Churg-Strauss Syndrome following PTU Treatment
Propylthiouracil (PTU) is a frequently prescribed drug in the treatment
of hyperthyroidism. The use of PTU is, however, accompanied by numerous potentially
serious side effects including vasculitis. PTU-related vasculitides can present as
haematuria, pulmonary haemorrhage, or cutaneous lesion together with aspecific
symptoms such as fever, myalgia, arthralgia, and fatigue. Cerebral involvement is
seldom observed. We present a 49-year-old female with Graves' disease and
asthma, who developed paresis of the proximal extremities, eosinophilia, pulmonary, and cutaneous lesions following treatment with PTU. A cerebral vasculitis consistent with Churg-Strauss syndrome (CSS) was suspected. Although cerebral involvement is seldom observed with PTU treatment, cerebral vasculitis should be considered in patients developing CNS symptoms
Cooling of Active Components in Structurally Integrated Phased Arrays Antennas
This paper presents an innovative cooling solution for active phased array antennas,
using a 3D printed liquid cooling device integrated in the structureâs PCB
High renin and prorenin in plasma and pleural exudate of a patient with the ovarian hyperstimulation syndrome
We present the case of a 35-year-old woman with a severe ovarian hyperstimulation syndrome (OHSS) as a complication of ovulation induction for primary infertility. The clinical picture showed massively enlarged ovaries, pleural effusion and haemoconcentration. She needed a thoracentesis for evacuation of the large pleural effusion. High levels of renin and prorenin were observed in plasma and pleural exudate
NATALIA: a satcom phased array in Ku-band
This article presents the prototype of an electronically steerable receive-only array antenna realised within the ESA-project NATALIA. This project is funded under contract number 18612/04/NL/US. The antenna is conceived for the reception of mobile satellite services in Ku-band and its design targets the market of automotive applications. The design of the prototype is based on an innovative polarisation agile phased array concept and exhibits an excellent RF-performance as well as a very compact envelope
Collecting Service-Based Maintainability Metrics from RESTful API Descriptions: Static Analysis and Threshold Derivation
While many maintainability metrics have been explicitly designed for
service-based systems, tool-supported approaches to automatically collect these
metrics are lacking. Especially in the context of microservices,
decentralization and technological heterogeneity may pose challenges for static
analysis. We therefore propose the modular and extensible RAMA approach
(RESTful API Metric Analyzer) to calculate such metrics from machine-readable
interface descriptions of RESTful services. We also provide prototypical tool
support, the RAMA CLI, which currently parses the formats OpenAPI, RAML, and
WADL and calculates 10 structural service-based metrics proposed in scientific
literature. To make RAMA measurement results more actionable, we additionally
designed a repeatable benchmark for quartile-based threshold ranges (green,
yellow, orange, red). In an exemplary run, we derived thresholds for all RAMA
CLI metrics from the interface descriptions of 1,737 publicly available RESTful
APIs. Researchers and practitioners can use RAMA to evaluate the
maintainability of RESTful services or to support the empirical evaluation of
new service interface metrics.Comment: Accepted at CSE/QUDOS workshop (collocated with ECSA 2020
Red cell distribution width in adults with congenital heart disease: A worldwide available and low-cost predictor of cardiovascular events
Background: Red cell distribution width (RDW) is a standard component of the automated blood count, and is of prognostic value in heart failure and coronary heart disease. We investigated the association between RDW and cardiovascular events in patients with adult congenital heart disease (ACHD). Methods and results: In this prospective cohort study, 602 consecutive patients with ACHD who routinely visited the outpatient clinic were enrolled between 2011 and 2013. RDW was measured in fresh venous blood samples at inclusion in 592 patients (median age 33 [IQR 25-41] years, 58% male, 90% NYHA I) and at four annual follow-up visits. During 4.3 [IQR 3.8-4.7] years of follow-up, the primary endpoint (death, heart failure, hospitalization, arrhythmia, thromboembolic events, cardiac intervention) occurred in 196 patients (33%). Median RDW was 13.4 (12.8-14.1)% versus 12.9 (12.5-13.4)% in patients with and without the primary endpoint (P < 0.001). RDW was significantly associated with the endpoint when adjusted for age, sex, clinical risk factors, CRP, and NT-proBNP (HR 1.20; 95% CI 1.06-1.35; P = 0.003). The C-index of the model including RDW was slightly, but significantly (P = 0.005) higher than the model without (0.74, 95% CI 0.70-0.78 versus 0.73, 95% CI 0.69-0.78). Analysis of repeated RDW measurements (n = 2449) did not show an increase in RDW prior to the occurrence of the endpoint. Conclusions: RDW is associated with cardiovascular events in patients with ACHD, independently of age, sex, clinical risk factors, CRP, and NT-proBNP. This readily available biomarker could therefore be considered as an additive biomarker for risk stratification in these patients

Victimization of patients with severe psychiatric disorders: prevalence, risk factors, protective factors and consequences for mental health. A longitudinal study

Abstract

Background

Victimization among people with a Severe Mental Illness is a common phenomenon. The objectives of this study proposal are: to delineate the extent and kind of victimization in a representative sample of chronic psychiatric patients; to contribute to the development and validation of a set of instruments registering victimization of psychiatric patients; to determine risk factors and protective factors; and to gain insight into the possible consequences of victimization.

Methods/Design

An extensive data set of 323 patients with Sever Mental Illness (assessed 4 years ago) is used. In 2010 a second measurement will be performed, enabling longitudinal research on the predictors and consequences of victimization.

Discussion

The consequences of (re)victimization have barely been subjected to analysis, partially due to the lack of a comprehensive, conceptual model for victimization. This research project will contribute significantly to the scientific development of the conceptual model of victimization in chronic psychiatric patients.</p
