    Post-partum surgical wound infections: incidence after caesarean section in an Italian hospital

    Introduction. Despite international recommendations and gen- eral agreement on the fact that more complications arise after caesarean section, Italy ranks first in the number of caesarean sections performed each year. Aim of this cohort study was to estimate the incidence of post-partum wound infections following caesarean section in a sample of low-risk women and to examine the main risk factors correlated. Methods. 430 mothers were included in the study. A data col- lection form was completed with woman?s obstetric history, details of the operation and of any infection that occurred during hospital staying. A post-discharge telephone call-up surveillance after delivery was also performed Results. A total of 20 (4.7%) SSIs were recorded. Through post-discharge surveillance, 85% of infections were identified. The time between membrane rupture and start of the operation was found to be associated with the development of infection (p = 0.04). No statistically significant association with any of the other risk factors was found. Discussion and conclusion. From the comparison of current practices with international guideline recommendations we could identify critical points that will need to be addressed in correc- tive and training interactions, specifically, choice and timing of administration of antibiotics in antimicrobial prophylaxis and timing of showering and shaving

    La valle del Rio della Rocca nel Comune di Castellarano (Reggio Emilia) è stata interessata a partire dagli anni ‘50 del secolo scorso da attività estrattive che in parte ne hanno profondamente modificato le caratteristiche ambientali, in particolare quelle geomorfologiche. In considerazione dell’interesse paesaggistico della valle e ai fini di una riqualificazione ambientale del territorio, è stato realizzato uno studio multidisciplinare volto a delineare i principali aspetti geologici, paleontologici, floristici e faunistici. Specifica attenzione è stata posta alla valutazione dell’instabilità dei versanti e alla individuazione dei geositi presenti nell’area. Sulla scorta dei risultati delle ricerche effettuate sono state elaborate proposte di riqualificazione del territorio che prevedono una valorizzazione della valle a fini geoturistici e ricreativi

    Circulating mucosal-associated invariant T cells identify patients responding to anti-PD-1 therapy

    Immune checkpoint inhibitors are used for treating patients with metastatic melanoma. Since the response to treatment is variable, biomarkers are urgently needed to identify patients who may benefit from such therapy. Here, we combine single-cell RNA-sequencing and multiparameter flow cytometry to assess changes in circulating CD8+ T cells in 28 patients with metastatic melanoma starting anti-PD-1 therapy, followed for 6 months: 17 responded to therapy, whilst 11 did not. Proportions of activated and proliferating CD8+ T cells and of mucosal-associated invariant T (MAIT) cells are significantly higher in responders, prior to and throughout therapy duration. MAIT cells from responders express higher level of CXCR4 and produce more granzyme B. In silico analysis support MAIT presence in the tumor microenvironment. Finally, patients with >1.7% of MAIT among peripheral CD8+ population show a better response to treatment. Our results thus suggest that MAIT cells may be considered a biomarker for patients responding to anti-PD-1 therapy

    Validation of a web-based questionnaire for pregnant women to assess utilization of internet: survey among an Italian sample

    Abstract BACKGROUND: Objective of this pilot study was to evaluate the reliability and validity of the web-based questionnaire in pregnant women as a tool to examine prevalence, knowledge and attitudes about internet utilization for health-related purposes, in a sample of Italian pregnant women. METHODS: The questionnaire was composed by 9 sections for a total of 73 items. Reliability analysis was tested and content validity was evaluated using Cronbach's alpha to check internal consistency. Statistical analysis was performed through SPSS 13.0. RESULTS: Questionnaire was administered to 56 pregnant women. The higher value of Cronbach's alpha resulted on 61 items: alpha = 0.786 (n. 73 items: alpha = 0.579). High rate of pregnant women generally utilized internet (87.5%) and the 92.1% confirmed to use internet with the focus to acquire information about pregnancy (p < 0.0001). CONCLUSIONS: The questionnaire showed a good reliability property in the pilot study. In terms of internal consistency and validity appeared to have a good performance. Given the high prevalence of pregnant women that use internet to search information about their pregnancy status, professional healthcare workers should give advice regarding official websites where they could retrieve safe information and learn knowledge based on scientific evidence

    A developmentally regulated nerve growth factor-induced gene, VGF, is expressed in geniculocortical afferents during synaptogenesis

    The expression of the nerve growth factor-inducible gene VGF has been examined by in situ hybridization. Western blot and immunohistochemical studies in the developing and adult rat central nervous system, with particular emphasis on the visual system. Both the messenger RNA and the protein are particularly abundant in the developing dorsal lateral geniculate nucleus, appearing, respectively, at embryonal day 16 and 18. After its onset at E16, VGF messenger RNA expression increases progressively in the dorsal lateral geniculate nucleus and remains high during the first two post-natal weeks; afterwards, it gradually decreases and, at the offset of the plasticity period, it reaches very low levels maintained in adulthood. A similar time course has been observed for VGF protein in the dorsal lateral geniculate nucleus area, by semi-quantitative Western blots. In addition to the presence of the protein in the geniculate neurons, a strong, transient immunoreactivity has been found at the embryonic cortical subplate at E18, reflecting the presence of the antigen in axonal terminals originating from thalamic neurons. Interestingly, we found that the blockade of afferent electrical activity by intraocular injection of tetrodotoxin strongly reduces the level of VGF messenger RNA in the dorsal lateral geniculate nucleus. Although the function of the VGF protein is not known, it had been previously proposed that VGF could be a precursor for neuropeptide/s. The spatiotemporal expression of VGF, together with the observation of a regulation by electrical activity, suggest that this protein may be relevant in the process of synaptogenesis and/or synaptic stabilization in the developing geniculocortical connections

Considering the high scenic and environmental value of the study area, multidisciplinary investigations concerning the main geological, botanical and faunistic aspects were carried out in order to plan the environmental upgrading of the whole valley. Specific attention was given to assessing slope instability and identifying the geosites of the area. On the basis of the results attained, proposals of territorial upgrading have been developed which take into account also appraisal measures for geotourism and recreational purposes

    Effects of miglustat treatment in a patient affected by an atypical form of Tangier disease.

    BACKGROUND: Tangier disease (TD) is a rare autosomal recessive disorder, resulting from mutations in the ATP binding cassette transporter (ABCA1) gene. The deficiency of ABCA1 protein impairs high density lipoprotein (HDL) synthesis and cholesterol esters trafficking. CASE REPORT: A 58 year-old female, presenting with complex clinical signs (splenomegaly, dysarthria, dysphagia, ataxia, tongue enlargement, prurigo nodularis, legs lymphedema, pancytopenia and bone marrow foam cells), was misdiagnosed as Niemann-Pick C (NPC) and treated with miglustat (300 mg/day), normalizing neurological symptoms and improving skin lesions and legs lymphedema. Subsequently filipin-staining and molecular analysis for NPC genes were negative. Lipid profiling showed severe deficiency of HDL, 2 mg/dl (n.v. 45-65) and apoAI, 5.19 mg/dl (n.v. 110-170), suggesting TD as a probable diagnosis. Molecular analysis of ABCA1 gene showed the presence of a novel homozygous deletion (c.4464-486_4698 + 382 Del). Miglustat treatment was then interrupted with worsening of some neurological signs (memory defects, slowing of thought processes) and skin lesions. Treatment was restarted after 7 months with neurological normalization and improvement of skin involvement. CONCLUSIONS: These results suggest miglustat as a possible therapeutic approach in this untreatable disease. The mechanisms by which miglustat ameliorates at least some clinical manifestations of TD needs to be further investigated.
