Effectiveness of training on de-escalation of violence and management of aggressive behavior faced by health care providers in a public sector hospital of Karachi
Background: Considering high burden of violence against healthcare workers in Pakistan APPNA Institute of Public Health developed a training to prevent reactive violence among healthcare providers. The purpose of this training was to equip healthcare providers with skills essential to control aggressive behaviors and prevent verbal and non-verbal violence in workplace settings. This study assesses the effectiveness of training in prevention, de-escalation and management of violence in healthcare settings.Methods: A quasi-experimental study was conducted in October, 2016 using mixed method concurrent embedded design. The study assessed effectiveness of de-escalation trainings among health care providers working in emergency and gynecology and obstetrics departments of two teaching hospitals in Karachi. Quantitative assessment was done through structured interviews and qualitative through Focus Group Discussions. Healthcare providers` confidence in coping with patient aggression was also measured using a standard validated tool .Results: The overall self-perceived mean score of Confidence in Coping with Patient Aggression Instrument (CCPAI) scale was significantly higher in intervention group (Mean= 27.49, SD=3.53) as compared to control group (Mean= 23.92, SD=4.52) (p\u3c0.001). No statistically significant difference was observed between intervention and control groups with regard to frequency of violence faced by HCPs post training and major perpetrators of violence.Conclusion: De-escalation of violence training was effective in improving confidence of healthcare providers in coping with patient aggression
Recommendations for diagnostics and therapy of gastrointestinal stromal tumors (GIST) in 2008
Nowotwory podścieliskowe przewodu pokarmowego (gastrointestinal stromal tumors – GIST) są najczęstszymi nowotworami
pochodzenia mezenchymalnego w obrębie przewodu pokarmowego. Nowotwory te, dzięki postępom w diagnostyce
patologiczno-molekularnej, powszechnie rozpoznawane są dopiero od kilku lat. W pracy przedstawiono zalecenia dotyczące
diagnostyki i terapii tych nowotworów, opracowane na podstawie danych naukowych oraz doświadczenia ekspertów,
które są powszechnie akceptowane i warte rekomendacji. Nadekspresja błonowego receptora KIT, będąca następstwem
mutacji protoonkogenu KIT, jest wysoce specyficzna dla GIST, i możliwa do wykrycia metodami immunohistochemicznymi
(CD117) w preparatach histopatologicznych z guza, stanowiąc najważniejsze kryterium w jego diagnostyce mikroskopowej
i wskazaniach do leczenia drobnocząsteczkowymi inhibitorami kinaz tyrozynowych. W każdym przypadku zalecane jest
przesłanie materiału do badań molekularnych (w celu analizy mutacji KIT i PDGFRA). Radykalne leczenie operacyjne jest nadal najskuteczniejszą metodą leczenia pierwotnych GIST, jednakże nowotwór ten cechuje duży potencjał nawrotowości.
W przypadkach zmian nieoperacyjnych/rozsianych leczeniem z wyboru jest zastosowanie inhibitora kinaz tyrozynowych –
imatinibu, leku, który stanowi pierwsze efektywne leczenie systemowe w zaawansowanym GIST CD117(+). Zalecana dawka
początkowa powinna wynosić 400 mg raz dziennie (800 mg dziennie w przypadku mutacji w eksonie 9 KIT). Monitorowanie
leczenia musi opierać się na powtarzanym badaniu tomografii komputerowej jamy brzusznej, z uwzględnieniem zmian
wielkości i gęstości. W przypadku progresji zaleca się zwiększenie dawki imatinibu do 800 mg na dobę, a w przypadku
braku skuteczności zastosowanie sunitynibu w dawce początkowej 50 mg dziennie. Obecnie trwają badania kliniczne nad
ustaleniem roli leczenia chirurgicznego w skojarzeniu z imatinibem oraz skuteczności innych leków celowanych molekularnie
w przypadku występowania oporności w czasie leczenia imatinibem. Dostępne dane odnośnie leczenia uzupełniającego
wskazują na poprawę przeżyć wolnych od nawrotu, ale wpływ tej strategii na przeżycia całkowite i optymalna długość czasu
leczenia są nadal przedmiotem badań. Przedstawione zasady postępowania diagnostyczno-terapeutycznego powinny być
praktycznie wprowadzane przez lekarzy zajmujących się chorymi na GIST w Polsce. Zaleca się rejestrowanie przypadków
chorych na GIST w ramach narodowego Rejestru Klinicznego ([email protected]) oraz standardowe leczenie chorych
w wielodyscyplinarnych zespołach, mających doświadczenie w terapii GIST i włączanie nowych przypadków GIST do

GIST as a distinct sarcoma entity. The paper describes guidelines for diagnostics and therapy of these tumors based on
scientific basis and experts’ experience, which are commonly accepted and worth to recommend. Overexpression of KIT
receptor, as a consequence of mutation of protooncogene KIT, is highly specific for GIST and enable for detection by
immunohistochemistry staining (CD117) in tumor specimens. It is the most important criterion in microscopic diagnostics
and indications for treatment with small-molecule tyrosine kinas inhibitors. Sending material for molecular analysis is
strongly recommended (for KIT and PDGFRA genotyping). Radical surgery is still the mainstay treatment for primary,
localized, resectable GISTs, although high percentages of the patients after potentially curative operations develop recurrent
or metastatic disease. In inoperable/metastatic lesions the treatment of choice is tyrosine kinase inhibitor – imatinib mesylate
– the first effective systemic therapy in advanced CD117(+) GIST. Recommended initial dose should be 400 mg daily
(800 mg for exon 9 KIT mutants). Monitoring of the therapy should be based on serial computed tomography imaging of
abdominal cavity with the assessment of changes of tumor size and density. In case of disease progression the increase of
imatinib dose to 800 mg daily is recommended and if further progression exists – sunitinib in the initial dose 50 mg daily
should be introduced. Clinical trials evaluating the role of surgery in combination of imatinib and the efficacy of other
molecular targeted drugs in resistant cases are ongoing. Existing data indicate beneficial role of adjuvant imatinib therapy
in terms of relapse-free survival, but the impact of this policy on overall survival, optimal duration of adjuvant therapy and
criteria for its implementation are currently investigated. Presented recommendations for diagnostics and therapy of GIST
should be practically implemented by physicians involved in management of GIST patients in Poland. The including GIST
cases in national Clinical Registry ([email protected]) and standard treatment of patients in multidisciplinary team with
expertise in GIST therapy, as well as enrollment of new cases to prospective clinical trials, are recommended
Zalecenia dotyczące stosowania badań pozytonowej emisyjnej tomografii w onkologii
Positron emission tomography (PET) is a modern functional imaging method with proven value in diagnosing, staging, evaluating of response to anticancer therapy and detecting of relapses in numerous neoplasms. Utility, sensitivity and specificity of PET has increased by its use in combination with computed tomography (CT) or magnetic resonance in form of fusion PET-CT or PET-MR and the introduction of new radiotracers. This paper, prepared based on scientific evidence by a multidisciplinary group of authors, presents the utility and clinical recommendations for the application of PET-CT in oncology. PET-CT is particularly useful for:
— appropriate diagnosis and initial staging of patients with head and neck, lung, pancreatic and esophageal cancers as well as lymphomas, advanced melanomas and tumors of unknown primary site;
— detection of relapses inpatients with colorectal, thyroid, ovarian, head and neck, and breast cancers, as well as lymphomas;
— monitoring of response to therapy in patients with testicular and lung cancers, lymphomas and some types of sarcomas. PET-MR is particulary useful in pediatrics. Pozytonowa emisyjna tomografia (PET) jest nowoczesną metodą czynnościowego obrazowania o potwierdzonej wartości w rozpoznawaniu i określaniu stopnia zaawansowania, ustalaniu kategorii odpowiedzi na leczenie przeciwnowotworowe oraz wykrywaniu nawrotów wielu nowotworów. Użyteczność, czułość i swoistość PET znacznie zwiększyły się po połączeniu z badaniem za pomocą komputerowej tomografii (KT) lub magnetycznego rezonansu (MR) oraz wprowadzeniu nowych radiofarmaceutyków. Obecne opracowanie, przygotowane na podstawie dowodów naukowych przez wielodyscyplinarny zespół autorów, przedstawia użyteczność i zalecenia kliniczne stosowania badania PET-KT w onkologii. Badanie PET-KT jest szczególnie przydatne w:
— rozpoznawaniu i określaniu wyjściowego zaawansowania u chorych na nowotwory głowy i szyi, płuca, trzustki, przełyku oraz chłoniaki, zaawansowane czerniaki i nowotwory o nieznanym punkcie wyjścia;
— wykrywaniu nawrotów u chorych na nowotwory jelita grubego, tarczycy, jajnika, głowy i szyi, piersi oraz chłoniaki;
— określaniu odpowiedzi na leczenie u chorych na nowotwory jądra, płuca oraz chłoniaki i niektóre mięsaki. Badanie PET-MR jest szczególnie istotne w zastosowaniach pediatrycznych

Prevalence and determinants of violence against health care in the metropolitan city of Peshawar: a cross sectional study.
BackgroundViolence against healthcare personnel is a major public health problem. Healthcare personnel are at the frontline dealing with people in stressful and unpredictable situations. Therefore, this study was conducted to determine the prevalence and associated factors of violence against health care personnel.MethodsA cross sectional study was conducted in the district Peshawar. Healthcare personnel from public and private sectors working in both the primary and tertiary levels of healthcare were invited to participate. Violence was assessed through a structured questionnaire previously used in Pakistan and was defined as experiencing and/or witnessing any form of violence in the last 12 months. Mental health was assessed through the General Health Questionnaire. Logistic regression was used to estimate the association of violence against healthcare personnel with psychological distress and demographic characteristics. Data entry and analysis were conducted in STATA 14.ResultsA total of 842 healthcare personnel participated in the study. The prevalence of violence experienced and/or witnessed by healthcare personnel in Peshawar was 51%. Verbal violence remained the predominant form of violence and almost half of the healthcare personnel (45%) were exposed to it. A quarter of the respondents (24%) reported physical violence alone or in combination with other forms of violence. In almost two third of the incidents the perpetrators were either attendants, relatives or the patients. The emergency unit and wards within healthcare facilities were the most common places where violent events took place. The major factors responsible for the violent incidents were communication failure, unreasonable expectations and perceived substandard care. No uniform policy/procedure existed to manage the incidents and the healthcare personnel adopted different responses in the wake of violent events targeting health care. Working in public healthcare facilities and having a larger number of co-workers/colleagues significantly increased the risk of violence in the healthcare settings while being a paramedic significantly reduced the risk as compared to physicians.ConclusionsViolence against healthcare personnel is a serious public health issue and the prevalence is quite high. A holistic effort is needed by all stakeholders including healthcare community, the administration, lawmakers, law enforcement, civil society, and international organizations

The management of peritoneal surface malignancy is a significant clinical problem. The hyperthermic intraperitoneal chemotherapy (HIPEC) combined with cytoreductive surgery seems to be the most promising method, leading to better local control or improved overall survival in selected patients with a peritoneal neoplasm. In this paper the authors present a consensus for the optimal indications and technical guidelines for performing HIPEC in Poland.Leczenie nowotworów powierzchni otrzewnej stanowi istotny problem w onkologii.
