. Of the remaining six, three were PUL treated with methotrexate and three were ectopic (salpingectomy=2, methotrexate=1). In three cases, the baseline ultrasound indicated a probable early intrauterine pregnancy. CONCLUSIONS: Ectopic pregnancies are uncommon among women presenting for abortion. The value of routine ultrasound in excluding ectopic pregnancy in symptom-free women without significant risk factors is questionable as it may aid detection of some cases but may provide false reassurance that a pregnancy is intrauterine
An Investigation of Single- and Dual-Rotation Propellers at Positive and Negative Thrust, and in Combination with an NACA 1-series D-Type Cowling at Mach Numbers up to 0.84
An investigation has been made to determine the aerodynamic characteristics of the NACA 4-(5)(05)-041 four-blade, single-relation propeller and the NACA 4-(5)(05)-037 six- and eight-blade, dual-rotation propellers in combination with various spinners and NACA d-type spinner-cowling combinations at Mach numbers up to 0.84. Propeller force characteristics, local velocity distributions in the propeller planes, inlet pressure recoveries, and static-pressure distributions on the cowling surfaces were measured for a wide range of blade angles, advance ratios, and inlet-velocity ratios. Included are data showing: (a) the effect of extended cylindrical spinners on the characteristics of the single-rotation propeller, (b) the effect of variation of the difference in blade angle setting between the front and rear components of the dual-rotation propellers, (c) the negative- and static-thrust characteristics of the propellers with 1 series spinners, and (d) the effects of ideal- and platform-type propeller-spinner junctures on the pressure-recovery characteristics of the single-rotation propeller-spinner-cowling combination

Testimonials Do Not Convert Patients from Brand to Generic Medication
Objectives: To assess whether the addition of a peer testimonial to an informational mailing increases conversion rates from brand-name prescription medications to lower-cost therapeutic equivalents, and whether the testimonial’s efficacy increases when information is added about an affiliation the quoted individual shares with the recipient.
Research Design and Methods: 5,498 union members were randomly assigned to receive one of three different informational letters: one without a testimonial (No Testimonial Group), one with a testimonial from a person whose shared union affiliation with the recipient was not disclosed (Unaffiliated Testimonial Group), and one with a testimonial from a person whose shared union affiliation with the recipient was disclosed (Affiliated Testimonial Group).
Results: The conversion rate for the No Testimonial Group was 12.2%, which is higher than the Unaffiliated Testimonial Group rate of 11.3% and the Affiliated Testimonial Group rate of 11.7%. The differences between the groups are not statistically significant.
Conclusions: Short peer testimonials do not increase the impact of a mailed communication on conversion rates to lower-cost, therapeutically equivalent medications, even when the testimonial is presented as coming from a more socially proximate peer.Economic
FUSE Observations of a Full Orbit of Hercules X-1: Signatures of Disk, Star, and Wind
We observed an entire 1.7 day orbit of the X-ray binary Hercules X-1 with the
Far Ultraviolet Spectroscopic Explorer (FUSE). Changes in the O VI 1032,1037
line profiles through eclipse ingress and egress indicate a Keplerian accretion
disk spinning prograde with the orbit. These observations may show the first
double-peaked accretion disk line profile to be seen in the Hercules X-1
system. Doppler tomograms of the emission lines show a bright spot offset from
the Roche lobe of the companion star HZ Her, but no obvious signs of the
accretion disk. Simulations show that the bright spot is too far offset from
the Roche lobe to result from uneven X-ray heating of its surface. The absence
of disk signatures in the tomogram can be reproduced in simulations which
include absorption from a stellar wind. We attempt to diagnose the state of the
emitting gas from the C III 977, C III 1175, and N III 991 emission lines. The

The X-ray Reflectors in the Nucleus of the Seyfert Galaxy NGC 1068
(abridged) Based on observations of the Seyfert nucleus in NGC1068 with ASCA,
RXTE and BeppoSAX, we report the discovery of a flare (increase in flux by a
factor of ~1.6) in the 6.7 keV Fe K line component between observations
obtained 4 months apart, with no significant change in the other (6.21, 6.4,
and 6.97 keV) Fe K_alpha line components. During this time, the continuum flux
decreased by ~20%. The RXTE spectrum requires an Fe K absorption edge near 8.6
keV (Fe XXIII - XXV). The spectral data indicate that the 2-10 keV continuum
emission is dominated (~2/3 of the luminosity) by reflection from a previously
unidentified region of warm, ionized gas located <~ 0.2 pc from the AGN. The
remaining ~1/3 of the observed X-ray emission is reflected from optically
thick, neutral gas. The inferred properties of the warm reflector (WR) are:
size (diameter) ~ 10^{5.5} /cm3, ionization parameter
xi approx 10^{3.5} erg cm/s, and covering fraction 0.003 (L_0/10^{43.5}
erg/s)^{-1} < (Omega/4 pi) < 0.024 (L_0/10^{43.5})^{-1}, where L_0 is the
intrinsic 2-10 keV X-ray luminosity of the AGN. We suggest that the WR gas is
the source of the (variable) 6.7 keV Fe line emission, and the 6.97 keV Fe line
emission. The 6.7 keV line flare is assumed to be due to an increase in the
emissivity of the WR gas from a decrease (by 20-30%) in L_0. The properties of
the WR are most consistent with an intrinsically X-ray weak AGN with L_0 approx
10^{43.0} erg/s. The optical and UV emission that scatters from the WR into our
line of sight is required to suffer strong extinction, which can be reconciled
if the line-of-sight skims the outer surface of the torus. Thermal
bremsstrahlung radio emission from the WR may be detectable in VLBA radio maps

Vaccination Rates are Associated With Functional Proximity But Not Base Proximity of Vaccination Clinics
Background:
Routine annual influenza vaccinations are recommended for persons 6 months of age and older, but less than half of US adults get vaccinated. Many employers offer employees free influenza vaccinations at workplace clinics, but even then take-up is low.
Objective:
To determine whether employees are significantly more likely to get vaccinated if they have a higher probability of walking by the clinic for reasons other than vaccination.
Method:
We obtained data from an employer with a free workplace influenza vaccination clinic. Using each employee’s building entry/exit swipe card data, we test whether functional proximity—the likelihood that the employee walks by the clinic for reasons other than vaccination—predicts whether the employee gets vaccinated at the clinic. We also test whether base proximity—the inverse of walking distance from the employee’s desk to the clinic—predicts vaccination probability.
Participants:
A total of 1801 employees of a health benefits administrator that held a free workplace influenza vaccination clinic.
Results:
A 2 SD increase in functional proximity is associated with a 6.4 percentage point increase in the probability of vaccination (total vaccination rate at company=40%), even though the average employee’s desk is only 166 meters from the clinic. Base proximity does not predict vaccination probability.
Conclusions and Relevance:
Minor changes in the environment can have substantial effects on the probability of vaccination. If these results generalize, health systems should emphasize functional proximity over base proximity when locating preventive health services.Economic
Estimating the Porosity of the Interstellar Medium from Three-Dimensional Photoionization Modeling of H II Regions
We apply our three dimensional photoionization code to model Wisconsin
H-alpha Mapper observations of the H II region surrounding the O9.5V star Zeta
Oph. Our models investigate the porosity of the interstellar medium around zeta
Oph and the effects of 3D densities on the H-alpha surface brightness and
variation in the [N II]/H-alpha line ratio. The Zeta Oph H II region has a well
characterized ionizing source, so it is an excellent starting point for 3D
models of diffuse ionized gas. We investigate various hierarchically clumped
density structures, varying the overall smoothness within the clumping
algorithm. By simulating the observations, we can estimate the porosity of the
medium in the vicinity of Zeta Oph and find that within the context of our
hierarchically clumped models, around 50% to 80% of the volume is occupied by
clumps surrounded by a low density smooth medium. We also conclude that in
order for O stars to ionize the diffuse Warm Ionized Medium, the O star
environment must be more porous than that surrounding Zeta Oph, with clumps
occupying less than one half of the interstellar volume. Our clumpy models have
irregular boundaries, similar to observed H II regions. However, in observed H
II regions it is difficult to identify the precise location of the boundary
because of the foreground and/or background emission from the widespread Warm
Ionized Medium. This complicates the interpretation of the predicted rapid rise
of some emission line ratios near the edge of uniform density H II regions and
combined with the three dimensional clumpy nature of the interstellar medium
may explain the apparent lack of distinctive emission line ratios near H I -- H

Increased Birth Weight is Associated with Altered Gene Expression in Neonatal Foreskin
Elevated birth weight is linked to glucose intolerance and obesity health-related complications later in life. No studies have examined if infant birth weight is associated with gene expression markers of obesity and inflammation in a tissue that comes directly from the infant following birth. We evaluated the association between birth weight and gene expression on fetal programming of obesity. Foreskin samples were collected following circumcision, and gene expression analyzed comparing the 15% greatest birth weight infants (n = 7) v. the remainder of the cohort (n = 40). Multivariate linear regression models were fit to relate expression levels on differentially expressed genes to birth weight group with adjustment for variables selected from a list of maternal and infant characteristics. Glucose transporter type 4 (GLUT4), insulin receptor substrate 2 (IRS2), leptin receptor (LEPR), lipoprotein lipase (LPL), low-density lipoprotein receptor-related protein 1 (LRP1), matrix metalloproteinase 2 (MMP2), plasminogen activator inhibitor-1 (PAI-1) and transcription factor 7-like 2 (TCF7L2) were significantly upregulated and histone deacetylase 1 (HDAC1) and thioredoxin (TXN) downregulated in the larger birth weight neonates v. controls. Multivariate modeling revealed that the estimated adjusted birth weight group difference exceeded one standard deviation of the expression level for eight of the 10 genes. Between 25 and 50% of variation in expression level was explained by multivariate modeling for eight of the 10 genes. Gene expression related to glycemic control, appetite/energy balance, obesity and inflammation were altered in tissue from babies with elevated birth weight, and these genes may provide important information regarding fetal programming in macrosomic babies
Near-Infrared Synchrotron Emission from Cas A
High energy observations of Cas A suggested the presence of synchrotron
radiation, implying acceleration of cosmic rays by young supernova remnants. We
detect synchrotron emission from Cas A in the near-infrared using Two Micron
All Sky Survey (2MASS) and Palomar 200 inch PFIRCAM observations. The remnant
is detected in J, H, and Ks bands, with Ks band brightest and J faint. In the J
and H bands, bright [Fe II] lines (1.24um and 1.64um) are detected
spectroscopically. The Palomar observations include Ks continuum, narrow-band
1.64um (centered on [Fe II]) and 2.12um (centered on H2(1-0)) images. While the
narrow-band 1.64um image shows filamentary and knotty structures, similar to
the optical image, the Ks image shows a relatively smooth, diffuse shell,
remarkably similar to the radio image. The broad-band near-infrared fluxes of
Cas A are generally consistent with, but a few tens of percent higher than, an
extrapolation of the radio fluxes. The hardening to higher frequencies is
possibly due to nonlinear shock acceleration and/or spectral index variation
across the remnant. We show evidence of spectral index variation. The presence
of near-infrared synchrotron radiation requires the roll-off frequency to be
higher than 1.5e14 Hz, implying that electrons are accelerated to energies of
at least 0.2 TeV. The morphological similarity in diffuse emission between the
radio and Ks band images implies that synchrotron losses are not dominant. Our
observations show unambiguous evidence that the near-infrared Ks band emission
of Cas A is from synchrotron emission by accelerated cosmic-ray electrons.Comment: accepted by Ap
