    FORCE- AND POWER-TIME RELATIONSHIP,EMG RESPONSES IN CONCENTRIG AND ISOMETRIC CONDITIONS,EFFECTS OF TRAINING AND INDIVIDUAL CRARACTERISTICS

    Introduction The relationship between EHG (temporal pattern, IEMG, power spectra ) and the biomechanical parameter in isometric and dynamic muscle tension represents an essential method in the analysis of the specificity of training effects and of the individual characteristics in motor control modelling and in practical motor learning. METHODS Force-time, power-time relationships under concentric and isometric conditions (MVC) with and without preloading were determined. For EHG purpose, surface electrodes (O= 5 mm; interelectrode distance 40 mm) were placed over the belly of each muscle (Tric.br.;Bic.br.; Delt.an.; Pect.ma.). Bipolar myoelectrical potentials were recorded with the passive electrode placed between the two actives: signals were preamplified and band-pass filtered (CMRRT 70 db, BP= 10 Hz- 1 KHz: Zin=1,5 MR. gain= 1000). EMG signals and the force were digitized on-line with a sampling frequency of 1000 Hz. For time structure analysis EMG signals were full-wave rectified and bandpass filtered (20-70 Hz) to obtain envelop curve patterns of each muscle. Power spectra analysis (median frequency MF) were performed using 1024 and pl2 data points. 3 groups (n=4) of sport students were involved in the investigation. Subjects performed a 8 week training period under different loading conditions. RESULTS # In the concentric tasks subjects show individual strategies in the muscular coordination patterns dependent on the load intensity. # Differences in the muscular interactivity pattern are not necessarily matching similar variation in the mechanical output. # For increasing loads there is a linear relationship between muscular activation level and mechanical power output. # In the power spectra, trends are consistent but also discontinuous. Median frequency could possibly indicate that some subjects have resources in motor units recruitment . # Other considerations are discussed referring to the muscular activation and the isometric MVC under preloading conditions

    Sclerotherapy with 3% polidocanol foam to treat second-degree haemorrhoidal disease. Three-year follow-up of a multicentre, single arm, IDEAL phase 2b trial

    Background Sclerotherapy with 3% polidocanol foam is becoming increasingly popular for the treatment of symptomatic I-II or III degree haemorrhoidal disease (HD). However, there are no studies that have reported a follow-up of more than 1 year. The purpose of this study was to analyse the long-term outcomes of sclerotherapy with 3% polidocanol foam in the treatment of II-degree HD. Methods This was an open label, single-arm, phase 2b trial conducted in 10 tertiary referral centres for HD. A total of 183 patients with II-degree HD, aged between 18 and 75 years with symptomatic HD according to the Goligher classification and unresponsive to medical treatment, were included in the study and underwent sclerotherapy with 3% polidocanol foam. The efficacy was evaluated in terms of bleeding score, haemorrhoidal disease symptom score (HDSS) and short health scale for HD (SHS-HD) score. Successful treatment was defined as the complete absence of bleeding episodes after 7 days (T1) according to the bleeding score. Results The overall success rate ranged from 95.6% (175/183) at 1 year to 90.2% (165/183) after the final 3 year follow-up. The recurrence rate, based on the primary outcome, ranged from 12% (15/125) to 28% (35/125). The greatest increase in recurrence (15) was recorded between 12 and 18 months of follow-up, then another five between 18 and 24 months. Both the HDSS and the SHS score remained statistically significant (p < 0.001) from a median preoperative value of 11 (10-13) and 18 (15-20) to 0 (0-2) and 4 (0-4), respectively. Symptom-free (HDSS = 0) patients, excluding patients converted to surgery, increased from 55.5% (101/182) at 1 year to 65.1% at 3 years (110/169). There were no intraoperative complications in redo-sclerotherapy nor additional adverse events (AEs) compared to the first 12 months. Conclusions Sclerotherapy with 3% polidocanol foam is gradually establishing itself in the treatment of bleeding HD due to its repeatability, safety, convenience in terms of direct and indirect costs with the absence of discomfort for the patient as well as AEs rather than an excellent overall success rate

    Efficacy of Mesoglycan in Pain Control after Excisional Hemorrhoidectomy: A Pilot Comparative Prospective Multicenter Study

    Introduction. Various pain management strategies for patients undergoing open excisional hemorrhoidectomy have been proposed, yet postoperative pain remains a frequent complaint. Objective. To determine whether mesoglycan (30 mg two vials i.m. once/day for the first 5 days postoperative, followed by 50 mg 1 oral tablet twice/day for 30 days) would reduce the edema of the mucocutaneous bridges and thus improve postoperative pain symptoms. Patients and Methods. For this prospective observational multicenter study, 101 patients undergoing excisional diathermy hemorrhoidectomy for III-IV degree hemorrhoidal disease were enrolled at 5 colorectal referral centers. Patients were assigned to receive either mesoglycan (study group SG) or a recommended oral dose of ketorolac tromethamine of 10 mg every 4–6 hours, not exceeding 40 mg per day and not exceeding 5 postoperative days according to the indications for short-term management of moderate/severe acute postoperative pain, plus stool softeners (control group CG). Results. Postoperative thrombosis (SG 1/48 versus CG 5/45) (p<0.001) and pain after rectal examination (p<0.001) were significantly reduced at 7–10 days after surgery in the mesoglycan-treated group, permitting a faster return to work (p<0.001); however, in the same group, the incidence of postoperative bleeding, considered relevant when needing a readmission or an unexpected outpatient visit, was higher, possibly owing to the drug’s antithrombotic properties. Conclusions. The administration of mesoglycan after an open diathermy excisional hemorrhoidectomy can reduce postoperative thrombosis and pain at 7–10 days after surgery, permitting a faster return to normal activities

    Potential for Circular Autopoietic Economy in High River Po Valley

    This Gigamap aims to describe and summarize a student work carried out during the semester course Open Systems Design at Politecnico di Torino. This is one of the outputs from an analysis of the High River Po Valley area in the Italian Piedmont (Southwest of Turin) and subsequently an in-depth study of the relationships and “flows” with certain “currencies” between some selected economic and public actors. The analysis was conducted through five economic sectors: nature & ecosystem services, tourism, mobility and infrastructure, local craft activities and agri-food. The investigation of the territorial economy was carried out by giving particular attention to the production sectors characteristic of the territory and examine their production lines. The holistic diagnosis has highlighted the existence of several problems related to the linearity of the production processes. Through the five types of system flows (material, CO2 emissions/energy, water, economic and social flows), we designed new opportunities, new activities and new potential companies, thinking circularly and systemically. The Gigamap will be presented to stakeholders in a public hearing and used to illustrate and incubate a more circular economy that is more resilient and more regenerative. Reading the map The reader can find in the first part an abstract with the aim of the Gigamap, the five topics for the investigation and actors’ selection, a timeline about important events and some peculiarities of the territory. In the middle, the territorial map of a suggested improved territorial economy based on circular flows: the 16 actors are localized on the territory with new circular flows “designed in” between them. The boxes describe connections for new circular opportunities, with flows explained under the territorial map—finally, some data about the territory and the three main outputs emerging from the new circular linkages
