    Effect Of Functional Endoscopic Sinus Surgery To The Flow Behavior In Nasal During Resting Breathing Condition

    Functional endoscopic sinus surgery (FESS) is a surgery to removeuncinate process in order to restore sufficient sinus ventilation and drainagein nasal. However, there were a few cases with side effects such as facialpain, reduction in sense of smell and sinusitis reoccurrence of infection. Inthis study, the effect of uncinate process removal is investigated. Images ofthe model were done through computational technique and then the flowwas simulated to predict the effect of the removal. Inhalation processes withresting breathing condition were modeled. The results show that smoothflow was observed at nasal area which indicates successful surgical process.However for post FESS model the result shows that the possibilities ofsinusitis reoccurrence of infection were high. Finally, velocity profile in theolfactory area show non-favorable flow condition for effective smell senses

    Environmental DNA reveals links between abundance and composition of airborne grass pollen and respiratory health

Standard pollen monitoring ‘count’ data were sourced from the MEDMI database, with the exception of data from Bangor which were produced as part of the present study and are available on request. Prescribing datasets are publicly available, as are weather, air pollution, deprivation (IMD) and rural-urban category data. Hospital episode statistics (HES) datasets are sensitive, individual-level health data, which are subject to strict privacy regulations and are not publicly available. The study did not generate any unique codeGrass (Poaceae) pollen is the most important outdoor aeroallergen, exacerbating a range of respiratory conditions, including allergic asthma and rhinitis (‘hay fever’). Understanding the relationships between respiratory diseases and airborne grass pollen with view to improving forecasting has broad public health and socioeconomic relevance. It is estimated that there are over 400 million people with allergic rhinitis and over 300 million with asthma, globally, often comorbidly . In the UK, allergic asthma has an annual cost of around US$ 2.8 billion (2017). The relative contributions of the >11,000 (worldwide) grass species to respiratory health have been unresolved, as grass pollen cannot be readily discriminated using standard microscopy. Instead, here we used novel environmental DNA (eDNA) sampling and quantitative PCR (qPCR) , to measure the relative abundances of airborne pollen from common grass species, during two grass pollen seasons (2016 and 2017), across the UK. We quantitatively demonstrate discrete spatiotemporal patterns in airborne grass pollen assemblages. Using a series of generalised additive models (GAMs), we explore the relationship between the incidences of airborne pollen and severe asthma exacerbations (sub-weekly) and prescribing rates of drugs for respiratory allergies (monthly). Our results indicate that a subset of grass species may have disproportionate influence on these population-scale respiratory health responses during peak grass pollen concentrations.     Obeticholic acid for the treatment of non-alcoholic steatohepatitis: interim analysis from a multicentre, randomised, placebo-controlled phase 3 trial

    Background Non-alcoholic steatohepatitis (NASH) is a common type of chronic liver disease that can lead to cirrhosis. Obeticholic acid, a farnesoid X receptor agonist, has been shown to improve the histological features of NASH. Here we report results from a planned interim analysis of an ongoing, phase 3 study of obeticholic acid for NASH. Methods In this multicentre, randomised, double-blind, placebo-controlled study, adult patients with definite NASH,non-alcoholic fatty liver disease (NAFLD) activity score of at least 4, and fibrosis stages F2–F3, or F1 with at least oneaccompanying comorbidity, were randomly assigned using an interactive web response system in a 1:1:1 ratio to receive oral placebo, obeticholic acid 10 mg, or obeticholic acid 25 mg daily. Patients were excluded if cirrhosis, other chronic liver disease, elevated alcohol consumption, or confounding conditions were present. The primary endpointsfor the month-18 interim analysis were fibrosis improvement (≥1 stage) with no worsening of NASH, or NASH resolution with no worsening of fibrosis, with the study considered successful if either primary endpoint was met. Primary analyses were done by intention to treat, in patients with fibrosis stage F2–F3 who received at least one dose of treatment and reached, or would have reached, the month 18 visit by the prespecified interim analysis cutoff date. The study also evaluated other histological and biochemical markers of NASH and fibrosis, and safety. This study is ongoing, and registered with ClinicalTrials.gov, NCT02548351, and EudraCT, 20150-025601-6. Findings Between Dec 9, 2015, and Oct 26, 2018, 1968 patients with stage F1–F3 fibrosis were enrolled and received at least one dose of study treatment; 931 patients with stage F2–F3 fibrosis were included in the primary analysis (311 in the placebo group, 312 in the obeticholic acid 10 mg group, and 308 in the obeticholic acid 25 mg group). The fibrosis improvement endpoint was achieved by 37 (12%) patients in the placebo group, 55 (18%) in the obeticholic acid 10 mg group (p=0·045), and 71 (23%) in the obeticholic acid 25 mg group (p=0·0002). The NASH resolution endpoint was not met (25 [8%] patients in the placebo group, 35 [11%] in the obeticholic acid 10 mg group [p=0·18], and 36 [12%] in the obeticholic acid 25 mg group [p=0·13]). In the safety population (1968 patients with fibrosis stages F1–F3), the most common adverse event was pruritus (123 [19%] in the placebo group, 183 [28%] in the obeticholic acid 10 mg group, and 336 [51%] in the obeticholic acid 25 mg group); incidence was generally mild to moderate in severity. The overall safety profile was similar to that in previous studies, and incidence of serious adverse events was similar across treatment groups (75 [11%] patients in the placebo group, 72 [11%] in the obeticholic acid 10 mg group, and 93 [14%] in the obeticholic acid 25 mg group). Interpretation Obeticholic acid 25 mg significantly improved fibrosis and key components of NASH disease activity among patients with NASH. The results from this planned interim analysis show clinically significant histological improvement that is reasonably likely to predict clinical benefit.     Effect of pretreatment process on thermal oxidation of biomedical grade cobalt based alloy

    Wear on Co-Cr-Mo biomedical implants is still a major issue especially for applications in articulation joints like in total ankle, knee and hip arthroplasty. Generation of excessive wear particles can coagulate in body tissues which later cause inflammation, bone loss and necrosis. Modification of implant surfaces is a common technique for increasing the hardness and thus minimizing these effects. In this study, thermal oxidation method was carried out on the Co-Cr-Mo to investigate the effects of different pretreatment processes and surface roughness on the hardness of oxide layer formed. Prior to oxidation process, all samples were annealed and pickled to remove residual stress and oxide scales respectively. The oxidation process was done inside furnace under atmospheric condition for 3 hours at 1160 °C. The metallic compositions, surface morphology and hardness of the oxide layer formed on the substrate were verified using X-ray diffraction (XRD), scanning electron microscope and micro-Vickers hardness analysis respectively. It is found that mechanical pretreatment provides oxide/carbide layer with higher hardness than chemical pretreatment method. It is believed that remnants of polishing diamond pastes trapped in roughness valleys react with metal matrix and later transform into carbides during oxidation process. In contrast, initial surface roughness of the substrate has no significant effect on the hardness of oxide/carbide layer

    A Study and Comparison of the Preparation of Gadolinium Aluminate Nanoparticles Using γ-Irradiated and Unirradiated Precursors

    The effects of γ-irradiation and the application of different precursors on the formation of gadolinium aluminate (GdAlO3) nanoparticles (NPs) have been studied in detail. GdAlO3 NPs were prepared by using different gadolinium-based precursors including gadolinium acetate (Gd(CH3COO)3·4H2O) and gadolinium nitrate (Gd(NO3)3·6H2O), while Al2O3 and Al(NO3)3·9H2O were used as the source of Al3+. The preparation of GdAlO3 was carried out by two different methods, solid-state reaction and sol-gel process. To study the effect of γ-irradiation, both irradiated and unirradiated Gd(CH3COO)3·4H2O have been tested for the preparation of gadolinium aluminate (GdAlO3). Notably, Gd(CH3COO)3·4H2O did not produce GdAlO3 in both solid-state and sol-gel processes even after optimizing various parameters, including the application of γ-irradiation. However, single-phase nanocrystalline GdAlO3 NPs were successfully obtained from the reaction of gadolinium nitrate Gd(NO3)3·6H2O and Al(NO3)3·9H2O by a sol-gel process. The formation of NPs has been confirmed by X-ray diffraction analysis (XRD) and Fourier-transform infrared (FT-IR) spectroscopy. The results indicate towards the formation of an orthorhombic perovskite structure of GdAO3 in the Pbnm space group. Transmission electron microscopy (TEM) has been employed for the particle-size analysis, which revealed the formation of spherical-shaped nanoparticles with the size range of 50–70 nm. Surface morphology of the sintered pellet was obtained from high-resolution scanning electron microscopy (HR-SEM). Besides, the effect of irradiation with γ-rays on the quality of resultant NPs has also been studied

    Stereoselective synthesis of diazaspiro[5.5]undecane derivatives via base promoted [5+1] double Michael addition of N,N-dimethylbarbituric acid to diaryliedene acetones

    The nitrogen containing spiro-heterocycle is one of the privileged synthetic motif that constitutes various naturally occurring molecules and displays a broad range of pharmaceutical and biological activities. A new methodology was developed for the synthesis of 2,4-diazaspiro[5.5]undecane-1,3,5,9-tetraones spiro-heterocyclic derivatives via cascade cyclization of [5+1] double Michael addition reaction of N,N-dimethylbarbituric acid with the derivatives of diaryldivinylketones in the presence of diethylamine at ambient temperature. The developed protocol is highly capable of furnishing diazaspiro[5.5]undecane derivatives 3a–m in excellent yields (up to 98%), from easily accessible symmetric and non-symmetric divinylketones 2a–m, containing aryl and heteroaryl substituents. The diazaspiro-heterocyclic structure was mainly elucidated by NMR and X-ray crystallographic techniques. The single-crystal X-ray studies revealed that, the cyclohexanone unit of spirocycles often prefers a chair conformation rather than twisted conformation. The intermolecular hydrogen bonding and CArH⋯π, π–π stacking interactions driving forces are mainly responsible for the crystal packing

    There the purpose is to develop the techniques for the selection of rational range of parts and for the estimation of efficiency of use of Multimission Machines (MM) at the stage of Engineering Preparation of Production (EPP). The causes of non-rational use of MM under the particular production conditions have been brought out. The basic scheme for the determination of rational range of parts for the making by MM, the estimation technique of labour-intensiveness of part making with regard to their structural-engineering characteristics at the EPP stage have been developed. The general and particular indicators of use efficiency of MM have been ascertained; the estimation procedure for the use efficiency of MM has been developed. The software, which was realized by CA CLIPPER language, 5.2 version (for the data base management system), has been created.
