    Serous carcinoma cervix: a rare and aggressive carcinoma mimicking ovarian malignancy

    Cervical cancers are the most common gynaecological cancers affecting women in India. Only rarely it is seen that cervical cancers metastasize to the ovaries. Serous carcinoma of cervix is a rare histological variant of adenocarcinoma of cervix, which is an aggressive tumour which usually presents in advanced stages and with metastasis to local and distant organs. Its abnormal and metastatic presentation disguising as ovarian malignancy often deceits the clinical and surgical decisions. Here we reported a rare presentation of primary endocervical cancer masquerading as ovarian carcinoma, which was treated with staging laparotomy followed by chemotherapy and patient had a good response with resolution of tumour masses

    Trabeculectomy Augmented with Limited Deep Sclerectomy and Cyclodialysis with Use of Scleral Tissue as a Spacer

    Trabeculectomy remains the most commonly performed surgery for medically uncontrolled glaucoma. Its success in primary open angle glaucoma is approximately 82% in the initial year after surgery and 64% at the end of five years. Lower success rates have been found in secondary glaucomas like neovascular glucoma, uvietic glaucoma, post-traumatic glaucoma, and for repeat surgeries. To illustrate improvement of the efficacy of trabeculectomy, enhancement with cyclodialysis has been introduced. This involves the creation of a cyclodialysis cleft in a controlled manner to allow additional suprachoroidal drainage of the aqueous. Cyclodialysis is the result of the separation of the longitudinal ciliary muscle fibers from the scleral spur, which creates an additional pathway for aqueous humor drainage. However, such a cleft often closes on its own due to associated inflammation caused by the filtration surgery. Deep sclerectomy is a non-penetrating surgery that involves dissection of a scleral patch and excision of a block of scleral tissue, retaining a thin membrane for aqueous drainage. In this study, we introduce a novel surgical technique of combining trabeculectomy with a limited deep sclerectomy and a cyclodialysis in two pseudophakic patients who developed secondary glaucoma after vitreo-retinal surgery with silicone oil insertion. In this technique the excised scleral tissue obtained after deep sclerectomy was utilized as a spacer to maintain the patency of the cyclodialysis cleft

    Lockdown and Insomnia among Undergraduate Healthcare Students: A Cross-Sectional Study

    INTRODUCTION: Insomnia is a risk factor for various physical and mental disorders as well can affect the academic performance of a student(s). AIM: To assess the prevalence of insomnia among university going students (medical, dental and nursing streams) in the South Asian continent during the lockdown due to the COVID-19 pandemicMATERIALS AND METHOD: The present study was conducted amongst 743 medical, dental and nursing undergraduate students residing in South Asia using convenience sampling. Data was collected using a pre-tested and pre-validated questionnaire [the Athens Insomnia Scale (AIS)] using google forms and had a total of 8 questions (score range 0-3) . Final scores (the individual AIS score) were obtained by adding the scores for each question (range 0 -24). The higher the score was, the worse was the sleep quality; students with score of ≥ 6 were considered insomniac. Data Analysis was done using SPSS version 21.012 by using the independent samples t-test, and multiple logistic regression.RESULTS: A total of 921 entries were recorded, out of which 743 were complete and hence, were included in the study (response rate: 80.7%). Insomnia was reported in 421 (56.7%) students, out of which, the highest was seen among dental (62.7%), followed by medical (59.8%) and nursing (45.3%) undergraduates. The highest range of AIS was observed among females (6-22) and dental students (6-21). Gender differences revealed a significant association among females in both range obtained (t-test) (p=0.03) as well as the multiple linear regression analyzing insomnia in relation to gender (p=0.03).CONCLUSION: There is a need to regularly assess insomnia among students and to take preventive measures incase of high prevalence is found among them, especially while pursuing academics online and from their homes due to the pandemic

    Manual loop in laparoscopic appendectomy: A retrospective cohort study and literature review

    Objectives: To determine the incidence of complications [Surgical site infection (SSI), intra-abdominal abscess (IAA), stump leak] related to stump ligation with manual loop of sliding extracorporeal suture knot in laparoscopic appendectomy. Methods: This cohort study was conducted on patients who underwent laparoscopic appendectomy from June 2014 to November 2020 performed by the same surgeon with almost similar technique. Stump was ligated with manual loops, applied by the surgeon or trainee or both (one by surgeon and other by trainee). SSI and IAA were classified according to Centers for Disease Control and Prevention (CDC) criteria. Results: Total 120 patients were included with median (Interquartile range, IQR) age of 24 (19-35) years and male predominance i.e. 81 (67.5%). Median (IQR) for the duration of symptoms, time from presentation to surgery and duration of surgery was 2(1-4) days, 10 (4-15) hours and 60 (44-70) minutes, respectively. SSI was documented in 9(7.5%) patients, managed by wound hygiene and antibiotics. IAA was observed in one(0.8%) patient who required readmission for antibiotics and radiology guided drain placement. No stump leak was observed.Conclusions: Manual endo-loop is a safe, reliable and cost effective technique for stump ligation in LA, and can safely be incorporated into teaching of surgical trainees

    Characteristics and Prevalence of Latent Autoimmune Diabetes in Adults (LADA)

    Diabetes, one of the most commonly seen metabolic disorders, is affecting a major area of population in many developing as well as most of the developed countries and is becoming an alarming concern for the rising cost of the healthcare system. Latent Autoimmune Diabetes in Adults (LADA) is a form of diabetes which is less recognized and underdiagnosed type of diabetes which appears to have characteristics of both type 1 (autoimmune in nature) and type 2 diabetes (adult age at onset and initial response to oral hypoglycemic agents). An epidemiological study was carried out on 500 patients in the western region of India. Various parameters such as age at onset, duration of diabetes, gender, basal metabolic index (BMI), type of diabetes, family history, HbA1c levels, cholesterol levels, and current treatment regimen were evaluated and correlated with type 1 and type 2 diabetes. Moreover, diagnostic markers for LADA, namely, GAD autoantibodies and C-peptide levels, were determined for 80 patients selected from the epidemiological study. Some of the results obtained were found to be consistent with the literature whereas some results were found to be contradictory to the existing data

    Textual Query Based Image Retrieval

    As digital cameras becoming popular and mobile phones are increased very fast so that consumers photos are increased. So that retrieving the appropriate image depending on content or text based image retrieval techniques has become very vast. Content-based image retrieval, a technique which uses visual contents to search images from large scale image databases according to users interests, has been an active and fast advancing research area semantic gap between the low-level visual features and the high-level semantic concepts. Real-time textual query-based personal photo retrieval system by leveraging millions of Web images and their associated rich textual descriptions. Then user provides a textual query. Our system generates the inverted file to automatically find the positive Web images that are related to the textual query as well as the negative Web images that are irrelevant to the textual query. For that purpose we use k-Nearest Neighbor (kNN), Decision stumps, and linear SVM, to rank personal photos. For improvement of the photo retrieval performance, we have used two relevance feedback methods via cross-domain learning, which effectively utilize both the Web images and personal images.     Population differentiation in dromedarian camel: a comparative study of camel inhabiting extremes of geographical distribution

    Abstract: The camel has not been subject to selective pressures and is not differentiated. The present study was undertaken to evaluate differentiation of camels inhabiting the extreme geographical habitats for the species. In this paper we compared the camel populations of India and Southern region of Africa to adjudge the differentiation between the two populations. Utilizing 12 microsatellite loci it has been demonstrated that the populations i.e., of India and South Africa are very well differentiated. The camel population of India shows differentiation among themselves owing to selection pressure and breeding for specific economic traits/parameters like carting, baggage, riding etc. The differentiation among the two camel populations was 23% while that of among population, within the two groups was 11%. A small group of Bactrian camel exhibited closeness to hill camels. Based on the published literature and in the present study it is evident that there is a general trend of increased population differentiation from South region of Africa to Indian subcontinent with India having highly differentiated populations

    Conceptual Design Of Cargo Airplane

    The main goal of the work is to design the military cargo aircraft that fulfils all the requirements. Current work includes weight estimation of an aircraft, selection of airfoil and suitable wing configuration, selection of tail, fuselage sizing and power plant selection. From the available details, weight estimation of the aircraft was started, by assuming the soldiers weight and baggage allowance, with the available empirical relations, weight estimation was done. There are many conditions to select feasible airfoil for the aircraft, with the consideration of design Mach number and design lift coefficient airfoil was selected, then for aircraft flight regime, suitable wing configurations was selected. Primary objective of fuselage is to accommodate the soldiers, crew members in cockpit, and cargo, to place all these in the fuselage, space was sized and proper aisle was given in between with reference to the military standards. Need of Empennage is to provide the stability for aircraft, by checking the required stability for aircraft, horizontal and vertical tail was sized and suitable configuration was selected from the historical trends and requirement. In the design stage feasible engine for the aircraft was selected
