    Qualitative evaluation of a practice-based experience pilot program for master of pharmacy students in Scotland

    Objective. To determine the views of pharmacists in central Scotland regarding experiential education for MPharm students. Methods. A thematic analysis was completed by Ms. Gillian Hendry and Dr. Sally Wiggins of interviews conducted with ten practicing pharmacists paired with first-year master of pharmacy (MPharm) students during the 2011-2012 academic year. Relevant comments from the interviews were manually sorted in a Microsoft Excel spreadsheet to bring similarly themed material together to facilitate the identification and naming of recurring themes and subthemes. Results. The pharmacists were unanimous in their opinion that experiential education was valuable for MPharm students and, in particular, that it helped students to develop self-confidence. The pharmacists derived personal satisfaction in developing mentor/mentee relationships with students. They also recognized the value that students provided to the workforce as well as the educational value to themselves in supervising students. The participants’ primary dissatisfaction was that the pharmacy workflow limited the time they could spend mentoring students. Conclusion. The results provide guidance to the academic community and the pharmacy practice community in the United Kingdom (UK) regarding the design and integration of experiential education courses in MPharm degree programs

    Redesigned and chemically-modified hammerhead ribozymes with improved activity and serum stability

    BACKGROUND: Hammerhead ribozymes are RNA-based molecules which bind and cleave other RNAs specifically. As such they have potential as laboratory reagents, diagnostics and therapeutics. Despite having been extensively studied for 15 years or so, their wide application is hampered by their instability in biological media, and by the poor translation of cleavage studies on short substrates to long RNA molecules. This work describes a systematic study aimed at addressing these two issues. RESULTS: A series of hammerhead ribozyme derivatives, varying in their hybridising arm length and size of helix II, were tested in vitro for cleavage of RNA derived from the carbamoyl phosphate synthetase II gene of Plasmodium falciparum. Against a 550-nt transcript the most efficient (t(1/2 )= 26 seconds) was a miniribozyme with helix II reduced to a single G-C base pair and with twelve nucleotides in each hybridising arm. Miniribozymes of this general design were targeted to three further sites, and they demonstrated exceptional cleavage activity. A series of chemically modified derivatives was prepared and examined for cleavage activity and stability in human serum. One derivative showed a 10(3)-fold increase in serum stability and a doubling in cleavage efficiency compared to the unmodified miniribozyme. A second was almost 10(4)-fold more stable and only 7-fold less active than the unmodified parent. CONCLUSION: Hammerhead ribozyme derivatives in which helix II is reduced to a single G-C base pair cleave long RNA substrates very efficiently in vitro. Using commonly available phosphoramidites and reagents, two patterns of nucleotide substitution in this derivative were identified which conferred both good cleavage activity against long RNA targets and good stability in human serum

    Vibrating grid as a tool for studying the flow of pure He II and its transition to turbulence

    We report a detailed experimental study of the flow of isotopically-pure He II, generated by a vibrating grid. Our measurements span a wide range of temperatures (50 mK < T < 1.37 K) and pressures (2 bar < p < 15 bar). The response of the grid was found to be of a Lorentzian form up to a sharply-defined threshold value. This threshold value does not change appreciably with pressure; the form of the resonant response of the grid is qualitatively the same for all temperatures while the threshold value is a monotonically increasing function of temperature. We discuss the measured variation of the resonant frequency of the grid as a function of applied pressure (density) of He II and relate this to a hydrodynamic effective mass of the grid. These measurements extend our previously reported studies [Nichol et al, Phys. Rev. E 70, 056307 (2004)] and form an integral part of a series of experiments aimed at providing a better understanding of classical and quantum turbulence

    Real-world evaluation of an acceptance and commitment therapy–based group programme for breast cancer survivors with fear of cancer recurrence

    Purpose: To evaluate the effectiveness and acceptability of a 6-week acceptance and commitment therapy (ACT)–based group programme on participants’ fear of cancer recurrence (FCR), quality of life (QoL), psychological distress and psychological flexibility at the end of the programme and 12-week follow-up. Methods: A one-group, post-test service evaluation of a real-world psychological programme was carried out to evaluate collected outcome measures and attendance for a total of 21 groups facilitated between 2017 and 2019. Participants were breast cancer survivors who attended a 6-week group programme led by NHS clinicians. Descriptive statistics and repeated measures ANOVA analyses were carried out for each outcome measure. Attendance levels were examined to assess acceptability. Results: A total of 97 group participants who had completed curative treatment for breast cancer took part. Of whom, 89% completed at least 4 of the 6 weekly group sessions and 76% attended the 12-week follow-up session. Eighty-four (87%) participants returned outcome measures at all three time points relative to group participation (T1 = pre, T2 = post T3 = 12-week follow-up). Group participants were female, mean age 51.9 years. FCR was highest at T1 (mean 25.2, SD 4.7), reduced T2 (mean 21.2, SD 5.4) and further lowered T3 (mean 19.5, SD 6.2). This difference was statistically significant (p &lt; 0.001). QoL was lowest at T1 (mean 62.4, SD 15.7), increased T2 (mean 71.7, SD 18.1) and further increased at T3 (mean 75.9, SD 17.5). This difference was statistically significant (p &lt; 0.001). Psychological distress measures were shown to reduce, and psychological flexibility increased. Conclusions: This real-world evaluation of an ACT-based group programme led to improvements in FCR, QoL, psychological distress and psychological flexibility in this population. This evaluation provides basis for further investigation to determine if these results can be replicated by controlled research design across diverse populations

    ‘On the outside I’m smiling but inside I’m crying’: communication successes and challenges for undergraduate academic writing

    Student difficulties with the transition to writing in higher education are well documented whether from a ‘study skills’, an ‘academic socialisation’ or an ‘academic literacies’ perspective. In order to more closely examine the challenges faced by students from widening participation backgrounds and diverse routes into undergraduate study, this project focuses on first-year undergraduate experiences of developing academic literacies on an Education Studies programme at one university in England. It highlights the impact of different support and guidance within and beyond their degree programme where attempts to embed academic literacy development are part of subject modules. The paper reports the findings generated using a mixed methods interpretive approach. Questionnaires were collected at the beginning (n = 48) and end of the students’ first year (n = 44), and interviews and visual data collection methods (n =19) were used at the mid-point of the academic year. Key findings highlight students’ expectations of achievement on entry to university and the influence of the emotional journey of students as they begin to make progress as academic writers. Identifying, selecting and applying academic reading were an enduring concern whilst some students struggled with the digital literacy implicit in undergraduate work. Importantly, some strategies developed to support student transition to academic writing in higher education may have unintended consequences as they progress through the first year

    The Arctic and the UK: Climate, research and engagement

    • The Arctic has warmed by around 2°C since 1850, approximately double the global average. Even if the Paris Agreement successfully limits global warming to a further 0.5°C, the Arctic is expected to warm by at least another 1°C. • The United Kingdom’s (UK) weather is linked to conditions in the European Arctic. For example, high atmospheric pressure in the Nordic Seas divert damaging storms across the UK and mainland Europe, with the potential to cause societal disruption from flooding. • It is possible, although presently unconfirmed, that alterations in Arctic conditions provoked the ‘Beast from the East’ winter storm in 2018. • Scientists need to take observations and improve their understanding of climatic processes in the Nordic Seas and the Arctic Ocean to fill gaps in knowledge about the links between the Arctic climate and the UK’s weather; a risk identified by the Intergovernmental Panel on Climate Change (IPCC). • The UK has significant research expertise and experience to understand how global warming will change the Arctic’s environment and affect the UK. • This strength, allied with the capabilities of the UK’s new polar research ship the RRS Sir David Attenborough, warrants an integrated programme of research, including advanced numerical modelling, to improve predictions of future extreme weather events. • Such a programme must acknowledge that the Arctic is politically an increasingly congested and contested space. It should be designed in collaboration with key Arctic and near-Arctic nations to increase the UK’s influence and abilit

    Disaster preparedness and response capacity of regional hospitals in Tanzania: a descriptive cross-sectional study

    Background Tanzania has witnessed several disasters in the past decade, which resulted in substantial mortality, long-term morbidity, and significant socio-economic losses. Health care facilities and personnel are critical to disaster response. We assessed the current state of disaster preparedness and response capacity among Tanzanian regional hospitals. Methods This descriptive cross-sectional survey was conducted in all Tanzanian regional hospitals between May 2012 and December 2012. Data were prospectively collected using a structured questionnaire based on the World Health Organization National Health Sector Emergency Preparedness and Response Tool. Trained medical doctors conducted structured interviews and direct observations in each hospital. Results We surveyed 25 regional hospitals (100% capture) in mainland Tanzania, in which interviews were conducted with 13-hospital doctors incharge, 9 matrons and 4 heads of casualty. All the hospitals were found to have inadequate numbers of all cadres of health care providers to support effective disaster response. 92% of hospitals reported experiencing a disaster in the past 5 years; with the top three being large motor vehicle accidents 22 (87%), floods 7 (26%) and infectious disease outbreaks 6 (22%). Fifteen hospitals (60%) had a disaster committee, but only five (20%) had a disaster plan. No hospital had all components of surge capacity. Although all had electricity and back-up generators, only 3 (12%) had a back-up communication system. Conclusion This nationwide survey found that hospital disaster preparedness is at an early stage of development in Tanzania, and important opportunities exist to better prepare regional hospitals to respond to disasters

    Reanalysis and Simulation Suggest a Phylogenetic Microarray Does Not Accurately Profile Microbial Communities

    The second generation (G2) PhyloChip is designed to detect over 8700 bacteria and archaeal and has been used over 50 publications and conference presentations. Many of those publications reveal that the PhyloChip measures of species richness greatly exceed statistical estimates of richness based on other methods. An examination of probes downloaded from Greengenes suggested that the system may have the potential to distort the observed community structure. This may be due to the sharing of probes by taxa; more than 21% of the taxa in that downloaded data have no unique probes. In-silico simulations using these data showed that a population of 64 taxa representing a typical anaerobic subterranean community returned 96 different taxa, including 15 families incorrectly called present and 19 families incorrectly called absent. A study of nasal and oropharyngeal microbial communities by Lemon et al (2010) found some 1325 taxa using the G2 PhyloChip, however, about 950 of these taxa have, in the downloaded data, no unique probes and cannot be definitively called present. Finally, data from Brodie et al (2007), when re-examined, indicate that the abundance of the majority of detected taxa, are highly correlated with one another, suggesting that many probe sets do not act independently. Based on our analyses of downloaded data, we conclude that outputs from the G2 PhyloChip should be treated with some caution, and that the presence of taxa represented solely by non-unique probes be independently verified

    Metamorphism obscures primary taphonomic pathways in the early Cambrian Sirius Passet Lagerstätte, North Greenland

    Correct interpretation of soft-bodied fossils relies on a thorough understanding of their taphonomy. While the focus has often been on the primary roles of decay and early diagenesis, the impacts of deeper burial and metamorphism on fossil preservation are less well understood. We document a sequence of late-stage mineral replacements in panarthropod fossils from the Sirius Passet Lagerstätte (North Greenland), an important early Cambrian Burgess Shale–type (BST) biota. Muscle and gut diverticula were initially stabilized by early diagenetic apatite, prior to being pervasively replaced by quartz and then subordinate chlorite, muscovite, and chloritoid during very low- to low-grade metamorphism. Each new mineral replicates the soft tissues with different precision and occurs in particular anatomical regions, imposing strong biases on the biological information retained. Muscovite and chloritoid largely obliterate the tissues’ original detail, suggesting that aluminum-rich protoliths may have least potential for conserving mineralized soft tissues in metamorphism. Overall, the fossils exhibit a marked shift toward mineralogical equilibration with the matrix, obscuring primary taphonomic modes. Sequential replacement of the phosphatized soft tissues released phosphorus to form new accessory monazite (and apatite and xenotime), whose presence in other BST biotas might signal the prior, more widespread, occurrence of this primary mode of preservation. Our results provide critical context for interpreting the Sirius Passet biota and for identifying late-stage overprints in other biotas
