Management of a ruptured epidural catheter, an anesthesiologist's dilemma: a case report
Epidural anesthesia is a widely used anesthetic technique in lower extremity surgeries although it is a relatively safe procedure, it can have complications, such as rupture of the epidural catheter. This is a 69-year-old male patient with a diagnosis of Wagner IV diabetic foot is presented, which was scheduled for left supracondylar amputation in which after epidural block, retention of the catheter tip in the epidural space at level L2-L3 was seen, so hemi laminectomy was performed in a second surgical stage in L2 and removal of the epidural catheter. Ideally a broken needle should be removed as soon as possible
ColonizaciĂłn nasal por Staphylococcus aureus resistente a la meticilina en pacientes sometidos a cirugĂa cardiovascular en un hospital universitario de BogotĂĄ, Colombia
Introduction: Methicillin-resistant Staphylococcus aureus (MRSA) is a microorganism that colonizes nostrils and different parts of the body, which is considered a risk factor to acquire invasive infections, especially in cardiovascular surgery patients.Objective: To determine the frequency of nasal colonization by MRSA and to establish the clinical characteristics in patients scheduled for cardiovascular surgery.Materials and methods: This was a descriptive study conducted between February and December, 2015. We included adult patients scheduled for cardiovascular surgery at the Hospital Universitario San Ignacio in BogotĂĄ, Colombia. Colonization was identified by real-time PCR from nasal swabs. Colonized patients were treated with mupirocin 2.0% intranasally twice a day and bathed with chlorhexidine 4% from the neck downwards for five days. At the end of this treatment, PCR control was carried out.Results: We included 141 patients with a percentage of nasal colonization of 13.4% (19/141). There were 52 hospitalized patients and 89 outpatients with a percentage of nasal colonization of 17.3% (9/52), and 11.2% (10/89), respectively. All colonized patients who received treatment had a negative PCR at the end of the regime and none of the participating patients had a surgical site infection by S. aureus at the end of the study.Conclusions: Nasal colonization was observed both in hospitalized patients and outpatients. Decolonization treatment with mupirocin was effective to eradicate the carrier state in the short term, which could impact the rates of surgical wound infection associated with cardiovascular surgery.IntroducciĂłn. Staphylococcus aureus resistente a la meticilina (SARM) es un microorganismo que coloniza las fosas nasales y diferentes partes del cuerpo, lo cual se considera un factor de riesgo para adquirir infecciones invasivas, especialmente en pacientes sometidos a cirugĂa cardiovascular.Objetivo. Determinar la colonizaciĂłn nasal por SARM y establecer las caracterĂsticas clĂnicas en pacientes programados para cirugĂa cardiovascular.Materiales y mĂ©todos. Se hizo un estudio descriptivo entre febrero y diciembre de 2015. Se incluyeron pacientes adultos programados para cirugĂa cardiovascular en el Hospital Universitario San Ignacio de BogotĂĄ. La colonizaciĂłn se identificĂł mediante reacciĂłn en cadena de la polimerasa (Polymerase Chain Reaction, PCR) en tiempo real en muestras obtenidas mediante hisopados nasales. Los pacientes fueron descolonizados con mupirocina al 2,0 % intranasal dos veces al dĂa y baños con gluconato de clorhexidina al 4 % del cuello hacĂa abajo durante cinco dĂas, al cabo de lo cual se hizo una PCR de control.Resultados. Se incluyeron 141 pacientes, 52 hospitalizados y 89 ambulatorios. Del total, 19 (13,4 %) tenĂan colonizaciĂłn nasal por SARM, correspondientes a 9 (17,3 %) de los 52 hospitalizados y 10 (11,2 %) de los 89 ambulatorios. Todos los pacientes sometidos a descolonizaciĂłn tuvieron resultado negativo en la PCR al final del proceso y ninguno presentĂł infecciĂłn del sitio operatorio por S. aureus.Conclusiones. Se demostrĂł colonizaciĂłn nasal por SARM tanto en los pacientes hospitalizados como en los ambulatorios. La descolonizaciĂłn con mupirocina fue efectiva para erradicar el estado de portador a corto plazo, lo que podrĂa tener efecto en las tasas de infecciĂłn del sitio operatorio en las cirugĂas cardiovasculares

Hidradenitis suppurativa (HS) is a chronic inflammatory disorder that is characterized by recurrence, as well as the characteristic location of skin lesions. Patients usually develop very painful inflammatory nodules that generally end in the formation of multiple abscesses and fistulas that typically occur in the skin of the axillary, inguinal, buttock, and perianal folds. It significantly affects the quality of life of patients, leaving physical, economic and psychological sequelae. There is a wide therapeutic arsenal available, but each patient must be individualized and the best possible treatment determined. Early assessment and intensive treatment of the disease can prevent and even avoid significant sequelae and permanent deformities
Biodegradable electrospun scaffolds for skin wound regeneration: a review
Over the years, skin substitutes have been sought as an alternative for the treatment of different pathologies. In this article, we focus on describing the use of different biodegradable nanofibrillar polymers as skin substitutes in the treatment of acute and chronic wounds, obtained by the electrospinning technique. Electrospinning is a tissue engineering technique used to generate nanofibers of different polymers that are characterized by having a high surface area, low molecular weight, high resistance rates, and nanoporosity, which is why they are particularly interesting for biomedicine, with potential applicability. in the replacement of skin and tubular organs. In this context, the skin created by tissue engineering has high expectations of application in the study of treatment of skin wounds
Current strategies for the reconstruction of the nipple-areola complex: a review
The reconstruction of the nipple-areola complex after a mastectomy is essential for the bio-psycho-social recovery of the patient, it is generally performed 4 to 6 months after surgery and there are multiple surgical reconstruction techniques depending on the experience of the surgeon and of the individual characteristics of the patients. The most widely used for its safety and for having shown the best results is the local flap technique combined with the use of autologous, alloplastic and allograft grafts. However, currently there is still no technique that shows long-term lasting results. For this reason, in this article we describe the five categories of reconstruction techniques for the nipple-areola complex that currently exist, their advantages and disadvantages, as well as the lines of research in tissue engineering in which the world is working to find a therapeutic strategy that can reproduce a nipple-areola complex with the characteristics of the biologic.
Use of rivaroxaban and acetylsalicylic acid as a combined treatment for peripheral arterial disease in Central Military Hospital
Background: The objective of this research was to evaluate the behavior of 3 risk indicators for peripheral arterial disease in patients under oral treatment with rivaroxaban 2.5 mg every 12 hours plus, acetylsalicylic acid 100 mg every 24 hours. It was hypothesized that the oral combination of rivaroxaban and acetylsalicylic acid presents a therapeutic advantage over other treatments.Methods: A prospective longitudinal and non-randomized study of a single center was performed. 59 patients with peripheral arterial disease were included and treated with acetylsalicylic acid + rivaroxaban. Peak systolic velocity, ankle-brachial index and C reactive protein index were evaluated.Results: Significant changes were found at month 1 and 3 of follow-up in maximum systolic velocity, ankle-arm index and C-reactive protein index. The baseline peak systolic velocity (PSV) in the anterior tibial artery had significant differences after one month of treatment (p=0.001) and after 3 months (p=0.001). The baseline PSV in the posterior tibial artery had significant differences compared to the values found at the month of treatment (p=0.001) and 3 months (p=0.001). In the ankle-brachial index a baseline median of 0.790 was found, one month after the treatment of 0.795 (p=0.147) and 3 months after 0.800 (p=0.019). The mean baseline C-reactive protein obtained was 73.142 mg/l, at one month 87.233 mg/l (p=0.001) and at 3 months at 79.009 mg/l (p=0.294) with a standard deviation of 67.18, 74.78 and 69.69 respectively.Conclusions: The combined use of acetylsalicylic acid and rivaroxaban allows a clinical improvement in patients with peripheral arterial disease
