    Estimating transmission probability in schools for the 2009 H1N1 influenza pandemic in Italy

    BACKGROUND: Epidemic models are being extensively used to understand the main pathways of spread of infectious diseases, and thus to assess control methods. Schools are well known to represent hot spots for epidemic spread; hence, understanding typical patterns of infection transmission within schools is crucial for designing adequate control strategies. The attention that was given to the 2009 A/H1N1pdm09 flu pandemic has made it possible to collect detailed data on the occurrence of influenza-like illness (ILI) symptoms in two primary schools of Trento, Italy. RESULTS: The data collected in the two schools were used to calibrate a discrete-time SIR model, which was designed to estimate the probabilities of influenza transmission within the classes, grades and schools using Markov Chain Monte Carlo (MCMC) methods. We found that the virus was mainly transmitted within class, with lower levels of transmission between students in the same grade and even lower, though not significantly so, among different grades within the schools. We estimated median values of R 0 from the epidemic curves in the two schools of 1.16 and 1.40; on the other hand, we estimated the average number of students infected by the first school case to be 0.85 and 1.09 in the two schools. CONCLUSIONS: The discrepancy between the values of R 0 estimated from the epidemic curve or from the within-school transmission probabilities suggests that household and community transmission played an important role in sustaining the school epidemics. The high probability of infection between students in the same class confirms that targeting within-class transmission is key to controlling the spread of influenza in school settings and, as a consequence, in the general population

    The course of cancer related fatigue up to ten years in early breast cancer patients. What impact in clinical practice?

    Little is known about the cancer related fatigue (CRF) along cancer course and risk factors that could predict CRF development and persistence in breast cancer (BC) survivors. This prospective study detected incidence, timing of onset, duration of CRF, impact on QoL and psychological distress. Seventy-eight early BC patients, undergoing chemotherapy (CT) followed or not by hormonal therapy were assessed for QoL and psychological distress by EORTC QLQC30 and HADs questionnaires. Fatigue was investigated with mix methods, structured interview and psychometric measures. A qualitative analysis was added to assess the behavioral pattern of CRF. Low fatigue levels were identified after surgery (9%), increasing during (49%) and at the end of CT (47%), maintaining after 1 year (31%) and declining up to ten years of follow-up. Prevalence of CRF was higher at the end of CT and lower at follow-up. At the end and after 1 and 2 years from CT, persistence of CRF was associated to anxiety in 20%, 11% and 5% and to depression in 15%, 10% and 5% respectively. A relationship between CRF and psychological distress was observed; patients presenting depression and anxiety before CT were at higher risk for fatigue onset at a later period. A relationship between fatigue and QoL was noted at the end of CT. Our study shows the fatigue timely trend in early BC patients from surgery, CT and follow-up. Identification of biological, psychological, social predictor factors related to fatigue could be helpful for early interventions in patients at higher risk of developing fatigue

    Heterogeneous pair approximation for voter models on networks

    For models whose evolution takes place on a network it is often necessary to augment the mean-field approach by considering explicitly the degree dependence of average quantities (heterogeneous mean-field). Here we introduce the degree dependence in the pair approximation (heterogeneous pair approximation) for analyzing voter models on uncorrelated networks. This approach gives an essentially exact description of the dynamics, correcting some inaccurate results of previous approaches.     Circular motion of neutral test particles in Reissner-Nordstr\"om spacetime

    We investigate the motion of neutral test particles in the gravitational field of a mass MM with charge QQ described by the Reissner-Nordstr\"om (RN) spacetime. We focus on the study of circular stable and unstable orbits around configurations describing either black holes or naked singularities. We show that at the classical radius, defined as Q2/MQ^2/M, there exist orbits with zero angular momentum due to the presence of repulsive gravity. The analysis of the stability of circular orbits indicates that black holes are characterized by a continuous region of stability. In the case of naked singularities, the region of stability can split into two non-connected regions inside which test particles move along stable circular orbits.Comment: 23 pages, 22 figures.     Measuring Egyptian Farmers’ Attitude Towards Staying Organic

    Organic agriculture (OA) in Egypt is well-developed and still fast growing. Improving the relation between organic farmers and the other agents in the chain can provide a positive contribution to the whole organic chain competitiveness. One possible approach to investigate the farmers’ perceived role and satisfaction within the organic system is to explore the factors influencing their decision to stay organic. In particular, the aim of the present study was to measure the farmers’ attitude towards staying organic. Organic agricultural experts and institutional stakeholders were interviewed to complete a literature review and to obtain information about the Egyptian context. The survey questionnaire was pre-tested (n = 13) and then administered to a different sample (n = 232). A split-half validation procedure was used to evaluate and then confirm the factor structure. Explorative and confirmatory factor analysis yielded a final 29-item measure consisting of 8 distinct factors showing how organic agriculture influences a broad range of farmers’ life dimensions (environmental, economic, social, psychological). The significant role played by psychological and social factors in defining the farmers’ decision to stay organic emerged as a relatively unexpected outcome. The study supports the sustainable development of small family farmers, providing a useful tool to support the growth of organic production and consumption, mostly in developing countries. By monitoring farmers’ attitudes and perception towards OA, the instrument proposed in the present study can support policy makers, farmers’ organizations, civil society organizations (NGOs) and organic chains focal companies when defining policies, advocating campaigns, and chain coordination strategies for farmers involved in the organic food system development

    Lymphocyte Subsets and Inflammatory Cytokines of Monoclonal Gammopathy of Undetermined Significance and Multiple Myeloma

    Almost all multiple myeloma (MM) cases have been demonstrated to be linked to earlier monoclonal gammopathy of undetermined significance (MGUS). Nevertheless, there are no identified characteristics in the diagnosis of MGUS that have been helpful in differentiating subjects whose cancer may progress to a malignant situation. Regarding malignancy, the role of lymphocyte subsets and cytokines at the beginning of neoplastic diseases is now incontestable. In this review, we have concentrated our attention on the equilibrium between the diverse lymphocyte subsets and the cytokine system and summarized the current state of knowledge, providing an overview of the condition of the entire system in MGUS and MM. In an age where the therapy of neoplastic monoclonal gammopathies largely relies on drugs capable of acting on the immune system (immunomodulants, immunological checkpoint inhibitors, CAR-T), detailed knowledge of the the differences existing in benign and neoplastic forms of gammopathy is the main foundation for the adequate and optimal use of new drugs

    Diabetes and pancreatic cancer-a dangerous liaison relying on carbonyl stress

    Both type 2 (T2DM) and type 1 (T1DM) diabetes mellitus confer an increased risk of pancreatic cancer in humans. The magnitude and temporal trajectory of the risk conferred by the two forms of diabetes are similar, suggesting a common mechanism. Carbonyl stress is a hallmark of hyperglycemia and dyslipidemia, which accompanies T2DM, prediabetes, and obesity. Accumulating evidence demonstrates that diabetes promotes pancreatic ductal adenocarcinoma (PDAC) in experimental models of T2DM, a finding recently confirmed in a T1DM model. The carbonyl stress markers advanced glycation end-products (AGEs), the levels of which are increased in diabetes, were shown to markedly accelerate tumor development in a mouse model of Kras-driven PDAC. Consistently, inhibition of AGE formation by trapping their carbonyl precursors (i.e., reactive carbonyl species, RCS) prevented the PDAC-promoting effect of diabetes. Considering the growing attention on carbonyl stress in the onset and progression of several cancers, including breast, lung and colorectal cancer, this review discusses the mechanisms by which glucose and lipid imbalances induce a status of carbonyl stress, the oncogenic pathways activated by AGEs and their precursors RCS, and the potential use of carbonyl-scavenging agents and AGE inhibitors in PDAC prevention and treatment, particularly in high-risk diabetic individuals

    Diabetic complications and oxidative stress: A 20‐year voyage back in time and back to the future

    Twenty years have passed since Brownlee and colleagues proposed a single unifying mechanism for diabetic complications, introducing a turning point in this field of research. For the first time, reactive oxygen species (ROS) were identified as the causal link between hyperglycemia and four seemingly independent pathways that are involved in the pathogenesis of diabetes-associated vascular disease. Before and after this milestone in diabetes research, hundreds of articles describe a role for ROS, but the failure of clinical trials to demonstrate antioxidant benefits and some recent experimental studies showing that ROS are dispensable for the pathogenesis of diabetic complications call for time to reflect. This twenty‐year journey focuses on the most relevant literature regarding the main sources of ROS generation in diabetes and their role in the pathogenesis of cell dysfunction and diabetic complications. To identify future research directions, this review discusses the evidence in favor and against oxidative stress as an initial event in the cellular biochemical abnormalities induced by hyperglycemia. It also explores possible alternative mechanisms, including carbonyl stress and the Warburg effect, linking glucose and lipid excess, mitochondrial dysfunction, and the activation of alternative pathways of glucose metabolism leading to vascular cell injury and inflammation
