    A rare cause of shoulder pain: Osteoid osteoma of the scapular glenoid treated with cryoablation

    We report a case of an osteoid osteoma at the scapular glenoid that was treated with cryoablation. The patient presented with shoulder pain with subsequent CT and MRI imaging findings suspicious for an osteoid osteoma. The patient complained of persistent pain despite medical treatment and was referred to interventional radiology for consideration of ablation. CT-guided biopsy confirmed the diagnosis of osteoid osteoma and the patient underwent cryoablation of the lesion in the same session. The patient reported a substantial relief in her pain at a follow-up 4 weeks after the procedure. MRI performed 8 weeks after the procedure also confirmed radiographic remission of the lesion

    Quality Control of Danggui Buxue Tang, a Traditional Chinese Medicine Decoction, by H-NMR Metabolic Profiling

    Danggui Buxue Tang (DBT) is one of the simplest traditional Chinese medicine (TCM) decoctions, first described in China in 1247 AD. DBT is composed of 2 herbs, Astragali Radix (AR) and Angelica Sinensis Radix (ASR), boiled together in a 5 : 1 ratio. Clinically, DBT is prescribed to women as a remedy for menopausal symptoms. Here, H-NMR metabolic profiling was conducted for DBT and the water extracts of AR or ASR, to evaluate the potential of this chemical profiling method for quality control of the herbal decoction. Principal component analysis (PCA) showed that DBT could be readily distinguished from the water extracts of its constituent herbs by the metabolic profiles. More interestingly, the metabolic profile of DBT was not a simple sum of that of AR and ASR. Asparagine was found at significantly higher concentration in DBT than that in either AR or ASR extract, contributing mainly to the discrimination of DBT sample. In addition, we employed the same method to profile a commercial DBT powder, verifying its authenticity as compared to our prepared DBT. This study is the first to employ H-NMR metabolic profiling for the quality control of traditional Chinese medicine decoctions

    Influence of surface water availability on mammal distributions in Nonggang National Nature Reserve, Guangxi, China

    Surface water is a major limiting factor affecting animal activities in karst ecosystems. From March, 2006 to June, 2007 and from October, 2010 to May, 2011, infra-red camera traps were installed along animal trails and temporary rain pools in Nonggang National Nature Reserve, Guangxi, China, to monitor mammal diversity and relative abundance. In total, 19 species from 17 genera, 12 families, and 5 orders were recorded, including two State Key Protection Class I species, the François’ langur ( Trachypithecus francoisi ) and Assam macaque ( Macaca assamensis ). Although 42% of species only occurred in one of the microhabitats, differences in species assemblages between trails and pools were not significant. The results of our observation indicated that camera trapping was effective in monitoring medium to large sized mammals, and for recording illegal hunting. In addition, our results suggest that authorities should reinforce patrolling, especially at water pools during the dry season, and eradicate unsustainable extraction of underground water. Moreover, based on the advantages of large inhibited environments to animal species, especially to large predators, we also recommend connecting the three isolated sections of the reserve to promote species recovery and dispersal

    Trends in Prevalence and Mortality of Dementia in Elderly Hong Kong Population: Projections, Disease Burden, and Implications for Long-Term Care

    Background. We describe the trends in prevalence and mortality of dementia among older people in Hong Kong over time. Projections of the number of older people with dementia through 2039 and estimation of the disease burden are also included. Methods. Prevalence data were extracted from previous studies in Hong Kong. Mortality data were obtained from the Department of Health of Hong Kong. Projections of the number of people with dementia were calculated by applying the prevalence rates of dementia obtained from previous studies to Hong Kong population projections. The burden of dementia was measured by Disability-Adjusted Life Years (DALYs). Results. The number of people aged 60 and above with dementia is projected to increase by 222%, from 103,433 in 2009 to 332,688 in 2039, with a large proportion of those living in institutions. The number of deaths due to dementia among people aged 60 and above has more than doubled between 2001 and 2009. Mortality rates for dementia have also risen. In 2006, about 286,313 DALYS were lost due to dementia. Conclusions. The information presented may be used to formulate a long-term care strategy for dementia of the ageing population in Hong Kong

    Immediate effects of modified landing pattern on a probabilistic tibial stress fracture model in runners

    Background: Tibial stress fracture is a common injury in runners. This condition has been associated with increased impact loading. Since vertical loading rates are related to the landing pattern, many heelstrike runners attempt to modify their footfalls for a lower risk of tibial stress fracture. Such effect of modified landing pattern remains unknown. This study examined the immediate effects of landing pattern modification on the probability of tibial stress fracture. Methods: Fourteen experienced heelstrike runners ran on an instrumented treadmill and they were given augmented feedback for landing pattern switch. We measured their running kinematics and kinetics during different landing patterns. Ankle joint contact force and peak tibial strains were estimated using computational models. We used an established mathematical model to determine the effect of landing pattern on stress fracture probability. Findings: Heelstrike runners experienced greater impact loading immediately after landing pattern switch (P 0.986). Interpretation: Immediate transitioning of the landing pattern in heelstrike runners may not offer timely protection against tibial stress fracture, despite a reduction of impact loading. Long-term effects of landing pattern switch remains unknown
