Mixing with the radiofrequency single-electron transistor
By configuring a radio-frequency single-electron transistor as a mixer, we
demonstrate a unique implementation of this device, that achieves good charge
sensitivity with large bandwidth about a tunable center frequency. In our
implementation we achieve a measurement bandwidth of 16 MHz, with a tunable
center frequency from 0 to 1.2 GHz, demonstrated with the transistor operating
at 300 mK. Ultimately this device is limited in center frequency by the RC time
of the transistor's center island, which for our device is ~ 1.6 GHz, close to
the measured value. The measurement bandwidth is determined by the quality
factor of the readout tank circuit.Comment: Submitted to APL september 200
Patient attitudes towards analgesia and their openness to non-pharmacological methods such as acupuncture in the emergency department
Aims: To investigate patient attitudes to analgesia, opioids and non-pharmacological analgesia including acupuncture, in the ED.
Methods: ED patients with pain were surveyed regarding: pain scores, satisfaction, addiction concern, non-pharmacological methods of pain relief, and acupuncture. Data were analysed using logistic regression.
Results: Of 196 adult patients, 52.8% were ‘very satisfied’ with analgesia. Most patients (84.7%) would accept non-pharmacological methods including acupuncture (68.9%) and 78.6% were not concerned about addiction. Satisfaction was associated with male gender, and ‘adequate analgesia’ but not with opioids.
Conclusion: Most patients were generally satisfied with ED analgesia and were open to non-pharmacologic analgesia including acupuncture

Mussels can both outweigh and interact with the effects of terrestrial to freshwater resource subsidies on littoral benthic communities.
Litterfall is an important resource subsidy for lake ecosystems that primarily accumulates in littoral zones. Bivalves are abundant within littoral zones and may modify the effects of terrestrial resource subsidies through trophic interactions and engineering their surrounding habitat. Leaf inputs to lakes and freshwater mussel abundances are changing throughout the boreal ecoregion so we set out to investigate how the co-occurring benthic community might respond. We set up an in situ mesocosm experiment in Ramsey Lake, Sudbury, ON, Canada. Mesocosms contained sediments of either 5% or 35% terrestrial organic matter (tOM), into which we placed mussels (Elliptio complanata) at differing densities (0, 0.4 and 2musselsm-2, with a sham mussel treatment at 0.4musselsm-2). Over one month we recorded the sediment chemistry (dissolved organic carbon, nitrogen and phosphorus), littoral organisms (benthic algae and zooplankton) and mussel growth. At high mussel densities we recorded a 90%, 80%, 45% and 40% reduction in phosphorus, dissolved organic carbon, nitrogen and benthic diatoms, respectively, whereas at low mussel densities we observed a 3-fold increase in zooplankton. We discuss that these results were caused by a combination of bioturbation and trophic interactions. Benthic diatom concentrations were also reduced by 20% in sediments of 35% tOM, likely due to shading and competition with bacteria. Mussel growth increased at high mussel densities but was offset at high tOM, likely due to the organic matter interfering with filter feeding. Our results suggest that mussels can alter the geochemical composition of sediments and abundances of associated littoral organisms, in some cases regardless of tOM quantity. Therefore, the dominant top-down control exerted by freshwater mussels may outweigh bottom-up effects of tOM additions. Generally, our study reveals the importance of considering dominant species when studying the effects of cross-ecosystem resource fluxes
Applications of graphics to support a testbed for autonomous space vehicle operations
Researchers describe their experience using graphics tools and utilities while building an application, AUTOPS, that uses a graphical Machintosh (TM)-like interface for the input and display of data, and animation graphics to enhance the presentation of results of autonomous space vehicle operations simulations. AUTOPS is a test bed for evaluating decisions for intelligent control systems for autonomous vehicles. Decisions made by an intelligent control system, e.g., a revised mission plan, might be displayed to the user in textual format or he can witness the effects of those decisions via out of window graphics animations. Although a textual description conveys essentials, a graphics animation conveys the replanning results in a more convincing way. Similarily, iconic and menu-driven screen interfaces provide the user with more meaningful options and displays. Presented here are experiences with the SunView and TAE Plus graphics tools used for interface design, and the Johnson Space Center Interactive Graphics Laboratory animation graphics tools used for generating out out of the window graphics
Estimating the ground truth from multiple individual segmentations incorporating prior pattern analysis with application to skin lesion segmentation
Noise-enabled precision measurements of a Duffing nanomechanical resonator
We report quantitative experimental measurements of the nonlinear response of
a radiofrequency mechanical resonator, with very high quality factor, driven by
a large swept-frequency force. We directly measure the noise-free transition
dynamics between the two basins of attraction that appear in the nonlinear
regime, and find good agreement with those predicted by the one-dimensional
Duffing equation of motion. We then measure the response of the transition
rates to controlled levels of white noise, and extract the activation energy
from each basin. The measurements of the noise-induced transitions allow us to
obtain precise values for the critical frequencies, the natural resonance
frequency, and the cubic nonlinear parameter in the Duffing oscillator, with
direct applications to high sensitivity parametric sensors based on these

Dermatology undergraduate skin cancer training: a disconnect between recommendations, clinical exposure and competence
<p>Abstract</p> <p>Background</p> <p>Skin cancers are the most common malignancies in Caucasian populations. Non-specialists are responsible for the initial assessment of skin lesions and are required to act as the gatekeepers to dermatological cancer services in many healthcare systems. The majority of such physicians receive very limited formal undergraduate or postgraduate dermatology training. The British Association of Dermatologists (BAD) has produced guidelines that list the lesions that students should be able to diagnose on graduation and the majority of UK medical schools’ operate curricula in keeping with these. There is, however, virtually no evidence as to whether these competencies are being achieved. We set out to determine students’ competence at skin lesion diagnosis and to quantify their clinical exposure to examples of such lesions during their dermatology attachment.</p> <p>Methods</p> <p>Three linked studies were undertaken. In the first, students’ competence was tested by randomized slideshows of images containing the 16 lesions recommended in the UK guidelines. Students’ accuracy was tested at the beginning (Day 1) and end (Day 10) of their clinical placement, with a random sample of students retested 12 months later. Secondly, students’ exposure to these lesions was recorded during their attachments. Finally a survey of the additional dermatological resources used by the students was undertaken.</p> <p>Results</p> <p>Study 1: Students’ diagnostic accuracy increased from 11% on Day 1 to 33% on Day 10 (effect size +2.72). After 12 months half of this effect had disappeared and the students accuracy had dropped to 24%. Study 2: Students’ exposure to the recommended lesions was poor with 82% not even witnessing a single example of each of the 3 major skin cancers. Despite these measurements, only a minority of students reported that they were not confident at diagnosing skin tumours. Study 3: The majority of students use additional resources to supplement their learning.</p> <p>Conclusions</p> <p>In the light of what we know about learning in dermatology, our data would suggest, that the current (traditional) undergraduate attachment is inadequate to meet the UK recommendations for graduate competence. As well as critically examining the basis for these recommendations, we need more empirical data on student performance and exposure, in order to improve teaching and learning.</p
Siren songs or path to salvation? Interpreting the visions of web technology at a UK regional newspaper in crisis, 2006-11
A 5-year case study of an established regional newspaper in Britain investigates journalists about their perceptions of convergence in digital technologies. This research is the first ethnographic longitudinal case study of a UK regional newspaper. Although conforming to some trends observed in the wider field of scholarship, the analysis adds to skepticism about any linear or directional views of innovation and adoption: the Northern Echo newspaper journalists were observed to have revised their opinions of optimum Web practices, and sometimes radically reversed policies. Technology is seen in the period as a fluid, amorphous entity. Central corporate authority appeared to diminish in the period as part of a wider reduction in formalism. Questioning functionalist notions of the market, the study suggests cause and effect models of change are often subverted by contradictory perceptions of particular actions. Meanwhile, during technological evolution, the ‘professional imagination’ can be understood as strongly reflecting the parent print culture and its routines, despite pioneering a new convergence partnership with an independent television company
Dedicated outreach service for hard to reach patients with tuberculosis in London: observational study and economic evaluation.
OBJECTIVE: To assess the cost effectiveness of the Find and Treat service for diagnosing and managing hard to reach individuals with active tuberculosis. DESIGN: Economic evaluation using a discrete, multiple age cohort, compartmental model of treated and untreated cases of active tuberculosis. SETTING: London, United Kingdom. Population Hard to reach individuals with active pulmonary tuberculosis screened or managed by the Find and Treat service (48 mobile screening unit cases, 188 cases referred for case management support, and 180 cases referred for loss to follow-up), and 252 passively presenting controls from London's enhanced tuberculosis surveillance system. MAIN OUTCOME MEASURES: Incremental costs, quality adjusted life years (QALYs), and cost effectiveness ratios for the Find and Treat service. RESULTS: The model estimated that, on average, the Find and Treat service identifies 16 and manages 123 active cases of tuberculosis each year in hard to reach groups in London. The service has a net cost of £1.4 million/year and, under conservative assumptions, gains 220 QALYs. The incremental cost effectiveness ratio was £6400-£10,000/QALY gained (about €7300-€11,000 or 16 000 in September 2011). The two Find and Treat components were also cost effective, even in unfavourable scenarios (mobile screening unit (for undiagnosed cases), £18,000-£26,000/QALY gained; case management support team, £4100-£6800/QALY gained). CONCLUSIONS: Both the screening and case management components of the Find and Treat service are likely to be cost effective in London. The cost effectiveness of the mobile screening unit in particular could be even greater than estimated, in view of the secondary effects of infection transmission and development of antibiotic resistance
