    Investigating family context: an exploratory study to research how therapists use genograms as a therapeutic tool with individuals in one-to-one therapy

    My project focuses on context as a key aspect of my theoretical framework as a counselling psychologist and integrative psychotherapist. In particular, literature about system’s theory and research about transgenerational transmission highlight the importance of paying attention to a client’s family context in the therapeutic process. My practice-based research addresses the need to investigate how therapists work actively with a client’s family system in one-to-one therapy. I chose to explore therapists’ use of genograms (family trees) to understand a client’s family history and dynamics. I conducted semi-structured interviews with nine therapists who use genograms regularly within their ongoing therapeutic work with individuals. My method of data collection and analysis utilised a full, social constructivist version of grounded theory that incorporated an awareness of my experiences and influences using reflexivity. The three main categories I constructed from my data centred on how genograms become integrated into the evolving therapeutic process and relationship, how they create impact and change, and the influence of the therapist’s approach and interventions. Whilst each category can be understood separately, they are inter-related in terms of the overall use and effect of genograms in one-to-one work. My findings suggest that genograms can be powerful and, if used sensitively and appropriately, can enhance the therapeutic relationship and process of personal change. By demonstrating the beneficial use of a holistic and relational tool that has the potential for quicker work by understanding the context of the client’s presenting issues, my research has relevance within current debates about the provision of psychological therapy in statutory services and to the fields of counselling psychology and integrative psychotherapy with regards to how they address context in practice

    Public school art teacher autonomy in a segregated city: Affordances and contradictions

    Over the past two decades, the Chicago Public Schools have seen a lot of change. First there was the opening of magnet schools, and other gestures at reform, followed by school closures and the flourishing of charter schools. In this essay, two former Chicago art teachers, one who taught in a prominent college prep magnet high school on the north side, and one who taught in an under-resourced neighborhood high school on the south side, examine the commonalities of their otherwise divergent experiences, particularly with regard to the freedom allotted to both them and their students by the administrative affordances in their respective situations. While the schools were starkly different in numerous respects, the surprising ability of both teachers to lead collaborative projects that they and their students found engaging, partly through reaching outside the bounds of the institution, may offer an example of teacher autonomy and emergent pedagogy that seems particularly relevant to the public school setting

    Does particle decay cause wave function collapse: An experimental test

    We describe an experimental test of whether particle decay causes wave function collapse. The test uses interference between two well separated, but coherent, sources of vector mesons. The short-lived mesons decay before their wave functions can overlap, so any interference must involve identical final states. Unlike previous tests of nonlocality, the interference involves continuous variables, momentum and position. Interference can only occur if the wave function retains amplitudes for all possible decays.     Changes in serum and synovial fluid biomarkers after acute injury (NCT00332254)

    INTRODUCTION: Acute trauma involving the anterior cruciate ligament is believed to be a major risk factor for the development of post-traumatic osteoarthritis 10 to 20 years post-injury. In this study, to better understand the early biological changes which occur after acute injury, we investigated synovial fluid and serum biomarkers. METHODS: We collected serum from 11 patients without pre-existing osteoarthritis from a pilot intervention trial (5 placebo and 6 drug treated) using an intra-articular interleukin-1 receptor antagonist (IL-1Ra) therapy, 9 of which also supplied matched synovial fluid samples at presentation to the clinic after acute knee injury (mean 15.2 ± 7.2 days) and at the follow-up visit for reconstructive surgery (mean 47.6 ± 12.4 days). To exclude patients with pre-existing osteoarthritis (OA), the study was limited to individuals younger than 40 years of age (mean 23 ± 3.5) with no prior history of joint symptoms or trauma. We profiled a total of 21 biomarkers; 20 biomarkers in synovial fluid and 13 in serum with 12 biomarkers measured in both fluids. Biomarkers analyzed in this study were found to be independent of treatment (P > 0.05) as measured by Mann-Whitney and two-way ANOVA. RESULTS: We observed significant decreases in synovial fluid (sf) biomarker concentrations from baseline to follow-up for (sf)C-Reactive protein (CRP) (P = 0.039), (sf)lubricin (P = 0.008) and the proteoglycan biomarkers: (sf)Glycosaminoglycan (GAG) (P = 0.019), and (sf)Alanine-Arginine-Glycine-Serine (ARGS) aggrecan (P = 0.004). In contrast, we observed significant increases in the collagen biomarkers: (sf)C-terminal crosslinked telopeptide type II collagen (CTxII) (P = 0.012), (sf)C1,2C (P = 0.039), (sf)C-terminal crosslinked telopeptide type I collagen (CTxI) (P = 0.004), and (sf)N-terminal telopeptides of type I collagen (NTx) (P = 0.008). The concentrations of seven biomarkers were significantly higher in synovial fluid than serum suggesting release from the signal knee: IL-1β (P < 0.0001), fetal aggrecan FA846 (P = 0.0001), CTxI (P = 0.0002), NTx (P = 0.012), osteocalcin (P = 0.012), Cartilage oligomeric matrix protein (COMP) (P = 0.0001) and matrix metalloproteinase (MMP)-3 (P = 0.0001). For these seven biomarkers we found significant correlations between the serum and synovial fluid concentrations for only CTxI (P = 0.0002), NTx (P < 0.0001), osteocalcin (P = 0.0002) and MMP-3 (P = 0.038). CONCLUSIONS: These data strongly suggest that the biology after acute injury reflects that seen in cartilage explant models stimulated with pro-inflammatory cytokines, which are characterized by an initial wave of proteoglycan loss followed by subsequent collagen loss. As the rise of collagen biomarkers in synovial fluid occurs within the first month after injury, and as collagen loss is thought to be irreversible, very early treatment with agents to either reduce inflammation and/or reduce collagen loss may have the potential to reduce the onset of future post-traumatic osteoarthritis.     Interference in Exclusive Vector Meson Production in Heavy Ion Collisions

    Photons emitted from the electromagnetic fields of relativistic heavy ions can fluctuate into quark anti-quark pairs and scatter from a target nucleus, emerging as vector mesons. These coherent interactions are identifiable by final states consisting of the two nuclei and a vector meson with a small transverse momentum. The emitters and targets can switch roles, and the two possibilities are indistinguishable, so interference may occur. Vector mesons are negative parity so the amplitudes have opposite signs. When the meson transverse wavelength is larger than the impact parameter, the interference is large and destructive. The short-lived vector mesons decay before amplitudes from the two sources can overlap, and so cannot interfere directly. However, the decay products are emitted in an entangled state, and the interference depends on observing the complete final state.     Geometric representations for minimalist grammars

    We reformulate minimalist grammars as partial functions on term algebras for strings and trees. Using filler/role bindings and tensor product representations, we construct homomorphisms for these data structures into geometric vector spaces. We prove that the structure-building functions as well as simple processors for minimalist languages can be realized by piecewise linear operators in representation space. We also propose harmony, i.e. the distance of an intermediate processing step from the final well-formed state in representation space, as a measure of processing complexity. Finally, we illustrate our findings by means of two particular arithmetic and fractal representations.Comment: 43 pages, 4 figure

    Effect of Injection Rate and Post-Fill Cure Pressure on Properties of Resin Transfer Molded Disks

    The effects of flow rate andpost-fill cure pressure, i.e., packing pressure, on the mechanical properties of resin transfer molded disks are experimentally investigated. An experimental molding setup is constructed to fabricate fiber-reinforced, center-gated, disk-shaped composite parts. Disks are molded at different flow rates and packing pressures in order to observe the effects of these parameters on the mechanical properties andvoidcontent of the final parts. Specimens are cut from three different locations in the molded disks for testing. Specimens from the first two locations are tensile testedto obtain strength and stiffness properties, and the third location is usedfor microscopic analysis to determine void content and void properties. Increased injection rate is found to reduce both the strength and stiffness of the molded parts due to more voids induced by the faster moving fluidfront. Packing pressure is also foundto have a significant effect on specimen properties. At higher packing pressures fewer voids and improved strength andstiffness are observed. Mechanical properties are correlatedwith total void fraction for disks molded at different packing pressures. Exponential decrease in both tensile strength andelastic modulus is observedwith increasing voidfraction. Doubling the voidvolume fraction from 0.35 to 0.72% results in a 15% decrease in strength and a 14% decrease in stiffness. The results demonstrate that selection of suitable injection rate and addition of packing pressure to resin transfer molding (RTM) process can improve mechanical properties of molded parts considerably.Yeshttps://us.sagepub.com/en-us/nam/manuscript-submission-guideline

    Genomic and phenotypic analyses of recent Acinetobacter baumannii isolates from tertiary care hospitals in Thailand

    Antibiotic resistant strains of Acinetobacter baumannii are responsible for a large and increasing burden of nosocomial infections in Thailand and other countries of Southeast Asia. New approaches to their control and treatment are urgently needed and we are actively seeking biological agents that remove the polysaccharide capsules that protect these pathogens from the host’s immune system. To examine phylogenetic relationships, distribution of capsule chemotypes, acquired antibiotic resistance determinants, susceptibility to complement and other traits associated with systemic infection, we sequenced 191 recent isolates from three tertiary referral hospitals in Thailand and used phenotypic assays to characterise key aspects of infectivity. Several distinct lineages were circulating in three hospitals and the majority belonged to global clonal group 2 (GC2). Very high levels of resistance to carbapenems and other front-line antibiotics were found, as were a number of widespread plasmid replicons. A high diversity of capsule genotypes were encountered with only three (KL6, KL10 and KL47) above 10% frequency. Almost 90% of GC2 isolates belonged to the most common capsule genotypes and were fully resistant to the bactericidal action of human serum complement; we attribute this trait to the presence of a substantial protective capsule and for this to represent a key determinant of virulence for systemic infection. We conclude that current Thai nosocomial isolates represent potential targets for therapeutic strategies designed to remove the polysaccharide capsule from extensively drug-resistant A. baumanii during the course of systemic infection

    Ethnically diverse urban transmission networks of Neisseria gonorrhoeae without evidence of HIV serosorting

    Objective We aimed to characterise gonorrhoea transmission patterns in a diverse urban population by linking genomic, epidemiological and antimicrobial susceptibility data. Methods Neisseria gonorrhoeae isolates from patients attending sexual health clinics at Barts Health NHS Trust, London, UK, during an 11-month period underwent whole-genome sequencing and antimicrobial susceptibility testing. We combined laboratory and patient data to investigate the transmission network structure. Results One hundred and fifty-eight isolates from 158 patients were available with associated descriptive data. One hundred and twenty-nine (82%) patients identified as male and 25 (16%) as female; four (3%) records lacked gender information. Self-described ethnicities were: 51 (32%) English/Welsh/Scottish; 33 (21%) white, other; 23 (15%) black British/black African/black, other; 12 (8%) Caribbean; 9 (6%) South Asian; 6 (4%) mixed ethnicity; and 10 (6%) other; data were missing for 14 (9%). Self-reported sexual orientations were 82 (52%) men who have sex with men (MSM); 49 (31%) heterosexual; 2 (1%) bisexual; data were missing for 25 individuals. Twenty-two (14%) patients were HIV positive. Whole-genome sequence data were generated for 151 isolates, which linked 75 (50%) patients to at least one other case. Using sequencing data, we found no evidence of transmission networks related to specific ethnic groups (p=0.64) or of HIV serosorting (p=0.35). Of 82 MSM/bisexual patients with sequencing data, 45 (55%) belonged to clusters of ≥2 cases, compared with 16/44 (36%) heterosexuals with sequencing data (p=0.06). Conclusion We demonstrate links between 50% of patients in transmission networks using a relatively small sample in a large cosmopolitan city. We found no evidence of HIV serosorting. Our results do not support assortative selectivity as an explanation for differences in gonorrhoea incidence between ethnic groups
