    Alachlor oxidation by the filamentous fungus Paecilomyces marquandii

    Alachlor, a popular chloroacetanilide herbicide, can be a potential health risk factor. Soil microorganisms are primarily responsible for conversion and migration of alachlor in natural environment, but knowledge concerning alachlor biodegradation is not complete. Therefore, we studied the ability of Paecilomyces marquandii, soil fungus tolerant to heavy metals, to eliminate alachlor and proposed a new pathway of its transformation. After 7 days of incubation only 3.3% of alachlor was detected from an initial concentration 50 mg L-1 and 20.1% from a concentration 100 mg L-1. The qualitative IDA LC-MS analysis showed the presence of ten metabolites. All of them were dechlorinated mainly through oxidation, but also reductive dechlorination was observed. The main route of alachlor conversion progressed via N-acetyl oxidation resulting in the formation of mono-, di- and trihydroxylated byproducts. N-acetyl oxidation as a dominant route of alachlor metabolism by fungi has not been described so far. The toxicity of alachlor tested with Artemia franciscana did not increase after treatment with P. marquandii cultures. Paecilomyces marquandii strain seems to be an interesting model for the research on alachlor conversion by soil microscopic fungi, due to its dechlorination and hydroxylation ability as well as high tolerance to heavy metals.Grant of the National Centre for Science in Cracow, Poland, No UMO-2011/01/B/NZ9/0289

    Analysis of the unique structural and physicochemical properties of the DraD/AfaD invasin in the context of its belonging to the family of chaperone/usher type fimbrial subunits

    <p>Abstract</p> <p>Background</p> <p>DraD invasin encoded by the dra operon possesses a classical structure characteristic to fimbrial subunits of the chaperone/usher type. The Ig-fold of the DraD possesses two major characteristics distinguishing it from the family of fimbrial subunits: 1) a distortion of the β-barrel structure in the region of the acceptor cleft, demonstrated by a disturbance of the main-chain hydrogen bonds network, and 2) an unusually located disulfide bond connecting B and F strands - the localization exclusively observed in the subfamily of DraD/AfaD-type subunits.</p> <p>Results</p> <p>To evaluate the influence of the DraD-sc specific structural features on its stability and mechanism of thermal denaturation, a series of DSC and FT-IR denaturation experiments were performed giving following conclusions. 1) The DraD-sc is characterized by a low stability (standard Gibbs free energy and enthalpy of unfolding of 18.4 ±1.4 kJ mol<sup>-1 </sup>and 131 ±25 kJ mol<sup>-1</sup>, respectively) that contrasts strongly with almost infinite stability of the described previously DraE-sc fimbrial protein. 2) The DraD-sc unfolds thermally according to the two state equilibrium model, in contrast to the irreversible kinetically controlled transition of the DraE-sc. 3) The DraD specific disulfide bond is crucial at the folding stage and has little stability effect in the mature protein.</p> <p>Conclusions</p> <p>Data published so far emphasize unique biological properties of the DraD invasin as fimbrial subunit: a chaperone independent folding, an usher independent surface localization and the possibility to exist in two forms: as unbound subunits and as loosely bound at fimbrial tip.</p> <p>Presented calorimetric and FT-IR stability data combined with structural correlations has underlined that the DraD invasin is also characterized by unique physicochemical and structural attributes in the context of its belonging to the family of fimbrial subunits.</p

    The chain of survival in hypothermic circulatory arrest : encouraging preliminary results when using early identification, risk stratification and extracorporeal rewarming

    BACKGROUND: The prognosis in hypothermic cardiac arrest is frequently good despite prolonged period of hypoperfusion and cardiopulmonary resuscitation. Apart from protective effect of hypothermia itself established protocols of treatment and novel rewarming techniques may influence on outcome. The purpose of the study was to assess the outcome of patients with hypothermic circulatory arrest treated by means of arterio-venous extracorporeal membrane oxygenation (ECMO) according to locally elaborated protocol in Severe Accidental Hypothermia Center in Cracow, Poland. METHODS: Prospective observational case-series study – all patients with confirmed hypothermic cardiac arrest consulted with hypothermia coordinator were accepted for extracorporeal rewarming, unless contraindications for ECMO were observed (active bleeding). RESULTS: The study population consisted of 10 patients (7 male, median age 48.5 years). The core temperature measured esophageally was 16.9–28.4 °C, median 22 °C. On admission 5 patients presented with asystole and 5 with ventricular fibrillation. Duration of circulatory arrest before ECMO implantation was 107 to 345 min (median 156 min). The duration of ECMO support was 1.5 to 91 h (median 22 h). Cardiorespiratory stability and full neurologic recovery was achieved in 7 patients. The duration of mechanical ventilation was 88–437 h (median 177 h) and the length of stay in the ICU was 8–26 days (median 15 days). All survivors had mildly impaired (1 patient, LVEF 40 %) or preserved (6 patients, LVEF 55–65 %) left ventricular systolic function at the time of discharge from ICU. The cause of death of non-survivors (three patients) was acute myocarditis, massive retroperitoneal bleeding, and massive gastrointestinal bleeding. DISCUSSION AND CONCLUSIONS: Our data confirm the high survival rate (70 %) and excellent neurologic outcome in hypothermic cardiac arrest. The following key elements seem to impact the final prognosis: the appropriate coordination of the rescue operation, immediate high-quality CPR (preferably using mechanical chest compression system) and application of ECMO for rewarming and cardiorespiratory support

    Introduction: Orthorexia nervosa (ON) is a common phenomenon. Its prevalence in society ranges from 1% to 60%. Yet, there are no specific diagnostic criteria or effective screening tests. Moreover, orthorexia does not appear in any of the available international psychiatric classifications such as the ICD (International Statistical Classification of Diseases and Related Health Problems) and DSM (Diagnostic and Statistical Manual of Mental Disorders). Due to this fact many people remain undiagnosed, which significantly affects their standard of living, and later puts a strain on the health care system. Material and method: The article is based on a study and analysis of available publications in the last few years, published in the PubMed database. Conclusions: It is crucial to create specific diagnostic criteria for orthorexia. This will enable efficient detection of patients at an early stage of the disorder, which will significantly shorten the diagnostic path. Moreover, specifying potential risk factors will help to reduce new cases

    Modern methods of treating marijuana addiction (cannabis use disorders) and its influence on health

    Introduction: Worldwide, cannabis is the most widely used illegal psychoactive substance and the third most common drug, after alcohol and tobacco. Cannabis use disorder (CUD) affects approximately 10% of the 193 million cannabis users worldwide. In recent years, cannabinoids have become more and more popular. The CUD is becoming a growing social and health problem. The search for effective forms of pharmacotherapy is still ongoing, and an effective combination of psychotherapy and pharmacotherapy may be crucial for the future of CUD treatment. &nbsp; Material and method: The article was based on a review and analysis of publications and discoveries in recent years, which were available in the PubMed and Google Scholar database. &nbsp; Results: 82 study participants attempted abstinence from cannabis use. In the final primary endpoint analysis, both 400 mg of CBD and 800 mg of CBD were more effective than placebo in reducing cannabis use.&nbsp; 50 people who were addicted to cannabis treated with gabapentin showed a significant reduction in cannabis use and cannabis withdrawal compared to placebo. Moreover, 70 men addicted to cannabis were randomized and those, who used FAAH inhibitors reported fewer cannabis withdrawal symptoms, less cannabis use and lower THC urine levels than placebo users. Conclusions: Gabapentin has been proven to show a reduction in CUD. The use of CBD, FAAH inhibitors and psychotherapy MET/CBT/CM also appear to be effective. In addition, the use of the principles of People-First Language, avoiding slang and idioms, using medical and curing vocabulary, respects the addicted person, and also positively influences the chances of abstinence

    Giardia intestinalis - atypical symptoms of infection in the daily practice of various specialists

    Giardia intestinalis infection is the most common parasitic infection in the world. The parasite's reservoir is mainly humans and many species of mammals. Giardiasis is a parasitic disease of the duodenum and small intestine. Infection spreads easily through the oral-fecal route. Its most frequent symptom is chronic diarrhea, but it can also be asymptomatic. This article analyzes atypical symptoms of Giardia Intestinalis infection based on available studies in the PubMed database. Observations have shown that giardiasis can manifest in various types of dermatoses such as urticaria, angioedema, atopic dermatitis, erythema nodosum, or Wells syndrome. A relationship between Giardia Intestinalis infection and various allergic syndromes, manifesting as chronic urticaria or angioedema, has also been observed, as well as an increased occurrence of food allergies due to increased exposure to antigens.
