    An IoT system for remote health monitoring in elderly adults through a wearable device and mobile application

    With the increase in global life expectancy and the advance of technology, the creation of age-friendly environments is a priority in the design of new products for elderly people healthcare. This paper presents a proposal for a real-time health monitoring system of older adults living in geriatric residences. This system was developed to help caregivers to have a better control in monitoring the health of their patients and have closer communication with their patients’ family members. To validate the feasibility and effectiveness of this proposal, a prototype was built, using a biometric bracelet connected to a mobile application, which allows real-time visualization of all the information generated by the sensors (heart rate, body temperature, and blood oxygenation) in the bracelet. Using these data, caregivers can make decisions about the health status of their patients. The evaluation found that the users perceived the system to be easy to learn and use, providing initial evidence that our proposal could improve the quality of the adult’s healthcare.PRODEP | Ref.     Efecto de la aplicación intrauterina de ozono sobre la migración de neutrófilos y la endometritis subclínica en ganado lechero

    The objective was to determine whether an ozonized saline solution (O3SS) increases endometrial polymorphonuclear neutrophils (PMNN) (Exp 1) and to challenge the preventive effect of O3SS on subclinical endometritis (SCE) (Exp 2). In Exp 1, 38 primiparous Holstein cows were used. Cows with (WHM) and no history of postpartum metritis (NHM) were included; they were then divided into control (CTRL, saline solution) or O3SS (6.7 ± 0.3 ppm) subgroups. At 55 d postpartum, 50 ml of CTRL or O3SS was applied intrauterinely and at 48 h, the quantity of PMNN was recorded by endometrial cytology. In Exp 2, 26 primiparous NHM Holstein cows were used. The cows were randomly distributed in CTRL or O3SS. Two doses of 50 ml were administered with an interval of 7 d (first application 11.3 ± 0.4 d postpartum). At d 30 postpartum, SCE (≥6 % PMNN) was diagnosed. WHM cows had higher numbers of endometrial PMNN compared to NHM cows (13.9 ± 6.2 vs 1.0 ± 0.46, P0.05) and the NHM-CTRL and NHM-O3SS groups (0.1 ± 0.1 and 1.8 ± 4.8, P>0.05) were not different. A statistical trend (P=0.09) of lower percentage of SCE was observed in the CTRL group compared to O3SS (15.4 and 46.2 %, respectively). In conclusion, transcervical O3SS does not increase the number of endometrial PMNN and preventive treatment with O3SS applied to NHM cows did not decrease SCE. The results suggest a possible anti-inflammatory effect of the ionized saline solution treatment.El objetivo fue determinar si una solución salina ozonizada (SSO3) incrementa los neutrófilos polimorfonucleares (NPMN) endometriales (Exp 1) y desafiar el efecto preventivo de SSO3 sobre la endometritis subclínica (ESC) (Exp 2). En el Exp 1 se utilizaron 38 vacas Holstein primíparas. Se incluyeron vacas con (CAM) y sin antecedentes de metritis postparto (SAM); posteriormente se repartieron en subgrupos control (CTRL, solución salina) o SSO3 (6.7 ± 0.3 ppm). A los 55 días postparto se aplicó 50 ml de CTRL o SSO3 intrauterinamente y a las 48 h se registró la cantidad de NPMN por citología endometrial. En el Exp 2 se utilizaron 26 vacas Holstein primíparas SAM. Las vacas se repartieron aleatoriamente en CTRL o SSO3. Se administraron dos dosis de 50 ml con intervalo de 7 días (primera aplicación 11.3 ± 0.4 días postparto). Al día 30 postparto se diagnosticó ESC (≥6% NPMN). Las vacas CAM tuvieron mayor número de NPMN endometriales comparado con vacas SAM (13.9 ± 6.2 vs 1.0±0.46, P0.05) y SAM-CTRL y SAM-SSO3 (0.1 ± 0.1 y 1.8 ± 4.8, P>0.05) no fueron diferentes. Se observó una tendencia estadística (P=0.09) de menor porcentaje de ESC en grupo CTRL comparado con SSO3 (15.4 y 46.2 %, respectivamente). En conclusión, la SSO3 por vía transcervical no incrementa el número de NPMN endometriales y el tratamiento preventivo con SSO3 aplicado a vacas SAM no disminuyó la ESC.     Yield and nutraceutical quality of tomato fruits in organic substrates/Rendimiento y calidad nutracéutica de tomate en sustratos orgánicos

    The objective of this study was to determine the proportion of sand, solarized manure, vermicompost and soil capable of increasing tomato fruit yield and lycopene content. The treatments were: T1: 80 % Sand- 20 % Vermicompost (SV), T2: 80 % Sand- 20 % Solarized manure (SSM1), T3: 80 % Sand - 20 % Pelite ?Steiner solution (SPSS), T4: 80 % Sand- 5 % Soil - 15 % Vermicompost (SSoV), T5: 85 % Sand - 15 % Solarized manure (SSM2) and T6: 80 % Sand - 5 % Soil - 15 % Solarized manure (SSoSM). The organic substrates T5, T4, T1 and the control (T3) obtained larger fruits and yields. The lycopene content in tomato fruits grown in the organic substrates was 26 % higher than that obtained in the control treatment.     Direct endonasal approach with partial upper posterior septectomy. a “rescue flap” technique modification

    Objective Report a modification of the “rescue flap” technique using a direct endonasal approach with a partial superior septectomy for approaching pituitary tumors developed in our institution. Design Prospective study. Setting Hospital Universitario “Dr. José Eleuterio González,” Universidad Autónoma de Nuevo León. Main Outcome Measures Since April 2015, we have performed 19 cases employing a direct endonasal approach with partial superior septectomy. Results and a technical note are described below. Results Nineteen patients were included in this report. Six patients presented transoperatory cerebrospinal fluid (CSF) leak, so a nasoseptal rescue flap was harvested. No patients developed postoperative CSF leak in this group. Two patients were submitted to a second surgical procedure. Nasoseptal flap was harvested without complications. In both patients, the size of the flap was enough to cover the dural defect and avoid CSF leak. Conclusion Direct endonasal approach with a partial posterior septectomy allows enough exposition of the sphenoidal sinus while preserving the nasoseptal septum with the possibility of a successful rescue flap when needed

    Compromising between European and US allergen immunotherapy schools: Discussions from GUIMIT, the Mexican immunotherapy guidelines

    Background: Allergen immunotherapy (AIT) has a longstanding history and still remains the only disease-changing treatment for allergic rhinitis and asthma. Over the years 2 different schools have developed their strategies: the United States (US) and the European. Allergen extracts available in these regions are adapted to local practice. In other parts of the world, extracts from both regions and local ones are commercialized, as in Mexico. Here, local experts developed a national AIT guideline (GUIMIT 2019) searching for compromises between both schools. Methods: Using ADAPTE methodology for transculturizing guidelines and AGREE-II for evaluating guideline quality, GUIMIT selected 3 high-quality Main Reference Guidelines (MRGs): the European Academy of Allergy, Asthma and Immunology (EAACI) guideines, the S2k guideline of various German-speaking medical societies (2014), and the US Practice Parameters on Allergen Immunotherapy 2011. We formulated clinical questions and based responses on the fused evidence available in the MRGs, combined with local possibilities, patient's preference, and costs. We came across several issues on which the MRGs disagreed. These are presented here along with arguments of GUIMIT members to resolve them. GUIMIT (for a complete English version, see Supplementary data) concluded the following: Results: Related to the diagnosis of IgE-mediated respiratory allergy, apart from skin prick testing complementary tests (challenges, in vitro testing and molecular such as species-specific allergens) might be useful in selected cases to inform AIT composition. AIT is indicated in allergic rhinitis and suggested in allergic asthma (once controlled) and IgE-mediated atopic dermatitis. Concerning the correct subcutaneous AIT dose for compounding vials according to the US school: dosing tables and formula are given; up to 4 non-related allergens can be mixed, refraining from mixing high with low protease extracts. When using European extracts: the manufacturer's indications should be followed; in multi-allergic patients 2 simultaneous injections can be given (100% consensus); mixing is discouraged. In Mexico only allergoid tablets are available; based on doses used in all sublingual immunotherapy (SLIT) publications referenced in MRGs, GUIMIT suggests a probable effective dose related to subcutaneous immunotherapy (SCIT) might be: 50–200% of the monthly SCIT dose given daily, maximum mixing 4 allergens. Also, a table with practical suggestions on non-evidence-existing issues, developed with a simplified Delphi method, is added. Finally, dissemination and implementation of guidelines is briefly discussed, explaining how we used online tools for this in Mexico. Conclusions: Countries where European and American AIT extracts are available should adjust AIT according to which school is followed

    Viruses of Economic Impact on Tomato Crops in Mexico: From Diagnosis to Management—A Review

    Tomato is the most economically important vegetable crop worldwide and the second most important for Mexico. However, viral diseases are among the main limiting factors that affect the productivity of this crop, causing total losses in some cases. This review provides key information and findings on the symptoms, distribution, transmission, detection, and management of diseases caused by viruses of major importance in tomato crops in Mexico. Currently, about 25 viruses belonging to nine different families have been reported infecting tomato in Mexico, but not all of them cause economically significant diseases. Viruses of economic importance include tomato brown rugose fruit virus (ToBRFV), tomato spotted wilt virus (TSWV), tomato yellow leaf curl virus (TYLCV), pepino mosaic virus (PepMV), and tomato marchitez virus (ToMarV). The topics discussed here will provide updated information about the status of these plant viruses in Mexico as well as diverse management strategies that can be implemented according to the specific circumstances of each viral pathosystem. Additionally, a list of tomato-affecting viruses not present in Mexico that are continuous threats to the crop health is included

    Identification and Quantification of Lipopeptide Homologues Induced and Produced by <i>Bacillus amyloliquefaciens</i>

    Cyclic lipopeptides (LPs) are potentially promising in the agricultural, industrial and pharmaceutical sectors. LPs have a variable hydrophilic cyclic peptide part attached to a variable fatty acid chain. One limitation of these compounds is their low availability due to their limited production by bacteria. The objective of this study was to identify and quantify homologues of LPs biosynthesized by Bacillus amyloliquefaciens using ultra-performance liquid chromatography (UPLC–MS/MS) after inducing the synthesis of these secondary metabolites using different inducers, including chemical compounds and inactive cells of Colletotrichum sp. Four homologues were identified in the iturin family (bacillomycin D), and the iturin homologue with the highest synthesis was the molecular ion m/z 1031.54, with 173.1 µg mg−1 crude extract. In addition, seven homologues were detected in the fengycin family (four of fengycin A and three of fengycin B), and the homologue with the highest content was the molecular ion m/z 1463.69 (fengycin A), with 3288 ± 528.5 ng mg−1 crude extract. Finally, five homologues were identified in the surfactin family, where the highest concentration was observed for the molecular ion m/z 1036.68, with 61.5 ± 3.01 µg mg−1 crude extract. The highest concentration of LP homologues (iturin, fengycin and surfactin) synthesized by B. amyloliquefaciens was detected in the presence of inactive cells of Coletotrichum sp., suggesting that the inducing substance is associated with the inducer’s cell envelope and could be a single protein or a structure that includes protein components
