    Helping abused women access a protection order: the role of religious, traditional and community leaders

    Abstract: Religious,2 traditional and community leaders (RTCL) have an important role to play in the social protection of women who disclose abuse. If abused women disclose abuse to RTCL, their actions on the one hand, could either prevent further abuse and femicide; as well as protect children exposed to domestic violence. Or on the other hand, if they do not respond appropriately at critical moments of disclosure, they allow abuse to continue by not assisting survivors. This article aims to assist RTCLs, and other potential systems of support, utilize the legislation to assist survivors of domestic violence. The landmark Domestic Violence Act (116 of 1998) is the result of extensive advocacy and lobbying by women’s groups to locate gender based violence firmly in the public domain. It provides one very important avenue that can be utilized by RTCLs to help abused women access their human rights, by employing the law. This article provides an outline of process and the details of the legal remedy available to survivors of abuse provided by the DVA. It is however important to remember, that whilst it is imperative for RTCLs to understand how to assist survivors utilizing the DVA to obtain protection this must be accompanied by a package of other services, since it is not effective as a standalone instrument

    Impact of Computer-Aided Warfarin Dosing in a Saudi Arabian Cardiac Centre

    Purpose: To compare the efficacy of computer-aided dosing using Coagclinic (a web-based software) with physician dosing in patients receiving warfarin for various cardiac indications.Methods: In order to calculate the effectiveness of physician managed anticoagulation dosing, we calculated the “percentage of time international normalized ratio, INR, was in the therapeutic range" (TTR) for a random sample of 70 patients in the center. For each patient, 4 INR values were taken at 4 consecutive visits, before and after the installation of Coagclinic. Data on the doses given by physicians were collected at each visit and c omp ar ed them with the pharmacy-based computer aided dosing system data.Results: After performing paired samples t-test of doses based on the physician group with those based on the computer system (mean -1.5654 ± 2.09 ), a statistically significant difference was found (p < 0.015). Furthermore, the patients kept at TTR by physicians amounted to 26 % compared to 70 % by the software.Conclusion: The difference between TTR managed by physicians and those managed using CoagClinic™ software is statistically significant.     In vitro activity of fosfomycin against uropathogen multi-drug resistant (MDR) Pseudomonas aeruginosa and Acinetobacter baumannii

    Urinary tract infections caused by multidrug resistant Gram negative bacilli constitute a major global healthcare problem. Fosfomycin is considered the best treatment option for such infections. Urine samples were collected and cultured in a tertiary care hospital (Urology). Identification of these uropathogens and their antibiotic sensitivity screening were performed according to CLSI guidelines. Urine samples (n=436) were selected in which Ps. aeruginosa and Acinetobacter baumannii werefound to be the significant pathogens and treated-exposed to fosfomycin. Sixty six (15%) were identified as Acinetobacter baumannii, Ps. aeruginosa n=370(85%). Forty four percent of all Ps. aeruginosa were found to be multidrug resistant while 48.5% of the Acinetobacter baumannii strains were found multidrug resistant. Polymyxin B was found to be the most effective drug (100%) against all uropathogens and fosfomycin was found effective against 73% of the multidrug resistant Acinetobacter baumannii isolates and 70% of the multidrug resistant Pseudomonas aeruginosa strains. Identification de ces uropathogènes et programmation de leur sensibilité aux  antibiotiques ont été réalisés selon les directives (CLSI). Les échantillons d’urine (n = 436) ont été choisis dans laquelle Ps. aeruginosa et Acinetobacter baumannii se sont trouves être l’agent pathogène important et traités – exposés a Fosfomycine. Soixante – six (15%) ont été identifies comme  Acinetobacter baumannii, Ps. aeruginosa = 370 (85%). Quarante – quatre pourcent de tous les Ps. aeruginosa se sont trouvés être multi résistants et 48,5% des souches Acinetobacter baumannii sesont trouves multi résistants. Polymyxine B a été trouvé d’être le médicament le plus efficace (100%) contre tous les uropathogènes et Fosfomycine a été trouvé efficace contre 73% des isolats de multi résistants Acinetobacter baumannii et 70% des souches de multi résistants Pseudomonas aeruginosa. On peut conclure que l’activité antimicrobienne (in vitro) de Fosfomycine est très efficace, particulièrement contre les uropathogenes MDR.Mots – clés : Fosfomycine, les bacilles de Multi résistants Gram négatifs, les infections des voies urinaires, Ps. aeruginosa, Acinetobacter baumannii.

    Results of the Mariner 6 and 7 Mars occultation experiments

    Final profiles of temperature, pressure, and electron density on Mars were obtained for the Mariner 6 and 7 entry and exit cases, and results are presented for both the lower atmosphere and ionosphere. The results of an analysis of the systematic and formal errors introduced at each stage of the data-reduction process are also included. At all four occulation points, the lapse rate of temperature was subdadiabatic up to altitudes in excess of 20 km. A pronounced temperature inversion was present above the surface at the Mariner 6 exit point. All four profiles exhibit a sharp, superadiabatic drop in temperature at high altitudes, with temperatures falling below the frost point of CO2. These results give a strong indication of frozen CO2 in the middle atmosphere of Mars

    Cost Analysis of Medications Used in Upper Respiratory Tract Infections and Prescribing Patterns in University Sans Malaysia, Penang, Malaysia

    Purpose: To conduct a cost analysis, a narrow cost-utility study, for upper respiratory tract infection medications in University Sans Malaysia’s clinics.Methods: Retrospective analysis was done for all medical claims of upper respiratory tract infections in the period 2008 - 2009. The study was done in the clinics under University Sains Malaysia. A total of 302 patients  suffering from upper respiratory tract infection were enrolled. Cost analysis of the pattern of antibiotic prescriptions for upper respiratory tract infections was done. For cost analysis, costs were calculated based on the acquisition cost from the perspective of the medical center (USM) as the provider.Results: The research results states that there were 90% among the patients were prescribed an antibiotic, 81 percent were prescribed an  antihistamine, 81 percent were prescribed an analgesic, and 21 percent were prescribed a mucolytic or an expectorant.Conclusion: In comparison to the previously revised researchers, the percentage of the prescribed antibiotics is very high which is unacceptable. A tremendous amount of expenditure and other adverse events can be avoided by halting this behavior. A local guideline for prescribing antibiotics and implementing the same is the need of the hour.Keywords: Prescribing patterns, Cost analysis, Upper respiratory tract,  Infections, Antibiotic
