Follow-up of patients with Hodgkin's disease following curative treatment: the routine CT scan is of little value
A total of 10-40% of patients with Hodgkin's disease relapse following initial curative therapy. Intensive follow-up is resource intensive and may identify false relapses. We performed a retrospective review of all patients with Hodgkin's disease treated at our centre between 1990 and 1999 to evaluate the utility of the components of follow-up. A total of 107 patients met the inclusion and exclusion criteria. The median age was 33 years and the median duration of follow-up 38 months. The total number of follow-up visits was 1209 and total number of CT scans 283. There were 109 suspected relapses of which 22 proved to be true relapses. Of the latter, 14 were identified clinically, six radiologically and two via lab testing. The routine CT scan detected only two relapses (9%), yet accounted for 29% of the total follow-up costs. Based on data from our centre, the cost per true relapse was $6000 US, 49% incurred by radiological tests. The majority of the cost of follow-up was incurred by routine follow-up (84%) as opposed to the investigation of suspected relapses (16%). We conclude that most true relapses are clinically symptomatic and that the routine CT is an expensive and inefficient mode of routine follow-up
Working group written presentation: Trapped radiation effects
The results of the Trapped Radiation Effects Panel for the Space Environmental Effects on Materials Workshop are presented. The needs of the space community for new data regarding effects of the space environment on materials, including electronics are listed. A series of questions asked of each of the panels at the workshop are addressed. Areas of research which should be pursued to satisfy the requirements for better knowledge of the environment and better understanding of the effects of the energetic charged particle environment on new materials and advanced electronics technology are suggested
An assessment of the readiness of ablative materials for preflight application to the shuttle orbiter
The shuttle orbiter relies primarily on a reusable surface insulation (RSI) thermal protection system (TPS). The RSI is very efficient in its thermal performance; however, the RSI tile system has shown poor mechanical integrity. The state-of-the-art of the ablative TPS is reviewed, and an assessment made of the ablator's readiness for use on the shuttle orbiter. Unresolved technical issues with regard to the ablative TPS are identified. Short time, highly focused analytical and experimental programs were initiated to: (1) identify candidate ablation materials; (2) assess the data base for these materials; (3) evaluate the need and kind of waterproof coating; (4) calculate thermal and other stresses in an ablator tile; (5) identify an acceptable ablator/RSI tile joint filler; and (6) assess the sensitivity of the ablator to sequential heat pulses. Results from some of these programs are discussed
Controlling and predicting alkyl-onium electronic structure
X-ray photoelectron spectroscopy (XPS) and ab initio calculations show that fully alkylated onium cation electronic structure can be tuned using both the alkyl chains and the central onium atom. The key for tuning the central onium atom is methyl versus longer alkyl chains, allowing selection of the optimum cation for a wide range of applications, including catalysis and biocides
Impact of climate variability on human health: A pilot study in tertiary care hospital of Eastern Uttar Pradesh, India
This study is an attempt to find out the effect of climate variables on respiratory, cardiovascular, vector-borne and diarrheal diseases from 2004-2013 carried out at Sir Sunder Lal hospital, Varanasi, Uttar Pradesh with focus on eastern Uttar Pradesh. The study shows that cases of Chronic Obstructive Pulmonary Disorder (COPD) and Cardiovascular Disorders (CVD) didn't show any significant relation with any of the climate variables. With increase of 1 C mean maximum monthly temperature the estimated decrease in number of Tuberculosis (TB) patients was 4 (95% CI = 4.95-3.05) while a 1C increase in minimum monthly temperature showed increase of TB patients by 4 (95% CI = 4.95-3.05). One percent increase of monthly averaged relative humidity is estimated to increase the one pneumonia patients (95% CI = 1.95-0.05) at any given month. One-degree increase in given monthly temperature will increase the load of one diarrhea patients (95% CI = 1.95-0.05) monthly. Dengue and Malaria patients showed increasing monthly malaria cases by 5 (95% CI = 5.95-4.05) with 1C rise in minimum monthly temperature and by 1 patient (95% CI = 1.95-0.05) with increase in 1% relative humidity. Encephalitis showed an increase of one patient load (95% CI = 1.95-0.05) with monthly increase of 1C in maximum temperature. The study shows advance knowledge of health information, on timescales of seasons to decades ahead, would aid effective planning of health response measures and infrastructure at local and regional scale
Interactions between vaccinia virus and sensitized macrophages in vitro
The action of peritoneal exudate cells (PEC) from normal and vaccinia virus infected mice on infectious vaccinia virus particles was investigatedin vitro. PEC from immune mice showed a significantly higher infectivity titre reduction (virus clearance, VC) than normal cells. This effect could be clearly attributed to the macrophage. Vaccinia virus multiplied in PEC from normal animals while there was no virus propagation in cells from immunized mice. The release of adsorbed or engulfed virus was reduced significantly in PEC from immunized animals. Anti-vaccinia-antibodies seem to activate normal macrophages to increased virus clearance. This stimulating effect was demonstrable only in the IgG fraction of the antiserum.
The activity of macrophages from mice injected three times over a period of 14 days with vaccinia virus could be entirely blocked with anti-mouse-IgG, while PEC from mice injected one time six days previously were not inhibited
Adiabaticity Conditions for Volatility Smile in Black-Scholes Pricing Model
Our derivation of the distribution function for future returns is based on
the risk neutral approach which gives a functional dependence for the European
call (put) option price, C(K), given the strike price, K, and the distribution
function of the returns. We derive this distribution function using for C(K) a
Black-Scholes (BS) expression with volatility in the form of a volatility
smile. We show that this approach based on a volatility smile leads to relative
minima for the distribution function ("bad" probabilities) never observed in
real data and, in the worst cases, negative probabilities. We show that these
undesirable effects can be eliminated by requiring "adiabatic" conditions on
the volatility smile

constants of beta decay and allow for several stringent tests of the standard

Development and geometry of isotropic and directional shrinkage crack patterns
We have studied shrinkage crack patterns which form when a thin layer of an
alumina/water slurry dries. Both isotropic and directional drying were studied.
The dynamics of the pattern formation process and the geometric properties of
the isotropic crack patterns are similar to what is expected from recent
models, assuming weak disorder. There is some evidence for a gradual increase
in disorder as the drying layer become thinner, but no sudden transition, in
contrast to what has been seen in previous experiments. The morphology of the
crack patterns is influenced by drying gradients and front propagation effects,
with sharp gradients having a strong orienting and ordering effect.Comment: 8 pages, 11 figures, 8 in jpg format, 3 in postscript. See also
