    Seasonal variation of Guillain-Barré syndrome in Iranian patients: a retrospective study

    Background: It seems that the incidence of Guillain-Barré syndrome (GBS) has seasonal trends depending on weather as well as environmental and demographic factors such as upper respiratory tract infection (URI). The aim of this study was to evaluate seasonality of GBS and its electrophysiological subtypes. Methods: In this cross-sectional study, the records of all admitted patients to all wards of Dr. Shariati Hospital from March 2009 to March 2019 according to ICD-10 codes for GBS and other similar neuropathies were investigated and 87 patients were registered based on fulfillment of Brighton criteria and symptom onset during the recent 4 weeks. Statistical analysis was performed by IBM SPSS version 20. A p-value < 0.05 was considered statistically significant. Results: Most of the patients (63.2%, n=55) were men. The mean age of them was 49.1±19.2 years, and 41.3% (n=36) and 10.3% (n=9) participants had recent URI and gastrointestinal infection, respectively. The frequency of GBS in different seasons was 35.6% (n=31) patients in the winter, 27.6% (n=24) in the autumn, 19.6% (n=17) in the spring, and 17.2% (n=15) in the summer. The most frequent electrophysiological subtype was acute inflammatory demyelinating polyneuropathy (AIDP) in all seasons. The most common GBS disability score was 1. Conclusion: The highest and the lowest occurrence was seen in the winter and summer, respectively. AIDP was the most common electrophysiological subtype in all seasons. More studies are suggested to evaluate other aspects of GBS on more details

    Under- and overreporting of energy in a group of candidates for CABG surgery and its association with some anthropometric and sociodemographic factors, Tehran, Iran

This paper examines the prevalence of under- and overreporting of energy intake in a group of candidates for coronary artery bypass graft (CABG) surgery and its association with body mass index (BMI) and some sociodemographic factors.Subjects and methods: Dietary assessment (using a food frequency questionnaire) and demographic evaluation of 449 CABG surgery candidates was performed. Weight and height was also measured. McCrory equation was used to identify inaccurate records of energy intake. With this equation, reporting energy intake less than 78% and more than 122% of predicted energy expenditure was considered as under- and overreporting, respectively.Results: Less than half of the participants reported energy intakes within the plausible limits. There were more overreporters than underreporters in this sample. The only significant association between misreporting and related factors was seen in BMI groups. As BMI increased, the number of underreporters increased significantly. Expressed as a percentage of total energy, mean carbohydrate intake was significantly lower and mean fat and protein intake was significantly higher in underreporters compared to overreporters.Conclusion: The high prevalence of misreporting suggests more research to examine the characteristics of misreporters. Calibrating data with these characteristics can help to improve intake estimates.Keywords: underreporting, overreporting, energy intake, CABG candidate

    The Role of Mitochondrial DNA (mtDNA) in the Development of Diabetic Retinopathy (DR): A Systematic Review

    Diabetic Retinopathy (DR) is the most prevalent health problem, which is influenced by environmental and genetic factors with an increasing prevalence. The current systematic review is focused on mtDNA modification, including polymorphism and mutation/deletion, with a direct effect on DR. This systematic search was initially done through PubMed, Cochrane, EMBASE, SCOPUS, and Web of Science without a restriction on the years of publication. The terms searched included ‘‘mtDNA’’, ‘‘mitochondrial DNA’’, ‘‘diabetes’’, ‘‘diabetic’’, ‘‘retina’’, and ‘‘diabetic retinopathy’’. Animal, cohort, cross-sectional, and in vitro studies, as well as case series, case reports, review articles, and Letters to Editor were excluded from this research. From 1528 resulting searched articles, only 12 papers were finally chosen as the case-control studies considering  mtDNA gene and DR. Actually, of these 12 articles, 8 studies were concerned with mtDNA polymorphisms (UCP1, UCP2, ROMO-1, and Mn-SOD) and 4 articles were related to mtDNA mutation (A3243G mutation in tRNALeu(UUR) gene and mtDNA deletion (ΔmtDNA 4977)). Some conflicting results were found between the selected genetic modifications of mtDNA, such as Mn-SOD, UCP1, ΔmtDNA 4977, tRNALeu (UUR), and ROMO-1. Finally, A3243G mutation in the tRNALeu (UUR) gene and rs660339 and V16A polymorphisms of UCP2 and Mn-SOD genes were respectively considered as the most important factors in the pathogenesis of DR.Â

    Knowledge, attitude and practice of urban and ruarl households towards principles of nutrition in Iran: results of NUTRIKAP Survey.

    Background: The aim of this study was to assess knowledge, attitude and practice of urban and rural households toward principles of nutrition in Iran. Methods: The study population was Iranian households who live in rural and urban areas in all provinces of the country. The sampling method at households’ level in each province was single stage cluster sampling with equal size clusters. The incumbent data was collected by a structured questionnaire and through the interview with the eligible subject in each household. Results: A total of 14,136 Iranian households were selected as total sample size, 9,149 urban households, and 4,987 rural households. Around 57.2% of urban and 49.5% of rural households was aware of food groups. Respectively in urban and rural households, about 35.1% and 39.7% had correct knowledge toward roles of food groups. Approximately 41.5% and 39.9% of households had accurate knowledge about reason of food eating in urban and rural areas, respectively. The results showed that 79.6% of them had favorable attitudes. The most of the households consumed red meat and poultry weekly whereas fish was eaten rarely. Fruits, vegetables and dairy were consumed daily in the most of households. Sugar intake was daily in the most of households and cream and butter intake was weekly. Conclusion: The most of households had moderate knowledge and good attitudes. Practice of families about food consumption was good. The results of this study can be used for proper intervention for improving of health society

    Bone mineral density in female professional athletes involved in weight bearing and non-weight bearing exercises

    The present study was designed to assess the effect of different types of exercises on the BMD values in female professional atheletes. The case control study was conducted on 59 healthy female atheletes aged between 20 and 30 years who were a member of the country’s national teams in the past three years. They were involved in weight-bearing (soccer and golf) and non-weight bearing (swimming and rowing) exercises. The BMD values of the L1–L4 anteroposterior lumbar spine and femoral subregions were recorded using a DXA bone densitometer and compared to that of a group of age and sex-matched non-athletes. Mean BMD values at all the studied sites were highest among the footballers and lowest among the golf players. Except for the spine, a significant difference between the BMD values at all the studied sites. As for spine, a significant difference was only seen in the BMD values of the footballers and that of golf players. There was no significant difference between the BMD values of the controls and those involved in either weight bearing or non-weight bearing exercises. The considerable difference noted in BMD values at different sites in footballers and golf players’ points out the great influence of weight-bearing exercises on the bone structure. The bones’ response to exercises is site-specific. High-impact weight bearing exercises stressing bones in a variety of directions are more effective in improving BMD values. Athletes involved in non-weight bearing exercises should do certain weight-bearing exercises to strengthen their bones

    Geographic risk of general and abdominal obesity and related determinants in Iranian children and adolescents: CASPIAN-IV Study

    Background: Obesity, as a risk factor for many noncommunicable diseases, is a common public health problem in developed and developing countries. Among Iranian children and adolescents, the prevalence of being overweight has increased by almost 50% in the past two decades. Aims: To visualize the geographic differences in general and abdominal obesity risks and related determinants among Iranian children and adolescents. Methods: Participants consisted of 14 880 students, aged 7-19 years, living in urban and rural areas of the Islamic Republic of Iran. Spatial patterns of obesity and its association with related risk factors were identified using Bayesian spatial modeling. Results: The highest spatial risks of general obesity (odds ratio 1.21-1.66 for males and 1.81-2.02 for females) and abdominal obesity (odds ratio 1.20-1.82 for males and 1.25-1.78 for females) were observed in the north, northwest and southwest of the country. Risk of obesity was significantly higher in areas with a higher rate of urban residence, active current smokers and prolonged screen time. Conclusion: Identification of high-risk regions for obesity and spatially related risk factors can be used as informative tools for decision-making and planning in health systems at national and subnational levels

    Identification of candidate genes and proteins in aging skeletal muscle (sarcopenia) using gene expression and structural analysis

    Sarcopenia is an age-related disease characterized by the loss of muscle mass and muscle function. A proper understanding of its pathogenesis and mechanisms may lead to new strategies for diagnosis and treatment of the disease. This study aims to discover the underlying genes, proteins, and pathways associated with sarcopenia in both genders. Integrated analysis of microarray datasets has been performed to identify differentially expressed genes (DEGs) between old and young skeletal muscles. Gene Ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were then performed to uncover the functions of the DEGs. Moreover, a protein–protein interaction (PPI) network was constructed based on the DEGs. We have identified 41,715 DEGs, including 19 downregulated and 41,696 upregulated ones, in men. Among women, 3,015 DEGs have been found, with 2,874 of them being upregulated and 141 downregulated genes. Among the top up-regulated and downregulated genes, the ribosome biogenesis genes and genes involved in lipid storage may be closely related to aging muscles in men and women respectively. Also, the DEGs were enriched in the pathways including those of ribosome and Peroxisome proliferator-activated receptor (PPAR) in men and women, respectively. In the PPI network, Neurotrophic Receptor Tyrosine Kinase 1 (NTRK1), Cullin 3 (CUL3) and P53 have been identified as significant hub proteins in both genders. Using the integrated analysis of multiple gene expression profiles, we propose that the ribosome biogenesis genes and those involved in lipid storage would be promising markers for sarcopenia in men and women, respectively. In the reconstructed PPI network, neurotrophic factors expressed in skeletal muscle are essential for motoneuron survival and muscle fiber innervation during development. Cullin E3 ubiquitin ligase (Cul3) is an important component of the ubiquitin–proteasome system—it regulates the proteolysis. P53 is recognized as a central regulator of the cell cycle and apoptosis. These proteins, which have been identified as the most significant hubs, may be involved in aging muscle and sarcopenia

    Bushehr Elderly Health (BEH) Programme, phase I (cardiovascular system)

    Purpose: The main objective of the Bushehr Elderly Health Programme, in its first phase, is to investigate the prevalence of cardiovascular risk factors and their association with major adverse cardiovascular events. Participants: Between March 2013 and October 2014, a total of 3000 men and women aged ≥60 years, residing in Bushehr, Iran, participated in this prospective cohort study ( participation rate=90.2%). Findings to date: Baseline data on risk factors, including demographic and socioeconomic status, smoking and medical history, were collected through a modified WHO MONICA questionnaire. Vital signs and anthropometric measures, including systolic and diastolic blood pressure, weight, height, and waist and hip circumference, were also measured. 12-lead electrocardiography and echocardiography were conducted on all participants, and total of 10 cc venous blood was taken, and sera was separated and stored at –80°C for possible future use. Preliminary data analyses showed a noticeably higher prevalence of risk factors among older women compared to that in men. Future plans: Risk factor assessments will be repeated every 5 years, and the participants will be followed during the study to measure the occurrence of major adverse cardiac events. Moreover, the second phase, which includes investigation of bone health and cognition in the elderly, was started in September 2015.
