    The potential of generic social outcomes in promoting the positive impact of the public library: Evidence from the National year of reading in Yorkshire

    Objective - To present the development, planning and implementation of a qualitative research project on the impact of National Year of Reading in Yorkshire. Methods - Generic Social Outcomes (GSO) were used to develop a theoretical framework. Data were gathered via in-depth interviews and focus groups with National Year of Reading (NYR) steering group partners in Calderdale and North Lincolnshire, selected as the two case study authorities. The use of MAXQDA computer-assisted qualitative data analysis software (CAQDAS) enabled data and coding structures to be stored and facilitated comparison in this longitudinal study. Results - The findings using the GSO framework show considerable evidence of NYR related activities in supporting the three first-tier social outcomes: 'Stronger and Safer Communities', 'Health and Well-Being' and 'Strengthening Public Life.' Conclusion - The GSO framework can be used by practitioners to help provide evidence for how public libraries contribute to diverse agendas and demonstrate their value to the community. Public library authorities can use this evidence for planning and for advocacy with a range of audiences including local and central government. Ā© 2012 Rankin

    Auscultating heart and breath sounds through patientsā€™ gowns: who does this and does it matter?

    Background Doctors are taught to auscultate with the stethoscope applied to the skin, but in practice may be seen applying the stethoscope to the gown. Objectives To determine how often doctors auscultate heart and breath sounds through patientsā€™ gowns, and to assess the impact of this approach on the quality of the sounds heard. Methods A sample of doctors in the west of Scotland were sent an email in 2014 inviting them to answer an anonymous questionnaire about how they auscultated heart and breath sounds. Normal heart sounds from two subjects were recorded through skin, through skin and gown, and through skin, gown and dressing gown. These were played to doctors, unaware of the origin of each recording, who completed a questionnaire about the method and quality of the sounds they heard. Results 206 of 445 (46%) doctors completed the questionnaire. 124 (60%) stated that they listened to patientsā€™ heart sounds, and 156 (76%) to patientsā€™ breath sounds, through patientsā€™ gowns. Trainees were more likely to do this compared with consultants (OR 3.39, 95% CI 1.74 to 6.65). Doctors of all grades considered this practice affected the quality of the sounds heard. 32 doctors listened to the recorded heart sounds. 23 of the 64 (36%) skin and 23 of the 64 (36%) gown recordings were identified. The majority of doctors (74%) could not differentiate between skin or gown recordings, but could tell them apart from the double layer recordings (p=0.02). Trainees were more likely to hear artefactual added sounds (p=0.04). Conclusions Many doctors listen to patientsā€™ heart and breath sounds through hospital gowns, at least occasionally. In a short test, most doctors could not distinguish between sounds heard through a gown or skin. Further work is needed to determine the impact of this approach to auscultation on the identification of murmurs and added sounds

    An evaluation of the National Year of Reading in Yorkshire was conducted by Leeds Metropolitan University in response to a brief from Museums, Libraries and Archives, Yorkshire. This paper outlines the development and planning of phase one of this small scale qualitative research project and the analysis of the initial results which looks at the impact of NYR on the organisations that delivered the campaign and their work with target groups. The Generic Social Outcomes and the National Indicators were used to develop a theoretical framework. Data were gathered via in depth interviews and focus groups with NYR steering group partners in Calderdale and North Lincolnshire, selected as the two case study authorities. The use of MAXQDA computer-assisted qualitative data analysis software (CAQDAS) enabled data and coding structures to be stored and will facilitate comparison in this longitudinal study. This evaluation will provide material that local library authorities can use for advocacy with a range of audiences including local and central government

    The power of the STAGS family of programs was greatly enhanced. Members of the family include STAGS-C1 and RRSYS. As a result of improvements implemented, it is now possible to address the full collapse of a structural system, up to and beyond critical points where its resistance to the applied loads vanishes or suddenly changes. This also includes the important class of problems where a multiplicity of solutions exists at a given point (bifurcation), and where until now no solution could be obtained along any alternate (secondary) load path with any standard production finite element code

    A remarkably precise observational relation for pulse core component widths of radio pulsars is used to derive stringent limits on pulsar radii, strongly indicating that pulsars are strange stars rather than neutron stars.     Anti-adrenergic effects of endothelin on human atrial action potentials are potentially anti-arrhythmic

    Endothelin-1 (ET-1) is elevated in patients with atrial fibrillation (AF) and heart failure. We investigated effects of ET-1 on human atrial cellular electrophysiological measurements expected to influence the genesis and maintenance of AF. Action potential characteristics and L-type Ca<sup>2+</sup> current (I<sub>CaL</sub>) were recorded by whole cell patch clamp, in atrial isolated myocytes obtained from patients in sinus rhythm. Isoproterenol (ISO) at 0.05 Ī¼M prolonged the action potential duration at 50% repolarisation (APD<sub>50</sub>: 54 Ā± 10 vs. 28 Ā± 5 ms; <i>P</i> < 0.05, <i>N</i> = 15 cells, 10 patients), but neither late repolarisation nor cellular effective refractory period (ERP) were affected. ET-1 (10 nM) reversed the effect of ISO on APD<sub>50</sub>, and had no basal effect (in the absence of ISO) on repolarisation or ERP. During repetitive stimulation, ISO (0.05 Ī¼M) produced arrhythmic depolarisations (<i>P</i> < 0.05). Each was abolished by ET-1 at 10 nM (<i>P</i> < 0.05). ISO (0.05 Ī¼M) increased peak I<sub>CaL</sub> from ā€“5.5 Ā± 0.4 to ā€“14.6 Ā± 0.9 pA/pF (P < 0.05; N = 79 cells, 34 patients). ET-1 (10 nM) reversed this effect by 98 Ā± 10% (P < 0.05), with no effect on basal I<sub>CaL</sub>. Chronic treatment of patients with a Ī²-blocker did not significantly alter basal APD50 or I<sub>CaL</sub>, the increase in APD50 or I<sub>CaL</sub> by 0.05 Ī¼M ISO, nor the subsequent reversal of this effect on APD50 by 10 nM ET-1. The marked anti-adrenergic effects of ET-1 on human atrial cellular action potential plateau, arrhythmic depolarisations and I<sub>CaL</sub>, without affecting ERP and independently of Ī²-blocker treatment, may be expected to contribute a potentially anti-arrhythmic influence in the atria of patients with AF and heart failure
