Abdominal wall plasty with synthetic mesh in conditions of septicity
Scopul lucrării. Coexistența defectelor mari și gigantice infectate ale peretelui abdominal și plastia lor cu plasă sintetică sunt într-o
discordanță discutabilă.
Materiale și metode. Sunt prezentați 8 pacienți: 5 bărbați și 3 femei, de vârsta medie, spitalizați în SCM “Sf. Arh.Mihail” între anii
2008-2018.
Rezultate. La spitalizare în 6 cazuri se determină eventrație postoperatorie de dimensiuni mari complicată cu flegmon și ocluzie
intestinală acută la examenul radiologic. În 2 cazuri de eventrație postoperatorie după by-pass gastric pentru obezitate morbidă, se
atestă hiperemia tegumentară, ireductibilitatea, lipsa nivelelor hidroaerice, semne peritoneale. Se pun indicații pentru tratamentul
chirurgical de urgență. Operație - excizia în bloc a flegmonului sacului de eventrație cu rezecția intestinului subțire și anastomoza L-L.
Plastia cu plasă sintetică în 6 cazuri. În 2 cazuri intraoperator se determină ulcer perforat duodenal, a stomacului exclus, peritonită
difuză. Suturarea ulcerului, aplicarea gastrostomei, plastia defectului abdominal cu plasă sintetică procedeu de substituție, drenarea
abdominală și a spațiului subtegumentar. Evoluție simplă.
Concluzii. În defectele parietale abdominale mari se poate aplica plasă de substituție în condiții septice. Drenaj adecvat, antibioterapie,
asigură evoluția favorabilă.Aim of study. The coexistence of large and giant infected defects of the abdominal wall and their plasty with synthetic mesh are in debatable discordance.
Materials and methods. 8 patients are presented: 5 men and 3 women, of average age, hospitalized in SCM “Sf. Arh.Mihail” between
2008-2018.
Results. During hospitalization in 6 cases, a large post-surgery eventration complicated with phlegmon and acute intestinal occlusion
was determined during the radiological examination. In 2 cases of post-surgery eventration after gastric bypass for morbid obesity,
in cutaneous hyperemia, irreducibility, lack of hydro-aerial levels, peritoneal signs are attested. Indications are given for emergency
surgical treatment. Surgery – en bloc excision of the phlegmon of the eventration sac with resection of the small intestine and L-L
anastomosis. Plastic surgery with synthetic mesh in 6 cases. In 2 intraoperative cases, perforated duodenal ulcer, stomach excluded,
diffuse peritonitis is determined. Ulcer suturing, gastrostomy application, abdominal defect plasty with synthetic mesh substitution
procedure, abdominal and subcutaneous drainage. Simple evolution.
Conclusions. Substitution mesh can be applied in large abdominal parietal defects in septic conditions. Adequate drainage, antibiotic
therapy, ensures favorable evolution
Primary operations in the tumoral mechanical jaundice

al III-lea Congres al Societății de Endoscopie, Chirurgie miniminvazivă și Ultrasonografie ”V.M.Guțu” din Republica MoldovaIntroducere: Discuțiile referitor la drenajul preventiv sau la operațiile primare în icterul mecanic tumoral persistă.
Materiale și metode: Studiul din perioada 2008 – 2018 cuprinde 53 cazuri de icter tumoral. Raportul barbați-femei e de 1 la 1, vârstă
înnaintată, peste 60 de ani - 75%. Tabloul clinico-paraclinic sugestiv de icter mecanic, e confirmat imagistic EUS în 100% cazuri,
CT/RMN – 37,7%, așa că 62,3% de cazuri au reclamat indicații operatorii de sindrom. Caracteristica biologică a pacientului agravat
arată astfel: bilirubina totală 259,4 +121,9; AlAT 188,4; AsAT 157,7 mmoli/l; Mt hepatice 10 caz., ascită canceroasă 6 caz., tromboză/
implicare v. portă 6 caz.; comorbiditățile fiind prezente în toate cazurile.
Rezultate: Intraoperator s-au depistat: tumoră Klatskin - 5 caz, cancer: de 1/3 medie de CBP 4 caz., de 1/3 distală -3 caz., de
cefalopancreas 24 caz., ampular 5 caz., de veziculă biliară 5 caz., gastric 3 caz., Mt de melanom în pancreas și duoden 1 caz.,Mt în
hil hepatic + ficat 6 cazuri.Operații radicale au solicitat 18 cazuri (33,9%) – 9 rezecții de cale biliară și 9 DPC; celelalte au fost operații
derivative biliodigestive interne; rezectii gastrice paliative (3caz.,) și colecistectomii ( 42caz.) . Biopsie s-a prelevat laparoscopic din Mt
hepatice în 6 cazuri. Mortalitate postoperatorie 3 cazuri – 5,6%.
Concluzii: Valorile bilirubinemiei (icterului) nu influențează asupra rezultatelor postoperatorii. Operatiile primare în icterul mecanic
tumoral pot fi efectuate, amploarea lor fiind impuse de biologia tumorii și a pacientului. Icterul mecanic prezintă indicații pentru
tratament chirurgical în urgență temporizată dacă demersul diagnostic nu progresează (3-5 zile).Introduction: Discussions on preventive drainage or primary surgery in tumoral jaundice persist.
Materials and method: The study from 2008 to 2018 includes 53 cases of tumor jaundice. The male / female ratio is 1 to 1, old age,
over 60 years - 75%. The suggestive clinico-paraclinical picture of mechanical jaundice is confirmed by EUS in 100%, CT / MRI -
37.7%, so 62.3% of cases have claimed indications of the syndrome and accordingly for surgery. The biological characteristic of the
aggravated patient is as follows: total bilirubin 259,4 +121,9; A1AT 188,4; AsAT 157,7 mmol / L; Hepatic Mt - 10 cases, Malignant
ascites - 6 cases, thrombosis / portal vein involvement 6 cases; comorbidities being present in all cases.
Results: Intraoperative was found: Klatskin tumor - 5 cases, cancer: 1/3 medium of CBP- 4 cases, 1/3 distal -3 cases, head of the
pancreas- 24 cases, ampullary - 5 case, gallbladder- 5 cases, gastric- 3 cases, melanoma Mt in the pancreas and duodenum-1 case,
hepatic hilum Mt + liver- 6 cases. Radical operations required 18 cases (33.9%) - 9 gallbladder resections and 9 DPC; the other were
internal billiodigestive derivative operations; palliative gastric resections (3 cases), and cholecystectomies (42 cases). The biopsy was
laparoscopically collected from hepatic Mt in 6 cases. Postoperative mortality 3 cases - 5.6%.
Conclusions: Values of bilirubin (jaundice) do not affect postoperative outcomes.Primary operations in tumoral jaundice can be
performed, their magnitude being imposed by the biology of the tumor and the patient

al III-lea Congres al Societății de Endoscopie, Chirurgie miniminvazivă și Ultrasonografie ”V.M.Guțu” din Republica MoldovaIntroducere: Obezitatea morbidă este o maladie cronică plurifactorială, cu o creștere spectaculoasă a incidenţei în ultimii ani la
nivel mondial. Tratamentul conservativ al obezităţii morbide este ineficient, impunând abordarea metodelor alternative de tratament.
Bypassul gastric (BG) este intervenţia ce induce corijarea semnificativă a comorbidităţilor, cu o pierdere ponderală adecvată menţinută
la distanţă.
Obiectivul: Evaluarea eficacităţii, riscurilor precoce și la distanță a bypass-ului gastric în tratamentul obezităţii morbide.
Material și metode: Studiul include 356 pacienți cu obezitate morbidă și perturbări metabolice, care au fost supuși BG pe perioada
2009-2019. Vârsta pacienților a oscilat în limitele 17-68 ani, cu media de 40,3±5,6 ani. Masa corporală a constituit 123,2±20,5 (80-
265)kg, IMC (indicelemaseicorporale) preoperator fiind de 44±7,3 (28,7-75,6) kg/m². Toți pacienții au fost supuși examinării clinice ș
iparaclinice de o echipa multidisciplinară de specialiști, în scopul diminuării riscurilor perioperatorii.
Rezultate: Sunt relatate rezultatele la distanță de 10 ani postoperator. La 12 luni postoperator IMC a constituit 29,5±5,6 kg/m²,
%EWL(% excess weight loss)-64,1±16,3, %EBMIL (excess BMI loss)-82,4±29,1. În 13 cazuri (4,9%) perioada postoperatorie precoce
s-a complicat cu hemoragie digestive acută. Anemia feriprivă și litiaza veziculară au fost cele mai frecvente complicații postoperatorii tardive inregistrate în 74 cazuri (20,8%) și respectiv 27 cazuri (7,6%). Decese postoperatorii nu s-au înregistrat.
Concluzii: Bypass-ulgastric este un procedeu sigur și efectiv ntratamentul obezității morbide cu menținerea rezultatelor la distanță.
Este necesară monitorizarea acestor pacienți, în scopu lprevenirii complicațiilor postoperatorii tardive.Introduction: Morbid obesity is a multifactorial chronic disease, with a dramatic increase in incidence in the last years. The conservative
methods are ineffective, enforcing the approach of alternative methods of treatment. Gastric bypass induces a significant correction of
comorbidities, with a sustained adequate weight loss.
Objective: Evaluation of efficiency, early and late risks of gastric bypass in the treatment of morbid obesity.
Materials and methods: The study includes 356 patients with morbid obesity and metabolic disturbances that underwent gastric
bypass during 2009-2019. Age of the patients varies from 17 to 68 years, with an average of 40,3±5,6 years. Body mass consists of
123,2±20,5(80-265) kg, preoperative BMI (body mass index) being 44±7,3(28,7-75,6) kg/m². All the patients underwent clinical and
paraclinical examination by a multidisciplinary team of specialists with the aim of decreasing the perioperative risks.
Results: The study includes postoperative results for a period of 10 years after the intervention. At 12 months postoperative, BMI
was 29,5±5,6 kg/m², % EWL (excess weight loss) – 64,1±16,3, %EBMIL (excess BMI loss) – 82,4±29,1. In 13 cases (4,9%), early
postoperative period included acute gastrointestinal bleeding. Iron deficiency anemia and cholelithiasis were the most frequent late
postoperative complications registered in 74 cases (20,8%) and respectively 27 cases (7,6%). Postoperative deaths have not been
registered.
Conclusions: Gastric bypass is an efficient and safe procedure in the treatment of morbid obesity with sustained maintenance of the
results. The monitoring of patients is necessary for avoiding late postoperative complications
Managementul complicațiilor postoperatorii precoce după Roux-en-Y gastric bypass
Background. Obesity is in a spectaculous increase over the last decades. Bariatric surgery is the only
option to significant and sustained weight loss and to improvement of comorbidities and mortality
decrease. Efficiency of bariatric surgery is well-studied, but its complications have been
underestimated. Objective of the study. Elaboration of both diagnostic and treatment algorithm for
early postoperative complications after Roux-en-Y gastric bypass. Material and Methods. The clinical
trial is based on the evaluation of early postoperative results of 433 patients who supported Roux-en-Y
gastric bypass during 2009-2020. Weight varied from 80 to 245 (121,8 ± 26,3) kg, age was of 18-68
(40,5±10,4) years, sex ratio was 3,5/1, with the prevalence of women. Results. Early postoperative
complications were recorded in 44 (10,1%) cases. The complications mostly consisted of bleeding of
the transit excluded stomach – 17 (38,6%) cases, wound suppuration – 11 (25%) cases and acute gastric
dilatation – 6 (13,6%) cases. All patients (n=6) with acute gastric dilatations were reoperated via parietal
gastrostomy. Conclusion. Postoperative complications after Roux-en-Y gastric bypass are dominated
by bleeding of the transit excluded stomach. Gastric acute dilatations is a medical emergency, the
solution being parietal gastrostomy.
Introducere. Obezitatea este în creștere spectaculoasă în ultimele decenii. Chirurgia bariatrică este
singura opțiune care duce la pierderea marcată și susținută a greutății corporale. Eficiența chirurgiei
bariatrice a fost bine studiată, dar complicațiile acesteia au fost subestimate. Scopul lucrării. Elaborarea
algoritmului de diagnostic și tratament în complicațiile postoperatorii precoce survenite după Roux-en-
Y gastric bypass. Material și Metode. Studiul clinic este bazat pe evaluarea rezultatelor postoperatorii
precoce a 433 pacienți care au fost supuși Roux-en-Y gastric bypass pe perioada 2009-2020. Masa
corporală a variat între 80 și 245 (121,8 ± 26,3) kg, vârsta medie - 40,5±10,4 ani, raport femei/bărbați
de 3,5/1. Rezultate. Complicații postoperatorii precoce s-au înregistrat în 44 (10,1%) cazuri.
Complicațiile au fost dominate de hemoragii din stomacul exclus din tranzit – 17 (38,6%) cazuri,
supurația plăgii – 11 (25%) cazuri și dilatarea acută gastrică, instalată în 6 (13,6%) cazuri. Toți pacienții
(n=6) la care a survenit dilatare acută gastrică au fost reoperați prin montarea gastrostomei
parietale. Concluzii. Complicațiile postoperatorii precoce după Roux-en-Y gastric sunt dominate de
hemoragiile din stomacul exclus din tranzit. Dilatarea acută gastrică este o urgență chirurgicală, soluția
fiind aplicarea gastrostomei parietale
Contemporary treatment methods in acute cholangitis

Republic of Moldova, The 8th International Medical Congress for Students and Young Doctors, September 24-26, 2020Introduction. Acute cholangitis is a medical emergency and is a potential life-threatening
condition, is an infectious process of the entire biliary tree or only of the intrahepatic system
developed almost always under the conditions of partial or complete obstruction of the main
biliary tract.. If this condition is not treated with antibiotic therapy and the earlier decline of
bile pressure, the risk of aggravation of the situation and mortality increases. The actuality of
the problem is determined by the increase of the number of diseases of the organs in the
hepatopancreatoduodenal area, which are accompanied by a lot of complications.
Aim of the study. Research of surgical tactical opportunities in acute cholangitis. Materials and methods. Scientific articles were searched in PubMed, Hinari and Cambridge
University press databases, using the descriptors "acute cholangitis", "biliary stenting", "nasobiliary drainage". The research was not delimited to a specific time period and was
supplemented with bibliographic data from statistical sites, from Ministry of Health, Labor and
Social Protection of Moldova.
Results. Reaching positive results in the treatment of cholangitis, is possible by performing the
diagnostic and therapeutic maneuvers in stages. A necessary condition is the strict observance
of the sequences and the time in each stage by the maximum use of the endosurgical and miniinvasive interventions. Early surgery in acute cholangitis is accompanied by an increased
frequency of complications. Advances in therapeutic endoscopy, such as balloon drainage
guided by balloon enteroscopy or endoscopic ultrasound-guided biliary drainage, have added
new extent for the endoscopic management of acute cholangitis, which avoids the need for
more invasive procedures. Bile drainage can be achieved by different methods and procedures:
endoscopic, percutaneous and open transhepatic. Endoscopic drainage is associated with a
shorter duration of hospitalization and a low morbidity rate.
Conclusions. The treatment tactic in acute cholangitis remains a current problem in
hepatobiliary surgery. So far, there are many questions for pathogenesis, diagnosis, treatment
and prevention, which need to be solved. Acute cholangitis requires an in-depth
multidisciplinary study, which allows the adoption of a correct management for each clinical
case, and the application of the stepped treatment tactic will allow us to obtain positive results
in this pathology
The role of fertiloscopy in the management of female inferility
Catedra Obstetrică-Ginecologie,USMF „Nicolae Testemiţanu”The diagnosis of the cause of infertility is not very easy especially when it is
necessary to establish the status of the Fallopian tubes. Hysterosalpingography(HSG) is
very often practised for this purpose, but the rate of false negative and even false positive
is very high as shown by laparoscopy. In contrast, laparoscopy is considered as the gold
standard to explore tubo peritoneal infertility. Unfortunately, laparoscopy presents some risks
which can be very serious. Fertiloscopy is a new surgical minimally invasive procedure and a
combination at the same time of a transvaginal hydropelviscopy, a dye-test, a
salpingoscopy, a microsalpingoscopy and lastly an hystéroscopy performed under local
anaesthesia.
Diagnosticarea cauzelor infertilităţii este dificilă, în special când este necesar de a
determina starea trompelor Fallopian. Histerosalpingografia este fregvent practicată în aceste
situaţii, dar fregvenţa rezultatelor fals negative şi chiar fals pozitive este foarte înaltă comparativ
cu laparoscopia. Laparoscopia rămâne a fi standartul de aur în explorarea infertilităţii peritonealtubare.
Din nefericire, laparoscopia prezintă riscuri ce pot fi foarte serioase. Fertiloscopia este o
metodă nouă, mini invazivă şi o cuplare în acelaş timp a unei hidropelvioscopii transvaginale,
dye-test, salpingoscopii, microsalpingoscopii şi la sfârşit a histeroscopiei efectuate sub anestezie
locală
Fifteen-month-olds accept arbitrary shapes as symbols of familiar kind tokens
Across three experiments, we show that 15-month-old infants understand that arbitrary objects can be used as symbols. Experiment 1 shows that infants map geometric shapes (e.g., a triangle) onto familiar discourse referents (e.g., a duck) based on labeling (e.g., “Look, a duck!”). Experiment 2 shows that infants do not generalize these mappings to a new speaker. This rules out the alternative hypothesis that infants interpret the labeling events literally. Experiment 3 shows that infants are sensitive to the conceptual identity of the discourse referent. After being told that one shape represents an agent (e.g., a duck) and another shape represents a patient (e.g., a cup), infants attend differentially when the agent symbol moves towards the patient symbol than the opposite. This rules out the alternative hypothesis that infants interpret the labeling events as referential pacts. The findings jointly indicate that symbolic relations are easily activated and available early in human development
Chirurgia metabolică în tratamentul diabetului zaharat de tip 2
Introducere: Diabetul zaharat este o problemă de sănătate
publică majoră, cu consecințe semnificative atât pentru
pacienți, cât și pentru sistemul de sănătate. Se estimează că,
până în 2040, numărul persoanelor cu diabet va atinge 642
de milioane. Chirurgia metabolică se impune ca metodă
alternativă de tratament. Scopul lucării: Analiza
comparativă a eficacității Roux-en-Y gastric bypass
(RYGB) în tratamentul diabetului zaharat tip 2 la pacienții
cu obezitate morbidă, obezitate redusă și non-obezi.
Material și metode: Studiu analitic retrospectiv a 36 de
pacienți din 951 cu obezitate morbidă (IMC≥35 kg/m²) și
pacienți cu IMC35 kg/m² în prezența diabetului zaharat
tip 2, care au fost supuși tratamentului chirurgical prin
RYGB în CSF „Galaxia” în perioada anilor 2009-2024.
Durata diabetului a fost de 6,19±3,67 ani, IMC fiind de
41,17±9,8 kg/m2. Din lotul total (n=36), 3 (8%) pacienți
administrau insulină (10-110 UI/zi), cu o medie de 50±52,9
UI/zi. În 29 cazuri s-a prelevat segment de jejun pentru
examen histologic, în 23 cazuri fiind suplimentat de
examenul histochimic. Rezultate: La 12 luni postoperator
glicemia bazală a fost în limitele 6,27±1,5 mmol/l, iar
HbA1C – 5,8±0,76%. Remisia în cazul IMC≥35 kg/m²
(n=20) a survenit în 15 (75%) cazuri, comparativ cu
IMC<35 kg/m² (n=16) – 8 (50%) cazuri. Examenul
histochimic a evidențiat un infiltrat inflamator cronic
constituit din limfocite T, preponderent CD4 și CD8.
Concluzii: Datele preliminare au evidențiat eficacitatea
înaltă a RYGB în tratamentul diabetului zaharat tip 2, rata
de remisie fiind superioară la pacienții cu obezitate morbidă
comparativ cu obezitate redusă și non-obezi
Particular form of simultaneous pancreas and liver toxic damage
USMF “N. Testemițanu”, Clinica Chirurgie “Sf. Arh. Mihail”, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011Studiul vizează 10 cazuri, observate în SCM ”Sf. Arh. Mihail” pe perioada ultimilor 10 ani. Toți sunt spitalizați în secția ATI cu manifestări clinicoparaclinice
de insuficiență hepatică acută; antecedente de băutori și cu afecțiuni hepatice cronice (hepatita cronică 2 cazuri, ciroza hepatică -8). Date
pentru pancreatita acută căutate intenționat absente, amilaza sanguine cifre joase/ absentă, date EUS (10 caz. ), cât și CT (2 caz.) fără modificări de
structură. Evolutțe gravă, icter, uremie, hipoprotrombinemie, trasamilazemie, ulterior MODS avansat. Deces în mediu la ziua a 8 . Necroptic – asociație
de ciroză hepatică cu pancreatită acută necrotică în toate cazurile. Expansie parapancreatică absentă. În acest context de afectare panceatica directă
au fost studiate și 55 acte medico-legale de deces subit (anii 2004-2006) cu diagnostic – pancreonecroză hemoragică. Rezultatul ulterior a alcoolemiei
arată a fi prezent în 39 cazuri ( 71%), până la 3% - 23 cazuri, peste 3% -15 cazuri. Concluzie: 1. Posibilitatea afectării toxice concomitente a ficatului și
pancreasului este o realitate clinică. 2. Clinico - paraclinic predomină insuficiența hepatică acută, manifestările pancreatice fiind minime. 3. PA are o evoluție necrotică de la început, fără răspândire peripancreatică și deasemenea fără complicații chirurgicale.The study included 10 cases observed in the SCM ” St. Arch. later shown to be present in 39 cases (71%), up to 3% - 23 cases over 3%-15 cases. Conclusion: 1. the possibility of toxic damage to liver and pancreas
simultaneous is a clinical reality. 2. Clinical - Acute liver failure prevails paraclinical, pancreatic manifestations were minimal. 3. PA has a necrotic
evolution from the beginning, and also without peripancreatic spread visa without surgical complications
