    Frequent and Recent Human Acquisition of Simian Foamy Viruses Through Apes' Bites in Central Africa

    Human infection by simian foamy viruses (SFV) can be acquired by persons occupationally exposed to non-human primates (NHP) or in natural settings. This study aimed at getting better knowledge on SFV transmission dynamics, risk factors for such a zoonotic infection and, searching for intra-familial dissemination and the level of peripheral blood (pro)viral loads in infected individuals. We studied 1,321 people from the general adult population (mean age 49 yrs, 640 women and 681 men) and 198 individuals, mostly men, all of whom had encountered a NHP with a resulting bite or scratch. All of these, either Pygmies (436) or Bantus (1085) live in villages in South Cameroon. A specific SFV Western blot was used and two nested PCRs (polymerase, and LTR) were done on all the positive/borderline samples by serology. In the general population, 2/1,321 (0.2%) persons were found to be infected. In the second group, 37/198 (18.6%) persons were SFV positive. They were mostly infected by apes (37/39) FV (mainly gorilla). Infection by monkey FV was less frequent (2/39). The viral origin of the amplified sequences matched with the history reported by the hunters, most of which (83%) are aged 20 to 40 years and acquired the infection during the last twenty years. The (pro)viral load in 33 individuals infected by a gorilla FV was quite low (<1 to 145 copies per 105 cells) in the peripheral blood leucocytes. Of the 30 wives and 12 children from families of FV infected persons, only one woman was seropositive in WB without subsequent viral DNA amplification. We demonstrate a high level of recent transmission of SFVs to humans in natural settings specifically following severe gorilla bites during hunting activities.     Molecular Ecology and Natural History of Simian Foamy Virus Infection in Wild-Living Chimpanzees

    Identifying microbial pathogens with zoonotic potential in wild-living primates can be important to human health, as evidenced by human immunodeficiency viruses types 1 and 2 (HIV-1 and HIV-2) and Ebola virus. Simian foamy viruses (SFVs) are ancient retroviruses that infect Old and New World monkeys and apes. Although not known to cause disease, these viruses are of public health interest because they have the potential to infect humans and thus provide a more general indication of zoonotic exposure risks. Surprisingly, no information exists concerning the prevalence, geographic distribution, and genetic diversity of SFVs in wild-living monkeys and apes. Here, we report the first comprehensive survey of SFVcpz infection in free-ranging chimpanzees (Pan troglodytes) using newly developed, fecal-based assays. Chimpanzee fecal samples (n = 724) were collected at 25 field sites throughout equatorial Africa and tested for SFVcpz-specific antibodies (n = 706) or viral nucleic acids (n = 392). SFVcpz infection was documented at all field sites, with prevalence rates ranging from 44% to 100%. In two habituated communities, adult chimpanzees had significantly higher SFVcpz infection rates than infants and juveniles, indicating predominantly horizontal rather than vertical transmission routes. Some chimpanzees were co-infected with simian immunodeficiency virus (SIVcpz); however, there was no evidence that SFVcpz and SIVcpz were epidemiologically linked. SFVcpz nucleic acids were recovered from 177 fecal samples, all of which contained SFVcpz RNA and not DNA. Phylogenetic analysis of partial gag (616 bp), pol-RT (717 bp), and pol-IN (425 bp) sequences identified a diverse group of viruses, which could be subdivided into four distinct SFVcpz lineages according to their chimpanzee subspecies of origin. Within these lineages, there was evidence of frequent superinfection and viral recombination. One chimpanzee was infected by a foamy virus from a Cercopithecus monkey species, indicating cross-species transmission of SFVs in the wild. These data indicate that SFVcpz (i) is widely distributed among all chimpanzee subspecies; (ii) is shed in fecal samples as viral RNA; (iii) is transmitted predominantly by horizontal routes; (iv) is prone to superinfection and recombination; (v) has co-evolved with its natural host; and (vi) represents a sensitive marker of population structure that may be useful for chimpanzee taxonomy and conservation strategies

    The effects of integrated care: a systematic review of UK and international evidence

    BACKGROUND: Healthcare systems around the world have been responding to the demand for better integrated models of service delivery. However, there is a need for further clarity regarding the effects of these new models of integration, and exploration regarding whether models introduced in other care systems may achieve similar outcomes in a UK national health service context. METHODS: The study aimed to carry out a systematic review of the effects of integration or co-ordination between healthcare services, or between health and social care on service delivery outcomes including effectiveness, efficiency and quality of care. Electronic databases including MEDLINE; Embase; PsycINFO; CINAHL; Science and Social Science Citation Indices; and the Cochrane Library were searched for relevant literature published between 2006 to March 2017. Online sources were searched for UK grey literature, and citation searching, and manual reference list screening were also carried out. Quantitative primary studies and systematic reviews, reporting actual or perceived effects on service delivery following the introduction of models of integration or co-ordination, in healthcare or health and social care settings in developed countries were eligible for inclusion. Strength of evidence for each outcome reported was analysed and synthesised using a four point comparative rating system of stronger, weaker, inconsistent or limited evidence. RESULTS: One hundred sixty seven studies were eligible for inclusion. Analysis indicated evidence of perceived improved quality of care, evidence of increased patient satisfaction, and evidence of improved access to care. Evidence was rated as either inconsistent or limited regarding all other outcomes reported, including system-wide impacts on primary care, secondary care, and health care costs. There were limited differences between outcomes reported by UK and international studies, and overall the literature had a limited consideration of effects on service users. CONCLUSIONS: Models of integrated care may enhance patient satisfaction, increase perceived quality of care, and enable access to services, although the evidence for other outcomes including service costs remains unclear.     An integrated multi-scale 3D seismic model of the Archaean Yilgarn Craton, Australia

    The collection of a range of different seismic data types has greatly improved our understanding of the crustal architecture of Australia's Archaean Yilgarn Craton over the last few years. These seismic data include broadband seismic studies, seismic receiver functions, wide-angle recordings and mine-scale to deep seismic reflection transects. Each data set provides information on the three-dimensional (3D) tectonic model of the Yilgarn Craton from the craton scale through to the mine scale. This paper demonstrates that the integration and rationalisation of these different seismic data sets into a multi-scale 3D geological/seismic model, that can be visualised at once in a single software package, and incorporating all available data sets, significantly enhances this understanding. This enhanced understanding occurred because the integrated 3D model allowed easy and accurate comparison of one result against another, and facilitated the integrated questioning and interrogation across scales and seismic method. As a result, there are feedback questions regarding understanding of the individual seismic data sets themselves, as well as the Yilgarn Craton as a whole. The methodology used, including all the data sets in the model range, had to allow for the wide range of data sets, frequencies and seismic modes. At the craton scale, P-wave, S-wave and surface wave variations constrained the 3D lithospheric velocity model, revealing noticeable large-scale velocity variations within and across the craton. An interesting feature of the data, easily identified in 3D, is the presence of a fast S-wave velocity anomaly (>4.8 km s-1) within the upper mantle. This velocity anomaly dips east and has a series of step-down offsets that coincide approximately with province and terrane boundaries of the Yilgarn Craton. One-dimensional receiver function profiles show variations in their crustal velocity across the craton. These crustal velocity variations are consistent with the larger-scale geological subdivision of the craton, and provide characteristic profiles for provinces and terranes. The receiver function results and the deep seismic reflection data both agree on the depth to the Moho, and both indicate an increase in Moho depth to the east. The 2D seismic refraction results in the south-west of the craton provide crustal thickness information, an indication of middle and lower crustal compositions, and information regarding the broad-scale architectural framework. At the province- and terrane-scale, the deep seismic reflection data and the mine-scale seismic data provide geometric constraints on crustal architecture, in particular the orientation of the region's fault systems as well as variations in the thickness ofthe granite-greenstone succession. Integration of the results from wide-angle seismic refraction data coincident with the deep seismic reflection data provided additional constraints on likely upper crustal lithologies. The integrated 3D seismic model implies the dominant geodynamic process involved the development of an orogenic belt that developed with a series of contractional (folding and thrusting) events, separated by equally important extensional events. The seismic reflection data in particular suggests that extensional movement on many shear zones was more common than previously thought. The seismic reflection data suggest that the dominant mineral systems involved deeply sourced fluid flowing up crustalpenetrating shear zones. These deeply sourced fluids were further focussed into sites located above fault-breached domal regions in the upper crust

    The timing of gold mineralization across the eastern Yilgarn craton using U–Pb geochronology of hydrothermal phosphate minerals

    The highly mineralized Eastern Goldfields of the eastern Yilgarn craton is an amalgamation of dominantly Neoarchaean granitoid-greenstone terranes and domains that record a history of early rifting, followed by westward directed collision with initial arc formation, collision and clastic basin formation, and final accretion to the western Yilgarn proto-craton between 2.66 and 2.60 billion years ago. The gold deposits that define this region as a world-class gold province are the product of orogenic processes that operated during accretion late in the tectonic history, after initial compressional deformation (D1–D2) and the majority of granitoid magmatism. Minor gold was also deposited throughout the entire tectonic history in magmatic-hydrothermal-related systems. However, such mineralization (mostly < 0.3 g/t gold) is nowhere economic unless it overprints, or is overprinted by, much higher-grade orogenic gold lodes.Robust SHRIMP U–Pb geochronology of gold-related hydrothermal xenotime and monazite supports structural studies that gold mineralization occurred during late transpressional events (D3–D4), shortly before cratonization. However, westward migration of collision and accretion produced a complementary diachroneity in the timing of gold mineralization of 5 to 20 m.y. between c. 2.65 Ma in the east (including Laverton District, Kurnalpi Terrane) to c. 2.63 Ma in the west (including Kalgoorlie Terrane) across the eastern part of the craton. The robust geochronology refutes previous suggestions that significant gold mineralization events extended from DE to D4 in the evolution of the orogen and that the Kalgoorlie gold deposits formed over a period of 45 m.y. The crustal continuum model is applicable within terranes where orogenic gold depositional events were penecontemporaneous, but must be modified to account for diachroneity of orogenic events and gold mineralization across the Eastern Goldfields
