One-Year Clinical, Microbiological and Immunological Results of Local Doxycycline or Antimicrobial Photodynamic Therapy for Recurrent/Persisting Periodontal Pockets: A Randomized Clinical Trial.
We evaluated, in this study, the clinical, microbiological and immunological effects of local drug delivery (LDD) or photodynamic therapy (PDT), adjunctive to subgingival instrumentation (SI) in persistent or recurrent periodontal pockets in patients enrolled in supportive periodontal therapy (SPT) after one year. A total of 105 patients enrolled in SPT with persistent/recurrent pockets were randomly treated with SI +PDT or SI + LDD or SI (control). The number of treated sites with bleeding on probing (n BOP+), probing pocket depths (PPD), clinical attachment level (CAL), full-mouth plaque and bleeding scores (gingival bleeding index, %bleeding on probing-BOP) was evaluated at baseline and after 12 months. Additionally, eight periodontopathogens and the immunomarkers IL-1ÎČ (interleukin)and MMP-8 (matrix metalloprotease) were quantitatively determined using real-time PCR and ELISA, respectively. All three treatments resulted in statistically significant clinical improvements (p < 0.05) without statistically significant intergroup differences (p > 0.05), which were maintained up to 12 months. The presence of BOP negatively affected the PPD and CAL. Moreover, statistically significantly fewer bleeding sites at 12 months were observed in the test groups (p = 0.049). Several periodontopathogens were reduced after 12 months. In conclusion, the present data indicate that in periodontal patients enrolled in SPT, treatment of persistent/recurrent pockets with SI alone or combined with either PDT or LDD may lead to comparable clinical, microbiological and immunological improvements, which are maintained up to 12 months. ERGONAUT: A Tool for Ergonomic Analyses in Virtual Environments
Ergonomic quality is becoming a crucial criterium for the success of many products. The problem of increasing the speed of product development cycles while keeping costs to a minimum could be solved by the possibility of the early evaluation of drafts regarding their ergonomic design. A digital platform is a prerequisite for the analysis of virtual prototypes. The analysis tool ERGONAUT not only fulfills this task in a Virtual Environment (VE) but also makes it possible to combine virtual and real data in a mixed mock-up. This implies that the designer is able to consider the ergonomic point of view at an early stage in the design process. In this context, a project together with the John Deere Company in Mannheim, Germany has been carried out using such an analysis tool in a pilot project. Within this first semi-productive industrial project ERGONAUT has shown its usefulness. It will help companies gain and maintain a long-term leading position in the field of ergonomics..
Comparison of Clear Aligner Treatment in First-Treatment and Re-Treatment Patients: A Retrospective Cohort Study
This retrospective real-world study aimed to evaluate the outcomes of clear aligner treatment (CAT) and patient demographics of patients seeking orthodontic re-treatment who had previously received orthodontic treatment in childhood or adolescence. The study was conducted using anonymized data from DrSmile (Berlin, Germany), a CAT provider in Europe. Data were collected from a total of 2080 patients in Germany (71.5% female, 28.5% male, ranging from 18â70 years, median 29.0 years), who completed their CAT between 1 June and 30 June 2022. Patients were divided into two groups: adult first-treatment patients (n = 1587) and adult re-treatment patients (n = 493). Results showed that re-treatment patients had a shorter treatment duration (p p p p = 0.068), types of insurance (p = 0.615), and treatment satisfaction (p = 0.673). The study highlights the need for orthodontists and dentists to provide adequate information to patients about long-term retention and decision-making for orthodontic treatment, especially for re-treatment
Dynamic Shader Generation for Flexible Multi-Volume Visualization
Imaging) dataset of a human head. The MRI dataset provides the skin and brain tissue. It is vertically cut and the two halves are moved away from each other to get insight to the inner structures. The CTA dataset contains the skull and the vessels which are rendered with different transfer functions. Images (a-c) show three stages of an interactive multi-volume visualization session and image (d) represents the render graph which corresponds to the final configuration in (c). Volume rendering of multiple intersecting volumetric objects is a difficult visualization task, especially if different rendering styles need to be applied to the components, in order to achieve the desired illustration effect. Real-time performance for even complex scenarios is obtained by exploiting the speed and flexibility of modern GPUs, but at the same time programming the necessary shaders turned into a task for GPU experts only. We foresee the demand for an intermediate level of programming abstraction where visualization specialists can realize advanced applications without the need to deal with shader programming intricacies. In this paper, we describe a generic technique for multi-volume render graph. By combining pre-defined nodes, complex volume operations can be realized. Our system efficiently creates GPU-based fragment shader and vertex shader programs âon-theflyâ to achieve the desired visual results. We demonstrate the flexibility of our technique by applying several dynamically generated volume rendering styles to multi-modal medical datasets
The impact of structure dimensions on initial bacterial adhesion
Substrate topography can have profound effects on initial bacterial adhesion during biofilm formation. We applied Staphylococcus epidermidis and Escherichia coli cells onto periodically structured substrates with different structure dimensions, structure types and wetting properties. We found a strong dependence of cell retention on the structure dimensions of the applied substrates. Periodicities in the range of the cell size increased, whereas smaller periodicities decreased cell retention, independent of contact time (minutes to hours) and hydrophobicity. These novel insights on the role of surface topography on bacterial retention might facilitate the development of non-fouling surfaces in the future
Relationships between multiple patient safety outcomes and healthcare and hospital-related risk factors in colorectal resection cases: cross-sectional evidence from a nationwide sample of 232 German hospitals
Objectives Studies analysing colorectal resections usually focus on a specific outcome (eg, mortality) and/or specific risk factors at the individual (eg, comorbidities) or hospital (eg, volume) level. Comprehensive evidence across different patient safety outcomes, risk factors and patient groups is still scarce. Therefore the aim of this analysis was to investigate consistent relationships between multiple patient safety outcomes, healthcare and hospital risk factors in colorectal resection cases.
Design Cross-sectional study.
Setting German inpatient routine care data of colorectal resections between 2016 and 2018.
Participants We analysed 54 168 colon resection and 20 395 rectum resection cases treated in German hospitals. The German Inpatient Quality Indicators were used to define colon resections and rectum resections transparently.
Primary outcome measures Additionally to in-hospital death, postoperative respiratory failure, renal failure and postoperative wound infections we included multiple patient safety outcomes as primary outcomes/dependent variables for our analysis. Healthcare (eg, weekend surgery), hospital (eg, volume) and case (eg, age) characteristics served as independent covariates in a multilevel logistic regression model. The estimated regression coefficients were transferred into ORs.
Results Weekend surgery, emergency admissions and transfers from other hospitals were significantly associated (ORs ranged from 1.1 to 2.6) with poor patient safety outcome (ie, death, renal failure, postoperative respiratory failure) in colon resections and rectum resections. Hospital characteristics showed heterogeneous effects. In colon resections hospital volume was associated with insignificant or adverse associations (postoperative wound infections: OR 1.168 (95% CI 1.030 to 1.325)) to multiple patient safety outcomes. In rectum resections hospital volume was protectively associated with death, renal failure and postoperative respiratory failure (ORs ranged from 0.7 to 0.8).
Conclusions Transfer from other hospital and emergency admission are constantly associated with poor patient safety outcome. Hospital variables like volume, ownership or localisation did not show consistent relationships to patient safety outcomes
