    Quiste neuroentérico y embarazo. Informe de un caso con seguimiento a largo plazo

    Introducción: Los quistes neuroentéricos son anomalías congénitas muy raras, secretoras, con una capa epitelial y características que semejan a las de la mucosa gastrointestinal y respiratoria. Se originan en el periodo embriogénico y resultan de la separación incompleta del canal neuroentérico del intestino anterior. Caso clínico: Seguimiento por 21 años de una paciente con antecedentes de alteraciones en el control de esfínter urinario y anal, además de dolor e hipoestesia en silla de montar, limitación de la marcha, constipación crónica y parestesias de miembros inferiores. Los hallazgos de la resonancia magnética fueron compatibles con quiste neuroentérico de la región sacra. La paciente fue sometida a cirugía en dos ocasiones. Se utilizó cirugía no radical debido a la conexión del plano profundo del quiste con el recto y nervios raquídeos. A la edad de 22 años, la paciente se embarazó; tuvo una gestación sin eventualidades con un producto sano. Discusión: La selección de la estrategia quirúrgica se orienta prioritariamente a la resolución del efecto de masa, sin embargo, resulta conveniente analizar las opciones terapéuticas de acuerdo con lo propuesto en la literatura en relación con el balance apropiado de los riesgos y de los beneficios, y con la finalidad de privilegiar la integridad funcional. Este informe demuestra la relevancia de la cirugía selectiva en comparación con la estrategia radical. Conclusiones: El objetivo terapéutico orientado a controlar el efecto de masa permitió mantener el control de los síntomas, sin deterioro neurológico y con la preservación sensoriomotriz a largo plazo y una calidad de vida funcional

    Eutrophic temperate and sub-tropical lakes often exhibit a marked vertical structure during the warm season that involves important spatial differences of physical, chemical and biological variables. Therefore, zooplankton is exposed to a highly heterogeneous environment in the vertical dimension. In this work, We analyze the depth-distribution of the cladoceran Daphnia ambigua in the eutrophic, monomictic lake El Plateado at midday and midnight, along with its relationship with the vertical distribution of water temperature and dissolved oxygen concentration. We also attempt to define whether or not this population exhibits a diel vertical migration. The results show significant changes in the day/night vertical distribution of D. ambigua during its growing season, with the exception of the last date. Also, the data revealed that average depth selected by D. ambigua becomes shallower with time, and the amplitude of the vertical migration decreases throughout the season. During the period of lake stratification, temperature appears positively correlated, and oxygen negatively correlated to the frequency of D. ambigua. It is suggested that oxygen concentration plays a crucial role in modulating the vertical migration behavior of D. ambigua in lake El Plateado, which has important consequences for understanding the atypical pattern of population dynamics exhibited by this speciesLos lagos templados y sub-tropicales a menudo presentan una marcada estructura vertical durante la estación cálida que involucra importantes diferencias espaciales de variables físicas, químicas y biológicas. El zooplancton se encuentra por ello expuesto a un ambiente marcadamente heterogéneo en su dimensión vertical. En este trabajo analizamos la distribución vertical del cladócero D. ambigua en el lago eutrófico y monomíctico El Plateado, a mediodía y a medianoche, y su relación con la distribución vertical de la temperatura del agua y concentración de oxígeno disuelto. Intentamos también definir si esta población exhibe o no una conducta de migración vertical. Los resultados muestran cambios significativos en la distribución vertical día/noche de D. ambigua durante su periodo de crecimiento, con excepción de la última fecha. Los datos revelan también que la profundidad promedio seleccionada por D. ambigua disminuye en el tiempo, así como la amplitud de la migración vertical. Dentro del periodo de estratificación del lago, la temperatura aparece positivamente correlacionada y el oxígeno negativamente correlacionado con la frecuencia de D. ambigua. Se sugiere que la concentración de oxígeno juega un papel crucial en la modulación de la conducta de migración vertical de D. ambigua en el lago El Plateado, con importantes consecuencias para el entendimiento del patrón atípico de dinámica poblacional que presenta esta especi

    We present experimental data and results supporting the conclusions of a cross-sectional quasi-experimental comparative educational intervention study that evaluated the impact of an educational intervention, which constituted a purpose-developed senescence simulator designed for us through a quasi-experimental trial. The intervention was presented in scheduled sessions to randomly selected representative groups of students from different professional fields in the health sciences from March to June 2021 in Guadalajara, Mexico, at the University of Guadalajara, the second largest public university in the country.</p

    Efficacy of Keyhole Approach to Carpal Tunnel Syndrome under Ambulatory Strategy

    The carpal tunnel syndrome is one of the most common entrapment neuropathies found in humans. Currently, the gold standard is surgical treatment using different modalities. The minimally invasive strategy with high resolution capacity and less morbidity is still a challenge. Methods. Prospective nonrandomised clinical trial in which a minimally invasive microsurgical approach was used following the keyhole principle in 55 consecutive patients and 65 hands under local anesthesia and ambulatory strategy. They were evaluated with stringent inclusion criteria with the Levine severity and functional status scale and with a 2-year follow-up. Results. 90% showed immediate improvement dropping to grades 1-2 in all items of the scale referring to pain and numbness. 97% reported improvement, as of the first month, and 3% reported persistence of symptoms, although at a lesser degree and with no functional limitation. No incidents were identified during the procedure and 98% of patients were discharged within an hour after the surgical procedure. Conclusions. The microsurgical approach described following the keyhole principle is a treatment option that, under local anesthesia and ambulatory management, may represent an alternative strategy of an effective treatment reducing the morbidity.     Combined approach for experimental Oto-neurosurgical procedures

    Background: Experimental procedures will continue to be a key element while going through the learning curve in the use of the endoscope and minimally invasive procedures. We describe the technical procedure of an experimental approach to middle ear in New Zealand rabbits through external auditory canal and its relevance as an ideal model to study graft materials and serve as a training tool for potential applications in otoneurology. Methods: A group of 28 adult New Zealand rabbits were subjected to an experimental myringoplasty, combining the transmeatal and retroauricular approach with endoscopic assistance and microsurgical technique. The different anatomical steps and systematization of the complete experimental procedure are described. Results: An experimental approach to middle ear live model and basic anatomic description was successfully used, standardizing the ideal technique. The eardrum could regenerate with no complications and with functional preservation in all the myringoplasty cases. This strategy involves a safe combined approach to the tympanic membrane and others neurosurgical as transcochlear and translaberyntic approaches and is useful as a test of other experimental procedures to evaluate biomaterials to repair the eardrum currently studied. This experimental myringoplasty model also facilitates functional tests such as impedanciometry and the endoscopic follow-up of the whole process. Conclusions: The method described to perform an experimental myringoplasty (type I tympanoplasty) in a New Zealand rabbit is an option to be used as a basic model to study the behavior of the graft in the tympanic membrane. Also, basic concepts for the use of combined instrumentation are established in the treatment of eardrum lesions, as a refinement of the technical training application in microsurgery and assisted endoscopy in the transcochlear and translaberintic approaches and otoneurology areas.
