The coupling coordination between health service supply and regional economy in China: spatio-temporal evolution and convergence
BackgroundThe coordination of health service supply and regional economy is an integral path to promote China’s prosperity.MethodsBased on the coupling mechanism of health service supply and regional economy, we sampled the data from 30 provinces in China from 2009 to 2021 in this study and constructed the evaluation index system. Additionally, we calculated the coupling coordination degree (HED) of the two through the coupling coordination degree model. We further used the kernel density estimation, Moran’s I index, and spatial β convergence model to assess the dynamic evolution trends, spatial aggregation effect, and spatial convergence characteristics of coupling coordination.Conclusion(1) HED in China showed a rising trend during the study period but with large regional differences, forming a gradient distribution pattern of “high in the east and low in the west.” (2) The results of Kernel density estimation show that HED has formed a gradient differentiation phenomenon within each region in China. (3) HED has modeled spatial clustering characteristics during the study period, with high-value clusters mainly appearing in the eastern region and low-value clusters appearing in the northwestern region. (4) There are absolute β-convergence and conditional β-convergence trends in HED in China and the three major regions during the study period, but there is an obvious regional heterogeneity in the control factors. The research provides a reference for accurately implementing policies according to different levels of health service supply and economic development, in addition to narrowing the regional differences of the coupling coordination between the regional economy and health service supply
Tuning Sn-Cu Catalysis for Electrochemical Reduction of CO<sub>2</sub> on Partially Reduced Oxides SnOx-CuOx-Modified Cu Electrodes
Copper-based bimetallic catalysts have been recently showing promising performance for the selective electrochemical reduction of CO2. In this work, we successfully fabricated the partially reduced oxides SnOx, CuOx modified Cu foam electrode (A-Cu/SnO2) through an electrodeposition-annealing-electroreduction approach. Notably, in comparison with the control electrode (Cu/SnO2) without undergoing annealing step, A-Cu/SnO2 exhibits a significant enhancement in terms of CO2 reduction activity and CO selectivity. By investigating the effect of the amount of the electrodeposited SnO2, it is found that A-Cu/SnO2 electrodes present the characteristic Sn-Cu synergistic catalysis with a feature of dominant CO formation (CO faradaic efficiency, 70~75%), the least HCOOH formation (HCOOH faradaic efficiency, <5%) and the remarkable inhibition of hydrogen evolution reaction. In contrast, Cu/SnO2 electrodes exhibit a SnO2 coverage-dependent catalysis—a shift from CO selectivity to HCOOH selectivity with the increasing deposited SnO2 on Cu foam. The different catalytic performance between Cu/SnO2 and A-Cu/SnO2 might be attributed to the different content of Cu atoms in SnO2 layer, which may affect the density of Cu-Sn interface on the surface. Our work provides a facile annealing-electroreduction strategy to modify the surface composition for understanding the metal effect towards CO2 reduction activity and selectivity for bimetallic Cu-based electrocatalysts
Predictive significance of systemic immune-inflammation index combined with prealbumin for postoperative pneumonia following lung resection surgery
Abstract Background We aimed to determine whether systemic immune-inflammation index (SII) combined with prealbumin can provide better predictive power for postoperative pneumonia in patients undergoing lung resection surgery. Methods We identified eligible patients undergoing lung resection surgery at the Affiliated Hospital of Nantong University from March 2021 to March 2022. Demographic characteristics, clinical data, and laboratory information were collected and reviewed from the electronic medical records of the patients. To test the effect of the combined detection of SII and prealbumin, we made an equation using logistic regression analysis. The receiver operating characteristic curve (ROC) was plotted to evaluate the predictive powers, sensitivity, and specificity of prealbumin, SII, and SII combined with prealbumin. Decision curve analysis (DCA) was used to determine the clinical validity and net benefit of different methods of detection. Results Totally 386 eligible patients were included with a median age of 62.0 years (IQR: 55.0, 68.0), and 57 (14.8%) patients presented with postoperative pneumonia within 7 days after surgery. The multivariate regression analysis showed that preoperative SII as continuous variable was associated with an increased risk of postoperative pneumonia (OR: 1.38, 95% CI: 1.19–2.83, P = 0.011), whereas the prealbumin as continuous variable remained as an independent protective predictor of postoperative pneumonia in the adjusted analysis (OR: 0.80, 95% CI: 0.37–0.89, P = 0.023). Compared to SII or prealbumin, the combined detection of preoperative SII and prealbumin showed a higher predictive power with area under curve of 0.79 (95% CI: 0.71–0.86, P < 0.05 for all). Additionally, DCA indicated that the combined detection was superior over preoperative SII or prealbumin alone in clinical validity and net benefit. Conclusion Both preoperative SII and prealbumin are independent influencing factors for postoperative pneumonia after lung resection surgery. The combined detection of preoperative SII and prealbumin can significantly improve prediction capability to identify potential postoperative pneumonia-susceptible patients, facilitating early interventions to improve postoperative quality of life for surgical lung resection patients
Neonatal Meningitis-Causing <i>Escherichia coli</i> Induces Microglia Activation which Acts as a Double-Edged Sword in Bacterial Meningitis
Bacterial meningitis is a devastating disease occurring worldwide, with up to half of survivors left with permanent neurological sequelae. Neonatal meningitis-causing Escherichia coli (NMEC) is the most common Gram-negative bacillary organism that causes meningitis, particularly during the neonatal period. Here, RNA-seq transcriptional profiles of microglia in response to NMEC infection show that microglia are activated to produce inflammatory factors. In addition, we found that the secretion of inflammatory factors is a double-edged sword that promotes polymorphonuclear neutrophil (PMN) recruitment to the brain to clear the pathogens but, at the same time, induces neuronal damage, which may be related to the neurological sequelae. New neuroprotective therapeutic strategies must be developed for the treatment of acute bacterial meningitis. We found that transforming growth factor-β (TGF-β) may be a strong candidate in the treatment of acute bacterial meningitis, as it shows a therapeutic effect on bacterial-meningitis-induced brain damage. Prevention of disease and early initiation of the appropriate treatment in patients with suspected or proven bacterial meningitis are the key factors in reducing morbidity and mortality. Novel antibiotic and adjuvant treatment strategies must be developed, and the main goal for new therapies will be dampening the inflammatory response. Based on this view, our findings may help develop novel strategies for bacterial meningitis treatment

Quality appraisal of clinical practice guidelines addressing massage interventions using the AGREE II instrument
Abstract Objective The purpose of this study was to systematically evaluate the methodological quality of massage-related clinical practice guidelines (CPGs)/consensus on massage using the Appraisal of Guidelines Research and Evaluation (AGREE II) instrument and to summarize the current status of recommendations in the CPGs. Methods The Chinese National Knowledge Infrastructure (CNKI), WanFang Data, China Science and Technology Journal Database (VIP), China Biology Medicine disc (CBM), PubMed, Embase, and guideline websites (such as the Chinese Medical Ace Base, the China Association of Chinese Medicine, the World Health Organization, Guideline International Network, National Institute for Health and Care Excellence, Scottish Intercollegiate Guidelines Network) were searched from inception to October 31, 2022. In addition, the reference lists of relevant studies were reviewed to identify domestic and overseas massage CPGs/consensus. The search terms adopted a combination of subject words and free words, mainly including traditional Chinese medicine, complementary therapies, Tuina, massage, manipulation, chiropractic/osteopathic, spinal, acupressure, guideline, and consensus. Two researchers independently completed the eligible records and extracted the data. Before the formal research, calibrations were performed twice on AGREE II, and all reviewers completed the pilot test three times until they understood and reached an agreement on the assessment items. Three researchers appraised the methodological quality of the included guidelines using the AGREE II instrument and calculated the overall intraclass correlation coefficient (ICC) of agreement. Results The evaluation results showed that among the 49 eligible CPGs/consensus, 4 (8.2%) CPGs/consensus were considered “recommended”, 15 (30.6%) CPGs/consensus were considered “recommended with modifications”, and 30 (61.2%) CPGs/consensus were considered “not recommended”, while the consensus was considered “not recommended”. Generally, the scores in the six domains of the guidelines were all higher than the consensus. Evaluation results for the overall quality of 36 CPGs showed that 4 (11%) were “good quality”, 15 (42%) were “sufficient quality” and 17 (47%) were “lower quality”. The AGREE II quality scores of domains ranged from 0.30 to 0.75 ([ICC = 0.993, 95% CI (0.992, 0.995)]). The domain of scope and purpose (domain 1), with a median score of 0.75 (0.52~0.91), performed best in the guidelines with AGREE II, and stakeholder involvement (domain 2) [median 0.39 (0.31~0.56)] and application (domain 5) [median 0.30 (0.17~0.47] obtained lower scores. The consensus score of domain 1 was better at 26.0 (21.6~44.8), followed by rigor of development (domain 3) with a score of 18.0 (10.0~28.9). A total of 119 massage-related recommendations were extracted from 49 guidelines/consensuses, including “in favor” (102, 85.7%), “against” (9, 7.6%), and “did not make recommendations” (8, 6.7%). Conclusion The overall quality of the included guidelines was low, and most of the guidelines were not “recommended”. In future guideline updates, the existing evidence should be used, the professional composition of members of the expert group should be enriched, and patients’ values and preferences should be fully considered. It is necessary to clearly propose recognizable recommendations and strengthen the rigor and standardization of guideline formulation. Thus, clear standard guidelines can be formulated to better guide clinical practice
