    MULTIPLE MICRONUTRIENT (ZINC, MAGNESIUM) THERAPY TO SEVERE MALNOURISHED CHILDREN: EFFECT ON GROWTH CATCH UP AND CLINICAL RECOVERY

    Objective: to supplement zinc and magnesium to severe malnourished children aiming to improve catch up growth and clinical recovery. Design: It was a case control study conducted among one hundred severely malnourished children who were purposively enrolled from the nutritional unit of Dhaka Shishu Hospital, Dhaka, Bangladesh. The children were distributed in four groups each comprising twenty-five children having uniform morbidity. Along with the recovering diet and nutrient supplements, the interventional three groups received single zinc, single magnesium and combination of zinc and magnesium therapy. The control group received only the recovery diet and supplement. Increment of growth catch up and hospital stay time was measured and recoded. Setting: Nutritional unit of Dhaka Shishu Hospital, Dhaka, Bangladesh. Subjects: one hundred severe malnourished children. Results: It was resulted in that single zinc or magnesium and combination of zinc-magnesium therapy significantly (p<0.05) improved the growth catch-up and clinical recovery of the malnourished children. Conclusions: Multiple Zn and Mg therapy was found to be more effective than the single Zn or Mg therapy in improving the growth catch-up and clinical recovery of the malnourished children

    A study on organic matter and nitrogen dynamics in wetland paddy soils of Bangladesh

    The present study was aimed to evaluate the effects of different sources organic matter along with various level nitrogen fertilizations on nutrient dynamics and physicochemical variation of soil at different incubation periods in the research field of department of soil science, BSMRAU. RS, VC, RHB, CD and PM were used @ 2 t C ha-1 along with N rates 0, 100 and 150 kg ha-1 in a factorial randomized complete block design. Combined application of VC and RHB with N100 dose significantly reduces soil bulk density. PM-treated plot resulted the highest amount of TN at 90 DAT, while RHB treated plot at 45 DAT. N fertilized plot showed maxi-mum TN content at 75 DAT with N150 treatment. Significant interaction effects of OM and N on TN content of soil were exhibited at the incubation period 15, 30 and 45 DAT. Different organic amendment showed a dissimilar nutrient release pattern. Significantly higher phosphorus content was detected in VC treated soil while the S content was in the CD-treated soil. The RHB treated plots provide a significantly higher exchangeable K content in post-harvest soil. The available P, S and exchangeable K contents in post-harvest soil increased positively in all treatments as compared to initial soils. Organic manures and N fertilization had no significant effect on different chemical properties like soil pH, TN, available S and exchangeable K. Significant P enrichment was occurred in organic and N treated soil. Thus, organic and inorganic fertilization had a significant positive influence on the enrichment of physiochemical properties of wetland paddy soil

    Effects of stocking density on the growth, survival and production of endangered bata, Labeo bata (Hamilton, 1822) in primary nursing

    Effect of stocking densities on the growth, survival and production of bata, Labeo bata fry and fingerlings were tested in a primary nursery rearing system. The experiment was conducted for a period of 4 weeks in six earthen nursery ponds having an area of 0.032 ha each. Four-day-old fry stocked at 1.0 million/ha was designated as treatment-1 (T1), 1.5 million/ha as treatment-2 (T2) and 2.0 million/ ha as treatment-3 (T3). At stocking, all fry were of same age with a mean length and weight of 1.03±0.03 cm and 0.12±0.01 g, respectively. Fry in all the treatments were fed with Mega commercial fish feed. Physico-chemical parameters such as water temperature, transparency, dissolve oxygen, pH and total alkalinity in all the treatments were suitable ranges for fry and fingerling rearing. Plankton population (both phytoplankton and zooplankton) were found to be at optimum level for fish culture. Highest weight gain was observed in T1 (3.46±0.08) and lowest in T3 (1.98±0.03). Final length, final weight and survival of fingerlings also followed the same trends as weight gain. Fingerlings in T1 produced significantly higher specific growth rate (12.15±0.08) than T2 (11.31±0.03) and T2 (10.22±0.05). Feed conversion ratio was significantly lower in T1 (0.26±0.01) than T2 (0.42±0.02) and T3 (0.65±0.01). Significantly higher number of fingerlings was produced in T3 (1177700±4700) than T2 (963300±9900) and T1 (717850±7350), respectively. Despite of this, consistently higher net benefits were found from T1 than T2 and T3. Overall, highest growth (3.60±0.16 g), survival (71.79 ± 1.04%) and net benefits (TK. 127,087.00) of fingerlings were obtained at a density of 1.0 million hatchlings/ha. Therefore, out of three stocking densities, 1.0 million fry/ha appears to be most suitable stocking density for nursing and rearing of L. bata fry and fingerlings in primary nursing

    Surveillance of avian influenza virus type A in semi-scavenging ducks in Bangladesh

    BACKGROUND: Ducks are the natural reservoir of influenza A virus and the central host for highly pathogenic avian influenza (H5N1), while domestic ducks rearing in semi-scavenging system could serve as re-assortment vessels for re-emerging new subtypes of influenza viruses between birds to human. Avian influenza virus (AIV) surveillance in Bangladesh has been passive, relying on poultry farmers to report suspected outbreaks of highly pathogenic H5N1 influenza. Here, the results of an active surveillance effort focusing on the semi-scavenging ducks are presented. RESULT: A total of 2100 cloacal swabs and 2100 sera were collected from semi-scavenging ducks from three wintering-sites of Bangladesh during three successive winter seasons, December through February in the years between 2009 and 2012. Virus isolation and identification were carried out from the cloacal swabs by virus propagation in embryonated hen eggs followed by amplification of viral RNA using Avian influenza virus (AIV) specific RT-PCR. The overall prevalence of avian influenza type A was 22.05% for swab samples and 39.76% ducks were sero-positive for avian influenza type A antibody. Extremely low sero-prevalence (0.09%) of AIV H5N1 was detected. CONCLUSIONS: Based on our surveillance results, we conclude that semi-scavenging ducks in Bangladesh might play important role in transmitting Avian Influenza virus (AIV) type A. However, the current risk of infection for humans from domestic ducks in Bangladesh is negligible. We believe that this relatively large dataset over three winters in Bangladesh might create a strong foundation for future studies of AIV prevalence, evolution, and ecology in wintering sites around the globe

    Emergence of mobile colistin resistance (mcr-8) in a highly successful Klebsiella pneumoniae sequence type 15 clone from clinical infections in Bangladesh

    The emergence of mobilized colistin resistance genes (mcr) has become a serious concern in clinical practice, compromising treatment options for life-threatening infections. In this study, colistin-resistant Klebsiella pneumoniae harboring mcr-8.1 was recovered from infected patients in the largest public hospital of Bangladesh, with a prevalence of 0.3% (3/1,097). We found mcr-8.1 in an identical highly stable multidrug-resistant IncFIB(pQil) plasmid of ∼113 kb, which belonged to an epidemiologically successful K. pneumoniae clone, ST15. The resistance mechanism was proven to be horizontally transferable, which incurred a fitness cost to the host. The core genome phylogeny suggested the clonal spread of mcr-8.1 in a Bangladeshi hospital. Core genome single-nucleotide polymorphisms among the mcr-8.1-positive K. pneumoniae isolates ranged from 23 to 110. It has been hypothesized that mcr-8.1 was inserted into IncFIB(pQil) with preexisting resistance loci, blaTEM-1b and blaCTX-M-15, by IS903B. Coincidentally, all resistance determinants in the plasmid [mcr-8.1, ampC, sul2, 1d-APH(6), APH(3′′)-Ib, blaTEM-1b, blaCTX-M-15] were bracketed by IS903B, demonstrating the possibility of intra- and interspecies and intra- and intergenus transposition of entire resistance loci. This is the first report of an mcr-like mechanism from human infections in Bangladesh. However, given the acquisition of mcr-8.1 by a sable conjugative plasmid in a successful high-risk clone of K. pneumoniae ST15, there is a serious risk of dissemination of mcr-8.1 in Bangladesh from 2017 onwards

    Analysis of Genetic Diversity in Some Rice Varieties and Their Performance in Bangladesh

    This study was conducted to evaluate the genetic diversity for several rice features and their association with yields, as well as to identify genotypes of short-duration rice. The experiment was conducted in the field in a natural environment, and data were collected on several plant parameters for each genotype at various phases of plant development. Twenty genotypes of rice were examined based on their morphological and physiological characteristics. From July through December of 2020, the experiment was conducted at the Bangladesh Rice Research Institute's regional station in Shyampur, Rajshahi. There was significant diversity among the twenty rice genotypes for all characteristics tested. The genotype BRRI dhan57 displayed the shortest days to flowering. In terms of days to maturity, the genotype BRRI dhan57 was the earliest, with a maturity time of 107.33 days, followed by BRRI dhan56 and BRRI dhan39. The days to blooming had the highest heritability (99.75%), followed by the days to maturity (99.58%), grain yield (85.30%), thousand grain weight (85.22%), grains per panicle (84.91%), plant height (82.21%), and tillers per hill (21.61%). High heritability scores indicated that the researched qualities were less influenced by the surrounding environment. As a percentage of the mean, the genetic gain was greatest for grain yield (36.33%) and lowest for tillers per hill (6.60%) among the yield-contributing factors. In days to flowering, days to maturity, grains per panicle, and plant height, high heritability and genetic progress were seen. According to the principal component analysis (PCA), the Eigen values of the first four components of the total variance accounted for 89.46% of the total variance, indicating that these components were mostly responsible for the genetic diversity of the current materials. It was the largest cluster, containing seven rice genotypes. Clusters II and V contained five and four genotypes of rice, respectively. Clusters III and IV were the smallest, with only two genotypes apiece. The pattern of distribution of genotypes among various clusters demonstrated the significant genetic variety present in the genotypes, which may be the result of adaptation of these genotypes to certain environmental conditions. The largest value of intercluster distance indicated that cluster III genotypes were extremely distinct from cluster IV genotypes. Negative values in both vectors for tillers per hill suggested that this feature contributed the least to the total diversity. The number of panicles per hill, panicle length, weight per thousand grains, and grain yield were all positive in both directions.     COVID-19 among staff and their family members of a healthcare research institution in Bangladesh between March 2020 and April 2021 : a test-negative case–control study

    Objective To identify factors associated with COVID- 19 positivity among staff and their family members of icddr,b, a health research institute located in Bangladesh. Setting Dhaka, Bangladesh. Participants A total of 4295 symptomatic people were tested for SARS-CoV-2 by reverse-transcription PCR between 19 March 2020 and 15 April 2021. Multivariable logistic regression was done to identify the factors associated with COVID- 19 positivity by contrasting test positives with test negatives. Result Forty-three per cent of the participants were tested positive for SARS-CoV-2. The median age was high in positive cases (37 years vs 34 years). Among the positive cases, 97% were recovered, 2.1% had reinfections, 24 died and 41 were active cases as of 15 April 2021. Multivariable regression analysis showed that age more than 60 years (adjusted OR (aOR)=2.1, 95% CI 1.3 to 3.3; pwere tested negative. Conclusions The study findings suggest that older age, fever, cough and anosmia were associated with COVID- 19 among the study participants.publishedVersionPeer reviewe

    Genomic insights into the mechanism of carbapenem resistance dissemination in Enterobacterales from a tertiary public heath setting in South Asia

    Background Given the high prevalence of multidrug resistance (MDR) across South Asian (SA) hospitals, we documented the epidemiology of carbapenem resistant Enterobacterales (CRE) infections at Dhaka Medical College Hospital between October 2016 to September 2017. Methods We enrolled patients and collected epidemiology and outcome data. All Enterobacterales were characterised phenotypically and by whole genome sequencing. Risk assessment for the patients with CRE were performed compared to patients with carbapenem-susceptible Enterobacterales (CSE). Results 10.6% of all 1831 patients with a clinical specimen collected had CRE. In-hospital 30-day mortality was significantly higher with CRE [50/180 (27.8%)] than CSE [42/312 (13.5%)] (p = 0.001); however, for blood-stream infections, this was insignificant. Out of 643 Enterobacterales isolated, 210 were CRE. blaNDM was present in 180 isolates, blaOXA-232 in 26, blaOXA-181 in 24 and blaKPC-2 in 5. Despite this, ceftriaxone was the most commonly prescribed empirical antibiotic and only 27% patients were prescribed at least one antibiotic to which their infecting pathogen was susceptible. Significant risk factors for CRE isolation included burns unit and ICU admission, and prior exposure to levofloxacin, amikacin, clindamycin and meropenem. E. coli ST167 was the dominant CRE clone. Clustering suggested clonal transmission of K. pneumoniae ST15 and the MDR hyper-virulent clone, ST23. The major trajectories involved in horizontal gene transfer were IncFII and IncX3, IS26, and Tn3. Conclusion This is the largest study from a SA public hospital combining outcome, microbiology and genomics. The findings indicate the urgent implementation of targeted diagnostics, appropriate antibiotic use and infection control interventions in SA public institutions

    Vaccination following the expanded programme on immunization schedule could help to reduce deaths in children under five hospitalized for pneumonia and severe pneumonia in a developing country

    BackgroundWorldwide, pneumonia is the leading cause of mortality in children under the age of five. An expanded program on immunization (EPI) is one kind of evidence-based tool for controlling and even eradicating infectious diseases.ObjectivesThis study aimed to explore the impact of EPI vaccination, including BCG, DPT-Hib-Hep B, OPV, IPV, and PCV-10, among children from the age of 4 to 59 months hospitalized for pneumonia and severe pneumonia. Additionally, we evaluated the role of 10 valent pneumococcal conjugate vaccines alone on clinical outcomes in such children.MethodsIn this retrospective chart review, children from the age of 4 to 59 months with WHO-defined pneumonia and severe pneumonia admitted to the Dhaka Hospital of the International Centre for Diarrheal Disease Research, Bangladesh (icddr,b) between August 2013 and December 2017 who had the information on immunization as per EPI schedule by 4 months of age were included in the analysis. A comparison was made between the children who were fully immunized (immunization with BCG, DPT-Hib-Hep B, OPV, and IPV from 2013 to 2015 and PCV-10 from 2015 to 2017) and who were not immunized (consisting of partial immunization and no immunization) during the study period.ResultsA total of 4,625 children had pneumonia and severe pneumonia during the study period. Among them, 2,605 (56.3%) had received the information on immunization; 2,195 (84.3%) were fully immunized by 4 months of age according to the EPI schedule and 410 were not immunized. In the log-linear binomial regression analysis, immunization of children from 4 to 59 months of age was found to be associated with a lower risk of diarrhea (p = 0.033), severe pneumonia (p = 0.001), anemia (p = 0.026), and deaths (p = 0.035). Importantly, the risk of developing severe pneumonia (1054/1,570 [67%] vs. 202/257 [79%], p < 0.001) and case-fatality rate (57/1,570 [3.6%] vs. 19/257 [7.4%], p = 0.005) was still significantly lower among those who were immunized with PCV-10 than those who were not.ConclusionChildren immunized as per the EPI schedule were at a lower risk of diarrhea, severe pneumonia, anemia, and death, compared to unvaccinated children. In addition, PCV-10 was found to be protective against severe pneumonia and deaths in vaccinated children. The overall results underscored the importance of the continuation of immunization, scrupulously adhering to the EPI schedule to reduce the risk of morbidities and mortalities in children, especially in resource-limited settings
