    Latest Quaternary palaeoceanographic change in the eastern North Atlantic based upon a dinoflagellate cyst event ecostratigraphy

    AbstractThe analyses of dinoflagellate cyst records, from the latest Quaternary sediments recovered from DSDP Core 610A taken on the Feni Ridge in the southern Rockall Trough, and part of core MD01-2461 on the continental margin of the Porcupine Seabight in the eastern North Atlantic Ocean, has provided evidence for significant oceanographic change encompassing the Last Glacial Maximum (LGM) and part of the Holocene. This together with other published records has led to a regional evaluation of oceanographic change in the eastern North Atlantic over the past 68 ka, based upon a distinctive dinoflagellate event ecostratigraphy. These changes reflect changes in the surface waters of the North Atlantic Current (NAC), and perhaps the deeper thermohaline Atlantic Meridional Overturning Circulation (AMOC), driving fundamental regime changes within the phytoplanktonic communities. Three distinctive dinoflagellate cyst associations based upon both factor and cluster analyses have been recognised. Associations characterised by Bitectatodinium tepikiense (between 61.1 ± 6.2 to 13.4 ± 1.1 ka BP), Nematosphaeropsis labyrinthus (between 10.5 ± 0.3 and 11.45 ± 0.8 ka. BP), and the cyst of Protoceratium reticulatum (between 8.5 ± 0.9 and 5.2 ± 1.3 ka. BP) indicate major change within the eastern North Atlantic oceanography. The transitions between these changes occur over a relatively short time span (c.1.5 ka), given our sampling resolution, and have the potential to be incorporated into an event stratigraphy through the latest Quaternary as recommended by the INTIMATE (INTegrating Ice core, MArine and TErrestrial records) group.     Improving Community Health through Hospital-Public Health Collaboration: Insights and Lessons Learned from Successful Partnerships

    From the introduction: Health care expenditures in the United States currently consume over 17 percent of the nation’s gross domestic product, a much larger share than other developed nations. Yet, despite this large investment, studies by Commonwealth Fund, the Institute of Medicine, and other organizations show the USA lags behind other developed nations on multiple metrics of population health such as infant mortality and life expectancy. Moreover, there is extensive evidence of disparities in access, cost, and quality of health care services. Thus, we are confronted by a striking paradox: the USA spends a large and growing proportion of our resources on health care, but the outcomes in terms of access to services, the quality of those services, and the health of our population do not match other countries whose spending per capita is lower. It is evident that many factors contribute to this paradox — demographic, environmental, genetic, lifestyle, and socioeconomic — and all warrant societal attention. Improving access to outpatient and inpatient medical services and the quality of those services, while important, cannot resolve the paradox. Across the country, there is growing awareness that restraining the increase in health expenditures and improving the health status of families, communities, and society at large will require a broader approach that addresses the full array of factors affecting health status. Greater attention and resources must be devoted to promoting a safer environment, healthy lifestyles, prevention of illnesses and injuries, and early detection and treatment of health problems, as well as dealing with the underlying determinants of health.     Ascorbic Acid Supplementation: Effects on the Growth and Packed Cell Volume of Broiler Chickens

    This study was conducted to observe the effect of natural and artificial ascorbic acid supplementation on the growth and packed cell volume using broiler chicks. The chicks were divided into groups A, B, C and D. They were fed with the same quantity of feed but different type of ascorbic acid in a litre of water were also administered to groups B to D. Group A which served as the control group received no ascorbic acid and groups B was given 150 g of natural and 150 g of artificial ascorbic acid per litre of water, C received 300 g of natural and 300 g artificial ascorbic acid while D was given 300 g of artificial ascorbic acid per litre of water. The result of the supplement effects on the birds showed that birds feed with natural ascorbic acid recorded the highest weight gain 1.22 kg followed by those feed with mixture of natural and artificial ascorbic acid 1.09kg. Birds that were feed artificial ascorbic acid recorded a weight gain of 0.89 kg while the control group produced the least weight gain of 0.79 kg. The mixture of natural and artificial ascorbic acid proved to be most effective on PCV 21.23, followed by that of artificial ascorbic acid 18.1. Whereas control group recorded a PCV level of 12.84 natural ascorbic acid recorded the least PCV level of 9.93. The result of the experiment indicated that natural ascorbic acid elevated body weight while mixture of natural and artificial ascorbic acid boosted PCV in broiler when given in the right proportion.     Early Clinical and Subclinical Visual Evoked Potential and Humphrey's Visual Field Defects in Cryptococcal Meningitis.

    Cryptococcal induced visual loss is a devastating complication in survivors of cryptococcal meningitis (CM). Early detection is paramount in prevention and treatment. Subclinical optic nerve dysfunction in CM has not hitherto been investigated by electrophysiological means. We undertook a prospective study on 90 HIV sero-positive patients with culture confirmed CM. Seventy-four patients underwent visual evoked potential (VEP) testing and 47 patients underwent Humphrey's visual field (HVF) testing. Decreased best corrected visual acuity (BCVA) was detected in 46.5% of patients. VEP was abnormal in 51/74 (68.9%) right eyes and 50/74 (67.6%) left eyes. VEP P100 latency was the main abnormality with mean latency values of 118.9 (±16.5) ms and 119.8 (±15.7) ms for the right and left eyes respectively, mildly prolonged when compared to our laboratory references of 104 (±10) ms (p<0.001). Subclinical VEP abnormality was detected in 56.5% of normal eyes and constituted mostly latency abnormality. VEP amplitude was also significantly reduced in this cohort but minimally so in the visually unimpaired. HVF was abnormal in 36/47 (76.6%) right eyes and 32/45 (71.1%) left eyes. The predominant field defect was peripheral constriction with an enlarged blind spot suggesting the greater impact by raised intracranial pressure over that of optic neuritis. Whether this was due to papilloedema or a compartment syndrome is open to further investigation. Subclinical HVF abnormalities were minimal and therefore a poor screening test for early optic nerve dysfunction. However, early optic nerve dysfunction can be detected by testing of VEP P100 latency, which may precede the onset of visual loss in CM

    An Evaluation of the Integrated Disease Surveillance and Response (IDSR) in Enugu State, Nigeria

    Background: The Integrated Disease Surveillance and Response (IDSR), adopted in 1988 at the 48th World Health Organization Regional Committee for Africa meeting in Harare, Zimbabwe, is a regional strategy to strengthen the weak national surveillance systems in the region. In Nigeria, and prior 1988, there was no coordinated system of disease reporting and surveillance system in place until after a major yellow fever outbreak in 1986/87 that claimed many lives. The IDSR in Enugu state, as supported by the WHO, has 17 disease surveillance and notification officers across the 17 LGAs and 89 reporting sites covering the 3000 health facilities. Method: This study is a prospective cross-sectional study using WHO and CDC assessment protocols modified to reflect the local settings. The study samples were selected based on the IDSR performance indicators for AFP, measles core indicators, status of epidemic-prone diseases, and adequacy of feedback and feedforward of surveillance activities. Using desk review of the EPI/IDSR surveillance reports between 2012 and 2017, and also questionnaires between April and July 2017 to eligible participants, data were obtained, cleaned, and analysed using the SPSS version 24. Results: The Overall average score of IDSR performance indicator for Enugu state from the pooled data was 39% against the expected of &gt;80%. These findings are in disagreement with the globally recommended standard IDSR practice and response. Conclusion: The outcome of this study highlights that Enugu state surveillance and IDSR practice are short of the standard practice as prescribed by WHO and CDC assessment protocols. Main reasons for this include poor disease reporting, poor documentation of conducted activities, and lack of adequate feedback system. Similarly, there is non-involvement of community and private health facilities that made up more than 95% of surveillance networks in the state. However, global polio eradication initiative and change management approaches identified remain a huge opportunity for the improvement of the system.     A Baseline for the Multivariate Comparison of Resting-State Networks

    As the size of functional and structural MRI datasets expands, it becomes increasingly important to establish a baseline from which diagnostic relevance may be determined, a processing strategy that efficiently prepares data for analysis, and a statistical approach that identifies important effects in a manner that is both robust and reproducible. In this paper, we introduce a multivariate analytic approach that optimizes sensitivity and reduces unnecessary testing. We demonstrate the utility of this mega-analytic approach by identifying the effects of age and gender on the resting-state networks (RSNs) of 603 healthy adolescents and adults (mean age: 23.4 years, range: 12–71 years). Data were collected on the same scanner, preprocessed using an automated analysis pipeline based in SPM, and studied using group independent component analysis. RSNs were identified and evaluated in terms of three primary outcome measures: time course spectral power, spatial map intensity, and functional network connectivity. Results revealed robust effects of age on all three outcome measures, largely indicating decreases in network coherence and connectivity with increasing age. Gender effects were of smaller magnitude but suggested stronger intra-network connectivity in females and more inter-network connectivity in males, particularly with regard to sensorimotor networks.
