    Melanoma: A new strategy to reduce morbidity and mortality

    BackgroundPublic awareness campaigns could address risk factors for melanoma to reinforce their sun protection message. The objective of this study is to prioritise risk factors associated with malignant melanoma (MM) to improve public awareness.MethodsDesign: A cross-sectional study with retrospective data analysis from 2004 to 2010. Setting: Western Australian Melanoma Advisory Service (WAMAS), a tertiary referral multidisciplinary organisation providing MM management advice. WAMAS data files were analysed with histologically confirmed cutaneous MM. Forty-seven patients had two or more melanomas, but the patient file was counted only once. Six MM data files with missing or incomplete information were excluded. Main outcome measures: The number of naevi, blood relatives with MM, and previous sunburns were the primary variables collected. ResultsThe results showed that 70.9 per cent (268/378) had previous sunburn; 40.2 per cent (152/378) had multiple naevi; and 22.5 per cent (85/378) had a positive family history. In the 110 MM data files not associated with sunburn, multiple naevi and a positive family history represented 34.5 per cent (38/110) and 20.0 per cent (22/110), respectively. ConclusionsThe results confirm the findings of previous studies that multiple naevi and a positive family history are important risk factors associated with MM. We suggest that MM can be detected earlier and its mortality decreased by focusing on these high-risk groups who are not targeted by current public awareness campaigns

    Intraoperative detection of blood vessels with an imaging needle during neurosurgery in humans

    Intracranial hemorrhage can be a devastating complication associated with needle biopsies of the brain. Hemorrhage can occur to vessels located adjacent to the biopsy needle as tissue is aspirated into the needle and removed. No intraoperative technology exists to reliably identify blood vessels that are at risk of damage. To address this problem, we developed an “imaging needle” that can visualize nearby blood vessels in real time. The imaging needle contains a miniaturized optical coherence tomography probe that allows differentiation of blood flow and tissue. In 11 patients, we were able to intraoperatively detect blood vessels (diameter, \u3e500 μm) with a sensitivity of 91.2% and a specificity of 97.7%. This is the first reported use of an optical coherence tomography needle probe in human brain in vivo. These results suggest that imaging needles may serve as a valuable tool in a range of neurosurgical needle interventions

    Treatment of white coat HYpertension in the Very Elderly Trial (HYVET 2) - feasibility of a randomized controlled trial (study protocol)

    The results of HYpertension in the Very Elderly Trial (HYVET) were crucial in providing evidence of benefit of the treatment of hypertension in those 80 years or older. Following a subsequent sub study analysis of the HYVET data there is a suggestion that 50% of patients in the main study had White Coat Hypertension (WCH), defined as clinic BP readings >140/90 mmHg and ambulatory BP readings <135/85 mmHg. Currently, definitive evidence in support of treatment for such individuals is not available. HYVET 2 has been designed in order to assess the feasibility of conducting a randomized controlled trial which might determine whether the treatment of WCH in the very elderly is clinically beneficial. One hundred participants aged ≥75 years diagnosed with WCH will be recruited from General Practices (GPs) in UK. Randomization will be 1:1 to a treatment arm (indapamide and perindopril) and control arm (no treatment) and follow up will be for 52 weeks. HYVET 2 will report on feasibility outcomes including participant recruitment, adherence and withdrawal rates, willingness of GPs to recruit and randomize patients and the frequency of a composite of cardiovascular events.     A diagnostic autoantibody signature for primary cutaneous melanoma

    Melanoma is an aggressive form of skin cancer that is curable by surgical excision in the majority of cases, if detected at an early stage. To improve early stage melanoma detection, the development of a highly sensitive diagnostic test is of utmost importance. Here we aimed to identify antibodies to a panel of tumour associated antigens that can differentiate primary melanoma patients and healthy individuals. A total of 245 sera from primary melanoma patients and healthy volunteers were screened against a high-throughput microarray platform containing 1627 functional proteins. Following rigorous statistical analysis, we identified a combination of 10 autoantibody biomarkers that, as a panel, displays a sensitivity of 79%, specificity of 84% and an AUC of 0.828 for primary melanoma detection. This melanoma autoantibody signature may prove valuable for the development of a diagnostic blood test for routine population screening that, when used in conjunction with current melanoma diagnostic techniques, could improve the early diagnosis of this malignancy and ultimately decrease the mortality rate of patients

    Elevated temperature and nutrients lead to increased N2O emissions from salt marsh soils from cold and warm climates

    Salt marshes can attenuate nutrient pollution and store large amounts of ‘blue carbon’ in their soils, however, the value of sequestered carbon may be partially offset by nitrous oxide (N2O) emissions. Global climate and land use changes result in higher temperatures and inputs of reactive nitrogen (Nr) into coastal zones. Here, we investigated the combined effects of elevated temperature (ambient + 5℃) and Nr (double ambient concentrations) on nitrogen processing in marsh soils from two climatic regions (Quebec, Canada and Louisiana, U.S.) with two vegetation types, Sporobolus alterniflorus (= Spartina alterniflora) and Sporobolus pumilus (= Spartina patens), using 24-h laboratory incubation experiments. Potential N2O fluxes increased from minor sinks to major sources following elevated treatments across all four marsh sites. One day of potential N2O emissions under elevated treatments (representing either long-term sea surface warming or short-term ocean heatwaves effects on coastal marsh soil temperatures alongside pulses of N loading) offset 15–60% of the potential annual ambient N2O sink, depending on marsh site and vegetation type. Rates of potential denitrification were generally higher in high latitude than in low latitude marsh soils under ambient treatments, with low ratios of N2O:N2 indicating complete denitrification in high latitude marsh soils. Under elevated temperature and Nr treatments, potential denitrification was lower in high latitude soil but higher in low latitude soil as compared to ambient conditions, with incomplete denitrification observed except in Louisiana S. pumilus. Overall, our findings suggest that a combined increase in temperature and Nr has the potential to reduce salt marsh greenhouse gas (GHG) sinks under future global change scenarios

As the crystallization temperature T-x increased, the PEO or PLLA lamellar crystal thickness d(L) increased as well as the reduced tethering density (σ) over tilde of the PS chains. The onset of tethered PS chains overcrowding in solution occurs at (σ) over tilde*similar to3.7-3.8 as evidenced by an abrupt change in the slope between (d(L))(-1) and T-x. This results from the extra surface free energy created by the tethered chain that starts to affect the growth barrier of the crystalline blocks

    What Ukraine Taught NATO about Hybrid Warfare

    Russia’s invasion of Ukraine in 2022 forced the United States and its NATO partners to be confronted with the impact of hybrid warfare far beyond the battlefield. Targeting Europe’s energy security, Russia’s malign influence campaigns and malicious cyber intrusions are affecting global gas prices, driving up food costs, disrupting supply chains and grids, and testing US and Allied military mobility. This study examines how hybrid warfare is being used by NATO’s adversaries, what vulnerabilities in energy security exist across the Alliance, and what mitigation strategies are available to the member states. Cyberattacks targeting the renewable energy landscape during Europe’s green transition are increasing, making it urgent that new tools are developed to protect these emerging technologies. No less significant are the cyber and information operations targeting energy security in Eastern Europe as it seeks to become independent from Russia. Economic coercion is being used against Western and Central Europe to stop gas from flowing. China’s malign investments in Southern and Mediterranean Europe are enabling Beijing to control several NATO member states’ critical energy infrastructure at a critical moment in the global balance of power.
