    Polymorphisms of the TUB Gene Are Associated with Body Composition and Eating Behavior in Middle-Aged Women

    BACKGROUND: The TUB gene, encoding an evolutionary conserved protein, is highly expressed in the hypothalamus and might act as a transcription factor. Mutations in TUB cause late-onset obesity, insulin-resistance and neurosensory deficits in mice. An association of common variants in the TUB gene with body weight in humans has been reported. METHODS/FINDINGS: The aim was to investigate the relationship of single nucleotide polymorphisms (SNPs) of the TUB gene (rs2272382, rs2272383 and rs1528133) with both anthropometry and self-reported macronutrient intake from a validated food frequency questionnaire. These associations were studied in a population-based, cross-sectional study of 1680 middle-aged Dutch women, using linear regression analysis. The minor allele C of the rs1528133 SNP was significantly associated with increased weight (+1.88 kg, P = 0.022) and BMI (+0.56 units, P = 0.05). Compared with non-carriers, both AG heterozygotes and AA homozygotes of the rs2272382 SNP derived less energy from fat (AG: -0.55+/-0.28%, P = 0.05, AA: -0.95+/-0.48%, P = 0.047). However, both genotypes were associated with an increased energy intake from carbohydrates (0.69+/-0.33%, P = 0.04 and 1.68+/-0.56%, P = 0.003, respectively), mainly because of a higher consumption of mono- and disaccharides. Both these SNPs, rs2272382 and rs1528133, were also associated with a higher glycemic load in the diet. The glycemic load was higher among those with AG and AA genotypes for the variant rs2272382 than among the wild types (+1.49 (95% CI: -0.27-3.24) and +3.89 (95% CI: 0.94-6.85) units, respectively). Carriers of the minor allele C of rs1528133 were associated with an increased glycemic load of 1.85 units compared with non-carriers. CONCLUSIONS: Genetic variation of the TUB gene was associated with both body composition and macronutrient intake, suggesting that TUB might influence eating behavior

    The development and validation of a scoring tool to predict the operative duration of elective laparoscopic cholecystectomy

    Background: The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods: Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results: After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p  90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion: The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care

    Fatigue crack initiation and growth behavior in a notch with periodic overloads in the low-cycle fatigue regime of FV566 ex-service steam turbine blade material

    The effect of periodic overloads on crack initiation, growth rate and fatigue life, within a notch stress field representative of a turbine blade root fixing has been investigated. Bend bars made from FV566 martensitic stainless steel were extracted from the root of ex-service power plant turbine blades and representative notches introduced. These notched plain bend bars were loaded plastically in the low cycle fatigue regime and were tested with overloads up to 150% of the cyclic baseload every 150 baseload cycles. A periodic overload of 50% of the cyclic baseload increased the number of cycles to crack initiation and slightly retarded the crack growth rates of both short and long cracks, leading to a slight improvement in fatigue life. The results suggest that small overloads (less than 10%) are not expected to significantly impact fatigue lifetimes or service scheduling of components such as Low Pressure steam turbine blades

    A synthesis of convergent reflections, tensions and silences in linking gender and global environmental change research

    This synthesis article joins the authors of the special issue "Gender perspectives in resilience, vulnerability and adaptation to global environmental change" in a common reflective dialogue about the main contributions of their papers. In sum, here we reflect on links between gender and feminist approaches to research in adaptation and resilience in global environmental change (GEC). The main theoretical contributions of this special issue are threefold: emphasizing the relevance of power relations in feminist political ecology, bringing the livelihood and intersectionality approaches into GEC, and linking resilience theories and critical feminist research. Empirical insights on key debates in GEC studies are also highlighted from the nine cases analysed, from Europe, the Americas, Asia, Africa and the Pacific. Further, the special issue also contributes to broaden the gender approach in adaptation to GEC by incorporating research sites in the Global North alongside sites from the Global South. This paper examines and compares the main approaches adopted (e.g. qualitative or mixed methods) and the methodological challenges that derive from intersectional perspectives. Finally, key messages for policy agendas and further research are drawn from the common reflection.Este artículo de síntesis une a los autores del número especial "Gender perspectives in resilience, vulnerability and adaptation to global environmental change" en un diálogo reflexivo común sobre las principales contribuciones de sus documentos. En resumen, aquí reflexionamos sobre los vínculos entre los enfoques feministas y de género a la investigación en adaptación y resiliencia en el cambio ambiental global (GEC). Las principales contribuciones teóricas de este número especial son tres: enfatizar la relevancia de las relaciones de poder en la ecología política feminista, incorporar los enfoques de medios de subsistencia e interseccionalidad al GEC y vincular las teorías de la resiliencia y la investigación feminista crítica. También se destacan las ideas empíricas sobre los debates clave en los estudios de GEC de los nueve casos analizados, de Europa, América, Asia, África y el Pacífico. Además, el número especial también contribuye a ampliar el enfoque de género en la adaptación a GEC mediante la incorporación de sitios de investigación en el Norte Global junto a sitios del Sur Global. Este documento examina y compara los principales enfoques adoptados (por ejemplo, los métodos cualitativos o mixtos) y los desafíos metodológicos que se derivan de las perspectivas interseccionales. Finalmente, los mensajes clave para las agendas políticas y la investigación adicional se extraen de la reflexión común.Aquest article de síntesi uneix els autors del número especial "Gender perspectives in resilience, vulnerability and adaptation to global environmental change" en un diàleg reflexiu comú sobre les principals contribucions dels seus documents. En resum, aquí reflexionem sobre els vincles entre els enfocaments feministes i de gènere a la investigació en adaptació i resiliència en el canvi ambiental global (GEC). Les principals contribucions teòriques d'aquest número especial són tres: emfatitzar la rellevància de les relacions de poder en l'ecologia política feminista, incorporar els enfocaments de mitjans de subsistència i interseccionalitat al GEC i vincular les teories de la resiliència i la investigació feminista crítica. També es destaquen les idees empíriques sobre els debats clau en els estudis de GEC dels nou casos analitzats, d'Europa, Amèrica, Àsia, Àfrica i el Pacífic. A més, el número especial també contribueix a ampliar l'enfocament de gènere en l'adaptació a GEC mitjançant la incorporació de llocs d'investigació al Nord Global al costat de llocs del Sud Global.
