The impact of South Korea’s new drug-pricing policy on market competition among off-patent drugs
A new pricing policy was introduced in Korea in April 2012 with the aim of strengthening competition among off-patent drugs by eliminating price gaps between originators and generics. Examine the effect of newly implemented pricing policy. Retrospectively examining the effects through extracting from the National Health Insurance claims data a 30-month panel dataset (January 2011 - June 2013) containing consumption data in four major therapeutic classes (antihypertensives, lipid-lowering drugs, antiulcerants and antidepressants). Proxies for market competition were examined before and after the policy. The new pricing policy didn’t enhance competition among off-patent drugs. In fact, price dispersion significantly decreased as opposed to the expected change. Originator-to-generic utilization increased to 6.12 times (p=0.000) after the new policy. The new pricing policy made no impact on competition among off-patent drugs. Competition in the off-patent market cannot be enhanced unless both supply and demand-side measures are coordinated
Global action for training in malaria elimination
The Rethinking Malaria Leadership Forum, held at Harvard
Business School in February 2017 with collaboration of the
Barcelona Institute for Global Health and the Swiss Tropical and
Public Health Institute, identified this training gap as a high
priority for both analysis and action. The gap in human resource
training for malaria elimination needs to be addressed in order
to assure continued progress. This paper identifies major gaps
in skills and human resources, suggests institutions that can
assist in filling the training gaps, and proposes global actions
to implement expanded training for malaria elimination in
endemic countries

Barriers to Treatment Access for Chagas Disease in Mexico
Background: According to World Health Organization (WHO) prevalence estimates, 1.1 million people in Mexico are infected with Trypanosoma cruzi, the etiologic agent of Chagas disease (CD). However, limited information is available about access to antitrypanosomal treatment. This study assesses the extent of access in Mexico, analyzes the barriers to access, and suggests strategies to overcome them. Methods and Findings: Semi-structured in-depth interviews were conducted with 18 key informants and policymakers at the national level in Mexico. Data on CD cases, relevant policy documents and interview data were analyzed using the Flagship Framework for Pharmaceutical Policy Reform policy interventions: regulation, financing, payment, organization, and persuasion. Data showed that 3,013 cases were registered nationally from 2007–2011, representing 0.41% of total expected cases based on Mexico's national prevalence estimate. In four of five years, new registered cases were below national targets by 11–36%. Of 1,329 cases registered nationally in 2010–2011, 834 received treatment, 120 were pending treatment as of January 2012, and the treatment status of 375 was unknown. The analysis revealed that the national program mainly coordinated donation of nifurtimox and that important obstacles to access include the exclusion of antitrypanosomal medicines from the national formulary (regulation), historical exclusion of CD from the social insurance package (organization), absence of national clinical guidelines (organization), and limited provider awareness (persuasion). Conclusions: Efforts to treat CD in Mexico indicate an increased commitment to addressing this disease. Access to treatment could be advanced by improving the importation process for antitrypanosomal medicines and adding them to the national formulary, increasing education for healthcare providers, and strengthening clinical guidelines. These recommendations have important implications for other countries in the region with similar problems in access to treatment for CD
SNP and Mutation Data on the Web – Hidden Treasures for Uncovering
SNP data has grown exponentially over the last two years, SNP database evolution has
matched this growth, as initial development of several independent SNP databases has
given way to one central SNP database, dbSNP. Other SNP databases have instead
evolved to complement this central database by providing gene specific focus and an
increased level of curation and analysis on subsets of data, derived from the central data
set. By contrast, human mutation data, which has been collected over many years, is still
stored in disparate sources, although moves are afoot to move to a similar central
database. These developments are timely, human mutation and polymorphism data both
hold complementary keys to a better understanding of how genes function and malfunction
in disease. The impending availability of a complete human genome presents us with an
ideal framework to integrate both these forms of data, as our understanding of the
mechanisms of disease increase, the full genomic context of variation may become
increasingly significant

Introduction of pentavalent vaccine in Indonesia: a policy analysis
The introduction of pentavalent vaccine containing Haemophilus influenzae type b antigen in Indonesia’s National Immunization Program occurred nearly three decades after the vaccine was first available in the United States and 16 years after Indonesia added hepatitis B vaccine into the program. In this study, we analyzed the process that led to the decision to introduce pentavalent vaccine in Indonesia. Using process tracing and case comparison, we used qualitative data gathered through interviews with key informants and data extracted from written sources to identify four distinct but interrelated processes that were involved in the decision making: (a) pentavalent vaccine use policy process, (b) financing process, (c) domestic vaccine development process and (d) political process. We hypothesized that each process is associated with four necessary conditions that are jointly sufficient for the successful introduction of pentavalent vaccine in Indonesia, namely (a) an evidence-based vaccine use recommendation, (b) sufficient domestic financing capacity, (c) sufficient domestic vaccine manufacturing capacity and (d) political support for introduction. This analysis of four processes that led to the decision to introduce a new vaccine in Indonesia may help policy makers and other stakeholders understand and manage activities that can accelerate vaccine introduction in the future

Leveraging public health nurses for disaster risk communication in Fukushima City: a qualitative analysis of nurses' written records of parenting counseling and peer discussions
Background: Local public health nurses (PHNs) have been recognized as the main health service providers in communities in Japan. The Fukushima nuclear disaster in 2011 has, however, created a major challenge for them in responding to mothers’ concerns. This was in part due to difficulties in assessing, understanding and communicating health risks on low-dose radiation exposure. In order to guide the development of risk communication plans, this study sought to investigate mothers’ primary concerns and possible solutions perceived by a core healthcare profession like the PHNs. Methods: A total of 150 records from parenting counseling sessions conducted between PHNs and mothers who have attended mandatory 18-month health checkups for their children at the Fukushima City Health and Welfare Center in 2010, 2011 (year of disaster) and 2012 were examined. Discussion notes of three peer discussions among PHNs organized in response to the nuclear disaster in 2012 and 2013 were also analyzed. All transcribed data were first subjected to text mining to list the words according to their frequencies and inter-relationships. The Steps Coding and Theorization method was then undertaken as a framework for qualitative analysis. Results: PHNs noted mothers to have considerable needs for information on radiation risks as they impact on decisions related to relocations, concerns for child safety, and experiences with interpersonal conflicts within the family owing to differing risk perceptions. PHNs identified themselves as the information channels in the community, recommended the building of their risk communication capacities to support residents in making well-informed decisions, and advocated for self-measurement of radiation levels to increase residents’ sense of control. PHNs also suggested a more standardized form of information dissemination and an expansion of community-based counseling services. Conclusions: Inadequate risk communication on radiation in the Fukushima nuclear incident has resulted in multiple repercussions for mothers in the community. Empowerment of local residents to assume more active roles in the understanding of their environment, increasing PHNs’ capacity in communication, and an expansion of health services such as counseling will together better address risk communication challenges in post-disaster recovery efforts
Surface Transitions for Confined Associating Mixtures
Thin films of binary mixtures that interact through isotropic forces and
directionally specific "hydrogen bonding" are considered through Monte Carlo
simulations. We show, in good agreement with experiment, that the single phase
of these mixtures can be stabilized or destabilized on confinement. These
results resolve a long standing controversy, since previous theories suggest

74 MHz Discrete HII Absorption Regions Towards The Inner Galaxy
At low radio frequencies (< 100 MHz), classical HII regions may become
optically thick (optical depth > 1) and can be observed as discrete absorption
regions against the Galactic nonthermal background emission created by Galactic
cosmic ray electrons spiraling around magnetic fields. In this work we present
74 MHz observations in the region 26>l>-15, -5<b<5, and report the detection of
92 absorption features associated with known HII regions, and derive the
brightness temperature of the Galactic cosmic ray electron synchrotron emission
emanating from the column behind these regions. For the 42 HII regions with
known distances, the average emissivity of the column behind the HII region is
derived. 74 MHz emissivity values range between 0.3 and 1.0 Kelvin per parsec
for a model assuming uniform distribution of emissivity. Methods for utilizing
this type of data to model the 3-dimensional distribution of cosmic ray
emissivity and the possibility of using this method to break the HII region
kinematic distance degeneracy are discussed.Comment: Accepted, The Astronomical Journa
Addressing conflicts of interest in Public Private Partnerships
<p>Abstract</p> <p>Background</p> <p>Many articles have been written on conflicts of interests (COIs) in fields such as medicine, business, politics, public service and education. With the growing abundance of Public Private Partnerships (PPPs), often involving complex relationships among the partners, it is important to understand how COIs can be mitigated and managed in PPPs.</p> <p>Discussion</p> <p>We wanted to study PPPs, particularly in the areas of global health and agriculture, but discovered no single source of information available to identify and compare various approaches for avoiding and managing COIs in PPPs. This is a significant gap, especially for those wishing to study, compare and strengthen existing COI policies related to PPPs. In order to bridge this gap, we reviewed how PPPs currently address COIs and highlight what might be considered good practice in developing COI policies. We reviewed the online COI policies of 10 PPPs in global health and agriculture, and interviewed two global health PPP chief executives.</p> <p>Summary</p> <p>Based on our review of policies and interviews, we conclude that there exists a range of good practices including attention to accountability and governance, acknowledgement and disclosure, abstention and withdrawal, reporting and transparency, and independent monitoring. There appears to be a need for PPPs to interact closely and learn from each other on these parameters and to also place more emphasis on independent external monitoring of COIs as a means of strengthening their major social objectives on which their activities are largely predicated. We also recommend the establishment of a web based database, which would serve as a forum to discuss COI issues and how they can be resolved.</p
