    Sexual and reproductive health needs assessment and interventions in a female psychiatric intensive care unit

    Aims and method To assess the sexual and reproductive health (SRH) needs of women admitted to a psychiatric intensive care unit (PICU), and acceptability of delivering specialist SRH assessments and interventions in this setting. Within a quality improvement framework, staff were trained, a clinical protocol developed and clinical interventions made accessible. Results Thirty per cent of women were identified as having unmet SRH needs and proceeded to a specialist appointment, representing a 2.5-fold increase in unmet need detection. Forty-two per cent of women were assessed, representing a 3.5-fold increase in uptake. Twenty-one per cent of women initiated SRH interventions, of which 14% had all their SRH needs met. Staff, patients and carers highlighted the acceptability and importance of SRH care, if interventions were appropriately timed and patients’ individual risk profiles were considered. Barriers to access included lack of routine enquiry, illness acuity and impact of the COVID-19 pandemic. Clinical implications SRH needs for PICU admissions are greater than previously realised. Providing a nurse-led SRH assessment is acceptable, feasible and beneficial for PICU patients

    How installers select and explain domestic heating controls

    Though central heating controls have the potential to reduce the energy consumed through domestic space heating, their installation does not guarantee savings. End users do not always understand their controls, or operate them in an energy-efficient way, but there is little appreciation of why this is. Drawing on an ethnographic study, this paper investigates how installers select and explain central heating controls. With reference to the concept of technology scripting, which suggests that the assumptions made about users during the design of devices can influence their eventual use, it shows how heating installers also draw on certain user scripts. Through these means the paper illuminates the significant role that heating installers play in influencing the control products fitted into homes, and how they might be used. Though their use of these scripts is understandable, it is not always conducive to ensuring that central heating systems are operated in the most energy-efficient way. It is suggested that industry and policy-makers might engage with how installers understand users and revise current guidelines to foster better communication between them

    Understanding the missing middlemen of domestic heating:Installers as a community of professional practice in the United Kingdom

    Despite indications that they could play an important part in shaping how people heat their homes, central heating installers have been largely overlooked in energy research. As a means of addressing this oversight, this paper draws on a British ethnographic study to explore the ways in which these 'missing middlemen' can be said to comprise a 'community of practice'. Two aspects of community membership are explored in detail: social learning processes and shared identities. This exercise shows how socially acquired understandings of their professional role and their relationship with homeowners can influence the selection and installation of heating products. The paper concludes with suggestions for how industry and policy makers might engage with this group. These suggestions focus on strategies aimed at reducing the energy used for home heating, and the installation of alternative heating technologies, both of which might benefit from an appreciation of the informal processes of community formation

    Objetivo: Evaluar el cumplimiento de la Guía de prevención de UPP en pacientes hospitalizados en el Servicio de Geriatría del Hospital Nacional Almanzor Aguinaga en los meses Agosto-Octubre de 2015. Material y Métodos: Estudio descriptivo transversal, cuya unidad de análisis fueron los procedimientos preventivos realizados a un paciente por turno para evitar las UPP realizados por el Personal de enfermería que labore y/o realice horas extras, incluyendo al cuidador presente en el turno evaluado que aceptó participar. Se utilizó la Lista de chequeo para el cumplimiento de Medidas preventivas de UPP de la Guía clínica de Es salud. Se realizó un muestreo aleatorio simple para obtener la cantidad de turnos a evaluar por cada mes; dando como resultado 15 turnos de evaluación distribuidos en los meses de Agosto-Octubre, para obtener un tamaño muestral de 272 pacientes. Resultados: El cumplimiento de la Guía de Prevención de UPP en el Servicio de Geriatría el Hospital Nacional Almanzor Aguinaga fue 68,06%(Optimo: 100%, Aceptable: 90%, Crítico:<90%); encontrándose un 68,98% de cumplimiento en el turno mañana, en la tarde 69,49% y la noche 65,60%.El cambio postural y el registro de cuidados aplicados se cumplieron en 3,31% y 0,37% respectivamente. Conclusiones: El cumplimiento de la Guía de prevención de UPP en el Servicio de Geriatría se encuentra en un nivel crítico, lo que indica la importancia de implementar y reforzar los procesos de promoción y prevención durante la hospitalización, incluyendo a todo el personal de salud en su ejecución

    If tourism is to become part of a more sustainable lifestyle, changes are needed to the patterns of behaviour adopted by the public. This paper presents the results of research conducted amongst members of the public in England on their understanding of sustainable tourism; their response to four desired tourism behaviour goals, and expectations about the role of government and the tourism industry in encouraging sustainable tourism. The research shows a lack of awareness of tourism’s impact relative to day-to-day behaviour, feelings of disempowerment and an unwillingness to make significant changes to current tourism behaviour
