    Health-related quality of life and health preference of Chinese patients with diabetes mellitus managed in primary care and secondary care setting: decrements associated with individual complication and number of complications

    Background: Health-related quality of life (HRQoL) and health preference of patients with diabetes mellitus (DM) are essential in health economic evaluations but data on Chinese population is rare. This study aims to evaluate HRQoL and health preference of diabetic patients with different diabetic complications in Chinese population. Methods: A cross-sectional study was conducted in 1275 patients with DM, including 518 subjects with various DM-related complications. HRQoL and health preference were estimated using SF-12 and SF-6D questionnaires, respectively. Disease status of DM and complications were identified from documented clinical diagnosis. Multivariable regression was used to investigate the effects of specific complications on HRQoL and health preference, adjusting for socio-demographic and clinical parameters. Results: The presence of any diabetic complication was associated with lower physical component summary (−3.81 points, P < 0.01), and end-stage renal disease (ESRD) showed greatest reduction (−7.05 points, P < 0.01). Mental component summary and mental health (MH) scores were not decreased in any of the diabetic complications. The health preference score for diabetic subjects without complications was 0.882 (95% CI, 0.778 to 0.989). The reductions of health preference score were significant for stroke (−0.042, 95% CI -0.072 to −0.012), ESRD (−0.055, 95% CI -0.093 to −0.017), and sight-threatening diabetic retinopathy (STDR) (−0.043, 95% CI -0.075 to −0.010), while heart disease had an insignificant reduction (−0.017, 95% CI -0.042 to 0.008). Conclusions: The presence of any of the four major diabetic complications (heart disease, stroke, ESRD and STDR) was associated with lower HRQoL and health preference scores. Findings of this study facilitated the cost-effectiveness studies of alternative management strategies for prevention of diabetic complications in Chinese population.published_or_final_versio

    We survey results on factorizations of non zero-divisors into atoms (irreducible elements) in noncommutative rings. The point of view in this survey is motivated by the commutative theory of non-unique factorizations. Topics covered include unique factorization up to order and similarity, 2-firs, and modular LCM domains, as well as UFRs and UFDs in the sense of Chatters and Jordan and generalizations thereof. We recall arithmetical invariants for the study of non-unique factorizations, and give transfer results for arithmetical invariants in matrix rings, rings of triangular matrices, and classical maximal orders as well as classical hereditary orders in central simple algebras over global fields.Comment: 50 pages, comments welcom

    Ascites due to right atrial myxoma in a haemodialysis patient

    BACKGROUND: Persistent fluid overload in patients on renal replacement therapy despite good dialysis adequacy or obvious cardiac dysfunction should prompt a search for rarer causes. CASE PRESENTATION: We report here a rare cause of persistent peripheral oedema and ascites in a well-dialysed patient. CT scanning revealed a right atrial myxoma that was later confirmed on an echocardiogram. CONCLUSION: Fluid overload states are common in patients on dialysis. Common causes are inadequacy of dialysis and non-compliance. Where aetiology is not easily apparent further investigations into rarer causes should be sought

    PEMANFAATAN LIMBAH ABU BATU BARA SEBAGAI FILLER PADA CAMPURAN LASTON

    Perencanaan campuran dilakukan dalam dua tahap pembuatan. Pembuatan benda uji tahap pertama dengan menggunakan bahan pengisi (filler) normal yaitu Semen Pertland. Tahap kedua pembuatan benda uji dengan menggunakan bahan pengisi Abu Batu Bara (bottom ash).Penelitian ini dilakukan dengan meninjau dampak dari perbandingan antara kedua campuran dengan menggunakan filler Abu Batu Bara dan Semen Portland dalam campuran lapisan AC-WC. Untuk kedua bahan pengisi tersebut masing-masing dibuat benda uji sebanyak 15 buah, dengan kadar aspal rentang 5-7% dan benda uji direndam selama 30 s/d 40 menit dengan suhu air 60 oC bertujuan untuk mendapatkan nilai kadar Aspal optimum dari kedua campuran tersebut. Dari hasil pengujian kedua campuran dengan filler yang berbeda, ternyata hasil yang di dapat memiliki perbedaan yang tidak terlalu jauh.Ternyata dari hasil kedua campuran dengan filler berbeda menunjukan hasil yang tidak jauh berbeda namun dari hasil tersebut  semuanya masih memenuhi standar spesifikasi umum 2010 dinas pekerjaan umum direktorat jendral bina marga. Hasil akhir menunjukan bahwa dengan menggunakan filler Abu Batu Bara ternyata tidak terlalu berpengaruh terhadap kekuatan campuran. Dan dari hasil kadar Aspal optimum yang didapat dengan menggunakan filler Abu Batu Bara memiliki hasil yg lebih rendah 6,55% dari filler pembanding yaitu 6,85%. Kata kunci: filler Abu Batu Bara, Asphalt Concrete-Wearing Course (AC-WC

    Occurrence of testicular microlithiasis in androgen insensitive hypogonadal mice

    &lt;b&gt;Background&lt;/b&gt;: Testicular microliths are calcifications found within the seminiferous tubules. In humans, testicular microlithiasis (TM) has an unknown etiology but may be significantly associated with testicular germ cell tumors. Factors inducing microlith development may also, therefore, act as susceptibility factors for malignant testicular conditions. Studies to identify the mechanisms of microlith development have been hampered by the lack of suitable animal models for TM.&lt;BR/&gt; &lt;b&gt;Methods&lt;/b&gt;: This was an observational study of the testicular phenotype of different mouse models. The mouse models were: cryptorchid mice, mice lacking androgen receptors (ARs) on the Sertoli cells (SCARKO), mice with a ubiquitous loss of androgen ARs (ARKO), hypogonadal (hpg) mice which lack circulating gonadotrophins, and hpg mice crossed with SCARKO (hpg.SCARKO) and ARKO (hpg.ARKO) mice.&lt;BR/&gt; &lt;b&gt;Results&lt;/b&gt;: Microscopic TM was seen in 94% of hpg.ARKO mice (n=16) and the mean number of microliths per testis was 81 +/- 54. Occasional small microliths were seen in 36% (n=11) of hpg testes (mean 2 +/- 0.5 per testis) and 30% (n=10) of hpg.SCARKO testes (mean 8 +/- 6 per testis). No microliths were seen in cryptorchid, ARKO or SCARKO mice. There was no significant effect of FSH or androgen on TM in hpg.ARKO mice.&lt;BR/&gt; &lt;b&gt;Conclusions&lt;/b&gt;: We have identified a mouse model of TM and show that lack of endocrine stimulation is a cause of TM. Importantly, this model will provide a means with which to identify the mechanisms of TM development and the underlying changes in protein and gene expression

    Evaluation of two interaction techniques for visualization of dynamic graphs

    Several techniques for visualization of dynamic graphs are based on different spatial arrangements of a temporal sequence of node-link diagrams. Many studies in the literature have investigated the importance of maintaining the user's mental map across this temporal sequence, but usually each layout is considered as a static graph drawing and the effect of user interaction is disregarded. We conducted a task-based controlled experiment to assess the effectiveness of two basic interaction techniques: the adjustment of the layout stability and the highlighting of adjacent nodes and edges. We found that generally both interaction techniques increase accuracy, sometimes at the cost of longer completion times, and that the highlighting outclasses the stability adjustment for many tasks except the most complex ones.Comment: Appears in the Proceedings of the 24th International Symposium on Graph Drawing and Network Visualization (GD 2016

    <p>Abstract</p> <p>Introduction</p> <p>Tuberculosis can present in many varied clinical situations in immunosuppressed patients. It has been reported that the sigmoid colon is the most common site for colonic perforation in renal transplant recipients and diverticulitis is its most common cause. Cecal perforation because of tuberculosis is extremely rare in a renal transplant recipient. We present the case of a renal transplant patient with cecal perforation due to tuberculosis, 10 years after renal transplantation.</p> <p>Case presentation</p> <p>A 39-year-old Caucasian man, who was a renal transplant recipient, was admitted to our emergency surgery unit with an acute abdomen. A cecal perforation was found at exploratory laparotomy, and a right hemicolectomy with an end ileostomy and transverse colonic mucous fistula were performed. Necrotizing granulomatous colitis due to tuberculosis was reported in the histopathologic examination.</p> <p>Conclusion</p> <p>Colonic perforations in immunosuppressed patients may have unusual presentations and unusual causes. Tuberculosis infection should be considered in the differential diagnosis during the histopathologic evaluation in immunocompromised patients such as renal transplant recipients.</p
