Signs of the aeolian influence on the formation of coastal-marine sand deposits in the Permian (Republic of Tatarstan, Russia)

Sandy deposits can have a different genesis and form in different sedimentation environments. The example of the tar sands of the Early Permian age of the Republic of Tatarstan (Russia) shows the possibility of their complex and polygenic formation with the cardinal processing of the initial sediments under the influence of superimposed processes. Using paleogeomorphological analysis and mineralogical and lithological studies, the effect of aeolian processes on the formation of a sandy horizon with a two-layer structure has been established. As a result of the short-term sea regression, the coastal-marine sands were partially above the sea level. The upper part of the sandy sediments was subjected to aeolian processes with the blowing of the clay component from the sands, which led to the formation of two benches – the upper sandy and the lower sandy-clayey ones. The boundary between them is the ancient groundwater table (Stokes surface), which can be used as a reference point in the development of geological models

Социально-экономические аспекты лечения детей с синдромом короткой кишки

For many years, organizing and giving parenteral feeding, clinical manifestations of the underlying disease, use of costly drugs and complex expensive methods of examination and treatment have been associated with significant social and economic impact on the families with children suffering from short bowel syndrome. Allocation of resources among families with these children hasn’t been examined in Russia recently. Purpose. To estimate social and economic factors that influence the family with children affected by short bowel syndrome. Methods. Adult members of the families with the affected children born and living in Russia were questioned. 8 families were surveyed in 2014, 8 families – in 2019. Results. Complex social and economic aspects of treating children with short bowel syndrome in Russia were found during the conducted study. Comparative analysis of social and economic factors made it possible to estimate changes in ‘patient-social environment’ relationships within five years, briefly describe the scope of the work performed, delineate unsolved problems and discuss the measures needed to solve the problems. Conclusion. To solve social and economic problems associated with SBS, it is necessary to promote further development and expansion of the existing network of specialized centers dealing with treatment of these patients based on large pediatric hospitals in cities and federal districts of the Russian Federation. The Russian register of patients with short bowel syndrome composed by the Russian association of pediatric surgeons is a key link necessary to improve the quality of diagnostics and treatment of patients with SBS and to develop the state system of funding and provision of the patients with parenteral nutrition. A complex approach that requires participation of the state, federal clinics, charity institutions, creation of specialized centers, and local involvement of doctors will improve treatment outcomes of patients with SBS.Организация и проведение парентерального питания, клинические проявления основного заболевания, а также применение дорогостоящих препаратов и сложных дорогостоящих методов обследования и лечения в течение многих лет связаны со значительной социально-экономической нагрузкой на семьи детей с синдромом короткой кишки. Ресурсное обеспечение семей, включающих детей с синдромом короткой кишки до настоящего времени в России не изучалось. Цель. Оценка социальноэкономических факторов, влияющих на семью, в которой имеется ребёнок с синдромом короткой кишки. Методы. Проведено анкетирование взрослых членов семей детей с синдромом короткой кишки, родившихся и проживающих на территории Российской Федерации. В 2014 году были опрошены 8 семей, в 2019 также 8 семей. Результаты. Проведенное исследование выявило сложные социальноэкономические аспекты лечения детей с синдромом короткой кишки в России. Сравнительная характеристика социально-экономических факторов позволила оценить изменения в системе взаимоотношений «пациент-социальная среда» за пять лет, тезисно показать объём выполненной работы, очертить нерешённые проблемы и обсудить необходимые меры по решению этих проблем. Заключение. Для решения социально-экономических проблем, ассоциированных с СКК, необходимо дальнейшее развитие существующих и расширение сети специализированных центров по лечению таких пациентов на базе крупных детских больниц в городах и федеральных округах Российской Федерации. Созданный ассоциацией детских хирургов России Федеральный регистр больных с синдромом короткой кишки является ключевым звеном для повышения качества диагностики и лечения пациентов с СКК и разработки государственной системы финансирования и обеспечения парентеральным питанием таких пациентов. Комплексный подход, включающий участие государства, федеральных клиник, благотворительных организаций, создание специализированных центров, участие врачей на местах, позволит улучшить результаты лечения пациентов с СКК

ПРИЧИНЫ РАЗВИТИЯ СИНДРОМА КОРОТКОЙ КИШКИ

The short bowel syndrome (SBS) is a severe disorder caused by a significant loss of absorption capacities of the intestine due to its resection manifested as pronounced chronic intestinal insufficiency. We aimed at the investigation of the SBS causes at different intervals. The study was conducted in collaboration with the department of pediatric surgery of the RCCH (Republican Children’s Clinical Hospital) and Mannheim clinics of pediatric surgery at Heidelberg University (Germany). The most frequent causes of SBS in children are necrotizing enterocolitis, gastroschisis, small intestine atresia, twisted bowel, etc. According to our studies, gastroschisis was the most frequently occurred cause whereas necrotizing enterocolitis occupied the 2nd position on the list. Atresia and twisted bowel held the 3rd and 4th positions. However, it should be mentioned that the share of patients with SBS following necrotizing enterocolitis has increased considerably lately.Синдром короткой кишки (СКК) – тяжелое заболевание, которое вызвано значительной потерей абсорбционных возможностей кишечника из‑за его резекции и проявляется выраженной хронической кишечной недостаточностью. Целью данной работы явилось изучение причин развития СКК в разные периоды времени. Исследование проводилось совместно с кафедрой детской хирургии РДКБ и клиникой детской хирургии Маннгейм университета Гайдельберга (Германия). В детском возрасте наиболее частыми причинами развития СКК являются некротический энтероколит, гастрошизис, атрезия тонкой кишки, заворот кишки и др. По результатам наших исследований наиболее частой причиной СКК был гастрошизис. Второй по частоте причиной СКК был некротический энтероколит.На третьем и четвертом месте находились атрезия тонкой кишки и заворот кишки. Однако следует отметить, что доля пациентов с СКК после некротического энтероколита значительно возросла в последние годы
