    Assessment of Hematological and Inflammatory Parameters in Patients Receiving Isotretinoin Therapy for Acne Vulgaris

    Objectives:Isotretinoin (ISO) is a synthetic analog of vitamin A used at the doses of 0.5–2 mg/kg for four to five months in the treatment of moderate to severe acne vulgaris. In the present study, both hematological and inflammatory parameters were retrospectively assessed in patients receiving ISO therapy for acne vulgaris.Method: This study included 352 patients who were admitted to the Dermatology Clinic and received oral ISO for the treatment of acne vulgaris were included in the study. Each patient’s hematological and inflammatory parameters (hemoglobin, hematocrit, thrombocyte, mean platelet volume, leukocyte, erythrocyte, neutrophil, lymphocyte, monocyte, eosinophil, basophil, neutrophil/basophil, thrombocyte/lymphocyte, RDW, PCT and PDW) were evaluated before the treatment and retrospectively evaluated four months after the onset of treatment. Results: After treatment, a significant decrease was detected in the levels of leukocyte, neutrophil and neutrophil to lymphocyte ratio (NLR) (an indicator of inflammation), and a significant increase was detected in the levels of thrombocyte and procalcitonin (PCT).Conclusion: In our study, isotretinoin inhibited the anti-inflammatory effect of isotretinoin treatment of acne in patients with NLR decline after treatment with inflammatory markers. Patients with markedly elevated platelet counts or low leukocyte counts should be closely monitored

    Organizational humor as making our work more meaningful: mediation by crafting job resources

    In the present study, we examined the impact of humor’s positive functions on the perception of a job’s meaningfulness. We argued that liberating and stress-relieving humor act as job resources enhancing job crafting to increase social and structural resources to experience meaningfulness. We hypothesized that crafting the job to increase structural and social resources would mediate the link between organizational humor functions (i.e., liberating and stress-relieving) and meaningfulness. We conducted a cross-sectional study among 200 Turkish employees from different occupations. Our results revealed that increasing structural resources mediated the relationship between liberating humor and meaningfulness, while this mediation was partially for stress-relieving humor. The mediating role of increasing social resources was partial and conditional for both types of organizational humor functions.     Right atrial appendage aneurysm is a very rare condition which can be asymptomatic or can cause arrhythmia or life-threatening thromboembolism. We report a case of newborn with right atrial appendage aneurysm who was diagnosed with fetal echocardiography. Anticoagulant therapy was applied to prevent thromboembolism and he is still going on follow-up without any complaint

    The Effect of Non-invasive Mechanic Ventilation Treatment on Lactate Level in Sleep-Related Breathing Disorders

    Objective Lactate, as an indicator of hypoxia, may be useful in sleep-related breathing disorders to identify higher risk groups and monitor the effectiveness of positive airway pressure (PAP) treatment. We aimed to investigate the change in arterial lactate level in blood after PAP treatment in patients with obstructive sleep apnea (OSA) and sleep-related hypoventilation and hypoxemic syndromes and its applicability for monitoring this change regarding the effectiveness of the treatment. Materials and Methods PAP titration was applied to 64 OSA patients diagnosed with polysomnography. Measurements included pulmonary function testing, overnight PAP titration, arterial blood gases, and analysis of arterial lactate before and after titration. Overnight lactate level change was calculated. Results Of the 64 patients in the study, 49 were in the OSA group and 15 were in the Obesity Hypoventilation syndrome plus OSA group. No statistically significant difference was detected among the patients in terms of lactate levels before and after PAP treatment [1.97 (±0.7) - 1.98 (±0.6)] (p>0.05). In the OSA patient group, we detected an inverse relationship between Lactate and pre-treatment non-rapid eye movement 1-2% (r=-0.328, p=0.021), T 90% (r=-0.356, p=0.012), and total apnea hypopnea index (AHI) (r=-0.424, p=0.002). Lactate is found to be positively correlated with pre-treatment measured oxygen saturation (r=0.396, p=0.005), and with minimum oxygen saturation (r=0.361, p=0.011). Conclusion Our study showed that there is a relationship between AHI, average and minimum oxygen saturation and T 90% and lactate difference, which are severity indicators for OSA. These results suggest that PAP treatment may prevent possible lactate increase
