    Multivariate statistical analysis of the polyphenols content for the discrimination of honey produced in Sicily (Southern Italy)

    The polyphenols content of 105 honey samples produced by black honeybees (Apis Mellifera ssp. Sicula) and common honeybees (Apis mellifera ssp. Ligustica) from Western Sicily (Southern Italy) was examined using TurboFlow™ liquid chromatography Orbitrap™ high-resolution mass spectrometry. The results showed very high kaempferol and quercetin contents, with average values higher than what was reported in literature (3967.9±2184.16 and 2206.1±1666.4μg kg⁠−1 for kaempferol and quercetin, respectively). The honey samples produced by Apis Mellifera ssp. Sicula subspecies showed polyphenols content up to two times higher than Apis mellifera ssp. Ligustica. The Principal Component Analysis (PCA) model calculated on the polyphenols content showed a satisfactory separation of the honey samples in terms of honeybee subspecies and production area. The model proposed in this work shows the possibility to safeguard the authenticity of the honey produced in the various geographic areas of Sicily

    Radio-Frequency Plasma Polymerized Biodegradable Carrier for in vivo Release of Cis-Platinum

    A low pressure plasma process based on plasma deposition has been used to develop a drug delivery strategy. In this study, a drug delivery system based on different layers of plasma co-polymerized Poly ε-caprolactone-Polyethylene glycol (PCL-PEG) co-polymers was deposited on biocompatible substrates. Cis-platinum (118 μgm/cm2) was used as an anti-cancer drug and incorporated for local delivery of the chemotherapeutic agent. The co-polymer layers and their interaction with cancer cells were analyzed by scanning electron microscopy. Our study showed that the plasma-PCL-PEG coated cellophane membranes, in which the drug, was included did not modify the flexibility and appearance of the membranes. This system was actively investigated as an alternative method of controlling localized delivery of drug in vivo. The loading of the anti-cancer drug was investigated by UV-VIS spectroscopy and its release from plasma deposited implants against BALB/c mice liver tissues were analyzed through histological examination and apoptosis by TUNEL assay. The histological examination of liver tissues revealed that when the plasma-modified membranes encapsulated the cis-platinum, the Glisson\u27s capsule and liver parenchyma were damaged. In all cases, inflammatory tissues and fibrosis cells were observed in contact zones between the implant and the liver parenchyma. In conclusion, low pressure plasma deposited uniform nano-layers of the co-polymers can be used for controlled release of the drug in vivo

    Clinical presentation and treatment of gastrointestinal stromal tumors.

    AIMS AND BACKGROUND: Gastrointestinal stromal tumors (GISTs), although rare, are the most common mesenchymal neoplasms affecting the gastrointestinal tract. We present our experience in the treatment of localized and metastatic disease and a review of literature. PATIENTS AND METHODS: Nine patients were observed from April 2002 to July 2004. Eight tumors were in the gastric area and 1 was in the small bowel. In 5 cases, complete surgical removal was performed, and none of these patients underwent adjuvant therapy. The remaining 4 cases, with locally advanced or recurrent disease, were treated with imatinib. RESULTS: The patients with localized disease treated only by surgery did not relapse. In the patients with locally advanced or metastatic disease treated by imatinib, we observed 3 partial responses, and one case was not assessable because he had no measurable disease. In 2 of 3 responders, it was possible to perform a new radical surgery. CONCLUSIONS: Our series is too small to draw any conclusion. According to our review of the literature, surgery remains the standard treatment for non-metastatic GISTs. Imatinib mesylate represents a major breakthrough in the treatment of advanced GISTs and is the first effective systemic therapy for the disease

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     The synchronous presence of liver metastases (LM) and peritoneal carcinomatosis (PC) from colorectal cancer (CRC) is associated with poor outcome and is traditionally considered a contraindication to any surgical approach. However, few series reported a prolonged survival after surgical management, reaching 3 years in selected patients thus suggesting that a curative surgical management may be possible. To date, no standard management pathway has been established, especially if a major liver and peritoneal surgery has to be performed. We postulated that liver regeneration after liver resection could promote PC growth. We constructed an immunocompetent animal model of limited PC. The objective of our study was to analyze the effects of major LR and liver regeneration after hepatectomy on peritoneal carcinomatosis growth and the associated angiogenesis process. Furthermore, we have analyzed a prospective international cohort of patients undergoing synchronous liver resection and cytoreductive surgery with HIPEC. The aim of this study was to describe the outcomes, to identify variables potentially related to poor outcome, in order to establish future guideline for the management of these patients, to optimize the selection of candidates for surgical treatment and determine the best surgical strategy

À ce jour, en cas de chirurgie majeure hépatique et péritonéale associée, aucune stratégie thérapeutique n'a été établie, Nous avons postulé que la régénération hépatique après une résection hépatique pourrait favoriser la croissance de la CP. Nous avons construit un modèle animal immunnocompetent de CP limitée. L'objectif de l’étude était d'analyser les effets de l’hépatectomie majeure et de la régénération hépatique dans notre modèle murin de PC et le processus d'angiogenèse associé. En outre, nous avons analysée une cohorte prospective multicentrique de patients ayant eu une résection hépatique et une chirurgie cytoréductive avec HIPEC synchrone. L'objectif de cette étude était d'évaluer les outcomes et identifier les facteurs pronostiques afin d'optimiser la sélection des candidats au traitement chirurgical et de déterminer la stratégie chirurgicale optimale
