    A new smart mobile system for chronic wound care management

    Nonhealing wounds pose a major challenge in clinical medicine. Typical chronic wounds, such as diabetic foot ulcers and venous leg ulcers, have brought substantial difficulties to millions of patients around the world. The management of chronic wound care remains challenging in terms of precise wound size measurement, comprehensive wound assessment, timely wound healing monitoring, and efficient wound case management. Despite the rapid progress of digital health technologies in recent years, practical smart wound care management systems are yet to be developed. One of the main difficulties is in-depth communication and interaction with nurses and doctors throughout the complex wound care process. This paper presents a systematic approach for the user-centered design and development of a new smart mobile system for the management of chronic wound care that manages the nurse's task flow and meets the requirements for the care of different types of wounds in both clinic and hospital wards. The system evaluation and satisfaction review was carried out with a group of ten nurses from various clinical departments after using the system for over one month. The survey results demonstrated high effectiveness and usability of the smart mobile system for chronic wound care management, in contrast to the traditional pen-and-paper approach, in busy clinical contexts

    Generalized Category Discovery in Semantic Segmentation

    This paper explores a novel setting called Generalized Category Discovery in Semantic Segmentation (GCDSS), aiming to segment unlabeled images given prior knowledge from a labeled set of base classes. The unlabeled images contain pixels of the base class or novel class. In contrast to Novel Category Discovery in Semantic Segmentation (NCDSS), there is no prerequisite for prior knowledge mandating the existence of at least one novel class in each unlabeled image. Besides, we broaden the segmentation scope beyond foreground objects to include the entire image. Existing NCDSS methods rely on the aforementioned priors, making them challenging to truly apply in real-world situations. We propose a straightforward yet effective framework that reinterprets the GCDSS challenge as a task of mask classification. Additionally, we construct a baseline method and introduce the Neighborhood Relations-Guided Mask Clustering Algorithm (NeRG-MaskCA) for mask categorization to address the fragmentation in semantic representation. A benchmark dataset, Cityscapes-GCD, derived from the Cityscapes dataset, is established to evaluate the GCDSS framework. Our method demonstrates the feasibility of the GCDSS problem and the potential for discovering and segmenting novel object classes in unlabeled images. We employ the generated pseudo-labels from our approach as ground truth to supervise the training of other models, thereby enabling them with the ability to segment novel classes. It paves the way for further research in generalized category discovery, broadening the horizons of semantic segmentation and its applications.     Development and external validation of a COVID-19 mortality risk prediction algorithm: a multicentre retrospective cohort study

    Objective This study aimed to develop and externally validate a COVID-19 mortality risk prediction algorithm.Design Retrospective cohort study.Setting Five designated tertiary hospitals for COVID-19 in Hubei province, China.Participants We routinely collected medical data of 1364 confirmed adult patients with COVID-19 between 8 January and 19 March 2020. Among them, 1088 patients from two designated hospitals in Wuhan were used to develop the prognostic model, and 276 patients from three hospitals outside Wuhan were used for external validation. All patients were followed up for a maximal of 60 days after the diagnosis of COVID-19.Methods The model discrimination was assessed by the area under the receiver operating characteristic curve (AUC) and Somers’ D test, and calibration was examined by the calibration plot. Decision curve analysis was conducted.Main outcome measures The primary outcome was all-cause mortality within 60 days after the diagnosis of COVID-19.Results The full model included seven predictors of age, respiratory failure, white cell count, lymphocytes, platelets, D-dimer and lactate dehydrogenase. The simple model contained five indicators of age, respiratory failure, coronary heart disease, renal failure and heart failure. After cross-validation, the AUC statistics based on derivation cohort were 0.96 (95% CI, 0.96 to 0.97) for the full model and 0.92 (95% CI, 0.89 to 0.95) for the simple model. The AUC statistics based on the external validation cohort were 0.97 (95% CI, 0.96 to 0.98) for the full model and 0.88 (95% CI, 0.80 to 0.96) for the simple model. Good calibration accuracy of these two models was found in the derivation and validation cohort.Conclusion The prediction models showed good model performance in identifying patients with COVID-19 with a high risk of death in 60 days.     Genome sequence of the palaeopolyploid soybean

    Soybean (Glycine max) is one of the most important crop plants for seed protein and oil content, and for its capacity to fix atmospheric nitrogen through symbioses with soil-borne microorganisms. We sequenced the 1.1-gigabase genome by a whole-genome shotgun approach and integrated it with physical and high-density genetic maps to create a chromosome-scale draft sequence assembly. We predict 46,430 protein-coding genes, 70% more than Arabidopsis and similar to the poplar genome which, like soybean, is an ancient polyploid (palaeopolyploid). About 78% of the predicted genes occur in chromosome ends, which comprise less than one-half of the genome but account for nearly all of the genetic recombination. Genome duplications occurred at approximately 59 and 13 million years ago, resulting in a highly duplicated genome with nearly 75% of the genes present in multiple copies. The two duplication events were followed by gene diversification and loss, and numerous chromosome rearrangements. An accurate soybean genome sequence will facilitate the identification of the genetic basis of many soybean traits, and accelerate the creation of improved soybean varieties.
