    Sensorineural Hearing Loss following Carbon Monoxide Poisoning

    A case study is presented of a 17-year-old male who sustained an anoxic brain injury and sensorineural hearing loss secondary to carbon monoxide poisoning. Audiological data is presented showing a slightly asymmetrical hearing loss of sensorineural origin and mild-to-severe degree for both ears. Word recognition performance was fair to poor bilaterally for speech presented at normal conversational levels in quiet. Management considerations of the hearing loss are discussed

My first chapter does two things. First, it centers on an exemplar to anchor a discussion regarding the substance and ubiquity of the image of queer affluence. Second, I provide context for the image by pointing to the tenuous status of GLTB issues in politics at the same time popular media is expanding its repertoire of GLTB images. The crux of Chapter Two, in addition to reviewing scholarly work, is to argue that queer representations are cumulative over time. The assumption is important because that quality provides the rationale for looking at selected images at particular times in order to understand images that came later. With that assumption in place, Chapter Three examines the construction of Leopold and Loeb as an early exemplar of queer affluence that was then recycled in the films noir of the 1940s and 1950s. I look at the construction of the queer men in these films and chart their relation to the stereotype of queer affluence seen in television programming today. In Chapter Four, I analyze queer celebrity spokespeople, figures that I believe are intertextually related to all the images that came before them. Finally, I conclude by painting queer affluence as a representational strategy that is deeply invested in both maintaining sexual differences and assimilating the affluent queer into mainstream culture at the same time minimizing any hints of radical politics and leaving many non-affluent queers behind.Includes bibliographical reference

    Hearing Loss in Osteogenesis Imperfecta: Characteristics and Treatment Considerations

    Osteogenesis imperfecta (OI) is the most common heritable disorder of connective tissue. It is associated with fractures following relatively minor injury, blue sclerae, dentinogenesis imperfecta, increased joint mobility, short stature, and hearing loss. Structures in the otic capsule and inner ear share in the histologic features common to other skeletal tissues. OI is due to mutations involving several genes, the most commonly involved are the COL1A1 or COL1A2 genes which are responsible for the synthesis of the proalpha-1 and proalpha-2 polypeptide chains that form the type I collagen triple helix. A genotype/phenotype relationship to hearing loss has not been established in OI. Hearing loss is commonly found in OI with prevalence rates ranging from 50 to 92% in some studies. Hearing loss in OI may be conductive, mixed, or sensorineural and is more common by the second or third decade. Treatment options such as hearing aids, stapes surgery, and cochlear implants are discussed

To date, the evidence for contraindications for SRT is limited to very few studies. The present study investigated the objective and subjective daytime consequences during the acute phase of SRT for adults diagnosed with Chronic Insomnia Disorder. Methods Sixteen adults (age = 36.3 ± 13.4 yrs, 12 females, 4 males) underwent SRT for their insomnia over a two week period based on recommendations by Miller and colleagues (2014) [6]. Participants completed sleep diaries, self-reported daytime sleepiness (Epworth Sleepiness Scale [ESS]), as well as objective measures of reaction time/inhibition (Go/NoGo task) and driving performance (AusEd driving simulator) at pre-, mid- (ie, after one week of SRT) and post-SRT (after two weeks of SRT). Results Sleep diary outcomes indicated participants complied with the restriction of time in bed, and that a similar amount of total sleep time (TST) was maintained from pre-to-post-treatment. There was no significant change in daytime sleepiness, and similarly no significant changes observed in objective performance on the Go/NoGo task and AusEd driving simulator. Conclusions These preliminary results suggest SRT during the acute phase does not appear to place insomnia patients at risk of significant impairments in sleepiness and reaction times. We note these findings can only be translated into clinical practice when sleep duration remains relatively unchanged. Future studies using objective measures of sleep and a control group are recommended

    Effectiveness of Dexmedetomidine for Sedation in Auditory Brainstem Response Testing

    Sedation is widely used for auditory brainstem response (ABR) testing for infants or young children who are unable to sleep or remain adequately quiet for testing. Because chloral hydrate is no longer readily available, dexmedetomidine has been proposed as an alternative medicinal agent to achieve moderate levels of sedation without risk of respiratory depression. The purpose of the study was to assess the effectiveness of dexmedetomidine in terms of the completeness of the audiologic data obtained in achieving moderate levels of sedation for auditory brainstem response testing. A retrospective chart review was conducted on ninety-nine patients at Kennedy Krieger Institute. Participants were administered either chloral hydrate or dexmedetomidine prior to ABR testing. Effectiveness was defined as having obtained thresholds for click and tone burst stimuli centered at 500, 2000, and 4000 Hz for both ears. Complete audiological data were obtained on 92.2% of patients sedated with dexmedetomidine whereas complete audiological data were obtained on 91% of patients sedated with choral hydrate in a period prior to the use of dexmedetomidine. It was concluded that dexmedetomidine is as effective as chloral hydrate in producing an appropriate state for sedated auditory brainstem response testing

    Unsaturated fatty acids lactose esters: cytotoxicity, permeability enhancement and antimicrobial activity

    Sugar based surfactants conjugated with fatty acid chains are an emerging broad group of highly biocompatible and biodegradable compounds with established and potential future applications in the pharmaceutical, cosmetic and food industries. In this work, we investigated absorption enhancing and antimicrobial properties of disaccharide lactose, mono-esterified with unsaturated fatty acids through an enzymatic synthetic approach. After chemical and cytotoxicity characterizations, their permeability enhancing activity was demonstrated using intestinal Caco-2 monolayers through transepithelial electrical resistance (TEER) and permeability studies. The synthesised compounds, namely lactose palmitoleate (URB1076) and lactose nervonate (URB1077), were shown to exhibit antimicrobial activity versus eight pathogenic species belonging to Gram-positive, Gram-negative microorganisms and fungi
