    Measuring Multidimensional Parameters of Poverty Using Alkire and Foster Methodology in Qasimabad: A Case Study

    This paper measures the multidimensional poverty using Alkire and Foster methodology for ten regions of Qasimabad on the primary data. No one indicator alone gives us clear picture of poverty as poverty is multidimensional in nature. We have taken three dimensions having equal weights, education, health and living standard. These dimensions are further divided in ten indicators, two for each, education and health, and six for living standards. Results suggest that region Gul Baig Chandio has the highest multidimensional poverty whereas Muslim Society has the lowest multidimensional poverty among the selected regions of Qasimabad. Results further suggest that the indicators which contribute more to multidimensional poverty are life expectancy, year of schooling, Assets, Improved sanitation, child mortality, flooring and child school attendance. Analyzing the data we came to know that the Percentage of people who are MPI poor in Qasimabad is 45(Incidence of poverty), whereas their average deprivations are 43.27% .Furthermore, Multidimensional poverty Index (MPI) is 19.47% in Qasimabad.     Knowledge about hepatitis B and C among patients attending family medicine clinics in Karachi

    Knowledge about hepatitis B and C was assessed in a cross-sectional study of 300 adults aged 18 or older attending family medicine clinics at The Aga Khan University Hospital, Karachi. Most knew that hepatitis B and C are viral diseases that primarily affect the liver, but knowledge about risk factors for disease transmission was poor. Approximately 70% knew that hepatitis B is vaccine preventable; 60% had the misconception that hepatitis C is also vaccine preventable. The majority incorrectly believed that people with hepatitis B or C should follow the diet \u27parhaiz\u27. Generally women knew more than men about the diseases. This study suggests that health education about these infections should be provided to the public. Family physicians can play an important role in educating people about the prevention of these diseases

    Client Satisfaction Towards Quality of Health Services: an Assessment at Primary Healthcare of District Gujranwala

    This survey designed to evaluate the satisfaction level and the factors that affect the patient satisfaction regarding health care delivery services with the aim to improve the services in the primary health care settings of Gujranwala. A Cross Sectional Study done on randomly selected patients attending the basic health units of Gujranwala, with more than18 years of age. Pretested structured "Liker scale questionnaire" was used for data collection. Out of total respondents, 62 (41.3%) clients were satisfied with the services provided by the basic health units of Gujranwala. The factors identified to determine patient satisfaction were accessibility of services, behavior of staff, health education, level of cleanliness, drug availability and miscellaneous services. Not a single ranked area of satisfaction noticed. Client\u27s occupation and income had significant relationship with the patient satisfaction level. Gender, age, and education of clients were not contributing factors; they not affect the client satisfaction level.Less than half clients were satisfied with the services provided by the basic health units.     An evaluation of topical and local anesthesia in phacoemulsification

    Objective: To assess the comparative efficacy of topical and local anesthesia in phacoemulsification.MATERIAL AND Methods: The medical records of 186 men and women between the ages of 45-85 years, who underwent elective cataract surgery by phacoemulsification technique, under the care of one surgeon, over a period of one year, from March 1999-March 2000 were reviewed.Results: Of 186 patients who underwent small incision, self-healing phacoemulsification cataract surgery, 124 received topical and 62 local anesthesia. The most common cataract types were nuclear sclerosis. The duration of surgery with topical anesthesia was shorter. Sutures and eye padding were more frequently applied for procedures done under local anesthesia. Uncorrected visual acuity in the first post-op week was between 20/20-20/50 for 53.6% of the cases done under topical compared to 30.9% in local anesthesia. A similar trend was noted in the visual acuity one month post operatively.CONCLUSION: The uncorrected visual acuity improves faster and the duration of surgery is shorter when topical anesthesia is used

    Serum Macromineral Levels in Estrual, Fertile, Subfertile and Pregnant Mares Kept Under Two Different Managemental Conditions

    This study was conducted on 300 mares kept under one of the two managemental conditions: field (individual management) and farm (organizational management). Mares were categorized as estrual, fertile, subfertile or pregnant. Any possible relationship between fertility and serum levels of sodium, magnesium, phosphorus, potassium and calcium was investigated. The serum sodium level differed significantly (P<0.05) among all groups of mares at both conditions, with pregnant mares having the highest and subfertile the lowest levels. Also, independent of the condition, the pregnant mares had significantly higher (P<0.05) serum potassium levels compared with subfertile ones. Serum calcium levels were significantly higher in estrual mares when compared with those of pregnant mares under farm management or subfertile mares under field conditions. In each group, mares kept under farm management had significantly higher serum magnesium levels but significantly lower serum calcium levels than those of mares kept under field. In estrual group, mares raised under field condition had significantly higher serum phosphorus levels. These results sufficiently provide the foundation for more rigorous and controlled studies to establish a firm basis for fertility versus serum-mineral-profile relationship. Moreover, due to marginally adequate serum mineral levels in mares kept under both managements, supplementation with mineral mixture was recommended for optimum fertility

    Peculiar histopathological features of giardiasis in distal duodenal biopsies

    Histological changes in 20 Giardia positive duodenal biopsies (Group A) were compared with 50, Giardia negative duodenal biopsies (Group B), taken during the same period. Stool examinations in Group B were negative for Giardia. Surface epithelium, villous and crypt architecture and cellular infiltrates were examined and compared between the groups. Atrophic changes in the villi were more common in Group A as compared to B(P \u3c 0.0001). Intraepithelial neutrophil infiltration (P \u3c 0.001), infiltration of the lamina propria with plasma cells (P \u3c 0.5), and presence of eosinophils in the lamina propria (P \u3c 0.001) were significant findings in group A. Some of the changes were related to the density of Giardia colonization e.g., the goblet cell depletion (P \u3c 0.05) and the density of plasma cell infiltration in lamina propria (P \u3c 0.01). Erosions and ulcerations were less commonly seen in group A. Thus we conclude that giardiasis manifests its peculiar features in the distal duodenal mucosa and a biopsy of this region is an important diagnostic tool for detection of this disease

    Efecto de la hormona de crecimiento sobre los parámetros seminales, el hemograma parcial y el nivel de testosterona en búfalos Nili Ravi

    The purpose of this trial was to examine the possible effects of exogenous growth hormone (GH) on semen quality, hematological variables and serum testosterone concentration in Nili Ravi buffalo. Bulls of the treatment group (n=3) were injected with recombinant GH 500 mg in a 15 day interval for 10 consecutive weeks, while bulls of control group (n=3) received equal volume of normal saline solution. Semen samples were collected twice per week and analyzed for physiological parameters. Blood samples collected fortnightly were analyzed for red blood cells count (RBC), hemoglobin concentration (Hb), packed cell volume (PCV), mean corpuscular volume (MCV), white blood cell (WBC), neutrophils, lymphocytes, and serum testosterone levels. Data were analyzed statistically using t-test. Results indicated that GH treatment of Nili-Ravi buffaloes significantly (p&lt;0.05) increased ejaculatory volume (8.8±0.2 vs 7.7±0.2 ml), sperm motility (72.6±0.4 vs 67.4±0.7%), mass activity (3.35±0.07 vs 2.52±0.08), sperm concentration (982.2±67.8 vs 731.9±50.5 million/ml), live sperm (80.1±0.3 vs 75.8±0.5%) and serum testosterone (4.02±0.21 vs 2.37±0.07 ng/ml) compared to control bulls. Among hematological variables, lymphocytes were increased (p&lt;0.05), but MCV, WBC count, and neutrophils decreased (p&lt;0.05). Hb, RBC and PCV remained unchanged. However, values of these variables were within normal ranges for buffalo bulls. In conclusion, treatment of Nili-Ravi bulls with GH improved semen quality and increased serum testosterone without ostensible adverse effects on the general state.El propósito de este ensayo fue examinar los posibles efectos de la hormona de crecimiento exógena (GH) sobre la calidad del semen, algunas variables del hemograma y la concentración de testosterona en suero de toros bubalinos Nili Ravi. Los búfalos del grupo tratado (n=3) fueron inyectados con 500 mg de somatotropina recombinante cada 15 días durante 10 semanas consecutivas, mientras que los del grupo control (n=3) recibieron igual volumen de solución fisiológica salina. Las muestras de semen fueron tomadas dos veces por semana y analizadas para evaluar los parámetros fisiológicos. Las muestras de sangre colectadas quincenalmente fueron analizadas para establecer el recuento de glóbulos rojos, la  concentración de hemoglobina, el hematocrito, el volumen corpuscular medio (MCV), recuento de glóbulos blancos (WBC), tasa de neutrófilos y linfocitos, así como los niveles séricos de testosterona. Los datos obtenidos fueron analizados estadísticamente por medio del test-t de Student. Los resultados indicaron que el tratamiento con somatotropina aumentó significativamente (p&lt;0,05) el volumen eyaculado (8,8±0,2 versus 7,7±0,2 ml), la motilidad del semen (72,6±0,4 versus 67,4±0,7%), la actividad de masa (3,35±0,07 versus 2,52±0,08), la concentración de espermatozoides (982,2±67,8 versus 731,9±50,5 millones/ml), los espermatozoides vivos (80,1±0,3 versus 75,8±0,5%) y el nivel de testosterona sérica (4,02±0,21 versus 2,37±0,07 ng/ml), comparados con los animales controles. En el hemograma aumentaron los linfocitos (p&lt;0,05) y disminuyeron variables como volumen corpuscular medio, recuento de leucocitos y tasa de neutrófilos (p&lt;0,05). Hemoglobina, concentración de eritrocitos y hematocrito permanecieron inalterados. Todos los parámetros investigados se situaron dentro de los rangos normales para toros bubalinos. En conclusión, el tratamiento de búfalos Nili-Ravi con somatotropina mejoró la calidad del semen y aumentó la testosterona sérica, sin provocar ostensibles efectos adversos en el estado general de los toro

    TRANSVERSE VAGINAL SEPTUM: A CONGENITAL MALFORMATION AND ITS MANAGEMENT IN A FEMALE DROMEDARY CAMEL

    A seven year old female dromedary camel was examined for the complaint of inability to breed due to problem during penile intromission. Vaginoscopy, using equine tube vaginoscope, revealed the presence of a tissue flap cranial to the urethral opening, buldging caudally and separating the cranial and caudal parts of vagina. Digital palpation was also performed and the condition was diagnosed as transverse vaginal septum. For treatment, the vaginal septum was grasped with an Allis tissue forceps and a circular piece was severed from the center with the Metzenbaum scissors. The remaining portion of septum was then carefully trimmed. About 28 days after surgery, the animal showed heat signs and was mated. On ultrasonographic examination three months, post mating the animal was found pregnant

    Inhibition of nitric oxide in LPS-stimulated macrophages of young and senescent mice by δ-tocotrienol and quercetin

    Abstract Background Changes in immune function believed to contribute to a variety of age-related diseases have been associated with increased production of nitric oxide (NO). We have recently reported that proteasome inhibitors (dexamethasone, mevinolin, quercetin, &#948;-tocotrienol, and riboflavin) can inhibit lipopolysaccharide (LPS)-induced NO production in vitro by RAW 264.7 cells and by thioglycolate-elicited peritoneal macrophages derived from four strains of mice (C57BL/6, BALB/c, LMP7/MECL-1-/- and PPAR-&#945;-/- knockout mice). The present study was carried out in order to further explore the potential effects of diet supplementation with naturally-occurring inhibitors (&#948;-tocotrienol and quercetin) on LPS-stimulated production of NO, TNF-&#945;, and other pro-inflammatory cytokines involved in the ageing process. Young (4-week-old) and senescent mice (42-week old) were fed control diet with or without quercetin (100 ppm), &#948;-tocotrienol (100 ppm), or dexamethasone (10 ppm; included as positive control for suppression of inflammation) for 4 weeks. At the end of feeding period, thioglycolate-elicited peritoneal macrophages were collected, stimulated with LPS, LPS plus interferon-&#946; (IFN-&#946;), or LPS plus interferon-&#947; (IFN-&#947;), and inflammatory responses assessed as measured by production of NO and TNF-&#945;, mRNA reduction for TNF-&#945;, and iNOS genes, and microarray analysis. Results Thioglycolate-elicited peritoneal macrophages prepared after four weeks of feeding, and then challenged with LPS (10 ng or 100 ng) resulted in increases of 55% and 73%, respectively in the production of NO of 46-week-old compared to 8-week-old mice fed control diet alone (respective control groups), without affecting the secretion of TNF-&#945; among these two groups. However, macrophages obtained after feeding with quercetin, &#948;-tocotrienol, and dexamethasone significantly inhibited (30% to 60%; P &lt; 0.02) the LPS-stimulated NO production, compared to respective control groups. There was a 2-fold increase in the production of NO, when LPS-stimulated macrophages of quercetin, &#948;-tocotrienol, or dexamethasone were also treated with IFN-&#946; or IFN-&#947; compared to respective control groups. We also demonstrated that NO levels and iNOS mRNA expression levels were significantly higher in LPS-stimulated macrophages from senescent (0.69 vs 0.41; P &lt; 0.05), compared to young mice. In contrast, age did not appear to impact levels of TNF-&#945; protein or mRNA expression levels (0.38 vs 0.35) in LPS-stimulated macrophages. The histological analyses of livers of control groups showed lesions of peliosis and microvesicular steatosis, and treated groups showed Councilman body, and small or large lymphoplasmacytic clusters. Conclusions The present results demonstrated that quercetin and &#948;-tocotrienols inhibit the LPS-induced NO production in vivo. The microarray DNA analyses, followed by pathway analyses indicated that quercetin or &#948;-tocotrienol inhibit several LPS-induced expression of several ageing and pro-inflammatory genes (IL-1&#946;, IL-1&#945;, IL-6, TNF-&#945;, IL-12, iNOS, VCAM1, ICAM1, COX2, IL-1RA, TRAF1 and CD40). The NF-&#954;B pathway regulates the production of NO and inhibits the pro-inflammatory cytokines involved in normal and ageing process. These ex vivo results confirmed the earlier in vitro findings. The present findings of inhibition of NO production by quercetin and &#948;-tocotrienol may be of clinical significance treating several inflammatory diseases, including ageing process.Peer Reviewe

    Suppression of nitric oxide induction and pro-inflammatory cytokines by novel proteasome inhibitors in various experimental models

    <p>Abstract</p> <p>Background</p> <p>Inflammation has been implicated in a variety of diseases associated with ageing, including cancer, cardiovascular, and neurologic diseases. We have recently established that the proteasome is a pivotal regulator of inflammation, which modulates the induction of inflammatory mediators such as TNF-α, IL-1, IL-6, and nitric oxide (NO) in response to a variety of stimuli. The present study was undertaken to identify non-toxic proteasome inhibitors with the expectation that these compounds could potentially suppress the production of inflammatory mediators in ageing humans, thereby decreasing the risk of developing ageing related diseases. We evaluated the capacity of various proteasome inhibitors to suppress TNF-α, NO and gene suppression of TNF-α, and iNOS mRNA, by LPS-stimulated macrophages from several sources. Further, we evaluated the mechanisms by which these agents suppress secretion of TNF-α, and NO production. Over the course of these studies, we measured the effects of various proteasome inhibitors on the RAW 264.7 cells, and peritoneal macrophages from four different strains of mice (C57BL/6, BALB/c, proteasome double subunits knockout LMP7/MECL-1<sup>-/-</sup>, and peroxisome proliferator-activated receptor-α,<sup>-/- </sup>(PPAR-α,<sup>-/-</sup>) knockout mice. We also directly measured the effect of these proteasome inhibitors on proteolytic activity of 20S rabbit muscle proteasomes.</p> <p>Results</p> <p>There was significant reduction of chymotrypsin-like activity of the 20S rabbit muscle proteasomes with dexamethasone (31%), mevinolin (19%), δ-tocotrienol (28%), riboflavin (34%), and quercetin (45%; <b><it>P </it></b>< 0.05). Moreover, quercetin, riboflavin, and δ-tocotrienol also inhibited chymotrypsin-like, trypsin-like and post-glutamase activities in RAW 264.7 whole cells. These compounds also inhibited LPS-stimulated NO production and TNF-α, secretion, blocked the degradation of P-IκB protein, and decreased activation of NF-κB, in RAW 264.7 cells. All proteasome inhibitors tested also significantly inhibited NO production (30% to 60% reduction) by LPS-induced thioglycolate-elicited peritoneal macrophages derived from all four strains of mice. All five compounds also suppressed LPS-induced TNF-α, secretion by macrophages from C57BL/6 and BALB/c mice. TNF-α, secretion, however, was not suppressed by any of the three proteasome inhibitors tested (δ-tocotrienol, riboflavin, and quercetin) with LPS-induced macrophages from LMP7/MECL-1<sup>-/- </sup>and PPAR-α,<sup>-/- </sup>knockout mice. Results of gene expression studies for TNF-α, and iNOS were generally consistent with results obtained for TNF-α, protein and NO production observed with four strains of mice.</p> <p>Conclusions</p> <p>Results of the current study demonstrate that δ-tocotrienol, riboflavin, and quercetin inhibit NO production by LPS-stimulated macrophages of all four strains of mice, and TNF-α, secretion only by LPS-stimulated macrophages of C57BL/6 and BALB/c mice. The mechanism for this inhibition appears to be decreased proteolytic degradation of P-IκB protein by the inhibited proteasome, resulting in decreased translocation of activated NF-κB to the nucleus, and depressed transcription of gene expression of TNF-α, and iNOS. Further, these naturally-occurring proteasome inhibitors tested appear to be relatively potent inhibitors of multiple proteasome subunits in inflammatory proteasomes. Consequently, these agents could potentially suppress the production of inflammatory mediators in ageing humans, thereby decreasing the risk of developing a variety of ageing related diseases.</p
